Background: A peripheral extracorporeal membrane oxygenator (p-ECMO) has been developed to support patients who are dying due to a serious cardiopulmonary condition. This analysis was planned to define the clinical situation in which the patient benefits most from a p-ECMO. Material and Methods: Between June 2007 and Aug 2009, a total of 41 adult patients used the p-ECMO. There were 23 males and 18 females (mean age $54.4{\pm}15.1$ years). All patients had very unstable vital signs with hypoxia and complex cardiac problems. We divided the patients into 4 groups. In the first group, a p-ECMO was used as a bridge to cardiac operation. In the second group, patients did not have the opportunity to undergo any cardiac procedures; nevertheless, they were treated with a p-ECMO. In the third group, patients mostly had difficulty in weaning from CPB (cardiopulmonary bypass) after cardiac operation. The fourth group suffered from many complications, such as pneumonia, bleeding, infections, and LV dysfunction with underlying cardiac problems. All cannulations were performed by the Seldinger technique or cutting down the femoral vessel. A long venous cannula of $DLP^{(R)}$ (Medtronic Inc, Minneapolis, MN) or $RMI^{(R)}$ (Edwards Lifesciences LLC, Irvine, CA) was used together with a 17~21 Fr arterial cannula and a 21 Fr venous cannula. As a bypass pump, a Capiox emergency bypass system ($EBS^{(R)}$; Terumo, Tokyo, Japan) was used. We attempted to maintain a flow rate of $2.4{\sim}3.0\;L/min/m^2$ and an activated clotting time (ACT) of around 180 seconds. Results: Nine patients survived by the use of the p-ECMO. Ten patients were weaned from a p-ECMO but they did not survive, and the remainder had no chance to be weaned from the p-ECMO. The best clinical situation to apply the p-ECMO was to use it as a bridge to cardiac operation and for weaning from CPB after cardiac operation. Conclusion: Various clinical results were derived by p-ECMO according to the clinical situation. For the best results, early adoption of the p-ECMO for anatomical correction appears important.

Objectives: This research is part of a study to be conducted over five years starting from 2017 by the Ministry of Environment on the development of technologies to evaluate the impact of chemical accidents on the human body. Methods: For this research, a five-stage specific study method was developed. Results: In brief, the developed health risk assessment method can be summarized as follows. First, a health risk assessment system was built based on the guidelines set forth by the USA NRC/NAS. Second, based on the disease manifestation theory, the health risk assessment method was divided into 1) a carcinogenic health risk assessment method focused on all carcinogens except non-genotoxic carcinogens and 2) a non-carcinogenic health risk assessment method focused on noncarcinogens including non-genotoxic carcinogens. Third, the detailed contents of the health risk assessment method were developed in four stages(hazard identification, dose-response assessment, exposure assessment, and risk determination) through theoretical consideration of the assessment of the level of health risk related to chemical exposure. Finally, a health risk assessment methodology, classified into stages to address acute, subacute/subchronic, and chronic conditions was developed after considering the physicochemical behavior of hazardous chemicals upon implementation of countermeasures after a chemical accident. Conclusions: A method to evaluate the health risks related to toxic chemicals generated by chemical accidents was developed. This study was performed with the purpose of developing a mathematical health risk assessment method to evaluate the health effects of exposure to hazardous chemicals upon implementation of emergency countermeasures after chemical accidents.

Korea Utility Helicopter(KUH) is a project to replace the aging helicopter presently being operated by the military with a domestic helicopter. The auxiliary power unit is a device that supplies emergency power to the system by an alternator installed when the main generator fails and the start motor converts the electrical energy of the battery into mechanical rotational energy to rotate the auxiliary power unit engine. With continuing power and operation, defects of the starting motor were found and improvements were carried out to solve it. In the failure mode analysis, the causes of possible defects were classified into 5 categories. Analysis of the 5 factors revealed that the main cause of defects is mechanical wear, which was found to be related to spring pressure. 250 tests were conducted through the theoretically determined pressure and regression analysis was performed with 4 sampling.The results showed that pressure was related to wear rate. In conclusion, early wear and breakage due to wear can be controlled through spring force and test showed the prediction of wear and the validity of the result were confirmed.

In this study, we conducted an assessment of the risks posed by the noxious chemicals often handled at the analysis centers of organizations involved in Work Environment Measurement (WEM) using the Chemical Hazard Risk Management (CHARM) system. For this purpose, six organizations involved in WEM located in Seoul and Gyeonggi, which conducted the risk assessment, were selected and, as a result, 29 materials were selected as chemical substances presenting a risk of exposure. In terms of their physical properties, there were 16 liquid and 10 solid materials, and for the special management materials (CMR), the findings were as follows: 'Carcinogenic' 1A - 11 and 1B - 1 (2 - 8); 'Mutagenic' 1A - 4 and 1B - 3 (2 - 8); and 'Repro-toxic' 1A - 0 and 1B - 1 (2 - 6). In the risk estimation, 30.4%, 66.1% and 3.6% of the materials presented 'low', 'average' and 'high' risks, respectively. In addition, two of these six institutions had materials presenting a 'high' risk. This study is significant in that the risk assessment was conducted using CHARM, a chemical substance assessment tool. It is expected that the results will be utilized as the basic data for safety assessment, the establishment of a plan for emergency measures and drawing up a safety management manual, and the institutional and legal management of accidents and risks in the analysis centers of organizations involved in WEM and the management of chemicals by drawing attention to the risks involved.
The purpose of this study is to examine the structural model between servant - transformational leadership, organizational citizenship behavior and organizational performance of private security organizations. The security guards working in private security companies in Seoul and Gyeonggi - do were selected as population, random sampling method. The survey was conducted from September 1, 2016 to November 30. Accoridng to the purpose of the study, this study conducted factor analysis(EFA/CFA), reliability analysis, convergence and discriminant validity analysis, and covariance structure analysis using SPSSWIN 21.0 and AMOS 21.0. The conclusions of this study are as follows. First, servant leadership has a positive (+) effect (${\beta}=.406$) on organizational citizenship behavior statistically at .001 level. Second, transformational leadership has a positive (+) effect (${\beta}=.373$) on organizational citizenship behavior statistically at .001 level. Third, organizational citizenship behavior has a positive (+) effect (${\beta}=.615$) on organizational performance statistically at .01 level. Fourth, servant leadership does not affect the organizational performance statistically (${\beta}=.211$). Fifth, transformational leadership does not affect the organizational performance statistically (${\beta}=.058$). Sixth, organizational citizenship behavior has statistically positive (+) mediation effect (${\beta}=.249$) in the relationship between servant leadership and organizational performance. Seventh, organizational citizenship behavior has statistically positive (+) mediating effects (${\beta}=.230$) on the relationship between transformational leadership and organizational performance. The existing Extracorporeal membrane oxygenation(ECMO) and Cardiopulmonary bypass system(CPB) have been developed and applied to various devices according to their respective indications. However, due to the complicated configuration and difficult usage method, it causes inconvenience to users and there is a risk of an accident. Therefore, smart all-in-one cardiopulmonary circulation device is being developed recently. The smart all-in-one cardiopulmonary assist device consists of a blood pump for cardiopulmonary bypass, a blood oxidizer for cardiopulmonary bypass, a blood circuit for cardiopulmonary bypass, and an artificial cardiopulmonary device. It is an integrated cardiopulmonary bypass device that can be used for a variety of purposes such as emergency, intraoperative, post-operative intensive care, and long-term cardiopulmonary assist, combined with CPB used in open heart surgery and ECMO used when patient's cardiopulmonary function does not work normally. The smart all-in-one cardiopulmonary assist device does not exist as a standard and international standard applicable to advanced medical devices. Therefore, in this study, we will refer to the International Standard for Blood Components, the International Standard for Blood, the Guideline for Blood Products, and prepare applicable performance and safety guidelines to help quality control of medical devices, and contribute to the improvement of the health of people. The guideline, which is the result of conducted a survey of the method of safety and performance test, is based on the principle of all-in-one cardiopulmonary aiding device, related domestic foreign standards, the status of domestic and foreign patents, related literature, blood pump(ISO 18242), blood oxygenator (ISO 7199), and blood circuit (ISO 15676) for cardiopulmonary bypass.The items on blood safety are as follows: American Society for Testing and Materials ASTM F1841-97R17), and in the 2010 Food and Drug Administration's Safety Assessment Guidelines for Medical Assisted Circulatory Devices. In addition, after reviewing the guidelines drawn up through expert consultation bodies including manufacturers / importers, testing inspectors, academia, etc. the final guideline was established through revision and supplementation process. Therefore, we propose guidelines for evaluating the safety and performance of smart all-in-one cardiopulmonary assist devices in line with growing technology.
This study aimed to investigate the relationship between frailty and health care utilization in a cross-sectional design of a population-based sample of community-dwelling older adults. We used the data of 516 participants who dwell in Daejon, aged between 65 and 84 years old. Using K-frailty index, frailty status were measured and categorized as three groups: robust, prefrail, and frail. Logistic regression analysis was used to examine if frailty affects emergency department(ED) visit or hospitalization. In addition, negative binomial regression was used to examine the association between outpatient visits and frailty. Our results showed that the frail elderly increased the ED visit and the number of outpatient visit significantly after controlling for demographic characteristics, socioeconomic status, the number of chronic diseases, and self-rated health status. Considering that frailty is an important independent factor affecting health care utilization, more attention is required to prevent the frailty in our health care system.
When a fire occurs in a road tunnel, it is likely to develop into a major disaster because of its sealed structural characteristics. Because of this, a range of disaster prevention facilities should be installed, and they should show their appropriate functions in fire prevention and response. In addition, the installation of essential disaster prevention facilities is missing due to the lack of considering an extension of continuous tunnels and soundproof tunnels. For these reasons, it was judged that an amendment to the law would be necessary. Therefore, this paper reviewed the changes in domestic laws related to the disaster prevention facilities of road tunnels, and the results of the study on the parts that need to be revised in the fire-related laws through a comparative analysis between laws are as follows. First, consecutive tunnels should consider the sum of the extensions of the individual tunnels and ensure that the soundproof tunnels are included in the category of tunnels in the law. In this way, the necessary disaster prevention facilities should be installed. Second, it is necessary to secure a legal justification for installing disaster prevention facilities in tunnels. Therefore, it is essential that water mist fire extinguishing system facilities, emergency broadcasting facilities, and leading lights, which are excluded from installation in tunnels, are specified in the Fire-related laws. Third, to specify in the law that air respirators must be provided in the tunnel is necessary from the viewpoint of the field response. Accordingly, it will make a great contribution to the fire-fighting activities and the life-saving efforts by fire-fighters.

This paper deals with the braking dynamic behavior of diesel electric locomotive pulling domestic cargo and passenger vehicles. Friction coefficient, pneumatic pressure, and running resistance affecting the braking system were tested. For the friction coefficient, the Dynamo test was performed with reference to UIC 541-4. The results are analyzed by multivariate regression and the relationship between braking force and ititial velocity is presented. The pneumatic pressure were classified into service braking and emergency braking. In order to reflect the characteristics of the brake valve and piping, the pressure rising over time was measured in the vehicle. In order to reflect the external force acting on the vehicle, we carried out the test of EN 14067-4 and presented the second order polynomial formula on a running resistance. The running resistance test results were compared with other countries. The dynamic behavior of a diesel electric locomotive running on a straight flat track based on vehicle resources, friction coefficient, braking pressure, and running resistance is simulated using the time integration presented in EN 14531-1. The simulation results were compared and verified with the vehicle braking test results. The results of this study can be used to analyze the dynamic braking behavior of a train. Also, it is expected that various parameters affecting braking in vehicle design can be analyzed and used as basic data for braking performance improvement.

In this age, where the social environment is changing rapidly and unpredictably, interest in safety from crime is increasing in Korean society. As the desire to live a life free from the fear of crime increases, interest in the construction of safe cities is also rising nationwide. To meet the national demand, the Korean government is promoting a project to link public disaster safety systems by involving municipalities, 112, 119, and other emergency services and institutions through the Smart City Integrated Platform in order to construct a smart safety net. This study investigates the linking of theSmart City Integrated Platform and theIntelligent Security Project. The results are as follows. 1. The linkage's objective is clear. 2. The system sector can provide information to accident-related organizations. 3. The scenario area can be expanded to a crime-prevention sector, and a long-term urban information integration infrastructure can be created. 4. Product testing is enabled by a smart city road map and through continuous consultation with relevant organizations. 5. Project diffusion to other local governments can be promoted with the continued addition of commercial products.
