보건소 내원 노인들의 영양상태 및 보건소 이용에 관한 연구 (Nutritional Status and Utilization of Public Health Center of Elderly)

  • This study was performed to assess the nutritional status, frequency of visiting and necessity of nutrition programs for 157 elderly(male:49, female:108) visiting public health centers in Puchon. The purpose of the study was to provide the basic data for developing a nutrition service model. The subject were investigated by interviews with a questionnaire to obtain dietary data and other information related to public health center. Blood tests for analyzing biochemical status were also carried out. The elderly showed low income status. Ninety two percent of them showed their monthly income was less than 400,000 won and 72.6% was observed as having 30,000 won/month as their pocket money. The most frequent disease reported as having or being treated were hypertension(32.6%), rheumatic arthritis(28.5%), diabetes(10.2%), and stomach disease(8.2%) for males and hypertension(33.1%), diabetes(19.4%), rheumatic arthritis(16.7%), anemia(11.1%) for females. The nutrients whose daily intakes were less than 2/3 of RDA were calcium(37.5% RDA),vitamin A(49.9% RDA), iron(60.0% of RDA) and protein(62.0% RDA) for males and vitamin A(27.7% RDA), vitamin B$_2$(33.3% RDA), calcium(44.1% RDA), iron(53.3% RDA), and niacin(60.0% RDA) for females, respectively. Prevalence of anemia, assessed by hemogloben using WHO definition, were 4.1% for males and 18.5% for females. The percentage of males with hypercholesterolemia( 220 mg/dl) was 2.1% and 19.4% fir females, Two percent of males and 12.0% of females were observed as having a LDL-C higher than 165 mg/dl. The mean fasting blood glucose(FBG) level of males and female was 84.2 mg/dl and 101.7 mg/dl respectively. Two percent of males and 8.3% of females were found with a FBG higher than 140 mg/dl. Seventy one percent of elderly reported they were visiting public health centers at least once per week or more frequently. They were satisfied most with the low medical bills but showed the lowest satisfaction for the facilities of the public health centers. What the nutrition service programs wanted most was nutrition counseling and guidance.

  • The purposes of this study were to investigate the effects of drug use & the health-related habits(alcohol drinking, smoking and exercise) on the nutritional status of elderly. subjects were 362 free-living elderly aged over 65(male 131, female 231) in Chungbuk area. Interviews with questionaire on drug use, health-related habits, and nutrient intakes, anthropometric measurement and biochemical blood test were carried out from August to October in 1996. Energy intakes of the males who used drug were lower than those of males who did not use any drug, while energy intakes of the females who used drug were higher than those of females who did not. In biochemical indices, LDL-cholesterol level of the males who used drug was higher than that of males who did not have drug. serum triglycerides, VLDL-cholesterol and obesity degree(BMI & TS) of the females who used drug were higher than those of females who did not have drug. Alcohol drinking, smoking and exercise habits of the elderly who used drug had little effects on the nutrient intakes while those habits had considerable effects on biochemical indices and obesity degree. Among drug users, male drinkers showed higher triglycerides & HDL-cholesterol level, and female drinkers showed higher triglycerides and lower total cholesterol and LDL-cholesterol level, compared to non drinkers. Among female drug users, smokers showed lower body mass index(BMI) compared to non smokers. Males and females who had both drinking and smoking habits showed higher serum triglycerides and VLDL-cholesterol level. Especially females who had both drinking and smoking habits showed lower BMI & Tricep skinfold thickness compared to those who had netiher drinking nor smoking habits.

도시지역 노인의 성에 따른 건강행위 및 관련요인의 차이 (Gender differences in Health Behaviors and Related Factors of the Urban Elderly)

  • The purpose of the study is to examine gender differences in health behaviors and the related factors for the urban elderly population. This study utilized the data from the Suwon city health survey of the elderly in 2001. The data consisted of a random sample of 979 (388 males, 591 females) elderly people aged over 65. The results of the study were as follows. First, the elderly people's health behaviors were different by gender. Concerning health risk-taking behavior, the frequencies of smoking and drinking were higher in males than in females. In terms of health promotive behavior, the regular exercise rate was higher in males than in females. Second, there were different factors that influenced health behaviors by gender. The factors that influenced the health risk-taking behaviors were health status factors for male, and socioeconomic factors for female. The factors that significantly influenced the health promotive behaviors were social supports networks for both gender. Therefore, program priorities need to be modulated in accordance with these gender differences in health behaviors. Moreover, different program strategies are needed that reflect the gender differences in health behavior determinants.

노인들의 골밀도와 신체계측 및 생활습관과의 관련성 연구 (Association of Bone Densities with Anthropometric Indices and Lifestyles in Elderly People)

  • This study was performed to investigate the relationships among bone mineral densities (BMD), anthropometric data and lifestyle factors in the elderly. Subjects included 138 elderly (male: 38, female: 100) aged over 65 years, who were home-(dwelling in a low-income area of Puchon City. The BMDs of the lumbar spines (LS), femoral necks (FN), Ward′s triangles (WT) and trochanters (TC) were measured by dual energy X-ray absorptiometry. The females showed significantly lower BMDs in four sites (p < 0.0001) . The elderly aged over 75 revealed significantly more decreased femoral BMDs than the elderly aged 65 to 74. Female with BMIs of 20 to 25, showed significantly higher BMDs in LS, FN and trochanter than those with BMIs of less than 20. However, males displayed significantly higher BMDs in only LS with increasing BMIs. THe BMDs of LS correlated with weights (r = 0.543, p < 0.001), heights (r = 0.477, p < 0.001), upper arm circumferences (r=0.368, p < 0.01), waist circumferences (r=0.367, p < 0.001), subscapular skinfold thicknesses (r=0.363, p < 0.001) and flip circumferences (r=0.231, p < 0.01). Non-smokers and non-drinkers shelved significantly higher BMDs in trochanters only in the case of the males. Female milk-drinkers showed significantly elevated LS BMDs. Eighteen percent of the males were assessed as having osteoporosis, as compared to fifty percent of the females. Ninety-three percent of the females and 81.6% of the males responded that they often or always had "difficulty in standing for a long time".

  • This study was conducted to assess nutritional status of the elderly from low-income urban area and to investigate environmental and health risk factors that may influence to nutrient intakes. One hindered eighty three(males 53, females 130) healthy homestaying subjects aged over 65 years in Seoul were assessed with anthropometric measurements and dietary intake in August, 1994. HT and BW for females were decreased with aging. The proportion of hypertension was 36% with indite of $SBP{\;}(SBP{\;}{\gg}{\;}160mmHg)$ and 52% with indice of DBP $(DBP{\;}\gg{\;}97mmHg)$. Distribution of BMI, OR and PIBW indicated that BMI was more sensitive for identifying underweight whereas PIBW was more sensitive for identifying overweight. for group aged over 75, males had significantly lower intake of energy, protein, fat, calcium, iron, vitamin A and $\beta$ carotene compared to the group aged 67~74, while females had lower intake of calcium and vitamin A. Elderly group with smoking, less familly size and living alone had lower nutrient intakes. Blood pressure in the elderly subjects did not influence to nutrient intakes, except elderly with normal blood pressure were taking increased amount of thiamin and niacin.

여성 노인과 성인의 보행안정성 비교 (The Comparative Analysis of Gait Safety between Elderly Female and Adult Female)

  • The purpose of this study was to investigate the different patterns of the lower limb between elderly and adult females to reduce the rate of falls. Ten old females(age: $73.1{\pm}2.69yrs$, height: $151.9{\pm}4.82cm$, mass: $57.36{\pm}5.36$) and ten adult females(age: $28\pm}4.76$ yrs, height: $160.6{\pm}6.83cm$, mass: $53.9{\pm}8.44$) were participated in this experiment. The gait motions were captured with Qualisys system and variables were calculated with Visual-3D. The following results were found. The elderly female group showed bigger inclination angle between COM and COP than the adult female group so that the dynamic stability was reduced in walking for the compensation with a bigger stride width. The elderly female group ensure for the necessary forward movement of COM in order to replace the decreased function of ankle and knee joint. If the distance between COM and COP is closer and the energy reduction of a specific joint is reduced, they could prevent the elderly female's falling rate by strengthening of muscles which were related the extension of ankle joint.

The Effect of Milk Supplementation on Bone Density and Iron Status of Elderly

  • This study was performed to investigate how milk supplementation can benefit the elderly by increasing bone density but possibly harming the iron status concomitantly. Forty one elderly subjects over 65 years of age(male : 9, female : 32) participated. All subjects were apparently healthy, home staying and attending meal service for lunch at the welfare center. They were from low income area of Puchon city. One cup of milk per day was served for 10 months. The mean intake of calcium was significantly increased for females after milk supplementation. Males showed significantly increased means of triceps skinfold thickness, suprailiac skinfold thickness and waist circumference. Females showed significantly increased measurements fo three kinds of skinfold thickness, waist circumference, and hip circumference. There were no significant change in the mean bone density of lumbar spine$(L_2~L_4)$, femoral neck, ward's triangle and torchanter, but the proportion of osteopenia estimated by the T score of lumbar spine bone density was lowered from 50.0% to 34.6% for females. The mean Hb level was significantly for males. The proportion of anemia estimated by Hb(<12g/dl), Hct(<36%) and serum ferritin(<15mg/ml) were increased from 17.2% to 51.7%, from 20.7% to 44.8% and from 10.3% to 17.2%, respectively for females. It looks like milk supplementation can effect the intakes of several nutrients considered to be commonly deficient in the Korean diet fo elderly people, increase some anthropometric measurements, and decrease the proportion of osteopenia. However it can have adverse effects on iron status of females.

Prevalence of Cognitive Impairment and Related Factors Among the Elderly in Rural Communities of Jeju Province

  • Purpose. The purpose of this study was to survey the prevalence rate of cognitive impairments and to identify the factors influencing cognitive impairment in the elderly in rural communities of Jeju Province. Methods. 590 elderly in 6 rural communities of Jeju Province were interviewed, using a questionnaire consisting of sociodemographic characteristics, health behavior, quality of life, and MMSE-K Results. Prevalence of cognitive impairment was 33.1 % (39.1 % of females, 16.76% of males). Prevalence of dementia was 12.4% (16.3% of females, 2.87% of males). Factors related to cognitive impairment were age, sex, education, standard of living, employment status, and subjective health state. Conclusions. In community health care for the elderly, factors relating to cognitive impairment have to be considered. When planning community health care, priority should be given to the elderly; who need care but live alone; who lack social support; who have a low standard of living; who experience discomfort in the activities of daily living; who believe they are not in a good state of health; or whose life satisfaction is low.

노인의 혈청 삼투압과 건강관련 요인, 생화학적 변수들과의 상관성 (Serum Osmolality and Its Association with Health-Related Factors and Biochemical Parameters in the Elderly)

  • This study was designed to determine serum osmolality and to investigate the association among serum osmolality, health-related factors and biochemical indices. Two hundred thirty seven elderly(86 male, 151 female : mean age 73.8) residing in the chungbuk area participated. Sociodemographic data and self-perceived health status were obtained by interview, and biochemical parameters were measured. The mean serum osmolaity was 301.2$\pm$10.1 mOsm/kg(range 240.8~328.9 mOsm/kg) and serum osmolality of the females(302.4 mOsm/kg) was significantly(p<0.05) higher than that of the males(299.0 mOsm/kg). Only 11.0% of the elderly were within the accepted normal range(280~295 mOsm/kg) and the majority of the subjects were in a hyperosmolar state(60.8% : 296~307 mOsm/kg). It was observed that serum osmolality was reduced with increasing age but not statistically significant. There was no significant difference in serum osmolality according to marital status. Serum osmolality was significantly lower(p<0.05) in the elderly with higher self-perceived health scores than the elderly with lower self-perceived health scores, however no significant difference was observed between the presence or absence of disease. Serum osmolality tended to be higher(p=0.06) in the elderly with difficulty in mobility than the elderly who have no problem in mobility. Serum osmolality tended to be higher(p=0.06) among drug users compared to that of non-users. There was a significant association between serum osmolality and serum albumin in females but not in males. Significant associations were shown between serum osmolality and serum transferrin in both sexes. However, there was no significant correlation between serum osmolality and hemoglobin or hematocrit except with hematocrit in females only. The results of this study indicated that the elderly were dehydrated and hydration state seems to influence health status and, the elderly should be encouraged to drink an adequate amount of fluids. Also, the hydration state is required to be assessed and considered for the interpretation of biochemical parameters.

한국노인을 위한 반정량적 식품섭취빈도조사지의 신뢰도 검증 (Repeatability of a Semi-quantitative Food Frequency Questionnaire or the Korean Elderly)

  • In order to evaluate the repeatability of nutrient values estimated from a semiquantitative food frequency questionnaire(SFFQ) for elderly Korean, a sample of 100 participants(45 men, 55 women) were interviewed to complete a SFFQ developed by this study twice at an approximately 3 month interval. The mean age of subjects was 70.4 years. Males(mein=8.75 years) had more education thu females(1.4 years). In males, Nutrient values calculated from the repeat questionnaire were not significantly different from the baseline results by paired t test. Correlation coefficients of most nutrients ranged from 0.4-0.6. For most nutrients,60-73% males classified into the lowest tertile in baseline data belonged to the same tertile in a repeat questionnaire, but these values appeared to be less than 50% for females. These results were comparable to previously published results and confirm that repeatability of nutrient intakes estimated using SFFQs was acceptable. The repeatability of SFFQ was higher for males than females, which implies that years of schooling should be considered in the development of SFFQ for the elderly. Only f3r males, Mean Adequacy Ratio(MAR) of nutrient intake was higher for those who were less nutritionally at risk and less depressed, and physically functioned better. Considering the educational level of the Korean elderly overall, more studies on the development of simpler dietary assessment methods f3r this population are needed.(Korean J Nutrition 31(7) : 1183-1191, 1998)
