  • Objectives: With the aging of the population, the incidence of a variety of diseases is increasing. Cerebrovascular disease is a major cause of mortality worldwide and therefore requires timely treatment and management. We aimed to evaluate the clinical effectiveness of herbal medicines in treating cerebrovascular disease by screening the most frequently prescribed herbal medicines and analyzing the clinical data of patients with cerebrovascular disease taking these medicines. Methods: In this retrospective study, we analyzed the electronic medical records of patients with cerebrovascular disease who received herbal medication at the Pusan National University Korean Medical Hospital from 10 March 2010 to 31 March 2017. The inclusion criteria were as follows: diagnosis of cerebrovascular disease, use of frequently prescribed herbal medicines, available records of laboratory tests and functional evaluation that was conducted within two weeks before and after admission and discharge, and herbal medicine usage exceeding half the period of hospitalization. Results: Of the total patients with cerebrovascular disease, the number of patients analyzed in this study was 49. There were some changes in the various variables examined after frequently used herbal medicines had been taken, such as complete blood count, liver function test, lipid test, and functional evaluation. No serious adverse events related to herbal medicine were found in this study. Conclusions: We concluded that herbal medicines frequently used for cerebrovascular disease may have some clinical effectiveness. However, because of the retrospective nature of this study and only one hospital involved, the generalizability of our findings is limited.

  • Certified e-Document Authority keep it with protection legal as a system a guarantee and identifies originality of an e-Record, It presume to be authenticity e-Records and contents of an e-Record prove what was not changed. But, e-Records has high medium degree of dependence and loss danger of information has very high problems. In addition, Because correction(attachment and deletion) and a revision of information are easy, a problem for integrity and the originality of an e-Record is caused. Existing system show the following inefficient. For the originality guarantee, an existing e-Documents encryption method accomplishes a encrypted process of a whole document with a symmetric key, if the information revised midway, the whole documents content must accomplish re-scanning and re-encryption process again. To get over such inefficient, this paper maximize efficiency which occurred at the time of partial information revision request by encryption and managing using the link information based on the linkage characteristics of the each page on the registered requested e-Documents, It was able to increase security configuration by minimizing problems on an information exposure through increasing complicated of the key management.

  • Diabetes Mellitus (DM) is one of common chronic diseases leading to severe health complications that may cause death. The disease influences individuals, community, and the government due to the continuous monitoring, lifelong commitment, and the cost of treatment. The World Health Organization (WHO) considers Saudi Arabia as one of the top 10 countries in diabetes prevalence across the world. Since most of the medical services are provided by the government, the cost of the treatment in terms of hospitals and clinical visits and lab tests represents a real burden due to the large scale of the disease. The ability to predict the diabetic status of a patient without the laboratory tests by performing screening based on some personal features can lessen the health and economic burden caused by diabetes alone. The goal of this paper is to investigate the prediction of diabetic and prediabetic patients by considering factors other than the laboratory tests, as required by physicians in general. With the data obtained from local hospitals, medical records were processed to obtain a dataset that classified patients into three classes: diabetic, prediabetic, and non-diabetic. After applying three machine learning algorithms, we established good performance for accuracy, precision, and recall of the models on the dataset. Further analysis was performed on the data to identify important non-laboratory variables related to the patients for diabetes classification. The importance of five variables (gender, physical activity level, hypertension, BMI, and age) from the person's basic health data were investigated to find their contribution to the state of a patient being diabetic, prediabetic or normal. Our analysis presented great agreement with the risk factors of diabetes and prediabetes stated by the American Diabetes Association (ADA) and other health institutions worldwide. We conclude that by performing class-specific analysis of the disease, important factors specific to Saudi population can be identified, whose management can result in controlling the disease. We also provide some recommendations learnt from this research.

  • As the use of information has emerged as the core of national competitiveness, major developed countries and the Korean government have realized the importance of data. They have pursued technical research and standard establishment for long-term preservation and continuously strived for systematic management and preservation of data. However, although various types of data are specified for the purpose of record management in the law, there is no specific method on how to collect, manage and preserve them, except standard electronic documents. In particular, management and preservation of huge datasets from the administrative information system have been strongly demanded above all. Any guidelines for datasets do not have been properly provided. After the framework for selecting preservation format must be prepared, the system can be supplemented and built. The framework considering the characteristics of the dataset should be specified more concretely, and empirical verification of the conversion and restoration for the dataset preservation format derived according to the selection criteria is necessary. Therefore, this study intends to propose a method for long-term preservation through empirical verification of the preservation format after deriving an evaluation the framework for the preservation format selection criteria considering the characteristics of the dataset.

  • Purposes: Pancreatic injury is rare in abdominal trauma patients (3%~12%). but it could result in significant morbidity and even mortality. Early and adequate decision making are very important in the management of patients with traumatic pancreatic injury. The purpose of this study was to assess the kinds of management and outcome through the review of our experience of pancreatic injury with multiple trauma. Methods: We reviewed 17 patients with traumatic pancreas injury via electronic medical records from Jan. 2002 and April. 2011. We collected demographic findings; the type, location and grade of pancreas injury, the treatment modality, and patient's outcomes, such as complications, length of hospital stay (LOS), and mortality. Results: Total 17 patients were reviewed, and man was 13 (88%). Traffic accident was the most common cause of injury. Pancreas neck was the most common injured site, and occured in 5 patients. Ductal injury was detected in 7 cases. Eleven patients were treated by surgical procedure, and in this group, 3 patients underwent the endoscopic retrograde pancreas drainage procedure coincidently. ERPD was tried in 8 patients, and failed in 2 patients. The major complications were post-traumatic fluid collection and abscess which accounted for 70 % of all patients. The hospital stay was 35.9 days, and it was longer in patient with ductal injury ($38.0{\pm}18.56$ vs. $34.5{\pm}33.68$ days). Only one patient was died due to septic shock associated with an uncontrolled retroperitoneal abscess. Conclusion: Early diagnosis is the most important factor to apply the adequate treatment option and to manage the traumatic pancreas injury. Aggressive treatment should be considered in patients with a post-operative abscess.

  • Purposes: The purpose of this study is to analyze the institutional and personal factors that affect the medical utilization of patients transferred to tertiary medical institutions. Methodology: We retrospectively analyzed the 2 weeks electronic medical records of 1,556 patients, who were referred to the tertiary hospital, from June 15 to 26, 2015. The patient's personal characteristics, referral hospital, referral path, medical experiences and expenses were analyzed for 6 months after the patient's first visit. Findings: The largest proportion (848; 54.5%) of referrals was referred from primary clinic but the referrals of the same tertiary hospital level were one in seven (228; 14.7%) of the patients. Most patients (1,401; 90%) were referred from the clinics and hospitals directly and only one in ten (155; 10%) of the patients utilized the medical referral center. Patients who had been referred from tertiary care institutions had significantly higher medical costs than those referred to primary care (7,560,000 vs 2,333,000 won). The institutional factors including the numbers of visits to outpatient clinic, previous history of hospitalization and operation, consultation to other medical departments and hospitalization fee significantly influenced on medical utility pattern. Personal factors including patient's medical diagnosis and department of disease have a highly correlation with patient's referrals. Practical implications: The medical utilization of medical expenses and experiences is influenced by institutional and individual factors, and it is important to establish a referral system considering the institutional factors of the type of referral hospital.

  • In this Paper. we describe a home health care service using electronic health questionnaires and routine checkup of vital signs Including ECG (Electrocardiography) , blood pressure. and SpO$_2$ (Oxygen Saturation) . This system is for patients at home with chronic diseases, discharged Patients, or any normal people for the Prevention of disease The service requires a home health care terminal and a PC with Interned connection installed at Patient home. The distance health care management center is equipped with a vital-sign and questionnaire interpreter as well as database, Web, and notification servers with UMS (Unified Messaging System). Participating Physician can access the servers at the center using a Web browser running on a PC available to them at any time. These components are linked together through various kinds of data and voice communication channels including PSTN (Public Switched Telephone Network) . CATV(Community Antenna TV) . Interned. and mobile communication network. Following the Physician's direction given to a Patient. he or she uses the home health care terminal to collect vital signs and fill out the questionnaire. When the terminal automatically transmits these data to the management center. the data interpreter and servers at the center process the information fo1lowing the Protocol implemented on the system. Physicians can retrieve and review data corresponding to their Patients and send back their diagnostic reports to the center. UMS at the center delivers the physician 's recommendation to the corresponding patient through the notification server. Patients can also reprieve and review their own records as well as diagnostic reports from physicians. The system Provides a new way of collecting diagnostic information and delivering doctor's recommendation to patients at home for their health management. • Purpose: This study was conducted to identify the clinical characteristics and risk factors on the occurrence of variant angina, and to examine the predicting factors on the vascular endothelial dysfunction of the patients with variant angina. Methods: A total of 134 patients diagnosed with variant angina were recruited from 2006 to 2008. The degrees of endothelial dysfunction were measured and recorded by the researcher using the values of flow-mediated vasodilation of their brachial arteries and Nitroglycerine-mediated dilation. Subjects' demographic data and risk factors were gathered after obtaining informed consent, and their electronic medical records were reviewed to collect laboratory data. Results: The mean age was $54.2{\pm}9.6$ years and 52% was male patients. More than 50% of the male patients were cigarette smokers and had hypercholesterolemia. 84% of the male patients and 70% of the female patients had more than one risk factor of cardiovascular disease. A stepwise multiple regression analysis showed that smoking and hypercholesterolemia predicted the decrease of flow-mediated vasodilation (Adjusted $R^2$ = .204, p < .001). Conclusion: Tailored educational interventions for smoking cessation and cholesterol management are needed to prevent recurrence of angina attack for patients with variant angina and to prevent cardiovascular disease for middle-aged workers.
