A Management Strategy for Emergency Medical Facilities in Domestic Disaster Sites (국내 재난현장 응급의료시설 관리방안에 관한 연구)

  • Purpose: Despite the fact that the needs for disaster emergency medical facilities to minimize casualties are increasing, research for emergency medical facilities is insufficient compared to research for DMAT investment. A management strategy for emergency medical facilities in disaster site needs to be proposed. Methods: Recently there was introduction of mobile hospital system using unit modular method, but it was found out to be used only in disaster emergency training. Emergency medical facilities in disaster site require various tasks. Examination for our research was carried out through consultations with emergency medical and mobile hospital expert because there is no practical use case to apply Emergency Medical Facilities in Korea. Result: In this research we proposed a management strategy to manage and operate emergency medical facilities composed of tent, unit module and container in the aspects of management efficiency. Implications: It is highly expected to establish a foundation for fast and accurate disaster emergency medical facilities deployment by applying the results of our research to Emergency response manual of disaster emergency medical service.

A Study on the Architectural planning of Spatial Organization Feature in Emergency Medical Facilities (응급의료시설의 공간구성요소에 관한 건축계획적 연구)

  • The purpose of this study was to spatial organization feature and area composition of the over 500beds general hospital that will be expected construct in Seoul, Gyeonggi-do area after 2000year. From this study, we can suggest architectural planning for the efficiency operating of emergency medical facilities that can be used as preliminary data. The Spatial organization of emergency medical facilities are categorize into 4 session as medical examination and treatment, diagonosticradiology, staff, public and the others. We can find spatial organization system and area composition that compare area composition of 6 hospital the subject of research in a emergency medical facilities. The result of this study were as follows : spatial distribution of its field, grasping of spatial organization factor and recognize its merits and demerits should be researched in advance. Through this kind of recognizing emergency system, realistic medical local and condition architectural planning for spatial organization will be operate.

Primary visit, transfer, and re-transfer to emergency department in patients with severe emergency diseases in Chungnam (충남에서 발생한 중증응급환자의 응급의료기관 일차 방문, 전원 및 재전원 현황)

  • Objective: Regionalization is one of the principal subjects for the advancement of rural emergency medical service systems in South Korea. This study shows the characteristics of interhospital transfer and status of the incidence of three major emergency disorders (acute myocardial infarction [AMI], acute stroke, and severe trauma) in one local province. Methods: A retrospective study was conducted for patients with three major emergency disorders who visited emergency medical facilities in one local province from January 2013 to December 2015, on the basis of the National Emergency Department Information System (NEDIS) data. Results: The incidence of three major emergency disorders had increased annually. Patients with each of these disorders tended to choose distinguishing methods of visiting emergency medical facilities. AMI patients tended to visited emergency medical facilities using private cars or on foot, while severe trauma patients usually visited by 119 ambulance, and acute stroke patients used 119 ambulance and private car in similar amounts. Overall, 65% of AMI patients were treated in intraregional medical facilities, but about 70% of acute stoke and severe trauma patients were transferred outside of the region. Conclusion: Because each of these disorders has an individual characteristic, it is difficult to expect a solution for the problems associated with emergency disorders just by assuring the availability of medical resources. Based on regionalization, a policy to provide the optimal treatment for those emergency disorders should be developed by planning public medical service systems based on the individual characteristics of emergency disorders, the standardized transfer plans of emergency patients and the assurance for mobilization and sharing of confined medical resources.

Knowledge and Current Status about AED in the Public Facilities - Focused on the Gwangju City - (다중이용시설에서의 AED에 관한 지식 및 운영실태에 관한 연구 - 광주광역시 중심으로 -)

  • Purpose: In this study, we investigated the better application of the law which is about the AED installation and more effective ways of emergency medical care system, to understand the law and to research the current condition of public facilities which belong to local governments, and to seize the aspect of safety guards who currently work in order to provide the installation of AED in the public facilities and to provide more efficient emergency medical service with the effectuation of the immunity law of the good intention of first-aid treatment. Methods: In Gwang-ju, 234 public facilities have been identified by 31 December, 2008. With the exception of the duplication, we researched 158 facilities and received the answers from 95 of them. Results: In the research, 53% of them have had internal emergency first-aid education, and 55% of them didn't have this education and a CPR education manual, and 30% of the facilities even didn't know how to connect with the manager of the company for the first-aid department. On the other hand, most of them were highly interested in CPR and AED education on the ratio of 91% and 93%. 88% of them have been trained about first-aid, 51% of them haven't been retrained, 17% have never been trained. so, the reality of emergency system at public facilities is serious. 78% of them knew they are working at public facilities, though 49% of them didn't know about AED installation. 57% of them didn't know the fact there is the immunity law related with good intentions for first-aid treatment. 63% of the facilities have security guards, and 30% of them didn't answer the questions. Also, many of them agreed to the opinion that all employees should have first-aid training. At representative survey report of participator of public-facility, emergency treatment is 61%, 16% of patients calling. Accordingly they importantly think better doing an on-site first-aid than evacuating the patient. And the rates show that 57% of them answerers tend to call Fire-Office(119) for evacuating the patients, and 28% of them EMIC(1339) for the first-aid. Conclusions: In this study, we are suggest to improve the details of the efficient operations and management after the grasp of the uninstallation, indifference, and unreliable conditions of AED. 1) Need a publicity of AED install cognition which is an emergency medical instrument at public facilities. 2) Arrangement of safety agents at facilities and concerns about them for good management from the parties concerned. 3) Need a designation of legal details according to the decision of the AED installation and the standard of the AED installation. 4) Training about first-aid of safety guards and the persons concerned in the facilities should be practiced participation with the positive and through this, first-aid treatment could be done by anyone who knows the immunity law related to medical emergency. 5) The brochures for the potential users and the results form practicing the instructions need to be improved in many ways through recording the emergency cases that have happened.

A Study on the Site Selection Process of Field Emergency Medical Facilities Based on Text Mining (텍스트마이닝 기반의 재난현장 응급의료시설 대상지선정 프로세스 연구)

  • Purpose: In the case of mass disaster, the establishment of temporary medical facilities for the first aid and treatment is required for the stable accommodation of patients caused by the disaster. However, the criteria for decision making related to the deployment of field emergency medical facilities are not specified. So, The purpose of this study is to draw considerable factors needed for the deployment of field emergency medical facilities and to make proposal for site selection process of field emergency medical facilities on the basis of the factor. Methods: This study performs text mining of disaster-related laws, guidelines and documents to derive key factors affecting site selection, also proposes a decision making process and conducts virtual deployment to validate the process. Results: The key factors for the site selection derived as the size of the damage, the size of the DMAT inputs, the location of available place, and distance to the disaster base hospital. As a result of virtual deployment following proposed decision making process, It is confirmed that the site of field emergency medical facilities is changed depending on the type of disaster, even if the scope of the disaster damage was the same. Implications: The deployment of field emergency medical facilities requires a separate criteria for each type of disaster, not uniform, as a future research a quantitative approach of the criteria needs to be performed.

An Estimation on the Economic Value of Emergency Medical Facilities (응급의료시설의 경제적 가치 추정)

  • We consider the economic value of emergency medical facilities. An emergency medical facility affects the medical environments in a community, and thus the social demand on the facility increases as the demand of qualified public health service increases. Regarding the increased demand and the limited resources of fiscal budget, it is important to scientifically evaluate the social benefit of the public investment on emergency medical facilities, as the results of evaluation can help make better budgetary decision on each public investment project of emergency medical facilities. In this paper, we try to estimate the economic value of emergency medical facilities based upon the estimated changes in preventable death rate by the facility and the statistical value of life. We hope the results contribute to improve the budgetary decision making on the emergency medical facility projects, thus the public health policies.

  • Purpose: Facilities for infection prevention according to the characteristics of the patients, caregivers and medical personnel are needed in the patient triage room of the emergency department as a space for the first medical examination and classification of the patients. This study focus on the patient tirage room with the highest frequency of use in the emergency department to prevent the hospital acquired infection of the emergency department. Methods:: This study analyzed the facilities standard needed for the infection prevention through interviews with the medical personnel and analyses on the facilities standard/cases of foreign hospitals and facilities standard at home and abroad Results: And based on this, it attempted to present improvement measures by analyzing the line of circulation and space used by infected patients in a hospital designated in the regional emergency medical center among hospitals whose emergency department overcrowding index is high. Implications: The facilities standard for the infection prevention among the courses for patient classification of the emergency medical centers could be identified and implemented to prevent infection.

A Research Study on the Medical-spaces Setting of Mobile-hospitals for Emergency Medical Response (긴급 의료 대응을 위한 이동형병원의 의료공간 설정에 관한 조사 연구)

  • Purpose: As the pandemic period continues, various attempts are being made to new medical spaces in the medical society. Many hospitals, including existing general hospitals, have been effected by infected patients and are showing limitations in patient care capacity. Mobile-hospitals may be the starting point for the development of new environment in the medical society and healthcare facilities which are not replacing the role of existing hospitals. Mobile-hospitals can possibly respond to situations that require medical services and provide emergency care for various demands in connection with existing healthcare facilities. Methods: Through a total of five investigations/analysis, medical functions that can be inserted into mobile-hospitals based on modular architecture are set. The first is the analysis of domestic legal guidelines, the second is the analysis of previous studies, the analysis of emergency medical facilities and other medical spaces of hospitals to be compared, the fourth is the analysis of medical spaces of actual mobile hospital projects. Results: Through five analyses, medical functions applicable to the modular building platform were finally established. Mobile hospitals can be used not only in disaster sites such as infectious diseases, but also in medical underprivileged areas or general hospitals. Therefore, it is necessary to establish medical functions that meet the specificity of mobile hospitals along with the functions of existing fixed medical facilities. Furthermore, various studies such as use in international aid, use in normal times, and connection with other platform-based medical facilities are considered necessary. Implications: Through 5 strategies of analysis, 41 medical functions which can be applied to UNIT are decided and these functions will be placed where medical services will be required.

Improvement of the pre-hospital emergency medical service system in China (중국의 병원 전 응급의료체계 개선방안)

  • Purpose : Emergency medical services in China are increase in demand by people and under the greater pressure than ever before. So it is, necessary to advance the pre-hospital system in order to promote the development of emergency medical services. Methods : This is based on China-related articles, books, journals, reports, statistical data and other literature. Results : First, pre-hospital emergency medical care with the introduction of specialist training program should be established. Second, to strengthen pre-hospital emergency services and to develop the EMS guidelines. Third, the "120" reporting systems unification and awareness activation. Fourth, the preparation of the EMS facilities equipment system. Fifth, the rapid transport system establishment to the selected medical institutions. Conclusion : It is necessary to strengthen the emergency medical personnel at the scene, rapid transport, rapid patient triage and to improve the survival rate of the patients.

Estimation of Economic Benefits Based on Appropriate Allocation of Emergency Medical Beds by Region in South Korea (지역별 응급의료병상 적정 분배에 따른 경제적 편익 추정)

  • Background: This study aimed to assess the appropriate allocation of emergency medical beds across 17 provinces and presume the economic benefits associated with such allocation. Methods: To estimate the optimal allocation of emergency medical beds by province, data from the Statistics Korea's "cause of death statistics (2014-2021)," regional statistics on "area, population, gender, age," and "population projections" were utilized. The "number of emergency beds by city and district" provided by the Health Insurance Review and Assessment Service was also used. In estimating the economic benefits of preventing avoidable emergency deaths due to the expansion of emergency medical facilities, guidelines from the Korea Development Institute and the Korea Transport Institute were referenced to calculate the wage loss costs associated with emergency deaths and estimate the economic benefits. Results: The optimal ratio of emergency medical beds allocation by region was highest in Gyeonggi, Seoul, Gyeongnam, Gyeongbuk, and Busan, while Daejeon, Jeju, and Sejong showed lower ratios. Additionally, the prevention of avoidable deaths and economic benefits resulting from the increase in emergency medical facilities were highest in Gyeonggi, Seoul, Gyeongbuk, Gyeongnam, and Busan. However, when standardized by population, the prevention of avoidable deaths and economic benefits were analyzed to be highest in Gyeongbuk, Chungnam, Jeonnam, Gyeongnam, and Busan. Conclusion: The results of this study can serve as foundational data for future policy measures aimed at addressing the imbalance in the supply of emergency medical facilities across regions. Considering regional characteristics in the distribution of emergency medical facilities is expected to ultimately increase the efficiency of national finances and yield economic benefits.
