경흉교감신경절부의 전기자극의 체표면 체열변화에 미치는 영향 (Influencing of Electrical Stimulation to Cervicothoracic Sympathetic Ganglion on the Temperature Change of Body Surface)

  • This study was carried out to determine the influencing of electrical stimulation to cervicothoracic sympathetic ganglion(CTSG; stellate ganglion) u the sympathetic tone. For the purpose of this study. the stimulation was given to both the interferential current stimulation(ICS: AMF 100Hz) group consisting of 10 person(males 8, females 2) and the transcutaneous nerve electrical stimulation(TENS: 100 Hz) group of 10(males 7, females 3) in the right side of the trachea with probe electrodes. Then. the temperature changes on the surfaces of the forehead. cheek, neck and internal ear of cephalocervix, which is subject to the influence of the cervicothoracic sympathetic ganglion, and the palm. which is the end of the upper limbs. measured before. immediately alter. 10 minutes after and 20 minutes after experiment. The results are summarized as follows. 1. The emergence of remarkable Horner's symptoms which appear due to the changes of the tone of cervicothoracic sympathetic ganglion was not seen. However, in the interferential current stimulation group there were two felt the sense of warmth in the facial region and one person who felt it in the upper limbs, and in the transcutaneous nerve electrical stimulation group there was each one person who felt the sense of warmth in the facial region and in the upper limbs, respectively. Both groups have each one person who felt the sense of oppression in the eyelids. Three persons of the interferential current stimulation group and two persons of the transcutaneous nerve electrical stimulation group have the sense of hoarse voice or numbness in the neck. These are the symptoms that appeared during stimulation, so it is difficult for them to be considered as the direct effects of the changes of the tone of cervicothoracic sympathetic ganglion. 2. The t-test was performed to determine the significance between the right, which is the experimental side, and the left, which is the non-experimental side. Significance between the right, which is the experimental side, and the left, which is the non-experimental side. Significant changes were seen in the necks of the interferential current stimulation group and in the cheeks and internal ears of the transcutaneous nerve electrical stimulation immediately after experiment(P<.05). And the interferential current stimulation group showed a very high significance in the cheeks immediately after experiment and in the necks ten minutes after experiment(p<.01). Therefore, it could be seen that the electrical stimulation had an influencing on the changes of body temperature of the cephalocervix. 3. In both the interferential current stimulation group and the transcutaneous nerve electrical stimulation group, the forecheads, checks and necks of the cephalocervix in the experimental side(right) rather than the non-experimental side(left) had mostly a statistically significant rise in temperature immediately after experiment. The one-way ANOVA was carried out to determine the temperature change of on the surface of the body with the lapse of time; before, immediately after, ten minutes after and tewenty minutes after experiment. But no statistical significance was found from both the right and left sides.

  • Study the variation of low frequency electrical stimulation on human metabolism which serum to test the increasement of creatine- kinase, myoglobin, lactate .and pyruvate. The results were as followed: 1) Creatine kinase increasement showed significant difference between 200 Hz stimulation group and control group, and also between 300Hz stimulation group and control group, but no significant increasement difference showed in 50 Hz stimulation group compared to control group. 2) Lactate increasement showed significant difference in 200 Hz stimulation group and 300 Hz stimulation group compared to control group, but no significant difference showed in 50 Hz stimulation group. 3) Pyruvate increasement showed significant difference in 50 Hz stimulation group and 200 Hz stimulation group compared to control group, but no significant increasement difference showed between 50 Hz stimulation group and control group. (P < 0.05) 4) Myoglobin increasement showed significant difference in 50Hz stimulation group and 200 Hz stimulation group compared to control group, and also in 200 Hz stimulation group and 300 Hz stimulation group compared to 50Hz stimulation group.   • Purpose: This study was to examine the effects of electrical muscle stimulation therapy on chronic knee pain in the elderly. Method: The research design was a one-group pretest-posttest design. The subjects were 45 (TE: 17, SE: 15, SY: 13) elderly, 65 years old and above with chronic knee pain. Pain was measured by the S-F McGill Pain Questuionnaire (S-F MPQ) and Arthritis Impact Measurement Scale (AIMS). Electrical muscle stimulation therapy experimental treatment was applied for 12 weeks, 3 times/week, 15 min/time. Data was collected from March 2005 to February 2006. Data was analyzed using the SPSS PC+ 12 version. Results: After receiving electrical muscle stimulation therapy, chronic knee pain in TE (S-F MPQ: t=-62.143, p=.000, AIMS: t=-29.155, p=.000), SE (S-F MPQ: t=-76.345, p=.000, AIMS: t=-39.323, p=.000), and SY (S-F MPQ: t=-43.691, p=.000, AIMS: t=-30.306, p=.000) groups were significantly decreased. Conclusion: Electrical muscle stimulation therapy can be a better effective primary nursing intervention for chronic knee pain for community dwelling elderly people with TE, SE, and SY.

The Effects of Balance Training with Functional Electrical Stimulation on Balance and Gait in patients with chronic stroke

  • Objective: The purpose of this study was to examine the effects of balance training with Functional Electrical Stimulation (FES) on balance and gait in patients with chronic stroke. Design: A cross over design Methods: Nine patients with stroke were recruited into this study. They were measuring their balance ability and gait ability. The intervention "A" included 4 weeks of balance training with Functional Electrical Stimulation (FES) for 40 m/d, 3 d/wk. Intervention "B" included 4 weeks of balance training with placebo Functional Electrical Stimulation (FES) for 40 m/d, 3 d/wk. Of the 9 patients who completed the study, 5 were randomly assigned to" group A-B", and 4 to group B-A. The crossover occurred after 4 weeks. Results: Following are the specific results of balance training with Functional Electrical Stimulation (FES) on patients with chronic stroke. First, patients who received treatment A showed improvement compared with patients who received treatment B in static balance. There were significant decreases in anterioposterior, mediolateral postural sway extension and velocity moment (p<0.05) with their eyes opened and closed conditions. Second, they had significantly improved in dynamic balance (p<0.05). Lastly, there were also improvement in their gait velocity and cadence (p<0.05). Conclusions: These findings suggest that, the Functional Electrical Stimulation (FES) combined with balance training more effectively improves the balance and gait ability, I'm convinced that it could be actively used in clinics added to the conventional physical therapy in the future.

  • Purpose : The purpose of this study was to investigate the effects of mirror therapy with functional electrical stimulation to improve upper extremity motor function and activities of daily living (ADL) in stroke patients. Method : Thirty patients were randomly assigned to the experimental and control groups 15 patients in each. All subjects received the general occupational therapy consisting of five 30 min sessions per week for 8 weeks, in addition to 30 min of mirror therapy with functional electrical stimulation for the experimental group and 30 min of functional electrical stimulation for the control group for each session. To measure the functions of the upper limb and performance capacities in ADL, the Fugl-Meyer Assessment (FMA), and Manual Function Test (MFT), Modified Barthel Index (MBI) were used before and after the interventions. Results : Both the experimental and control groups showed a statistically significant increase in post-treatment FMA, MFT and MBI scores compared to their pre-treatment scores (p<.05). In addition, intergroup comparisons revealed a statistically significant increase in the scores of all assessments for the experimental group compared to those for the control group (p<.05). Conclusion : Based on these results, it is reasonable to conclude that mirror therapy with functional electrical stimulation is an effective intervention for improving upper limb motor function and ADL performance in stroke patients.

  • Acupuncture has been used as a clinical treatment in Oriental medicine for various diseases. In the present study was carried out to investigate the effects of acupuncture and electrical stimulation on the change neuronal nitric oxide synthase(nNOS) immunoreactive cells in the periaqueductal gray(PAG) area of the male SD rats. Enhanced expression of nNOS was detected in the dorsolateral-PAG(DL-PAG) area of rat with stress by fixed body, and acupuncture and needle electrode electrical stimulation groups at Hapgok like acupoint decreased the stress-induced enhancement in the expression of nNOS. The present results demonstrate that acupuncture and needle electrode electrical stimulation is effective in the modulation of expression of nNOS in the DL-PAG area under stress conditions.

뇌경색환자의 연하곤란에 대한 전기자극 치료의 효과 (The Effect of Electrical Stimulation for Dysphagia Caused by Cerebral Infarction)

  • Purpose : The purpose of this study was to analysis the effect of electrical stimulation for dysphagia caused by cerebral infarction. Methods : We recruited nineteen adults after cerebral infarction(14 male, 5 female; mean age, 69y) for our study. Electrical stimulation was used for participants with electrical stimulator connected to two pair of electrodes positioned on the neck for one hour a day and 5 times a week for 4 weeks. Maximum tolerated stimulation levels were applied at rest while participants held their mouth closed. Videofluoroscopic recordings, physical examination of swallowing, and swallow function score were used to evaluate swallowing state of participants. All evaluate items were evaluated before and after treatment. Results : There was no significance difference of swallowing stage. The swallow function score significantly increased from 1.8 to 5.3(p<0.001). Also, cough reflex(p<0.001), laryngeal excursion(p<0.01) were significantly reduced compared to before electrical stimulation. Conclusion : Therefore, electrical stimulation for dysphasia is proved effective as it activates the function of swallowing muscle.

  • Functional Electrical Stimulation is the clinical application of a small electric current to the intact nerves of the body, in order to trigger a muscle contraction. This contraction is then incorporated into a functional activity, for example walling. A clear distinction needs to be made between therapeutic stimulation and functional stimulation. The former being of an exercise orientation, where one would relax while the stimulation works on its own. Functional electrical stimulation on the other hand, incorporates this elicited muscle movement into an everyday activity, like standing, walking, reaching out etc. But recently, New method used to FES which updated electrode insult in muscle and small nerve branch. A lot of new research build up many countries such as England, USA, Japan, Spain, Canada. So I had been write this paper for introduce new FES method, and I hope to more enhanced motivation of therapist for the functional electric stimulation.

The Effect of an Abdominal Drawing-In Maneuver Combined with Low·High Frequency Neuromuscular Electrical Stimulation on Trunk Muscle Activity, Muscle Fatigue, and Balance in Stroke Patients

  • Purpose: This study investigated the effects of an intervention that combined the abdominal drawing-in maneuver and frequency-specific neuromuscular electrical stimulation on changes in trunk muscle activity, muscle fatigue, and balance in stroke patients. Methods: Thirty stroke patients were randomly assigned to two groups. Fifteen subjects were assigned to group I which performed the abdominal drawing-in maneuver combined with low-frequency neuromuscular electrical stimulation and the other 15 subjects to group II where the abdominal drawing-in maneuver was combined with high-frequency neuromuscular electrical stimulation. Muscle activity and fatigue were measured using surface electromyography before the intervention. Balance was measured using the Trunk Impairment Scale and re-measured after six weeks of intervention for comparative analysis. Results: Both groups showed a significant increase in muscle activity and balance (p<0.05), and there was no significant difference between the groups (p>0.05). In the changes in muscle fatigue, only the experimental group II showed a significant increase in muscle fatigue (p<0.05). The difference between the groups was statistically significant (p<0.05). Conclusion: It was confirmed that among stroke patients, the combination of the abdominal drawing-in maneuver and low-frequency neuromuscular electrical stimulation was more effective in changing the muscle activity and balance of the trunk by minimizing the occurrence of muscle fatigue compared to the combination of the abdominal drawing-in maneuver and high-frequency stimulation. These results can be used as basic data for clinical trunk stabilization training.

  • The purpose of this study was to demonstrate the effects of silver spike point (SSP) low frequency electrical stimulation on plasma ${\beta}-endorphin$ activities measured by radio- immunoassay from normal volunteer and the effects of ${\beta}-endorphin$ on 5-hydroxytryptamine (5-HT, serotonin)-induced contraction investigated by isometric tension methode in rats. The current of 3 Hz continue type, but not 100 Hz continue type, of SSP low frequency electrical stimulation significantly increased in plasma ${\beta}-endorphin$ from normal volunteer. The endothelial cell-dependent 5-HT-induced contractions were inhibited by ${\beta}-endorphin$ $1{\mu}M$. These results suggest that the ${\beta}-endorphin$ regulates nociceptive-like substance, such as 5-HT, in part and that the SSP low frequency electrical stimulation, specifically current of low frequency of 3 Hz continue type, significantly increases plasma ${\beta}-endorphin$ from normal volunteer.
