This study was carried out to assess the food habit index of the long-lived elderly who were aged over 85 years living in Ganghwa-gun. A survey was conducted during December 2003. Dietary nutrient intake data was obtained through the 24-hr recall method. The subjects were 96 (32 males, 64 females) aged people and divided into three groups based on food habit index scores. Such as group A: good $16\∼20$, group B: fair $11\∼15$, and group C: poor $5\∼10$. The three groups of the subjects were 90.7 years of age and $21.3\;kg/m^{2}$ of BMI on average. Healthy eating index scores of the subjects were $9.4\%$ in group A, $54.2\%$ in group B and $36.5\%$ in group C. Their level of education were lower and their levels of living standards tended to have been middle and lower of the middle class. The percentage of living together with their families or spouses in all groups were over $60.0\%$ and the rates of the subjects who have responsibility for their meal preparations were also very high in all groups. Most subjects tended not to drink nor smoke, and spend $8\~10$ hours for their sleep. The percent of number of diseases in group A was lower than in groups B and C. Their dietary habits such as taking three meals a day regularly have shown that they have good eating habit in general. There was a positive correlation between the eating behaviors and nutrient intakes. The protein, animal protein, fat, PUFA, vitamin E, vitamin $B_{2}$, niacin, P and K intaks in group A were significantly higher than that of the groups B and C. For group A of good food habits aged had no sufficient intake of Ca and vitamin A than the Korean RDA and long-lived elderly of group C in Ganghwa areas had worse nutrients intakes, especially Ca, vitamin A, vitamin $B_{2}$ and vitamin E. 본 연구는 장기요양시설 노인의 삶의 질에 관련된 요인을 알아보기 위하여 조사대상자의 인구사회학적 특성, 건강관련행위특성, 구강건강영향지수(OHIP-14), 신체적 기능(ADL, IADL) 및 정신적 기능(CES-D, MMSE-K)과 삶의 질(WHOQOL-BREF)과의 관련성을 분석하였다. 조사대상은 장기요양인정자로 판정받은 장기요양시설급여노인 602명으로 하였으며, 2016년 5월 1일부터 6월 30일까지의 기간 동안에 구조화된 설문지를 이용한 직접 면접조사와 구강검사를 실시하였다. 연구결과, 조사대상자의 삶의 질은 남성보다 여성에서, 생활비조달을 본인이 하는 경우보다 자녀가 조달하거나 정부의 보조를 받는다는 군에서, 요양기간이 길수록, 비음주군보다 음주군에서, 규칙적인 운동을 한다는 군보다 하지 않는다는 군에서, 식사를 규칙적으로 한다는 군보다 하지 않는다는 군에서, 주관적인 건강상태가 건강하다는 군보다 건강하지 않다는 군에서, 1일 칫솔질 횟수가 적을수록, 구강건조증이 없다는 군보다 있다는 군에서, 구강건강영향지수(OHIP-14)가 낮을수록, 신체적 기능(ADL, IADL)이 낮을수록, 정신적 기능(CES-D, MMSE-K)이 낮을수록 유의하게 낮았다. 특히 삶의 질은 다른 요인보다 건강관련행위특성과 정신적 기능(CES-D, MMSE-K)에 의해 더 큰 영향을 받고 있는 것으로 나타났다. 따라서 장기요양시설 노인의 삶의 질을 증진시키기 위해서는 바람직한 건강관련행위를 비롯하여 우울 및 인지기능장해를 예방하기 위한 노력이 필요할 것으로 생각된다.
사람이 서있는 동안의 안정성 감소는 낙상에 영향을 끼치는 요인 중 하나이다. 따라서 본 논문은 낙상 발생률이 높은 65세 이상의 노인여성을 대상으로 균형기능 검사를 실시하고, 성인여성의 연령대별 비교를 통해 균형능력이 노화에 따라 변화해가는 정형외과 및 신경학적 요인들을 밝혀 낙상을 예방하고자 한다. 연구 대상자는 보조기나 약물복용 없이 독립보행이 가능한 65세 이상의 신체 건강한 노인여성 10명(평균연령: 71.9세)과 성인여성 10명(평균연령: 23.2세)으로 하였다. 균형능력 평가장치(Tetrax)를 이용하여 8가지 자세에 대한 안정성 지수(Stability Index, ST), 푸리에 지수(Fourier index, F), 체중분표(Weight Distribution (%), WD), 체중분포율 지수(Weight Distribution Index, WDI), 동기화 지수(Synchronization Index, SI), 낙상율지수(fall index, FI)를 측정하였다. 그 결과, 안정성 지수(ST)는 노인여성과 성인여성간 PO를 제외한 모든 자세에서 통계적으로 유의한 차이를 나타냈다(p<.01) 푸리에지수는 낮은주파수(low, F1)에서 PO를 제외한 모든 자세에서 노인여성과 성인여성은 유의한 차이를 나타냈다(p<.01). 또한 중간낮은 주파수(middle-low, F2-F4), 중간높은주파수(middle-high, F5-F6), 높은주파수(high, F7-F8) 모두에서 노인여성이 성인여성보다 통계적으로 유의하게 높은 수치를 나타냈다. 모든 자세에서 노인여성의 체중분포율지수가 더 높게 나타났으나, PO와 PC에서만 통계적으로 유의했다. 이는 노화에 따른 생리학적 변화로 체성감각기관, 전정기관, 중추신경계등 전반적인 부분에서 능력이 저하됨에 따라 나타난 것으로 사료된다.
Background: The purpose of this study is to determine how the cognitive state of the elderly influences the activities of daily living(ADL) in order to increase their achievement capacity and their ability to exercise this capacity in daily life. The MMSE-K(Mini-Mental State Examination-Korean) was used to evaluate the daily activities and cognitive state of a group of subjects more than 65 years of age. The goal was to understand (1) How their achievement level in their daily activities changes according to the increase and decrease of their cognition points by analyzing and comparing their achievement points in the mental state examination, and (2) What variables influence that daily achievement level. Methods: The survey was conducted by a group of students from AnSan College who were trained for this method of data collection. The group surveyed 224 senior citizens older than 65 to determine their ADL performance. The subjects were selected from Silver Care Centers in the Gyounggi Province area including centers in Suwon, Anyang, Kwachun, and Sanbon City. The subjects were given enough information to fully understand the purpose of the study and the method by which it would be conducted. The survey method was a direct interview: which involved an interviewer reading the questionnaire to the subject and recording the answer. Result: The following results were calculated using SPSS WIN ver. 13.0: In the lower score of the MMSE-K regarding ADL performance (K-MBI) for activities such as individual hygiene, bathing activity, food activity, toilet management, going up stairs, getting dressed ad undressed, controlling stools and urination, walking, and chair/bed movement, most of the subjects responded that they could not perform these activities by themselves (p<.01). In the higher scores for the same activities, most response that they could perform the tasks by themselves (p<.01). In the results of the multiple regression analysis, there influence of the variables for ADL performance (K-MBI) and MMSE-K score was higher for females than males. Conclusion: This study evaluated the cognitive state of the elderly using the MMSE-K, which is the most convenient method, and examined how the cognitive state influences the achievement capacity of the daily lives with other variables. In conclusion, the higher the score on the MMSE-K, the more independent the elders are in the activities of daily living, especially among women more than men.

Background: The purpose of this study was to evaluate the long-term survival rates of very elderly (age ≥80) critically ill patients admitted to a medical intensive care unit (MICU) at a regional tertiary-care hospital in Korea. Methods: We retrospectively analyzed data from patients who survived after discharged from the MICU of our hospital. Survival rates at 90 days, 1 year, 2 years, and 3 years were assessed between patients age ≥80 and those age <80. Survival status was evaluated using the National Health Insurance Service data. Results: A total of 468 patients were admitted, 286 (179 males, 97 females; mean age, 70.18±13.2) of whom survived and were discharged soon after their treatment. Among these patients, 69 (24.1%) were age ≥80 and 217 (75.9%) were age <80. The 90-day, 1-year, 2-year, and 3-year survival rates of patients age ≥80 were significantly lower than those in patients age <80 (50.7%, 31.9%, 15.9% and 14.5% vs. 68.3%, 54.4%, 45.6%, and 40.1%, respectively) (p<0.01). The Kaplan-Meier survival curves showed significantly lower survival rates in patients age ≥80 than in those age <80 (p=0.001). Conclusion: The poor rates of long-term survival in very elderly (age ≥80) and critically ill patients admitted to an ICU should be considered while managing and treating them.
Gallbladder carcinoma (GBC) is the commonest cancer of the biliary tree and the most frequent cause of death from biliary malignancies. The incidence of GBC shows prominent geographic, age, race, and gender-related differences and is 4-7 times higher in patients with gallstones. This prompted us to study the clinicopathological aspects of the disease and the incidence of gallstones in gallbladder carcinoma patients, in this part of India. In this, combined retrospective (Jan 2004-March 2010) and prospective study (April 2010-Dec 2011) of eight years, 198 patients of gallbladder carcinoma (50 males and 148 females), (range 28-82 years; mean 55 years) were studied. Most of the patients were poor and presented with abdominal pain and mass, with abnormal lab parameters. Gallstones were present in 86% of patients. Surgical exploration was performed in 130, with gallbladder resection in 60 (including 7 incidental GBC). Adenocarcinoma (87.7%) was the commonest histological type. The study indicates that GBC is common in our scenario. It is a disease of elderly females, has a strong association with gallstones and every cholecystectomy specimen should be examined histopathologically.
The nutrients intake and health-related habits of nonagenarian subjects in Korean representative longevity belts of Damyang, Gokseong, Kurye, Sunchang were evaluated for the purpose of providing the information on the desirable food selection and dietary pattern of elderly population. A survey was conducted with 91 subjects (26 males and 65 females) and their food intakes were measured by the combination of one meal weighing and 24-hr recall, and their general background information was collected by visit. The mean age of the subjects was 93.6 $\pm$ 2.2 (male) and 97.6 $\pm$ 4.6 (female) The average smoking rate was $20.9\%$, the rate of regular drinking was $26.4\%$. The average energy intake was 1,284.9 kcal comprising $77.1\%$ of RDA for elder people over 75. The energy ratio of carbohydrate, protein and fat were 66.5 : 18.2 : 15.3. The average intake of protein, calcium, iron and zinc were $107.4\%,\;59.9\%,\;106.3\%$ and $60\%$ of RDA respectively. The lower intake of vitamins as low as $70\%$ was found except vitamin $B_6$ and niacin. The majority of the subjects consumed rice as staple diet and mostly consumed white plain rice rather than mixed grain rice. The animal and plant food intakes were 88.4 : 11.6 in males and those of females were 83.0 : 17.0 showing a tendency of plant-based meals. Nutrients that showed NAR over 0.7 were protein, iron, vitamin $B_6$, niacin and phosphate, but those with INQ over 1. Nutrient with INQ below 0.7 was only vitamin E. Even though their nutritional quantity did not appear to be enough, but their nutritional quality was relatively high. The adding dairy products, nuts and fruits for the purpose of proving sufficient vitamin and minerals can achieve the optimal nutritional intake patterns. Further research on RDA of this age population should be followed.
Objectives: While there have been many quantitative studies on the public's attitude towards mental illnesses, it is hard to find quantitative study which focused on the contextual effect on the public's attitude. The purpose of this study was to identify factors that affect the public's beliefs and attitudes including contextual effects. Methods: We analyzed survey on the public's beliefs and attitudes towards mental illness in Korea with multi-level analysis. We analyzed the public's beliefs and attitudes in terms of prejudice as an intermediate outcome and social distance as a final outcome. Then, we focused on the associations of factors, which were individual and regional socioeconomic factors, familiarity, and knowledge based on the comparison of the intermediate and final outcomes. Results: Prejudice was not explained by regional variables but was only correlated with individual factors. Prejudice increased with age and decreased by high education level. However, social distance controlling for prejudice increased in females, in people with a high education level, and in regions with a high education level and a high proportion of the old. Therefore, social distance without controlling for prejudice increased in females, in the elderly, in highly educated people, and in regions with a high education and aged community. Conclusions: The result of the multi-level analysis for the regional variables suggests that social distance for mental illness are not only determined by individual factors but also influenced by the surroundings so that it could be tackled sufficiently with appropriate considering of the relevant regional context with individual characteristics.
Aim: An epidemiological shift has resulted in increase in the prevalence of non-communicable diseases (NCD). Unlike other NCDs which are easily and definitely preventable, the knowledge of cancer prevention is still limited at present. Various aetiological factors are difficult to control since those are habit forming. Hence an available remedy remains its secondary and tertiary prevention for which appropriate planning is of paramount importance. Evidence based planning requires careful analysis of data with a view to prioritize various cancers. Keeping in view the fact that the adaptation of smoking free status in Chandigarh city might have a far reaching positive effect on the cancer related morbidity of the people, the following study was undertaken to provide base line data to be used for future comparisons. Methods: The registers maintained in the Department of Radiotherapy were checked and those belonging to the years 1999 to 2009 were utilized to analyze the cancer morbidity in respect to age, sex, and year of presentation to health care facility. Results: A total of 4,600 cancer patients (males=2276, females=2324) demonstrated a gradual increase in the number of cancer cases from 150 in the year 1999 to 783 in the year 2009. The most common cancers amongst males were cancer of gastro-intestinal tract (GIT) and lung (including larynx) constituting 37.3% and 27.1% of the total, respectively. In females these were cancers of breast and cervix representing 33.3% & 17.6% of total cancer cases, respectively, and lung cancer constituted 5.3%. The maximum cases of bone cancer (53.8% of all bone cancers) were observed amongst children aged less than 20 years and lung cancer (48.2% of all lung cancers) among the elderly aged 60-69 years. Objectives: This study aimed to determine the nutritional status of residents, to examine the perception and needs of community nutrition care service, and to compare and analyze the results of females and males living in Daejeon, Korea. Methods: The subjects were 592 adults and elderly (278 men and 314 women) residents of Daejeon. The dietary quality was assessed using the nutrition quotient (NQ), and data of the perception and needs for community nutrition care service were obtained by a questionnaire survey. Descriptive analysis was applied to analyze the demographic characteristics, perception and needs of the respondents, and independent t-test and chi-square test were performed to analyze and compare the differences between female and male participants. Results: A nutritional status that required monitoring was established in 78.9% of the respondents in this study. The balance factor (29.1 points) was much lower than the national average. In the diversity factor, the NQ score of the female group was 47.6 points, which was 8 points lower than that of males; the NQ grade was 43.9%, showing a significant difference between females and males (P < 0.001). The respondents answered that those who needed community nutrition care services were residents experiencing health care difficulties. Additionally, dietitians (35.6%) were pointed out as the most suitable service providers. The most important factor of community nutrition care service operation was securing the budget (47.0%). Conclusions: The results of the present study show that Daejeon residents need overall nutrition support, and a demand for community nutrition care services exists. Our findings provide basic data and can serve as a foundation for the development and establishment of a community nutrition care service model.
