119구급대원의 현장 및 이송 중 응급처치 경험에 대한 현상학적 연구 (A phenomenological study on pre-hospital care of 119 EMT)

  • Purpose: This study aimed to provide baseline data for implementing advanced life-support for 119EMTs. This is done with an in-depth examination of the emergency care experiences of the 119EMTs on-site and during transportation. Methods: Interviews were conducted with twelve 119EMTs, and the data were analyzed using the phenomenological method. Results: The 34 topics had 19 sub-components and were analyzed according to seven categories: 'A work environment where you cannot focus only on emergency care on-site and during transportation', '119EMTs struggling with manpower shortage', 'Current status of education blocking the upward standardization of 119EMT', 'Implementation of professional emergency care was held back due to the limited legal scope of 119EMT', 'Medical oversight makes it difficult to implement emergency care due to low flexibility', '119EMTs who cannot choose hospital selection and find it difficult to receive help', 'An evaluation system not reflecting reality at all that only leaves high scores'. Conclusion: This study was undertaken to implement specialized emergency care for 119EMTs; not only the necessity of reviewing the expansion of the work scope of 119EMTs but also the necessity of effective 'Medical Oversight system' reorganization raised as well.

돌발성 재해에 대비한 긴급 피난 지원 시스템의 구현 (Implementation of Emergency Evacuation Support System in Panic-type Disaster)

  • 세계적으로 지진, 해일, 홍수, 폭설 등의 자연 재해와 화재, 방화, 테러 등의 인위적 재해가 빈번하게 발생하고 있으며, 이러한 다양하고 예측 불가능한 재해로 인해 다수의 사상자가 발생하고 있다. 재해 발생 시 빠른 시간에 재해 발생 여부를 파악하여 재해 발생 위치와 적절한 피난 경로 등의 정보를 피해자에게 제공한다면 신속히 안전한 장소로 대피가 가능하여 피해를 최소화 할 수 있다. 이에 본 논문에서는 실내공간에서 공황상태가 발생 했을 경우 사람들의 움직임을 파악하여 긴급 재해 발생 여부를 신속히 탐지하고 이를 통해 상황에 따라 신속하고 안전하게 대피가 가능하도록 긴급 피난 지원 시스템의 알고리즘을 제안하고 구현하였다. 이동 단말기에서는 실내 보행자의 행동과 위치를 분석하여 서버로 전송하고 서버에서는 전송받은 정보를 토대로 긴급상황 여부를 판단한다. 긴급상황이 발생하면 서버는 이동 단말기에게 이를 통보하고, 실내 보행자는 이동단말기를 이용하여 비상탈출을 실시하게 된다.

  • Purpose: The purpose of this study was to test the effect of simulation-based learning on knowledge, confidence, and critical thinking of paramedical students enrolled in the Korean Advanced Life Support (KALS) program. This study used a one group, pre-post test design. Methods: The subjects of this study were 79 paramedical students in D city. Data were collected before and after the simulation-based training using a structured questionnaire. The data were analyzed using SPSS version 22.0. Results: After the simulation-based KALS education, knowledge (t=-6.88, p<.001) and confidence (t=-10.12, p<.001) increased among paramedical students. There was a positive correlation between confidence and critical thinking disposition (r=0.37, p=.001). Conclusion: A practical module for simulation-based education that can improve knowledge, confidence, and critical thinking disposition related to professional resuscitation is needed for use with paramedical students. Additionally, follow-up studies should be conducted to verify the educational effects of such a program.

  • Phalloides syndrome has usually occurred after incidental mushroom ingestion. It is sometimes difficult to differentiate toxic mushrooms from edible ones, especially in lay person because of their morphological similarities. In Korea, Amanita virosa and Amanita subjunquillea have been reported as a cause of fulminant hepatic failure in mushroom poisoning (phalloides syndrome). We report a case of phalloides syndrome who came to death with fulminant hepatic failure even though we planned liver transplantation and used T-PLS(R) (Twin Pulse Life Support) for hemodynamic support while waiting for the donor.

  • 세계적으로 지진, 해일, 홍수, 폭설 등의 자연 재해와 화재, 방화, 테러 등의 인위적 재해가 빈번하게 발생하고 있으며, 이러한 다양하고 예측 불가능한 재해로 인해 다수의 사상자가 발생하고 있다. 본 논문에서는 비상시 긴급 구조 및 피난 지원 시스템 구축을 위하여 동일 지역 정보를 가지는 단말 사이에서 하나의 클러스터를 형성하고 각 클러스터의 부모 단말기끼리 통신을 하게하여 트래픽을 줄이는 영역 분할 클러스터링 방식의 멀티 홉 애드혹 통신을 제안한다.

원자력발전소 비상운전시의 운전원 인지오류 예측 지원체계의 개발 (A Framework for the Support of Predictive Cognitive Error Analysis of Emergency Tasks in Nuclear Power Plants)

  • This paper introduces m analysis framework and procedure for the support of the cognitive error analysis of emergency tasks in nuclear poler plants. The framework provides a new perspective in the utilization of influencing factors into error prediction. The framework can be characterized by two features. First, influencing factors that affect the occurrence of human error me classified into three groups, i.e., task characteristic factors(TCF), situation factors(SF), and performance assisting factors(PAF). This classification aims to support error prediction from the viewpoint of assessing the adequacy of PAF under given TCF and SF. Second, the assessment of influencing factors is made by each cognitive function. Through this, influencing factors assessment and error prediction can be made in an integrative way according to each cognitive function. In addition, it helps analysts identify vulnerable cognitive functions and error factors, and obtain specific nor reduction strategies. The proposed framework was applied to the error analysis of the bleed and feed operation of nuclear emergency tasks.

시뮬레이션 교육이 응급구조과 학생의 기본소생술 수행능력에 미치는 영향 (The Effect of Simulation-based Training on the Competence of Basic Life Support of the students Emergency Medical Technology)

  • Purpose : The simulation-based training in this research consists of theory and practice. Before the training, target students took a test on the competence of basic life support. Based on the result, they were separated as the subject group and comparison group. The simulation-based training was offered to the subject group and the traditional training was given to the comparison group. As soon as the training was completed, a follow-up study was conducted. Methods : This research aimed to figure out the effect of the simulation-based training on the competence of basic life support of the students Emergency Medical Technology. To this end, the nonequivalent pretest-posttest-quasi-experimental design using a comparison group was conducted. Results : The first hypothesis was that 'The group who took the simulation-based training would show higher points in the knowledge of basic life support than the group who took the traditional training.' Among those who took prior theory education, the subject group showed $69.38{\pm}20.43$ points while the comparison group showed $76.25{\pm}21.33$ points(t = -0.658, p = 0.531). Among those who took prior theory education and training, the subject group showed $82.86{\pm}10.86$ points while the comparison group showed $79.33{\pm}15.45$ points(t = 0.705, p = 0.487). Since there were no significant statistical differences between the two groups, the first hypothesis didn't hold. It showed few differences between the two training methods. The second hypothesis was that 'The group who took the simulation-based training would show higher points in the basic life support skills than the group who took the traditional training.' Among those who took prior theory education, the subject group showed $65.75{\pm}7.66$ points while the comparison group showed $46.88{\pm}13.48$ points(t = -3.442, p = 0.004). Among those who took prior theory education and training, the subject group showed $79.50{\pm}11.40$ points while the comparison group showed $62.13{\pm}11.44$ points(t = 4.091, p = 0.000). Since there were significant statistical differences between the two groups, the second hypothesis held. It showed substantial differences between the two training methods. Conclusion : The group who took the simulation-based training showed more positive effects on the competence of basic life support than those who took the traditional training. Therefore, it is confirmed that the simulation-based training is a useful method to improve clinical work performance of the students Emergency Medical Technology.

기본소생술 S-PBL 패키지 개발 및 적용 후 평가 (An Examples Development and Implementation of S-PBL Package in Basic Life Support)

  • Purpose: The purpose of this study was to develop and apply a simulation-problem based learning(S-PBL) package in basic life support(BLS). Methods: This study was used to developed BLS' S-PBL package throughout 12 steps of PBL package development model. Then, BLS' S-PBL methodology was implemented in second year student in the undergraduate emergency medical technology, and survey was done. Results: 1. S-PBL package model was presented based on conceptual model II of PBL ; PEL for professional action. 2. Quantitative analysis of survey was to the effectiveness of learning, 3.59 points ; 4.15 of BLS and 3.84 of integration and practical use of knowledge. 3. As to the satisfaction of student, S-PBL package management, tutor and self-satisfaction' score was 3.59, 3.82, 3.39 respectively. Conclusion: This study was suggested that S-PBL education would be necessary with improved tutor skill and achieved advantage of simulation and PBL.

응급실 간호사의 전문직 삶의 질: 공감만족, 소진, 이차 외상성 스트레스 (Emergency Nurses' Professional Quality of Life: Compassion Satisfaction, Burnout, and Secondary Traumatic Stress)

  • Purpose: Professional quality of life is the quality one feels in relation to their work as a helper. The purpose of this study was to describe professional quality of life among emergency nurses. Methods: A total of 15 emergency rooms were selected in three cities. Among 263 nurses working at these emergency rooms, 178 nurses consented to participate in this cross-sectional survey. Professional Quality of Life (ProQOL) Scale version 5 was used to measure compassion satisfaction, burnout and secondary traumatic stress. Cluster analysis was used to classify nurses according to professional quality of life. Results: The mean scores (SD) for compassion satisfaction, burnout, and secondary traumatic stress were 32.12 (5.45), 28.27 (4.28), and 28.20 (5.07), respectively. The result of cluster analysis according to standardized score of compassion satisfaction, burnout, and secondary traumatic stress yielded three clusters. Over 50% of participants showed low professional quality of life. Nurses who were included in a cluster representing low professional quality of life were younger, had shorter periods of nursing experience, and perceivedlower social support than other cluster. Conclusion: Education or support programs for emergency nurses are needed to enhance their professional quality of life.
