초등학교 고학년생의 이물질에 의한 완전기도폐쇄 응급처치 실기교육이 학습동기, 응급처치 지식과 기술에 미치는 영향 (The Effect of Complete Airway Obstruction Maneuver Training Program on the Learning Motivation, Knowledge and Skill of Choking Management)

  • The purpose of this study was to develope the Complete Airway Obstruction Maneuver Training Program and identify its effects on learning motivation, knowledge and skill of choking management in the primary school students. The subjects for the experimental group of 38 students and the control group of 39 students, all of whom are primary school students in Gwangju, the Republic of Korea. A non-equivalent control group pretest-posttest design was used and data were collected from November to December, 2003. During about 2 weeks, 2 times for a week with 80 minutes at a time, the complete airway obstruction maneuver training program was conducted in the experimental group. Experimental data were analyzed through SPSS/win 11.0 PC+, and the tests examining general characteristics between the experimental group and the control group were conducted through $x^2$-test. Fisher's exact probability test and t-test, and identifying the effect of the complete airway obstruction Maneuver training program was analysed through t-test and ANCOVA. The results of the study were as follows: 1. After intervention on the complete airway obstruction maneuver training program, Learning motivation in the experimental group was significantly higher than that of the control group. 2. After intervention on the complete airway obstruction maneuver training program, knowledge of choking management the experimental group was significantly higher than that of the control group(F=223.637, p=.000). 3. After intervention on the complete airway obstruction maneuver training program, skill of choking management the experimental group was significantly higher than that of the control group(t=46.800, p=.014). These findings suggest that the complete airway obstruction maneuver training program can facilitate learning motivation, knowledge and skill of choking management in the primary school students. Therefore it is considered that the complete airway obstruction maneuver training program can be utilized as a effective way to implement the 7th national curriculum for creative extra-activities.

  • Purpose: The purpose of this study is to develop a realistic illustration using watercolors on a complete airway obstruction maneuver that is at an appropriate level for elementary school students from the first grade through the third. The illustration is a series of cuts divided into specific frames designed to illustrate the method to be followed in an action or a practical skill that would otherwise be too difficult to explain in written form. Methods: The fundamental theory of the Jeong In-Sung's instruction learning program based on the WEB to construct an appropriate site of instruction learning was used after modification. In this study analysis, design, and production stages were realized. Results: 1. In Stage 1, impressions of the illustration, in sketch form, were received. In Stage 2, results of impressions from a modified illustration were received. After an appropriate level of understanding and accuracy in the practical steps for emergency maneuvers were achieved the illustrations entering Stage 3 and color was added to the sketches. 2. In Stage 3, the illustration was completed by adding color to the contents for the practical steps in the complete airway obstruction maneuver. 3. For composition evaluation, a result of 100% appropriateness was obtained for both items of harmony between illustration and subject being studied and sufficiency in quantity of illustrations. Also high results for appropriateness in items such as clarity, creating interest, illustration size, and tone preference as related to content were achieved and thus completed the production stages. Conclusion: The Complete Airway Obstruction Maneuver Illustration developed specifically for the characteristics and cognitive level of elementary school students grade 1 through grade 3 was appropriate for teaching and learning. There is a need for future research on developed illustrations aimed at elementary school students grade 1 through 3 on the complete airway obstruction maneuver. There is also a need for continued effort in developing certified evaluating tools that can judge the suitability of illustrations and developing illustrations related to emergency maneuvers for various subjects and appropriate for learner level.

수중함의 긴급기동 해석을 위한 유체력계수 모델링에 관한 연구 (A Study on the Modeling of Hydrodynamic Coefficient for the Emergency Maneuver Simulation of Underwater Vehicle)

  • This paper describes a hydrodynamic modelling study based on the Feldman's equation to predict the nonlinear and coupled maneuvering characteristics of high speed submarine. The hydrodynamic coefficients set is obtained from the modeling of the cross flow drag force and sail induced vorticity, and the captive model experiments(VPMM and RA test) results used to improved the accuracy. The results contained in this paper will be helpful to predict the behavior of tight turn maneuver and to improve the SOE(Safety Operational Envelope) analysis in case of emergency maneuver.

잠수함의 제어판 재밍에 대한 안전운항영역 설정 (A Study on the Safety Operational Envelope of a Submarine in Jamming)

  • Safety operational envelope (SOE) is the area which guarantees the safety of a submarine from the accident such as jamming and flooding. The maximum safe depth is set to prevent the damage to the hull from increasing water pressure with depth. A minimum safety depth is set to prevent a submarine from the exposure above the free surface and collision against surface ship. The prediction method for the SOE in the design phase is needed to operate the submarine safely. In this paper, the modeling and calculation methods of the SOE are introduced. Main ballast tank blowing modeling and propeller force modeling are conducted to simulate the accidents and the recovery process. The SOEs are established based on the crash stop and emergency rising maneuver simulation. From the simulation results, it can be known that the emergency rising maneuver is more effective recovery action than the crash stop.

The duodenal window approach to pancreatoduodenectomy

  • The pancreatoduodenectomy (PD) technique is yet to be standardized. One of the most difficult passages in PD is the mobilization of the second, third, and fourth parts of the duodenum. This maneuver is classically performed from the supramesocolic space after the division of the gastrocolic ligament, but traction on the transverse mesocolon and the superior mesenteric pedicle can cause bleeding from the venous and arterial branches of the pancreatic head and uncinate process. We hereby describe a technique to access and mobilize the distal duodenum and proximal jejunum (D2 to J1) through the duodenal window and the Treitz's foramen, performing an almost complete Kocher's maneuver before opening the gastrocolic ligament and mobilizing the hepatic flexure. The anatomical basis and the surgical technique of the duodenal-window-first PD are discussed. The duodenal-window-first approach is a standardizable step of PD that allows an easy and safe mobilization of D2 to J1. This technique has been applied to 15 cases of PD, both open and robotic, with no specific morbidity. Therefore, we propose the adoption of the duodenal-window-first technique as a routine standardized step of PD.

  • In order to develop an active safety system which avoids or mitigates collisions with preceding vehicles such as autonomous emergency braking (AEB), accurate state estimation of the nearby vehicles is very important. In this paper, an algorithm is proposed using 3 dynamic models to better estimate the state of a vehicle which has various dynamic patterns in both longitudinal and lateral direction. In particular, the proposed algorithm is based on the Interacting Multiple Model (IMM) method which employs three different dynamic models, in cruise mode, lateral maneuver mode and longitudinal maneuver mode. In addition, a Probabilistic Data Association Filter (PDAF) is utilized as a data association algorithm which can improve the reliability of the measurement under a clutter environment. In order to verify the performance of the proposed method, it is simulated in comparison with a Kalman filter method which employs a single dynamic model. Finally, the proposed method is validated using radar data obtained from the field test in the proving ground.

Experimental study on hydrodynamic coefficients for high-incidence-angle maneuver of a submarine

  • Snap rolling during hard turning and instability during emergency rising are important features of submarine operation. Hydrodynamics modeling using a high incidence flow angle is required to predict these phenomena. In the present study, a quasi-steady dynamics model of a submarine suitable for high-incidence-angle maneuvering applications is developed. To determine the hydrodynamic coefficients of the model, static tests, dynamic tests, and control surface tests were conducted in a towing tank and wind tunnel. The towing tank test is conducted utilizing a Reynolds number of $3.12{\times}10^6$, and the wind tunnel test is performed utilizing a Reynolds number of $5.11{\times}10^6$. In addition, least squares, golden section search, and surface fitting using polynomial models were used to analyze the experimental results. The obtained coefficients are presented in tabular form and can be used for various purposes such as hard turning simulation, emergency rising simulation, and controller design.

성상신경절 차단으로 치료했던 양성 발작성 현기증 1예 (A Case of Benign Paroxysmal Positional Vertigo Treated with Stellate Ganglion Block)

  • A fourty-year-old housewife was admitted to the Department of Ear, Nose, and Throat from the emergency room, because of severe vertigo associated with positional changes of the head. Hallpike maneuver produced typical patterns of bilateral nystagmus consistent with benign paroxysmal positional vertigo(BPPV), and further geotropic rotatory nystagmus with positional change of the head in each direction. Direction of nystagmus reversed when the patients was repositioned upright. Particle repositioning maneuver was administered to her left ear but failed to relieve the severe dizziness associated with the head turning to the right. She was then referred to the patient received 27 SGB treatments and the patients was almost completely healed. It is unusual for the BPPV to occur bilaterally therefore reference concerning the effectiveness of SGB for the treatment of BPPV was difficult to obtain. In conclusion we would like to report the successful treatment of BPPV with SGB.

초등학교 고학년생의 응급처치 교육실태 및 교육 요구도 (A Study on The Actual Condition and Demand Assessment of First Aid Education on Higher Grade Students in Elementary School)

  • Background and Purpose : Higher grade students in elementary schools are most adequate subjects for first aid training. The purpose of this study was to assess first aid education and needs of higher grade students in elementary schools. Method : The subjects of this study were 183 higher grade students from 8 elementary schools. Data were collected by the questionnaire during the period from March 19 to April 13, 2007. The data were analyzed through frequency, Cronbach's ${\alpha}$, Independent Two samples t-test, One Way ANOVA by SPSS win 12.0. Result : 1. It showed that 78.1%(143 persons) of sujects answered that they learned first aid. 65% of sujects learned in the school(65%). 61.2% of sujects were taught by health teachers. 36.7%(67 person) of subjects was educated using practice with demonstration including lecture. Learned contents were action at emergency(50.8%), CPR(36.6%), splint (33.9%). 2. It showed that 90.2%(165 persons) of subjects answered that first aid and CPR education are necessary. Also 74.9%(137 persons) of subjects answered that will be educated first aid and CPR if opportunities is given. The 53.3%(73 persons) of subjects wanted teaching method using practice with demonstration including lecture. 3. The total mean showed $2.29{\pm}.48$ in needs of first aid education by 3 points Likert scale. Needs of first aid education was ranked Heimlich maneuver($2.41{\pm}.65$), splint and bandaging($2.38{\pm}.59$). Priority of intensive training showed patient assessment(38.0%) and CPR(19.7%) in first, splint and bandging(22.6%), CPR(21.9%) and Heimlich maneuver(21.9%) in second. 4. The needs assessment of first aid education showed statistically significant differences according to teaching method(F = 2.563, p = .025), education necessity yes or no(F = 2.474, p = .015), attending future education yes or no(F = 2.253, p = .026). Conclusion : These results suggest that First aid education for higher grade students in elementary schools must be consisted of most adequate content and method based on current education condition and needs assessment.

다목적 회항조선 Double 30 ( woo ) Turn 조성법의 개발에 관한 연구 ( On the New Development of Double 30 ( woo ) Turn Maneuvers )

  • There are some turn maneuvers, such as hard Round Turn, Turning Short Round, Williamson Turn and Single Turn, of effectively returning a ship back close to the original track in case of emergency situations of man-overboard and intentional delay of port entrance time, and so forth. But the author developed the new turn maneuvers of Double 30 (woo) Turn originally by means of simulation experiments of Full-Mission Shiphandling Simulator. The conclusions of this study are as follows: 1) By comparison of existing Williamson Turn and Turning Short Round this Double 30(woo) Turn is all-weather multipurpose turn maneuvers in some respects, 2) According to simulation experiments this Double 30 (woo) Turn saved approximately 20-35% of total turning time to the original position and reduced maximum advances by 8-10% approximately in comparison to that of Williamson Turn, 3) Except for emergency men-overboard the Reverse Williamson Turn was proposed as a new reverse maneuver of returning a ship back to the original track.
