Reduction of Length of Stay in Emergency Room by Using Critical Pathway for Stroke Patients (Critical pathway 적용을 통한 급성 뇌졸중 환자의 응급실 체류시간 단축 효과)

  • Purpose: The purpose of this study was to demonstrate effects of a critical pathway (CP) for stroke patients seen in emergency rooms (ER). Method: The CP developed by the CP committee consisted of 8 criteria: behavior of doctors and nurses, laboratory tests, Image testing, medication, treatment, activity, and nutrition. According to application of CP, a control group (n=17) and experimental group (n=17) were defined. Time was checked by the electronic medical records. Result: Use of CP for stroke patients in the ER, resulted in a decreased length of stay in ER (t=2.341, p=.026), and time required for image testing (t=2.623, p=.021), and an increased number of patients using rtPA ($x^2$=4.802, p=.049). Time required for neurology doctor contact, for neurology doctor to see patient in the ER, and for report of blood tests decreased, but there were no statistical significance. Conclusion: Quick responses are most important in the ER, so CP for these patients is a very effective patient management tool. To reduce delay in stroke diagnosis, continuous education programs for similar symptoms are necessary. CPs for other patients in the ER should be developed, and studies on cost and satisfaction, as well as length of stay, should be done.

Job Description of the Nurses Working in Outpatient Department by DACUM Technique (DACUM 직무분석 기법을 이용한 외래간호사의 직무분석)

  • Purpose: The purposes of this study were: (a) to identify role definition, tasks, and duties of the nurses who work in outpatient department (OPD), (b) to do Development a Curriculum(DACUM) chart, and (c) to compare duties and tasks among nurses, nurse's assistants, and doctors. Method: The DACUM committee was organized with 10 nurses who worked in OPD. The committee derived the duties and tasks of OPD nurses from what they had done at workplace. Validity of the derived duties and tasks was tested by 23 nurses at 15 university hospitals. Results: Eleven kinds of duties were identified: preparation for outpatient's medical examination; support for outpatient's medical examination; management of outpatients; outpatient education; outpatient counseling; special examination of outpatient; OPD administration; management of OPD supplies; management of facilities and environments of OPD; management of OPD personnel, and self-improvement. Ninety-two tasks were classified. Conclusion: The abilities for education and emergency care of OPD nurses should be empowered. The results of this study would not only contribute to the effective OPD nursing care, but also be useful as basic data in hospital management.

Learning Needs of Registered Nurse for Insertive Education (실무교육에 대한 간호원의 학습요구)

  • The advanced knowledge is increasing day by day due to the progress of medicine and tectonics, the increase of nursing research nowadays. In parallel with these, nursing profession has a need of the nursing care with improvement of new Knowledge, tectonics and nursing procedure through the insertive education. Therefore, this study was attempted to investigate that to grasp learning need of nurses about inservice educational play a pivotal role in the progress of inservice education. This study was made from 242 clinical nurse at 4 hospitals in Seoul and through question- are consisting of 1) ideological part 2) basic sciences 3) nursing part 4) administrative part 5) the others from 10 July '75 to 7 Oct. '75. The data were treated by percentage, Licker method, arid chi- square examination. Significant difference p is 0.05. The results of this studies are as follows : A. learning needs of nurses 1) Generally the learning needs of nurses is very high. 2) Of the learning need of inservice education for nurses, the highest learning need is as follow (Table 2 ) 1 st : Charting 2 nd : The ability of grasping patient's needs and problems. 3 rd ; The contents and the methods of the advanced nursing skill. 4 th ; The importance of team work 5 th ; The general knowledge of the various disease 6 th : The decision of a propriety nursing diagnosis under the general condition. 7 th ; The introduction of the new nursing theory. 8 th : The Nurses role and the responsibility in emergency and disaster. 9 th : The improvement of nursing skill for the perfect interpersonal relationship. l0th ; Cultural education: B . Verification of hypothesis 1. Hypothesis I , learning needs of nurses and educational level of nurse will be correlated, is rejected (Table 3 ) 2. Hypothesis II, learning need of nurses and duties of post will be correlated, is rejected (Table 4) 3. A Survey on the Use of Visiting Nurses in Child Day Care Centers (지역사회 보육시설에서의 방문간호사 활용을 위한 기초조사연구)

  • Purpose: The purpose of this study was to contribute children's health care and health promotion by surveying the use of visiting nurses in child day care centers. Methods: This study is a descriptive study, and the study period covers July to September, 2010. The research subjects were 27 public health center managers, 166 visiting nurses and 137child care teachers. Results: Teachers' need of visiting health care services and visiting nurses' work performance of were statistically significantly different. The teachers' need of visiting health care was higher in all areas (health examination, health life practice, infectious disease control, safety accidents and disaster management, emergency measures and linkage, nutrition, parent education) but the visiting nurses had a low level of work performance. Conclusion: Child care teachers are not health professionals for child health care, and therefore they are in need of professional help. Thus, for the current public health centers in need of customized visit health care, new visiting nurses in charge of professional child health care need to be developed. Also, new models need to be developed for visiting nurses and child care teachers through the connection of community child health care.

New Graduate Nurses' Satisfaction with Transition Programs and Experiences in Role Transition (신규간호사의 실무적응지원 교육 만족도 및 역할 이행 경험에 관한 조사)

  • Purpose: The aim of this study was to investigate the satisfaction of newly graduated nurses with educational programs and their experiences in role transition. Methods: Data were collected from November 1 to December 15, 2018 and 483 new graduate nurses working at 15 tertiary hospitals and 10 general hospitals participated. For data collection, self-report questionnaires including the Casey-Fink Graduate Nurse Experience Survey tool and satisfaction with education were used. Data were analyzed using descriptive statistics, t-test, and one-way analysis of variance. Results: Satisfaction with education ranged from 3.09 to 3.27, and satisfaction with preceptors was 3.45(maximum 4). The skill that new nurses ranked as most difficult during the first 3 months was charting/documentation, and throughout a whole year, the top 4 difficult skills were cardiopulmonary resuscitation/emergency response, ventilator care, end-of-life care, and prioritization/time management. In comfort/confidence, new graduates felt most comfortable with support and least comfortable with patient safety. More than 50 percent of new graduates experienced stress during role transition, and the most frequently experienced stressors were related to job performance and personal life. Levels of satisfaction with education and comfort/confidence differed according to the hospital type and number of preceptors for new nurses. Conclusion: In order to facilitate the transition of new graduate nurses to professional nurse, an extended period of education, systematic and standardized transition programs, and continuous support during the first year of practice are required.

Survey on the Intention to Stay, Field Adaptation, and Educational Demands in New Nurses and Reemployment Nurses (신규간호사 및 재취업간호사의 재직의도, 현장적응 및 교육요구도 조사)

  • The purpose of this study is to compare the intention to stay, field adaptation, and educational demands between the new nurses and reemployment nurses. This research was done to create a base for a program that will improve field adaptation, and to prevent dropping out or moving. The followings are the results; There was a significant difference in the intention to stay of the 48 new nurses and 48 reemployment nurses (t = -2.10, p = .038), but there was no significant difference in field adaptation between the two groups (t = -1.48, p = .142 ). Finally, the subjects responded with the topics they would most like to learn about emergency patient care. Through the above results, this study provide basic data for developing a program that can enhance the intention to stay in hospital.

Influence of COVID-19-related Nursing Experience on Job Stress of Nurses (COVID-19 관련 업무 경험이 간호사의 직무 스트레스에 주는 영향)

  • Purpose: This study aimed to compare and analyze the job stress of nurses with and without in COVID-19-related work. Methods: A structured self-reported questionnaire survey was conducted to assess job stress. The extent of job stress was compared between nurses with COVID-19 (COVID-19 group) and those without such experience (non-COVID-19 group). Multiple regression analysis was performed to identify the factors influencing job stress. Results: Job stress was higher in the COVID-19 group compared to the non-COVID-19 group (t=2.54, p=.12). In sub-categorical comparison, stress driven by a taxing work environment, relationship conflict, and work schedule was higher in the COVID-19 group than the non-COVID-19 group. Multiple regression analysis revealed the job stress was higher among nurses with COVID-19-related work experience than that of non-experienced nurses. The factors affecting job stress of nurses with COVID-19-related work experience included emergency room work, providing nursing assistant for COVID-patients, and caring for these patients. Conclusion: Since COVID-19-related work experience is a major factor that affects nurses' job stress, it is imperative to provide various support measures for nursing assistants such as providing a break from working in an environment with high risk of infection, adjusting work schedules, resolving conflicts between personnel, and securing support.

Perception of Uncertainty and Satisfaction for Nursing Service of Children's Parents Visiting to Pediatrics Outpatient Department and Emergency Room (소아과 외래와 응급실 내원 아동 부모의 불확실성 인지와 간호서비스 만족도)

  • Purpose: This study was a descriptive research to provide fundamental data in ensuring a high quality of nursing service needed by children's parent according to perception of uncertainty and care satisfaction of them between pediatric outpatient department and emergency room. Methods: For this study, 192 children's parents were questioned in a general hospital located in Busan from August 1 to 31. 2009. The collected data were scored by use of frequencies, $x^2$ test, t-test, AONVA, Pearson's correlation coefficient via SPSS/WIN 17.0. Results: With the compare of characteristics related to children's parents visiting emergency room and pediatric outpatient department, there were statistically significant difference in relation and age of parent, main caregiver, cause of arrival, and waiting time from arrival to procedure. The perception of uncertainty for parent in the emergency room showed significant difference to outpatient department. In nursing service, the highest score was category of professional skill and technic. In addition, overall parents' perceptions on providing information showed lower than any other nursing service. Last but not least, a significant correlation indicated between the perception of uncertainty and satisfaction of nursing service. Conclusion: Efforts to improve parent satisfaction for nursing service are needed to decrease their perception of uncertainty. Providing information and communication by nurses are necessary to improve quality of nursing service.

Effects of Simulation-Based Education for Emergency Patient Nursing Care in Korea: A MetaAnalysis (응급환자 간호를 위한 시뮬레이션 교육효과: 메타분석)

  • Purpose: The purpose of this review was to evaluate the effects of emergency nursing simulation program on nursing students and nurses. Methods: This systematic review was performed as per the guidelines of the Preferred Reporting Items for Systematic Reviews and Meta-Analyses and using the R program meta package (version 4.9-2). RISS, KISS, and DBpia Library databases were searched for studies published between June 2000 and August 2018 using the following key words: ($Emerge^*$ OR nursing) AND ($nurs^*$ OR simulation). Selected studies were assessed for methodological quality using Risk of Bias for Non randomized Studies. Results: 7 studies were identified and all of them met the inclusion criteria. The outcome variables were significant clinical performance, self-efficacy except knowledge, and problem-solving ability. Conclusion: This review provides updated evidence of the simulation-based education program in emergency nursing. Further studies are required to increase generalizability using randomized population, research design and controlled trials with sufficient sample size. Moreover, valid measurements are needed to assess the main outcomes.

  • Overcrowding in an Emergency Department (ED) of hospital is a common phenomenon. To improve the service quality and system performance of the ED, a task assignment rule for the Registered Nurses (RNs) is proposed in this paper. At each task assignment point, the rule prioritizes all treatment requests based on the urgency which is determined by the multiple attributes such as accomplishment time of treatment task, elapsed time of treatment request, total remain time to patient discharge, and number of remain treatments. The values of partial urgency with a single criterion are determined and then overall urgency is computed to find the most urgent one among current requests with the importance weights assigned to the criteria. Through computer simulation, the performance of the proposed rule is compared with current rule in terms of the length of stay and system throughput in a simplified ED system of the hospital M.
