Objective: This study examined whether emergency department (ED) crowding influences the timing of the initial assessment and treatment in severe trauma patients, as well as their mortality rates. Methods: This retrospective, observational study was conducted between January 2015 and October 2016, and included adult severe trauma patients who presented to the ED. The emergency department occupancy rate (EDOR) was used to measure ED crowding. The patients were divided into four groups using the EDOR quartile. The timeliness of the initial assessment and treatment in the four groups as well as the mortality rates were compared. Results: This study investigated 307 patients. The timing of the first computed tomography (CT) and laboratory test order, CT and laboratory test result acquisition, first transfusion, and patient transfer from the ED to the operating room were similar in the four groups. Multivariable logistic regression analysis did not show a significant difference in mortality between the groups. Conclusion: ED crowding was not associated with delays in the initial assessment and treatment of severe trauma patients, or in their mortality rates.
Endosulfan, one of organochlorine insecticides, is $\gamma$-aminobutyric acid (GABA) antagonist. In sufficient dose, this pesticide lower the seizure threshold and produce CNS stimulation, with resultant seizures, respiratory failure, and death. In patients with endosulfan intoxication, the first manifestation of toxicity is largely a generalized seizure without prodromal signs or symptoms. So the management of airway and seizure control are essential for survival and prognosis of intoxicated patients. We report two cases of acute endosulfan poisoning who manifest 'status epilepticus' similarly, but have different prognosis.
Laundery detergent ingestions are generally considered to have minor consequences. But some detergent ingestions have severe consequences. Unlike household bleaches contain sodium hypochlorite, bleaching agents that contain sodium carbonate have the potential to cause significant mucosal damage to the gastrointestinal tract if ingested. Especially, when ingested in solid form of Sodium carbonate, corrosive injury is much heavier. Therefore, patient who ingest sodium carbonate peroxyhydrate need more intensive management than patient who ingest other bleaches. We experienced a case of serious caustic injury after ingestion of Sodium Carbonate Peroxyhydrate. We report this case with review of literature.
Carbon dioxide is the fourth most abundant gas in the earth's atmosphere, and it is widely used in the chemical industry. Solid carbon dioxide is commonly known as dry ice. At low concentration, carbon dioxide appears to have little toxicological effect. At higher concentrations, however, it can produce an increased respiratory rate, tachycardia, cardiac arrhythmia, loss of consciousness, convulsion, and even death. Management of carbon dioxide poisoning requires the immediate removal of an individual from the toxic environment and administration of oxygen. It is important to know the concentration of carbon dioxide to which a patient has been exposed. We report a case of acute poisoning from solid carbon dioxide in a patient presenting with drowsiness and diminished mental capacity when she arrived in the emergency department. She recovered completely after administration of oxygen with conservative treatment.
The necessity for services of emergency communication systems to be uninterrupted and reliable has brought forward strict requirements for express software upgrades of base stations. Based on reconfigurability technology, this paper presents a protocol reconfiguration scheme for emergency communication base stations. By introducing the concept of 'local safe state,' the proposed scheme could make the updating and replacement of the protocol software flexible and maintain interactions during the updating procedure to achieve seamless system upgrades. Further, taking TETRA protocol stack and VxWorks operating system as an example, specific processes and realization methods of reconfiguration are proposed in this paper, along with the measurements of the factors impacting on the system performance. Simulation results show that, compared with traditional technology, the method proposed significantly improves the system efficiency and does not interrupt system services.
Simulators were introduced in education as a tool to make advanced training standardized, less expensive, and without danger to those involved. In 1922 in the Unites States, Edward Link presented his homemade flight simulator, which became common place in both military and civilian aviation, known as the "Link Trainer". The development of mannequin simulators used for education, training, and research is reviewed, tracing the motivations, evolution to commercial availability, and efforts toward assessment of efficacy of those for teaching cardiopulmonary resuscitation, cardiology skills, anaesthesia clinical skills, and crisis management. This will provide a brief overview of simulators and trainers in several domains.
Purpose: The purpose of this study was to develop and apply a simulation-problem based learning(S-PBL) package in basic life support(BLS). Methods: This study was used to developed BLS' S-PBL package throughout 12 steps of PBL package development model. Then, BLS' S-PBL methodology was implemented in second year student in the undergraduate emergency medical technology, and survey was done. Results: 1. S-PBL package model was presented based on conceptual model II of PBL ; PEL for professional action. 2. Quantitative analysis of survey was to the effectiveness of learning, 3.59 points ; 4.15 of BLS and 3.84 of integration and practical use of knowledge. 3. As to the satisfaction of student, S-PBL package management, tutor and self-satisfaction' score was 3.59, 3.82, 3.39 respectively. Conclusion: This study was suggested that S-PBL education would be necessary with improved tutor skill and achieved advantage of simulation and PBL.
과거 수년간 엘리베이터를 이용한 피난은 지속적인 논쟁의 대상이었으며, 일반적으로 대부분의 국가에서는 화재 시 엘리베이터의 이용을 금지하고 있는 실정이다. 그러나 WTC 사고 이후 초고층건물에서 엘리베이터를 이용한 피난대책이 계획되고, 일부는 채택되고 있는 상황이다. 본 연구는 초고층 건물에서 엘리베이터를 이용한 피난이 얼마나 피난시간을 단축하는 지에 대하여 실제 건물 설계 사례를 중심으로 연구해 보고자 한다.

In Korea, overall disaster prevention system or 119 emergency rescue system has been established to protect life and fortune of citizens. This system supports command & control operation, emergency 119 caller location indicator, automatic formation of fire troops and dispatch, and emergency management. To do this, various new information technologies such as GIS, telematics, CTI and TTS are applied to implement the system. In the future, however, it is not impossible to prevent a large scale disasters caused by world climate environment change and complication of city culture using the current system. In this paper, we propose enhancement plan for overall disaster prevention system to solve this problem.
This research aims to formulate a mathematical model and develop an algorithm for solving a location problem in emergency medical service vehicle parking. To find an optimal parking location which has the least risk score or risk priority number calculated from severity, occurrence, detection, and distance from parking location for emergency patients, data were collected from Pratoom sub-district Disaster Prevention and Mitigation Center from October 2010 to April 2011. The criteria of risk evaluation were modified from Automotive Industry Action Group's criteria. An adaptive simulated annealing algorithm with multiple cooling schedules called multi-agent simulated quenching (MASQ) is proposed for solving the problem in two schemes of algorithms including dual agent and triple agent quenching. The result showed that the solution obtained from both scheme of MASQ was better than the traditional solution. The best locations obtained from MASQ-dual agent quenching scheme was nodes #5 and #133. The risk score was reduced 61% from 6,022 to 2,371 points.
