Evaluation of 3-Dimensional Exoscopes in Brain Tumor Surgery

  • Objective : Though the operating microscope (OM) has been the standard optical system in neurosurgery, a new technology called three-dimensional (3D) exoscope has emerged as an alternative. Herein, two types of 3D exoscopes for brain tumor surgery are presented. In addition, the advantages and limitations compared with the OM are discussed. Methods : In the present study, 3D exoscope VOMS-100 or VITOM 3D was used in 11 patients with brain tumor who underwent surgical resection; the Kinevo 900 OM was used only in emergency. After completion of all surgeries, the participants were surveyed with a questionnaire regarding video image quality on the display monitor, handling of equipment, ergonomics, educational usefulness, 3D glasses, and expectation as a substitute for the OM. Results : Among 11 patients, nine patients underwent neurosurgical resection with only 3D exoscope; however, two patients required additional aid with the OM due to difficulty in hemostasis. Regarding video image quality, VITOM 3D was mostly equivalent to the OM, but VOMS-100 was not. However, both 3D exoscopes showed advantages in accessibility of instruments in the surgical field and occupied less space in the operating theater. Differences in ergonomics and educational usefulness between the exoscopes were not reported. Respondents did not experience discomfort in wearing 3D glasses and thought the exoscopes could be currently, and in the future, used as a substitute for the OM. Conclusion : Although many neurosurgeons are not familiar with 3D exoscopes, they have advantages compared with the OM and similar image quality. Exoscopes could be a substitute for OM in the future if some limitations are overcome.

뉴스 데이터를 활용한 재난문자 요구사항 분석 (Requirement Analysis of Korean Public Alert Service using News Data)

  • 본 연구에서는 뉴스 데이터를 이용해 재난문자의 문제점에 대해 알아보았다. 이를 위해 한국언론진흥재단 뉴스 빅데이터 시스템을 통해 2005년 5월 15일부터 2020년 4월 30일까지의 '재난문자'의 주제어가 포함된 뉴스를 검색하여 자료를 수집하였다. 분석방법은 내용분석을 사용하였다. 조사 결과에 따르면, 재난문자 수신 문제점은 경보음, 내용, 기준, 빈도, 속도, 수신범위, 시간, 언어 등에 대한 것으로 범주화되었다. 재난문자 미수신 문제점은 권한, 단말기, 발송기준, 통신, 기타 등에 대한 것으로 범주화되었다. 재난문자 문제점에 대한 뉴스 게재 추이를 살펴보면, 최근 2~3년간 미수신 문제점에 대한 뉴스는 줄어드는 반면에 수신 문제점에 대한 뉴스는 많아지는 것으로 나타났다. 이는 재난문자에 대한 국민의 개선 요구가 수신 부분에 있다는 것을 말해준다. 특히, 내용, 빈도, 수신범위 문제에 대한 사회적 해결 요구가 급증한 상황이다.

글루포시네이트 중독 후 심장독성의 다양한 임상경과를 보인 1례 (A case of various clinical aspects associated with cardiotoxicity after glufosinate poisoning)

  • Glufosinate-containing herbicides is a non-selective herbicide commonly used worldwide. As the use of them increased gradually since paraquat was banned in 2012, the number of suicides by their ingestion is also increasing continuously. Complications of glufosinate-containing herbicide poisoning include various central nervous system (CNS) toxicities such as convulsions, loss of consciousness, memory impairment, and respiratory depression, which may be accompanied by hemodynamic changes such as bradycardia and hypotension. However, it is very rare that arrhythmias other than bradycardia occurred and Takotsubo cardiomyopathy was combined due to cardiotoxicity. A 71-year-old female patient was transferred to our hospital after ingesting 500 mL of glufosinate-containing herbicide and receiving 5 L of gastric lavage at a local hospital. A few hours later, she presented stuporous mentality, respiratory depression, and convulsions, and was accompanied by hypotension and bradycardia. On the second day of admission, electrocardiogram (ECG) showed bradycardia and QTc prolongation with hemodynamic Instability. Accordingly, we conducted the early treatment with continuous renal replacement therapy (CRRT) and the application of temporary cardiac pacemaker. An echocardiogram demonstrated decreased ejection fraction (EF) and Takotsubo cardiomyopathy on the third day of admission. Then, she was discharged safely with conservative treatment. At the follow-up after 1 year, Takotsubo cardiomyopathy, EF and QTc prolongation were recovered on echocardiogram and ECG. Because cardiac toxicity after glufosinate-containing herbicide poisoning may cause life-threatening consequences, caution is required while treating the patient. Therefore, if electrocardiogram changes are seen in the elderly with a large amount of glufosinate herbicide ingestion, additional cardiac function test through echocardiography should be concerned, and early treatment through CRRT or artificial cardiac pacing should be considered.

지역사회간호학 재난간호교육 콘텐츠 개발을 위한 델파이 조사 (A Delphi Study for Development of Disaster Nursing Education Contents in Community Health Nursing)

  • Purpose: This study was conducted to develop the contents of disaster nursing education in community health nursing at universities. Methods: To validate contents, the Delphi method was used. We categorized two domains(indirect disaster management and direct disaster management) and developed 48 draft items. This study applied two round surveys and 23 experts participated in this study. The content validity was calculated using content validity ratio and coefficient of variation. Results: Indirect disaster management domain was composed of three categories including 12 items: 1) Understanding of the disaster, 2) disaster management system, and 3) response by disaster stage and recovery. Direct disaster management domain was composed of nine categories including 30 items: 1) Ethical considerations, 2) communication in disasters, 3) nursing activity by disaster stage, 4) emergency nursing in disasters, 5) patient severity classification in disasters, 6) disaster nursing for vulnerable groups, 7) disaster nursing for victims, 8) psychosocial nursing and health in disasters, and 9) cases of disaster nursing in communities. Conclusion: This Delphi study identified the contents of disaster nursing education curriculum, and confirmed the validity for disaster education program in community health nursing. Based on the results, it will be helpful for training the disaster nursing and improving the competency on disaster nursing of the nursing students.

  • 재해 복구는 자연 재해와 같은 비상 상황에 대비하여 서비스의 연속성을 보장하고, 리소스와 재정의 손실을 최소화하기 위한 정책 및 절차를 의미한다. 특히, 클라우드 서비스 제공자에 의한 재해 복구 방법은 관리의 유연성과 고가용성, 비용효율성과 같은 장점을 지닌다. 하지만, 이러한 방법은 서비스 사업자에 대한 의존성을 가지며, 개인의 데이터에 대해 사용자가 관여할 수 없는 구조적 한계를 가진다. 본 논문에서는 블록체인과 분산 스토리지를 활용하여 사용자의 데이터를 백업함으로써 서비스 제공자에 대한 의존성을 제거하고, 데이터의 기밀성을 위해 프록시 재암호화를 이용한 프로토콜을 제시한다. 제안된 방법은 이더리움과 IPFS 환경에서 구현되었으며, 백업 및 복구 운영에 필요한 성능과 비용을 제시한다.

정신요양시설 거주인의 건강 실태 및 정책 대안 (Health Condition of Residents of Mental Health Sanatoriums and Policy Options)

  • Purpose : The purpose of this study is to directly understand the health condition of residents of mental health sanatoriums nationwide, which has been difficult to ascertain in surveys conducted to date. The study presents specific measures for improving the health of these residents. Methods : A "physical examination questionnaire for residents of mental health sanatoriums" was developed to check the basic physical condition of residents, and 20 out of 59 mental health sanatoriums nationwide were randomly selected. Medical personnel visited the sanatoriums, interviewing and examining the residents in person. A total of 396 health surveys were completed. Results : Many of the residents were underweight but had abdominal obesity. It was confirmed that chronic diseases among the residents were not diagnosed early or were not properly managed. Among the subjective symptoms complained of by the residents, musculoskeletal symptoms were the most common. Oral examinations revealed a serious level of oral health problems among the residents, including dental caries and missing teeth. Basic physical examinations found health problems that required additional examination or medical treatment. Blood pressure abnormalities made up the highest percentage of the health problems. Conclusion : Regular health surveys are needed to determine the health condition of residents of mental health sanatoriums. Access to and quality of primary medical services within the sanatoriums need to be dramatically improved. A delivery system for severe diseases and emergency medical care in the sanatoriums should also be specifically presented. The residents should be notified upon admission and during their stay that they have the right to the enjoyment of the highest attainable standard of mental and physical health. The issue of health rights should be addressed within a larger framework of reorganizing management plans for people in the community - not only residents - with chronic mental illness.

딥러닝 활용 원전 중대사고 진단 (Nuclear Power Plant Severe Accident Diagnosis Using Deep Learning Approach)

  • 원자력발전소의 중대사고 발생 시 신속하고 정확하게 사고 상황을 파악해야 하며, 이러한 사고진단 정보를 획득했을 때 적절한 사고관리 및 대응을 수행할 수 있다. 본 연구에서는 국가원자력 재난관리 시스템인 AtomCARE (Computerized technical Advisory system for a Radiological Emergency)로 전송되는 주요 발전소 정보로부터 중대사고 상황을 진단하는데 있어 딥러닝 기술의 접목을 고려하였다. 이를 위하여 주요 시나리오를 선정하고 사고 진행에 따른 상세 시나리오에 대하여 중대사고 해석 코드인 MAAP5 다량 계산을 통한 학습 DB를 구축하였다. 그리고 이 DB의 학습을 통하여 주요 발전소 정보로부터 중대사고 상세 시나리오를 분류할 수 있는, 즉 중대사고 상황을 진단할 수 있는 기술을 개발하였다. 또한 블라인드 테스트와 주성분분석을 통한 검증을 수행하였다. 본 연구에서 개발한 기술은 향후 전체 중대사고 시나리오로 확장 및 적용 가능할 것으로 판단되며 신속하고 정확한 사고진단의 기반기술로 활용 가치가 높을 것으로 기대된다.

Experience of Disaster Response Team in Jecheon Sports Center Fire

  • Purpose : This study aimed to determine the experiences of the disaster response team 9 participants who participated in the disaster after the fire incident occurred in Jecheon Sports Center and their meaning and essence. Methods : Nine disaster response teams were the subjects of the study, and Individual in-depth interviews were conducted. Data were collected online between January 6 and April 30, 2021, from the software ZoomTM. This study was conducted as a qualitative study by applying Giorgi's phenomenological experiential research method, which has an advantage in revealing the essential structure and meaning of experience. Results : Three major themes were derived from the study results: the stimulus through unfamiliar experiences, the grievances due to work environment and regional characteristics, and dullness resulting from hiding and enduring. The following 17 sub-themes were identified: learning through unfamiliar experiences, frustration due to unexpected circumstances, shock from unfamiliar experiences, doing my best in the present, confidence due to increased experience, disunified system, intervention of various interests, the atmosphere that puts responsibility on others, inactive help, unforgettable regional characteristics, working without time to settle, tolerating it in my own way, memories left in the unconsciousness, sudden suffering from memories that come to mind, movement in anxiety and tension, dullness, work with colleagues in the same situation. Conclusion : This study is meaningful in that it attempted to provide basic data in preparing a long-term strategy for effective policy direction and institutional protection based on the systematic mental health management of the disaster response team. Additionally, this study's results can be used as primary data for future research among Disaster Response Team.

시설 재배 농가 관리를 위한 데이터 서비스 기반의 비상 알림 스마트팜 서비스 모델 설계 (Design of Emergency Notification Smart Farm Service Model based on Data Service for Facility Cultivation Farms Management)

  • 2015년부터 정부에서는 한국형 스마트팜 보급에 노력하고 있다. 그러나, 기술 및 현행 재배 연구 데이터의 한계로 인해 대규모 시설 채소 농가에만 보급이 국한되고 있는 실정이다. 또한, 작물 생육 및 재배 환경을 고려하지 않은 IT 기술의 단순 적용으로 도입 비용 대비 효율성과 신뢰성이 낮은 상황이다. 따라서, 본 논문에서는 공공 및 외부 데이터를 기반으로 데이터 분석 서비스를 수행하고 이를 시설 재배 농가의 실정에 맞는 데이터 기반의 타깃형 스마트팜 시스템을 설계하였다. 이를 위해, 농장 위험 정보 알림 서비스를 개발하고, 적정 비료 시비를 위한 광환경지도를 제공하며, 시설 농장의 온습도 정보를 활용한 재배 작기별 병해 예측 모델을 설계하였다. 이를 통해, 기존의 스마트팜 센서 데이터와의 연계, 활용으로 스마트팜 데이터 서비스 구현이 가능하며, 데이터 활용에 경제적 효율성 및 데이터 신뢰성을 확보할 수 있다.

한국의 환자중심 의사 역량 연구 (Patient-Centered Doctor's Competency Framework in Korea)

  • With increasing demands for medical care by society, the medical system, and general citizens and rapid changes in doctor's awareness, the competencies required of doctors are also changing. The goal of this study was to develop a doctor's competency framework from the patient's perspective, and to make it the basis for the development of milestones and entrustable professional activities for each period of medical student education and resident training. To this end, a big data analysis using topic modeling was performed on domestic and international research papers (2011-2020), domestic newspaper articles (2016-2020), and domestic social networking service data (2016-2020) related to doctor's competencies. Delphi surveys were conducted twice with 28 medical education experts. In addition, a survey was conducted on doctor's competencies among 1,000 citizens, 407 nurses, 237 medical students, 361 majors, and 200 specialists. Through the above process, six core competencies, 16 sub-competencies, and 47 competencies were derived as subject-oriented doctor's competencies. The core competencies were: (1) competency related to disease and health as an expert; (2) competency related to patients as a communicator; (3) competency related to colleagues as a collaborator; (4) competency related to society as a health care leader (5) competency related to oneself as a professional, and (6) competency related to academics as a scholar who contributes to the development of medicine.
