119 구급대원의 노인응급처치 현황 및 노인에 대한 지식, 응급처치 수행자신감 (Current prehospital care status, knowledge, and prehospital care confidence toward the elderly among 119 emergency medical technicians)

  • Purpose: This study aimed to improve the quality of prehospital emergency care for elderly patients by determining current prehospital care status based on run sheets and level of knowledge, and prehospital care confidence toward the elderly through questionnaires answered by 119 emergency medical technicians (EMTs). Methods: This study was conducted with 4,946 elderly patients who were transferred to the hospital by 119 ambulances in D city from March 1, 2013 to February 28, 2014. This study was a descriptive study of 119 run sheets. The questionnaire was completed by 160 EMTs working in D city. All data were analyzed by using SPSS 21.0. Results: 74.2% was due to disease among the 4,946 elderly patients who were transferred by 119 ambulances, and a rest was due to injury. The mean knowledge score of the 119 EMTs regarding elderly patients was low. Their confidence on prehospital care of the elderly patients statistically significantly differed according to career at hospital and education. Conclusion: Theoretical knowledge, education on prehospital care of the elderly, and repeated technical prehospital care education and training accompanied by feedback assessment by 119 EMTs are needed.

119구급대의 스마트의료지도 직무수행능력 영향요인 (Factors influencing the performance of emergency medical technicians utilizing smart advanced life support (SALS))

  • Purpose: The present study was conducted to identify the factors influencing the perfromance of the emergency medical technicians (EMTs) utilizing smart advance life support (SALS) and to provide appropriate feedback to improve SALS performance. Methods: The study subjects were recruited from 119 EMT members working in G city participating in SALS project. A total of 155 data were used in the analysis. Results: Of the three areas, the mean score of importance was the highest (4.53±0.48). followed by educational needs (4.24±0.66), and SALS performance (4.08±0.55). Importance was identified as a significant factor influencing SALS performance (β=.187, p<.047).The length of education. work scope. amd role of level 1 EMTs vary significantly depending on the type of qualifications they have as emergency medical professionals, while the supply of level 1 EMTs has already reached a point of saturation. Accordingly, the current regulation on allowing level 2 EMTs with at least three years of experience to take the level 1 EMT test presents serious inequity for students just graduating from their emergency medical services program. It is also a factor that degrades the professionalism of level 1 EMTs. Conclusion: Based on the results of this study, it was found that mediation and education programs need to be implemented to highlight the importance of the tasks performed in SALS situations in order to improve performance.

구급대원의 재난대응역량에 관한 연구 (Study on disaster response competencies of 119 EMTs)

  • Purpose: This study was conducted to investigate the disaster response competencies of 119 emergency medical technicians (EMTs). Methods: The subjects of this study were 226 119 EMT's at the fire station located in G province. The questionnaire consisted of 5 sub-factors and 29 items for measuring disaster response competencies and the collected data were analyzed using SPSS program. Results: Disaster response competency comprising immediate response, patient triage, patient treatment, patient transport, and collaboration support were 3.48, 3.64, 3.52, 3.64, and 3.16, respectively. Advanced EMTs scored significantly higher in patient triage (p<.001), treatment (p<.001), and transport (p=.022) competency than basic EMTs. Conclusion: In order to improve disaster response competency, it is necessary not only to enhance individual ability but also to continue disaster management training with related organizations.

119 구급대원의 노인학대 인식, 신고 및 교육 경험에 관한 연구 (Perception, report, and education of elder abuse in emergency medical technicians in Korea)

  • Purpose: The study investigated the perception, report, and education of elder abuse in 119 emergency medical technicians (EMTs) in Korea. Methods: A self-reported questionnaire was distributed to 110 EMTs in K city, and 100 answers were collected from June 1 to 10, 2015. The study instrument was perception regarding elder abuse and education inventory developed by Kim and Kim. Data were analyzed using SPSS/WIN 18.0 version. Results: The elder abuse perception score was 3.81 on a Likert 4-point scale. Physical abuse was the most common type, followed by verbal, financial, negligence, and emotional abuse. Most EMTs recognized that is important for the prevention of elder abuse, but most of them had no experience in reporting abuse. A majority of the EMTs had attended an elder abuse program and recognized the importsnce of such program. Conclusion: This study suggests the necessity of more targeted education of elder abuse prevention combined with family counseling methods and practical approach toward awareness of elder abuse.

119구급대원의 직업기초능력 요구도 및 우선순위 분석 기반 직무기초역량 제안 연구 (Defining core competencies for 119 emergency medical technicians based on the analysis of requirements and priorities of the profession)

  • Purpose: This study aimed to determine the core competencies for 119 emergency medical technicians (EMTs) and to provide evidence for the development and utilization of 119 EMTs, education and training programs. Methods: Data were collected with a questionnaire that consisted of items on the general characteristics of the subjects (8 items) and importance and satisfaction levels for 10 competencies, including 34 items on subfactors. The Borich Needs Assessment Model was used in designing the questionnaire. A locus for focus model was used to derive the top priority competencies for the improvement of core competency in the profession. Data were analyzed with by SPSS ver.21 (IBM, Armonk, NY, USA). Results: The core competencies for 119 EMTs were derived from technical competency, information competency, communication competency, problem-solving competency, self-development competency and interpersonal competency. In addition, among the sub-factors of these competencies, 16 abilities (including leadership), ability to apply technical knowledge, technical comprehension, conflict management ability, ability to create documents, problem handling ability, ability to think, technical selection ability, and computer literacy were included in the priority category. Conclusion: This study derived and presented the core competencies for 119 EMTs to enable them to meet the modern day requirements of their roles, which may contribute to enhancing their professionalism.

에고그램을 통해 본 119 구급대원의 자아상태와 소진과의 관계 (The Correlations Between the Ego-states and the Burnout of 119 Emergency Medical Technicians by Ego-gram)

  • Purpose : The purpose of this study is to identify the correlations between the ego-states of the 119 emergency medical technicians(119 EMTs) and the burnout and help them grow as a profession. Method : This is a descriptive study and the subjects of this study were 81 EMTs in 119 fire station in G City. This study tried to reveal the burnout in 119 EMTs in accordance with ego-state. Results : The highest score of 119 EMTs was the Nutrition Parent(NP) which showed $32.90{\pm}2.07$. The second highest was of the Critical Parent(CP), $31.95{\pm}2.79$. The Ego-gram profile of 119 EMTs was slope N type. The burnout was $78.90{\pm}13.19$, the physical burnout was highest($28.86{\pm}4.14$) in the low rank territory. The ego state according to level, the comparison of the degree of burnout, Free Child(FC) all but the type of ego-states burnout of the high groups appeared to be much higher. The correlations between their ego-states and the burnout of 119 EMTs appeared to be related to the 'plus(+)' correlation for the Free Child(FC ; r=0.243, p=0.029) and the Adapted Child(AC ; r=0.567, p=0.000), and CP, NP and Adult(A) appeared to be unrelated to the correlation. Results of the regression analysis show that the personal characteristic that has the biggest effect on the burnout of 119 EMTs was the AC(0.717), which seems to have a measurable effect on the statistics(p<0.01). Conclusion : Understanding the personality of 119 EMTs and its the application in the workforce will increase the human resources management system for the fire station officials.

  • Purpose: This is a descriptive research study that aims to identify factors affecting fatigue by examining the degree of emotional labor, job stress, and fatigue of 119 emergency medical technicians (119 EMTs). The correlation between emotional labor, job stress, and fatigue was also confirmed. Methods: A total of 139 EMTs working in the Daejeon area participated in this study. Data were collected from July 6-15, 2021. Results: When the number of dispatches was high and the job was dissatisfying, the degree of emotional labor was high. In addition, the shorter the work experience and the lower the job satisfaction are, the higher the job stress is. The degree of fatigue was significantly higher in female than male 119 EMTs, and when they had short work experience and were dissatisfied with their job, the degree of fatigue was higher. Moreover, there was a significant correlation between emotional labor, job stress, and fatigue of the 119 EMTs. Conclusion: To reduce emotional labor, job stress, and fatigue of 119 EMTs, appropriate interventions for each factor and institutional improvements are required.

응급구조사의 업무스트레스와 우울과의 관계 - 자아존중감과 사회적지지의 매개효과를 중심으로 - (Associations with Job Stress and Depression among Emergency Medical Technicians - Focusing on the Mediating Effect of Self-Esteem and Social Support-)

  • Objectives: The purpose of this study was to examine the mediating effect of self-esteem or social support on job-related stress and depression among emergency medical technicians (EMTs). Methods: Data were collected from EMTs at emergency medical institutions and fire stations (n=257). The data were analyzed using descriptive statistics, t-test, Pearson correlation coefficient, and stepwise multiple regression with SPSS WIN 21.0. Results: Self-esteem and social support of EMTs were found to be at moderate levels (Self-esteem: M=3.04, SD=0.43; Social support: M=3.65, SD=0.43) Job-related stress was positively correlated with depression: the higher the job-related stress score, the higher the depression score. Moreover, self-esteem score or social support score was negatively associated with job-related stress and depression: the higher the self-esteem score or the higher the social support score, the lower the job related stress score and the depression score. Conclusions: Future research should address how social support and networks interact and together affect self-esteem for job-related stress and depression.

응급구조사의 DNR에 대한 인식과 윤리적 태도 (Awareness of and ethical attitude toward "Do Not Resuscitate"orders in 119 emergency medical technicians)

  • Purpose: This descriptive study examined awareness of and ethical attitudes associated with Do Not Resuscitate (DNR) orders in 119 emergency medical technicians (EMTs). Method: In total, 255 paramedics and basic EMTs completed questionnaires between March and May, 2013. Data were analyzed using the SPSS WIN 18.0 program. Results: A chi-square analysis revealed significantly different responses given by paramedics and basic EMTs: Paramedics gave "the reason in which DNR is not necessary", as being "due to unclear DNR decision time" whereas basic EMTs answered that this was "due to a legal problem" ($x^2$ = 12.680, p < .05). Paramedics disagreed with the statement, "It is natural for medical teams to have less interest in patients with DNT orders", whereas basic EMTs agreed with the statement ($x^2$ = 6.666, p < .05). Conclusion: A unified manual on attitude toward DNR orders, taking account of social and culture factors, needs to be developed. This research provides a base line for future research.

우리나라 응급의료센터 응급구조사의 직업적 방사선 노출 (Occupational Radiation Exposure of Emergency Medical Technicians in Emergency Medical Centers in Korea)

  • Objectives: This study aims to investigate the occupational radiation exposures of emergency medical technicians(EMTs) in emergency medical centers in Korea. The results will provide a basis for developing prevention programs to minimize adverse health effects relating to radiation exposure among emergency medical technicians working in this area. Methods: Radiation exposure doses were measured for twenty-two EMTs working in six emergency medical centers. Thermo Luminescent Dosimeters(TLD) were placed on three representative body parts, including chest, neck, and a finger. Measurements were conducted over the entire working hours of the participants for foor weeks. Dosimeters were analyzed according to a standard method by a KFDA-designated lab. Detection rate, annual radiation exposure dose, and relative levels to dose limit were derived based on the measured doses from the dosimeters. SPSS/Win 18.0 software(IBM, US) was used for statistical analysis. Results: Detection rates were 45.5%, 36.4%, and 45.5% for the dosimeters sampled from chest, neck, and a finger, respectively. The average annual doses were $2.39{\pm}3.44mSv/year$(range 0.38-10.0 mSv/year) for the chest, $2.72{\pm}3.05mSv/year$(2.00-11.34) for the neck, and $20.98{\pm}17.57mSv/year$(1.25-53.50) for the hand dose. The average annual eye dose was estimated to $3.61{\pm}2.37mSv/year$(1.50-8.34). The exposure dose levels of EMTs were comparable to those of radiologists, who showed relatively higher radiation dose among health care workers, as reported in another study. Conclusions: EMTs working in emergency medical centers are considered to be at risk of radiation exposure. Although the radiation exposure dose of EMTs does not exceed the dose limit, it is not negligible comparing to other professionals in health care sectors.
