Development of a prototype TL/OSL reader for on-site use in a large-scale radiological accident

  • This study presents the development and characterization of a prototype TL/OSL reader for the retrospective dose assessment of individuals in radiological emergencies. The reader is portable, semi-automatic, and capable of accurate measurements. The dimension of the reader is 25 × 25 × 37 cm3 and the weight is about 15 kg. The reader consists of a sample moving stage, a heating module, an optical stimulation module, a detection module, a data acquisition (DAQ) unit, a nitrogen gas control module, and a PC with a GUI program. The reader has three measurement modes: TL, CW_OSL, and custom mode. The reader was characterized using commercial thermal luminescence dosimeters (TLD, LiF:Mg,Cu,Si) and optically stimulated dosimeters (OSLD, Al2O3:C), as well as fortuitous materials, such as display glasses and resistors of mobile phone. The results showed that the reader is capable of measuring signals with a detection limit of up to 0.02 mGy using a commercial dosimeter. In the dose recovery test using fortuitous materials, the reconstructed doses obtained three days post-irradiation closely aligned with the initially administered doses. As a result, this study suggests that the developed TL/OSL reader is a promising instrument for emergency dose assessment at accident sites.

A Study on the Linkage Method between Emergency Simulation Model and Other Models (비상대비 시뮬레이션 모델의 타 모델 연동방안 연구)

  • This study is a study on the interlocking method between emergency preparedness simulation model and military exercise war game model. The national emergency preparedness government exercises are being carried out by a message exercise and technology development for simulation models is being carried out to create a situation similar to the actual practice. In order to create a situation similar to the actual war, the military situation must be reflected and to do so, a link with the military exercise war game model is needed. The military exercise war game model applies HLA/RTI, which is a standardized interlocking method for various models such as Taegeuk JOS, and it is necessary to apply HLA/RTI linkage method to link with these military exercise war game models. In addition, since the emergency preparedness simulation model requires limited information such as enemy location and enemy attack situation on major facilities in the military exercise model, a method of interlocking that can select and link information is required. Therefore, in this study, the interlocking interface design plan is presented in order to selectively link the interlocking method and information between the emergency preparedness simulation model and the military exercise war game model. The main functions of interlocking interface include federation synchronization, storage and recovery, object management service, time management, and data filtering functions.

Improved postoperative recovery profile in pediatric oral rehabilitation with low-dose dexmedetomidine as an opioid substitute for general anesthesia: a randomized double-blind clinical trial

  • Background: Low-dose dexmedetomidine may be a suitable alternative to opioids for pediatric ambulatory procedures under general anesthesia (GA). However, the recovery profile remains unclear. Herein, we aimed to evaluate the effects of low-dose dexmedetomidine on the recovery profile of children. Methods: Seventy-two children undergoing ambulatory oral rehabilitation under GA were randomly and equally distributed into two groups (D and F). Group D received an infusion of dexmedetomidine 0.25 ㎍/kg for 4 min for induction, followed by maintenance of 0.4 ㎍/kg/h. Group F received an infusion of fentanyl 1 ㎍/kg over 4 min for induction, followed by maintenance at 1 ㎍/kg/h. The primary outcome was the extubation time. The secondary outcomes were awakening time, end-tidal sevoflurane (ET-Sevo) requirement, change in hemodynamic parameters, Richmond Agitation-Sedation Scale (RASS), Children's Hospital of Eastern Ontario pain scale (CHEOPS) score, length of PACU stay, and incidence of adverse events. Results: Statistically significant differences were observed in the recovery profile between the groups: the median time for extubation was 3.65 (3.44-6.2) vs. 6.25 (4.21-7) minutes in groups D vs. F (P=0.001), respectively, while the corresponding awakening times were 19 (18.75-21) and 22.5 (22-24) minutes, respectively (P < 0.001). The mean ET-Sevo was low in group D (1.1 vs. 1.2; P < 0.001). The heart rate was significantly low across all time points in group D, without resulting in bradycardia. The median RASS and CHEOPS scores were also significantly lower in group D. No significant differences were observed in the mean arterial pressure, incidence of adverse events, or length of PACU stay. Conclusion: Low-dose dexmedetomidine was more effective than fentanyl as an opioid substitute at providing a better recovery profile in pediatric ambulatory oral rehabilitation under GA. Dexmedetomidine also significantly reduced sevoflurane consumption without causing adverse events or prolonging hospital stay.

Isoflurane Induces Transient Anterograde Amnesia through Suppression of Brain-Derived Neurotrophic Factor in Hippocampus

  • Objective : Transient anterograde amnesia is occasionally observed in a number of conditions, including migraine, focal ischemia, venous flow abnormalities, and after general anesthesia. The inhalation anesthetic, isoflurane, is known to induce transient anterograde amnesia. We examined the involvement of brain-derived neurotrophic factor (BDNF) and its receptor tyrosine kinase B (TrkB) in the underlying mechanisms of the isoflurane-induced transient anterograde amnesia. Methods : Adult male Sprague-Dawley rats were divided into three groups : the control group, the 10 minutes after recovery from isoflurane anesthesia group, and the 2 hours after recovery from isoflurane anesthesia group (n=8 in each group). The rats in the isoflurane-exposed groups were anesthetized with 1.2% isoflurane in 75% nitrous oxide and 25% oxygen for 2 hours in a Plexiglas anesthetizing chamber. Short-term memory was determined using the step-down avoidance task. BDNF and TrkB expressions in the hippocampus were evaluated by immunofluorescence staining and western blot analysis. Results : Latency in the step-down avoidance task was decreased 10 minutes after recovery from isoflurane anesthesia, whereas it recovered to the control level 2 hours after isoflurane anesthesia. The expressions of BDNF and TrkB in the hippocampus were decreased immediately after isoflurane anesthesia but were increased 2 hours after isoflurane anesthesia. Conclusion : In this study, isoflurane anesthesia induced transient anterograde amnesia, and the expressions of BDNF and TrkB in the hippocampus might be involved in the underlying mechanisms of this transient anterograde amnesia.

  • In this paper, To take advantage a variety of DC power as the boost DC-DC converter design specifications through the inductor L and capacitor C through PSPICE to calculate the best estimate of the value. Boost DC-DC converter with a switch device using IRF840 and reverse recovery time Schottky diodes with excellent with constant current controller using D10SC6M and resistance can be configured to considering the Power LED Module was driven by the production. Converter's switching frequency is 50 kHz, the first Duty Rate was made to increase gradually depending on the value of the detection were, 10 % in the output voltage. As a result, the simulated Boost Power LED driver characteristics is in comparison with the design specifications, 5% or less as the error was approximated. Finally, when input 15 V were offered, a stable output 24 V were obtained. and Dimming Control through the adjustment of brightness and current consumption were possible.

The Effects of Semi-Fowler's position on Post-Operative Recovery and Pain for Patients with Laparoscopic Abdominal Surgery (복강경 수술 후 반좌위가 수술회복성과 통증에 미치는 영향)

  • Purpose:This study was conducted to identify the effects of theSemi-Fowler's position on post-operative recovery and pain for patients with laparoscopic abdominal surgery Methods: This study utilized a non-equivalent control group non-synchronized design to validate the effects of theSemi-Fowler's position. After IRB approval, 56 patients took part in this study, 29 in the experimental group and 27 in the control group. Consent was obtained from the participants. The Semi-Folwer's position was applied to experimental group and the supine position was applied to the control group for 24 hours after surgery. All data were reviewed retrospectively from April to June 2016. Collected data, frequency, percentage, average, standard deviation, chi-squared test, independent t-test and repeated measures ANOVA were conducted using SPSS 20.0. Results: There was no significant difference between the experimental and control group with regard to recovery outcomes; however, there was a significance differencebetween groups and among check times with regard to post operational pain. Conclusion:The results of this study provide information that will be usefulto the development of strategies for improving recovery outcomes and pain for laparoscopic operation patients.

The Role of Occupational Therapist in Disaster Management (재난상황에서 작업치료사의 역할에 대한 고찰)

  • Objective : In October 2016, the most powerful earthquake, magnitude 5.1 and 5.8 struck the city of Gyeongju in Korea. Although it did not take a toll, this implicates potential disaster in the future. Taking this earthquake, this paper considers the healthcare system responding to disaster in non-government organization and other countries, and investigates the roles of occupational therapist in disaster management. Methods : This paper reviews literature related to healthcare system and roles of occupational therapist in disaster response. Results : Humanitarian recovery mission of Red Cross impacted and facilitated the recovery of vulnerable population including children, elderly and people with disabilities in disaster response. It was also emphasized by occurring large population with disabilities after disasters so that the concept of rehabilitation and occupational therapist's role was required. Occupational therapy practitioners play an important role in the stage of disaster preparedness, response and recovery and their target population is children, elderly and people with disabilities. Conclusion : The most of NGO and counties take the concept of rehabilitation into healthcare system responding to disaster. However, the system in Korea stays in emergency level. It is important to take the humanitarian recovery and rehabilitation concept to disaster relief. the survivors would be able to return to their normalcy and health life.

National Oil Pollution Response System : Current Issues and Policy Recommendation (우리나라 기름오염방제제도의 문제점과 개선방안)

  • After the Sea Prince oil spill accident in 1995, the korean government has taken a measure to establish an emergency response system and equip clean-up capacity against large spill, major contents of which are as follows: First, Korea Marine Pollution Response Corporation has been established as a non-government organization for recovery of spilled oil in order to improve private response capabilities. Second, clean-up equipments, such as large clean-up vessels and oil fences for the open sea operation has been expanded. Third, a national contingency plan on the large spill accidents has been established compliance with the provisions of Article 6 of the OPRC 1990. However, there exist some problems in the national response system, such as clearly roles definition between government and private agencies; propel amendment of the Marine Pollution Prevention Act to incorporate major contents of the OPRC 1990; and training and exercises of clean-up personnel. With the above problems in mind, this paper reviews the current issues on the national oil pollution response system and recommends policy-making to tackle to those problems.

Unusual Position and Presentation of Frontobasal Meningoencephalocela

  • We wish to show our experiance with threating a rare congenital brain malformation-encephalocele. It is a protusion of brain matter with greater incidence in the Far East. Our case is even more curious because of the site of occurrence-frontobasal. Most of encephalocele occur in the occipital region. In this article we report a case of a 57-year-old woman, without deformations on the face, which had epileptic seizures and in spite of receiving antiepileptic drug. She was also frequently treated for sinusitis. She never had rhinoliquorrhea, nor was she diagnosed to have meningitis. In the last few years she had difficulty breathing on her right nostril. After she was diagnosed with encephalocele and treated surgically her recovery was complete and she is without the seizures.

Spontaneous Spinal Subdural Hematoma with Simultaneous Cranial Subarachnoid Hemorrhage

  • Spontaneous spinal subdural hematoma is reported at a rare level of incidence, and is frequently associated with underlying coagulopathy or those receiving anticoagulant or antiplatelet agents; some cases accompany concomitant intracranial hemorrhage. The spontaneous development of spinal subdural hemorrhage (SDH) is a neurological emergency; therefore, early diagnosis, the discontinuation of anticoagulant, and urgent surgical decompression are required to enable neurological recovery. In this report, we present a simultaneous spinal subdural hematoma and cranial subarachnoid hemorrhage, which mimicked an aneurysmal origin in a female patient who had been taking warfarin due to aortic valve replacement surgery.
