  • 본 논문에서는 안드로이드 위치 센서를 기반으로 경기도 긴급재난지원금의 사용처에 대한 정보를 사용자들에게 제공하는 '가치살림' 애플리케이션을 설계하고 구현한다. 이 애플리케이션은 간소화 된 회원가입과 시간과 장소에 구애받지 않는 스마트폰 애플리케이션을 제공하여 접근성이 높인다. 또한 남녀노소 편리한 사용을 위하여 단순한 화면구현과 사용처 위치, 길 안내, 사용처 정보 제공, 내 위치 등의 필수 정보를 제공한다. 사용자는 즐겨 찾는 지역 또는 본인의 거주 지역, 현재 위치를 기반으로 긴급재난지원금 사용처의 정보를 알 수 있다. 경기도 각 시군의 지역화폐 가맹점 현황 정보는 경기도 데이터 드림의 지역화폐 가맹점 현황 Open API를 통해 가져온다. 또한, 긴급 재난 지원금의 사용처의 위치 정보와 사용자의 위치는 경기도 데이터 드림의 지역화폐 가맹점 현황 Open API에서 각 사용처별 위도와 경도 값과 Google Map을 통해 구현한다.

Needle Decompression for Trauma Patients: Chest Wall Thickness and Size of the Needle (외상 환자에 대한 바늘감압술에서 흉벽 두께와 바늘 길이의 관계)

  • Purpose: A tension pneumothorax is a fatal condition that requires immediate intervention. Although a definitive treatment for a tension pneumothorax is a tube thoracostomy, needle decompression can provide temporary relief, that is lifesaving. The traditional procedure for needle decompression involves inserting a needle or catheter at the second intercostal space, the midclavicular line. Recent evidence suggests that the commonly used catheters do not have sufficiently penetrate the chest wall. There are also claims that a lateral approach to needle decompression is easier and safer than the traditional anterior approach. The purpose of this study is to evaluate the optimal approach for needle decompression for the Korean population by measuring chest wall thicknesses at the points used for both the anterior and the lateral approaches. Methods: The chest wall thickness (CWT) of trauma victims who visited the Emergency Center of Pusan National University Hospital was measured by computed tomography (CT) images. The CWT was measured at the points used for the anterior and the lateral methods and was compared with the length of commonly used catheters, which is 45 mm. Results: The mean CWT at the second intercostal space, the midclavicular line, was shorter than the CWT at the 5th intercostal space, the anterior axillary line. However, the percentage of patients whose CWT was greater than 45 mm was larger when measured anteriorly (8.2%) that when measure laterally (5.7%). Female patients and those older than 60 were more likely to have an anterior CWT greater than 45 mm (28.2% for females and 15.5% for those older than 60). Conclusion: The percentage of trauma victims in Korea whose CWT is greater than 45 mm is lower than the values previously reported by other countries. However, females and older patients tend to have thicker chest walls, so the lateral approach would be suggested when performing needle decompression for such patients with suspected tension pneumothoraces.

Design and Implementation of a Protocol for u-Safety Service (u-안심 서비스 프로토콜 설계 및 구현)

  • u-safety service system inter-works with the diverse operation agencies, through CCTV network, such as the emergency call terminal with built-in GPS, the mobile communication network, u-safety service provider, relay system and CCTV control center. In the case of the emergency call, this service searches the location of caller in real time, and then continues to search the location of caller through the control of CCTV in the searched place, and can provide the several agencies like guardian, police office, fire station, hospitals, relief organizations and municipalities, with the diverse information necessary for the secure rescue through SMS and wired network. In this paper, a new protocol and specification for u-safety service relay system is designed and implemented. The effectiveness of presented protocol is verified by computer simulation. The designed protocol of u-safety service is applied to real 3GPP and 3GPP2 mobile communication networks to verify its performance.

A Study on the Direction of System Improvement for the Utilization of Residential Spaces of the Emergency Evacuation Facilities in Responding to War (비상대피시설의 거주 공간 활용을 위한 제도 개선 방향 설정에 관한 연구)

  • Caused by the Cheonanham attack and the Yeonpyeongdo bombardment occurred a few years ago, and the recent North Korea's nuclear test, the war crisis between South and North Korea has been increasing. Accordingly, an interest in obtaining a temporary residential space, for the case of the outbreak of war, where people can reside safely over a period of time has been escalating. However, in the disaster relief planning guidelines of the National Emergency Management Agency, the standards on the temporary residential spaces in preparation for storm and flood or earthquake are included, but the standards on the ones in responding to war are not yet equipped. In particular, the standards on the underground temporary residential spaces that can accommodate massive victims should be developed in preparation. In this study, through a requirements analysis based on the survey of war victims and a comparative analysis between disaster-related laws and regulations, the direction of relevant system improvement for the utilization of temporary residential spaces in responding to war is established.

Experience with Gabapentin for Neuropathic Pain -Case report- (신경병증성 통증 환자에서 Gabapentin 사용의 임상경험 -증례 보고-)

  • Gabapentin is an oral antiepileptic agent with an unknown mechanism of action. There have been many proposed uses for gabapentin, including neuropathic pain, reflex sympathetic dystrophy, postherpetic neuralgia, midscapular pain secondary to radiation myelopathy and migraine prophylaxis. This report presents patients who were treated with gabapentin when other pharmacologic interventions failed to relieve neuropathic pain 3 patients with neuropathic pain were included among these cases. All patients were started on 200 mg gabapentin. The maximum dose required for pain relief was between 800 mg and 2400 mg. Gabapentin may be a useful adjunct for treating neuropathic pain with minimum of side effects.

A Research on Clinical Use of the Xiexiafa (사하법의 내과적 임상응용)

  • Xiexiafa is the one of eight methodes of treatment, the purgatives of oriental medicine was originated from Shanghanlun written by Zhangzhongjing. Traditionally Xiexiafa has been used to treat constipation. But All kinds of researches on Xiexiafa made purgatives possible to use variously for the disease of internal medicine. In the research on clinical use of Xiexiafa, the results were as follow. Xiexiafa is available on emergency medicine because of instant relief. It has an ability of preventive medicine and economical effect. It was considered to treat hard to cure and chronic diseases made by Sushi, Rexie, Shuijie, Tanzhi, Xuxie etc. Considering strong side effects of Xiexiafa and patient conditions, it must be used carefully.

  • China had revised Foreign Trade Law in order to fulfill its WTO commitments and duties, and fully exercise its rights and benefits as a WTO member. The main contents of the revised Foreign Trade Law as followed. For the first time, individuals are allowed to conduct foreign trade. Intellectual property is an important component of foreign trade, and protecting intellectual property rights is a key issue. A chapter on Intellectual property protection is one of the major additions to the Foreign Trade Law. New sections have also been added to allow and provide guidance for conducting investigations and seeking relief. Finally, the new law beefs up and clarifies enforcement procedures. It regulates the setting up of early warning and emergency systems and a foreign trade statistics system.

Real-time collision-free landing path planning for drone deliveries in urban environments

  • This study presents a novel safe landing algorithm for urban drone deliveries. The rapid advancement of drone technology has given rise to various delivery services for everyday necessities and emergency relief efforts. However, the reliability of drone delivery technology is still insufficient for application in urban environments. The proposed approach uses the "landing angle control" method to allow the drone to land vertically and a rapidly exploring random tree-based collision avoidance algorithm to generate safe and efficient vertical landing paths for drones while avoiding common urban obstacles like trees, street lights, utility poles, and wires; these methods allow for precise and reliable urban drone delivery. We verified the approach within a Gazebo simulation operated through ROS using a six-degree-of-freedom drone model and sensors with similar specifications to actual models. The performance of the algorithms was tested in various scenarios by comparing it with that of stateof-the-art 3D path planning algorithms.

Usefulness of End-tidal Carbon Dioxide as a Predictor of Emergency Intervention in Major Trauma Patients (중증 외상 환자에서의 응급중재술 시행 예측 인자로서의 호기말 이산화탄소 분압의 유용성)

  • Purpose: If the survival of patients suffering from severe blunt trauma is to be improved, appropriate interventions should be taken immediately. The purpose of this study is to evaluate the clinical utility of end-tidal carbon dioxide ($ETCO_2$) as a surrogate marker for predicting both the need for intervention and the prognosis. Methods: This is a prospective observational study. Nasal cannula was applied to measure $ETCO_2$, and the following parameters, which are known to be related to the prognosis for a patient, were recorded: injury severity score (ISS), revised trauma score (RTS), arterial blood gas (ABG), lactate, and hemoglobin (Hb). To evaluate the outcome, we investigated the details of emergent interventions and expired patients. Results: A total of 93 patients were enrolled in this study. Emergent intervention was significantly associated with systolic blood pressure (sBP, p-value=0.001), $ETCO_2$ (p-value<0.001), serum lactate level (p-value<0.001), pH (p-value< 0.003), $HCO_3$ (p-value=0.004), base excess (p-value<0.002), ISS (p-value<0.001) and RTS (p-value=0.005). In the multivariate logistic regression, only $ETCO_2$ (odds ratio (OR): 0.897, 95% confidence interval (CI): 0.792-0.975, p-value= 0.048) and ISS (OR: 1.132, 95% CI: 1.053-1.233, p-value=0.002) were associated with emergent intervention whereas $ETCO_2$ (p-value=0.973) and ISS (p-value=0.511) were not statistically significant in predicting the survival of patients in the univariate analysis. An optimal ETCO cut-off of 29 mmHg on the ROC curve was determined, with the area under the ROC curve (AUC) being 0.824 (0.732-0.917)]. Conclusion: This study has revealed that $ETCO_2$, which can be rapid and easily measured through a nasal cannula, and the ISS may be prognostic indicators of emergent interventions in Emergency Departments.

A Study of Temporality of a Critical Discourse on the Modern in the Late Japanese Colonial Period (일제말기 근대비판 담론의 시간성 연구: 세계사·전통·비상시)

  • In the late Japanese colonial period, from the Sino-Japanese War until the Pacific War, critical discourses on the modern were prevalent in Japan and the Joseon. Despite the absence of a consensus about the specific definition of the modern, most thinkers agreed that the modern was something to be overcome. While some regarded naturalism and capitalism of the West as the essence of the modern, some others named scientism and humanism as the nature of the western modernity. Additionally, some criticized the temporal concept of historicism and brought new meanings of 'tradition' into relief, and some others advocated overcoming 'the West inherent in us'. This study is to consider the temporality of the theory of overcoming the modern focusing on the following three notions-world history, tradition, and emergency-, and examines the antinomy of them. The first notion to consider is 'world history'. The theorists of overcoming the modern, including the Kyoto school, discarded the progressive ideology that had led the Western modern history, and instead introduced 'world history' as a new notion. Although this resulted from the imperialistic embracement of the theories of Ranke, a major positivist historian from Germany, it contained antinomy of remaining in 'history' which was the modern temporal view. The second notion is 'tradition'. While the critical mind of 'world history' brought 'time of world' into question in the context of temporal realization, the notion of 'tradition' was to understand 'time of history' itself as the modern and overcome it. The critical mind of the notion involves the attempts to criticize regarding history as a 'progressive' process and to discover tradition as 'the present past' or 'the eternal present'. However, it also contained antinomy; the 'tradition' here was a notion that was created in the modern times, not passed down from ancient times. The third notion to consider is 'emergency', which was a method to define the present time as a transition period toward a new era, relating to states of war. However, the theorists of overcoming the modern did not regard 'emergency' as a particular time that strayed from normal states, instead they thought is as 'a regularized exceptional state', namely 'a state in which exceptions have become regulations'. However, the notion also contained antinomy since the word 'emergency' connotes abnormality.
