경사로 횡단 보행 시 경사로 각도에 따른 안·바깥쪽 장딴지근의 활성 비율 변화 (Changes in the Ratio of Medial to Lateral Gastrocnemius Muscle Activities According to Mediolateral Ramp Angles during Cross Ramp Walking)

  • Purpose: This study was conducted to predict the risks that arise while standing on mediolateral ramps at various ramp angles by identifying the ratio of medial to lateral gastrocnemius muscle activities. Methods: The subjects were 20 healthy adult men. Seven mediolateral ramp angles ($0^{\circ}$, $2^{\circ}$, $5^{\circ}$, $10^{\circ}$, $15^{\circ}$, $20^{\circ}$, and $25^{\circ}$) were applied for the experiment. The ratio of medial to lateral gastrocnemius muscle activities in each condition was measured using electromyography, and the measured data were converted to root mean square values to calculate the activity ratios. Results: The study results showed statistically significant differences in the ratio of mediolateral gastrocnemius muscle activities according to the selected mediolateral ramp angles. The results of a post hoc test confirmed that the multifidus muscles were asymmetrically used on the two sides at mediolateral ramp angles of $5^{\circ}$ or higher. Conclusion: This study found that an asymmetric use of the multifidus muscles began at a mediolateral ramp angle of $5^{\circ}$, and the provision of propulsion using the ankle joints on mediolateral ramps at $5^{\circ}$ or steeper was maneuvered differently from that on flat ground. This suggests that individuals with balance control disorder have the risk of falling due to ankle sprains and unstable balance control. Therefore, patients and elderly people are required to exercise caution when crossing $5^{\circ}$ or steeper ramps.

  • Purpose: The purpose of this study was to examine exercise program and outcome variables of exercise intervention studies from 1985 to 2004 in 9 major nursing journals in Korea. Method: From 81 articles, publishing year, research design, subjects, exercise program and outcome variables were analyzed. Results: The 57 papers(70.4%) were experimental research among 81 papers. Subjects of exercise intervention studies were demonstrated that patients were 55.6% while healthy person was 44.4%. Exercise type by the subjects which demonstrated the highest proportion was dance movement for the elderly, walking for the middle aged women and aqua exercise for the arthritis. The 40 papers(49.4%) included exercise duration, frequency and time for the exercise program which demonstrated the highest proportion. Outcome variables to determine the effect of exercise intervention were demonstrated to be physical function, physiological index, psychological and emotional variables, cardiopulmonary function, body composition, physical symptoms, variables related to exercise and behavior in order. Conclusion: Half of the exercise intervention papers included exercise duration, frequency, time and intensity for the exercise program. Frequently used outcome variables to determine the effect of exercise intervention were demonstrated to be physical function, physiological index and psychological emotional variables.

아웃도어 웨어 기능성 요구에 따른 스마트 아웃도어 재킷 설계시안 (Draft Proposal of Smart Outdoor Wear upon the Outdoor Wear Functionality Demand)

  • This study revealed the results related to the functionality of outdoor wear required when man and women in their 30s to 50s engage in outdoor activities. Based on the results of the survey, we proposed composition draft for a smart outdoor jacket with main functions of GPS device and light-emitting device using solar cell and EL. Absorption and release of sweat, functionality regarding rain and wind, and lightweightness, etc. from material functionalities of outdoor wear were found to be important. From function required in clothing for outdoor activity, location tracking, night glow, and lighting functions were found to be most important. For results investigating the necessity of the jacket's location tracking function and lighting function using solar cell, high scores of 3.9~4.0 were given. Purchase intentions for smart outdoor jacket with location tracking and lighting functions devised by this study were fairly positive and most responses indicated that the appropriate purchase price was between 200,000 to 300,000 won while possible problems of this smart outdoor jacket were listed in the order of washing inconvenience, high price, device weight, and discomfort in movement. The draft proposal to integrate with wearable devices for smart outdoor jacket prototype is as follows: Solar cell has been attached to the upper arm but placed inside a transparent pocket which has been detachable for washing convenience while the solar cell and controller have been integrated into a single unit. Using frequent movement exhibited by the arms, EL has been attached along the center line of the raglan sleeve for easy spotting when used as an emergency signal or for night lighting function during outdoor activity. GPS has been attached on the left sleeve so that the person can bend the left arm inward and operate the GPS screen with the right hand while walking or running outdoors.

파워 저항운동이 여성노인의 체력, 근육량 및 단기운동수행력에 미치는 영향 (Effect of power resistance intervention on fitness and muscle mass and short physical performance battery in older women adults)

  • 이 연구의 목적은 파워 저항운동이 여성노인의 체력, 근육량 및 단기운동수행력에 미치는 영향을 보고자 하였다. 이 연구에 참여한 피검자는 70세 이상의 천안시 거주 여성노인 30명을 파워 저항운동군 15명과 일반 저항운동군 15명으로 분류하였다. 파워 저항운동군은 속도를 빠르게 하는 저항운동을 일반 저항운동군은 평소속도의 운동을 주 3회 60분간 실시하였다. 연구결과 근력은 두 저항운동군에서 유의한 개선을 보였고, 전신지구력에서는 파워 저항운동군에서 유의한 개선을 보였다. 단기운동수행력 중 2.44m왕복걷기는 파워 저항운동군에서, 400m걷기에서는 두 저항운동군에서 유의한 개선을 보이는 것으로 나타났다. 결론적으로 체력과 근량 및 단기운동수행력에서 두 운동방법의 효과는 충분하나 파워저항운동이 다소 우세한 것으로 나타났다.

공간분석을 이용한 심뇌혈관질환 사망률에 영향을 미치는 지역요인 분석 (A Study on the Regional Factors Affecting the Death Rates of Cardio-Cerebrovascular Disease Using the Spatial Analysis)

  • Background: The purpose of this study was to analyze the relationship between the regional characteristics and the age-adjusted cardio-cerebrovascular disease mortality rates (SCDMR) in 229 si·gun·gu administrative regions. Methods: SCDMR of man and woman was used as a dependent variable using the statistical data of death cause in 2017. As a representative index of regional characteristics, health behavior factors, socio-demographic and economic factors, physical environment factors, and health care factors were selected as independent variables. Ordinary least square (OLS) regression and geographically weighted regression (GWR) were performed to identify their relationship. Results: OLS analysis showed significant factors affecting the mortality rates of cardio-cerebrovascular disease as follows: high-risk drinking rates, the ratio of elderly living alone, financial independence, and walking practice rates. GWR analysis showed that the regression coefficients were varied by regions and the influence directions of the independent variables on the dependent variable were mixed. GWR showed higher adjusted R2 and Akaike information criterion values than those of OLS. Conclusion: If there is a spatial heterogeneity problem as Korea, it is appropriate to use the GWR model to estimate the influence of regional characteristics. Therefore, results using the GWR model suggest that it needs to establish customized health policies and projects for each region considering the socio-economic characteristics of each region.

외측 쐐기 깔창이 골관절염 환자의 내반슬에 미치는 영향에 관한 고찰 (A Review of Effects of Osteoarthritic Patient with a Varus Deformity of the Knee on Laterally Wedged Insole)

  • Osteoarthritis has been considered a disease of the elderly because it is uncommon before the age of 40 years and is seen in approximately 80% of United States citizens older than 65 years. general population on kuri city in korea revealed that prevalence of knee osteoarthritis is 10.2%, increasing with age. High level of physical activity in men and age, post-menopause and obesity in women can be risk factor. Osteoarthritis is no evidence that a acquired process initiated much earlier in life through mechanical, metabolic, genetic, or other origins. A high tibial osteotomy alters static lower extremity alignment thereby decreasing medial compartment loading. As well, conservative treatment strategies, such as knee braces and valgus heel wedges, affect lover limb mechanics and attempt to reduce medial compartment loading. It was hypothesized that valgus heel wedges and modified orthoses would shift the center of pressure laterally on the foot during level walking, reducing the moment arm of the adduction moment in the frontal plane, thereby resulting in a decrease in the knee adduction moment. In the 1980s, the effect of wearing a laterally wedged insole on osteoarthritic patients with a varus deformity of the knee was firsted, and since then, kinematic and kinetic analyses concerning this condition have mainly focused on a static standing position. Since the early 1990s, the beneficial effect of wearing a laterally wedged insole to treat osteoarthritis of the knee has also been reported in dynamic conditions, but these studies did not answer the question of the kinematic and kinetic mechanisms that resulted in the reduced symptoms in patents with knee osteoarthritis. therefore, the effect of wearing laterally wedged insole has not been sufficiently studied.

족배굴곡 보조용 외골격 보조기가 보행자의 보행패턴 및 하지근육에 미치는 효과 (Effect of Exoskeleton Orthosis for Assistance of Dorsiflexion Torque in Walking Pattern and Lower-limb Muscle)

  • 본 연구에서는 하지마비 장애인의 족하수 방지를 위하여 족배굴곡 보조를 위한 외골격 보조기를 개발하였다. 본 보조기는 인공 근육형 공압 액추에이터를 이용한 동력 전달부, 동력부의 고정을 위한 무릎 관절 착용부 및 보행 신호 검출을 위한 발목 관절 착용부로 구성되었다. 보조기는 무릎 관절 착용부와 발목 관절 착용부에 고정되어 족배굴곡 토크를 발생시켰으며, 발바닥 압력센서를 통해 사용자의 보행 단계를 검출하고, 족배굴곡 보조 타이밍을 감지하도록 하였다. 보행에 이상이 없는 건강한 65세 이상 고령자 7명과 20대 성인 10명을 대상으로 외골격 보조기의 족배굴곡 보조에 대한 성능을 확인한 결과, 인공 근육형 공압 액추에이터의 보조 시 전경골근의 근활성이 감소하는 결과가 나타났다. 향후 연구에서는 하지마비 환자를 대상으로 외골격 보조기의 효과를 검증할 것이다.

The Interlimb Coordination During Movement Initiation From a Quiet Stance: Manipulation of Swing Limb Kinetics and Kinematics -A Preliminary Study

  • The purpose of the current experiment was to describe interlimb coordination when swing limb conditions are being manipulated by constraining step length or by adding a 5 or 10 pound weight to the swing limb distally. Subjects were asked to begin walking with the right limb to land on the primary target (normal step length) that is 10 cm in diameter. However, if, during movement, the light was illuminated, then the subject had to step on one of the secondary targets (long and short step length). These three step length conditions were repeated while wearing a 5 pound ankle weight and then when wearing a 10 pound ankle weight. Ground reaction force (GRF) data indicated that there were changes in the forces and slopes of the swing and stance Fx GRFs. Long stepping subjects had to increase the propulsive force required to increase step length. Consequently, swing and stance toe-off greatly increased in the long step length condition. Short step length subjects had to adequately adjust step length, which decreased the speed of gait initiation. Loading the swing limb decreased the force and slope of the swing limb. Swing and stance toe-off was longest for the long step length condition, but there was a small difference of temporal events between no weight and weight condition. It appears that subjects modulated GRFs and temporal events differently to achieve the peak acceleration force of the swing and stance limb in response to different tasks. The findings from the current study provide preliminary data, which can be used to further investigate how we modulate forces during voluntary movement from a quiet stance. This information may be important if we are to use this or a similar task to evaluate gait patterns of the elderly and patient populations.

저강도 타이치 운동이 시설거주노인의 체력, 골밀도 및 골절위험도에 미치는 효과 (Effects of Tai Chi Exercise on Physical Fitness, Bone Mineral Density, and Fracture Risk in Institutionalized Elderly)

  • Purpose: The purpose of this study was to examine the effects of Tai Chi exercise on flexibility, balance, walking ability, muscle strength, bone mineral density, and fracture risk in institutionalized elders. Method: A quasi-experimental pretest-posttest design with a nonequivalent comparison group was utilized, and 53 older adults living in one institution were recruited and assigned to one of two groups, experimental group (18) or comparison (20). Both groups completed posttest measures at 6 months. There was a 31% rate of dropouts. Tai Chi exercise was provided twice a week for 24 weeks. Outcome measurements were conducted by a physiotherapist at a university hospital health promotion center who did not know the group assignment. Results: At 6 months, the experimental group had significantly greater grip strength(t=2.12, p=.04), back muscle strength (t=2.42, p=.02), balance (t=5.31, p<001), and flexibility (t=3.57, p<.001). They also showed significantly greater bone mineral density of lumbar spine and femur, and reduced fracture risk. Conclusion: Tai Chi exercise was safely and effectively used with institutionalized elders for 6 months and significantly improved physical fitness, bone mineral density along with a reduction in fracture risk. Whether Tai Chi exercise would lead to prevention of fall episodes and fall related fractures in this population will require further study.

시니어전용 기능성 깔창의 체계적인 개발 및 효과분석 (Systematic Development and Effect Estimation of the Functional Insoles for Seniors)

  • Korea has already entered into an aging society. In recent decades, the health problems for seniors has received considerable interest. We often see the elderly who have been struggled from discomfort or illness of the foot. Those foot related problems mainly cause from the use of improper shoes. Recently, shoe makers sell the shoes for seniors, so called comfort shoes, but the shoes are too expensive for seniors to buy easily. In this paper, we develop cheap insoles for seniors as an alternative of the comfort shoes and suggest the systematic process for the insoles development. This systematic process is as follows: 1) Survey the literature about the market of insole, 2) Investigate the standard size of body and foot for the seniors in Korea, 3) Analyze the customer needs by survey, 4) Study the walking pattern by experiments, and 5) Develop the novel insole for the seniors to relieve the inconveniences related with foot. From the newly developed insoles, the company B gains the sales increasing effect approximately 30% over through the increase in consumer satisfaction and company reputation and secures for intellectual property rights. Using the database from Korea Research Institute of Standards and Science, the reliability of developing technology of the functional insole has been obtained. The seniors are also enable to choose an alternative of comfort shoes for foot health. In the future, the insoles developed from this study have wide applications in the medical, cosmetic fields, and leisure sports fields. Accordingly, it seems to require more systematic studies utilizing Walk Analyzer and Foot Pressure Meter.
