장기요양 서비스를 누가, 얼마나, 얼마에 원하고 있는가? - 장기요양 서비스의 욕구와 결정요인 및 지불의사금액 - (The Want, its Determinants and the Willingness to Pay of the Long Term Care Service)

  • Before introducing the national long-term care insurance in 2008, the want for long term care service has to be estimated and analysed. This study estimates the demand and analyses what determines the want of long term care service. This study investigated data of 3f6 elderlies, that was collected by age stratified random sampling. The elderies resided in Onyang 4 - dong (urban area) and Dogo-myun (rural area) In the city of Asan. The researchers visited the elderlies and their care giver, and assessed their demand for the long term care service and examined physical, mental, socio-economic status by the assessment tools for Korean Long-Term Care System. $64\%$ of the those who are entitled to be served refuse the long term care service. $26.7\%$ of them wants for home care service and $7.9\%$ want facility care service. It is estimated that the want of home care service are three or four times as much as that of facility care service. The demand for long term care service is 5.155 times higher for those who live in rural area (p=0.000), 3.040 times higher for those who do not have spouse(p=0.057), and 3.356 times higher for the people who is in medicaid than medical insurance(p=0.029). However, income(p=0.782), means(p=0.614), living alone(p=0.223), number of family to live with (p=0.341) and age of the elderly(p=0.420) are not related with the demand of long term care service. The assessment tools for Korean Long-Term Care System for need evaluation of the long term care service can reflect the demand well.(p=0.024) If medical care will cover $80\%$ of total cost, the willingness to pay of the out of pocket money of the people with medical insurance is 67,400 Korean Won(66.77 US$) for the home care service and 182,500 Korean Won(180.78 US$) for the facility care service. There is possibility that long term care demand is still small after Introducing the long term care Insurance due to the care given by family members. When developing service delivery system of long term care insurance, rural area has to be given more consideration than urban area because of the higher demand. The people who do not have spouse or are in medicaid have to be given special consideration as well.

한국 장기요양 방문간호의 정책적 함의와 일본 방문간호의 시사점 (Policy Implications for Home-Visit Nursing(HVN) of the Korean Long Term Care Insurance through the implications of the Japanese HVN)

  • Due to lack of an information system regarding the status of using home-visit nursing (HVN), it has barriers of providing improvement of the HVN for management of elderly health care in Korea. The twofold aims of the current review are to expose the existing agendas for HVN and to suggest the political implications for HVN of Korea based on the transition process and revised HVN system of Japan. This review suggests that an information evaluation system has to precede for HVN services in detail. And, the service provided per manpower should be assessed by separating the code of manpower (registered nurse, nurse aide, dental hygienist) as well as securing detailed and precise information on the HVN services. The other suggestion, development of a community-based home health care nursing model in order to provide necessary services for long-term health insurance beneficiaries. In addition, a master plan for health care for elderly should be established at the national level in order to establish an effective home health nursing delivery system.

한국과 일본의 노인장기요양보험에 관한 문헌고찰 (A literature study on long term care for the elderly in Korea and Japan)

  • Objectives: The purpose of this study is to look into the health insurance systems and application in Korea and Japan in order to improve elderly people's quality of life. Their quality of life can be improved by upgrading the long term care systems; and extending treatment and prevention. Methods: This study is to examine long term care systems articles through content analysis in the National Health Insurance Service. Data were collected from the printed Internet and analyzed. Results: A part-time dentist system in elderly care facilities has trouble in taking care of old people's oral health due to both lack of time and operational difficulties. It is urgent to arrange dental experts who can permanently stay in care facilities and to build systems which can be managed periodically and continually. Conclusions: As having staff training for the efficiency and using the manpower in care facilities (care workers), it is necessary to solve the unequal medical service in oral health care for the elderly in Korea.

  • 최근 노년기의 삶에 대비하기 위한 은퇴자금 마련이 중요한 개인적, 사회적 문제로 부각되고 있다. 특히, 앞으로 노년인구의 비율이 지속적으로 상승할 것이라는 전망과 더불어 이러한 개인의 재무설계 및 그와 관련한 리스크와 관련한 문제는 그 중요성이 날로 커질 것이다. 노년기의 질병에 따른 의료비 지출은 특히 재무적인 리스크와 밀접한 관련이 있는데, 유병 기간이 상대적으로 긴 질병의 경우에는 수발비용을 포함한 장기적인 의료비 지출로 인하여 재무적인 위험을 증가시키고 노년기의 삶의 질을 크게 떨어뜨릴 수 있다. 따라서, 각 개인이 장기적인 비용 지출을 요하는 질병에 대하여 예상되는 비용의 규모를 파악하고 이를 사전에 대비할 수 있는 방안을 모색하는 것이 필요하다. 본 연구에서는 노인장기요양보험의 실적 자료와 다중상태모형을 토대로, 노년기에 노인장기요양보험을 통하여 장기요양보호가 필요한 기간과 이에 따른 비용 규모의 추정을 통하여, 각 개인이 장기간병을 위해 준비해야 하는 필요금액을 도출하여 보았다.

  • Home health care system in Korea has been classified into three types of home care programs based on different laws and regulations; for example, home health care nursing(HHCN) is based on medical laws, visiting health care nursing (VHCN) is based on long-term health care insurance, and visiting health care(VHC) is based on the regional health care act. HHCN in Korea has taken on an important role under the mandate of the national health care system since 2000. VHCN will commence its role under the long term health care insurance system in 2008. The strengthening of VHC commanded health promotion and prevention for vulnerable families in the community in 2007. This is an important turning point for increasing quality management for home health care program; it suggests certain possibilities for building a foundation for further changes in the service delivery structure. Accordingly, the home health care policy makers in Korea have a major function and role that consists of developing an agenda and alternatives for policy making in a systematic manner and clearly presenting implementation strategies for elderly health care system.

노인장기요양보험제도에서의 방문물리치료 도입에 대한 노인요양시설 물리치료사의 인식조사연구 (Research for the Inclusion of Home-Based Physical Therapy in Long-Term Care Insurance System of Physical Therapists in Elderly Care Facilities)

  • 본 연구는 앞으로의 노인장기요양보험 서비스의 확대 시 우선적인으로 방문물리치료 서비스 도입의 기초자료를 제공하는데 그 목적이 있으며 그를 위해 노인장기요양보험 하에서 물리치료 서비스를 제공하고 있는 노인시설 물리치료사들의 제도에 대한 인식조사를 실시하였다. 그 결과 노인장기요양보험에 대한 인지도는 88.1%로 높았으며 필요성에서도 높았지만 40세 이상에서 절대 필요하다는 응답이 60.3%로 더 높았다. 또한 수가는 방문간호를 기준으로 높게 책정해야 한다는 의견이 59.7% 가장 높았다. 방문물리치료서비스 중 가장 중요한 치료접근으로는 일상생활 지도가 40%로 가장 높았고, 서비스 주체로는 방문간호센터처럼 방문재활센터로 해야 한다는 의견이 69.4%를 차지했다. 방문물리치료 시행에 따른 기대효과는 50점 만점에 전체 평균 41.44점으로 나타났으며 특히 40세 이상에서 42.48점으로 나타나 전체적인 기대효과가 높은 것으로 나타났다.

우리나라 노인 방문 구강건강관리 서비스의 현황과 향후과제 (Current status and future tasks of visiting oral health care services for elders)

  • Objectives: This study confirms the current status of visiting oral health-care services for the elderly to draw policy implications for revitalization of the visiting oral health care services in the future. Methods: First, a survey was conducted on health centers about the current status of the elderly visiting oral health-care service and how to revitalize it. Next, the number of oral hygiene services provided to the elderly was checked in the long-term care insurance system. Results: Oral health education (100%) was the most common practice in visiting oral health-care service for the elderly, and the most difficult thing in providing services was the lack of dental hygienists (38.9%). The status of oral health-care services in the long-term care insurance system for elderly revealed that the total number of service claims has been confirmed to be zero since the introduction of the system. Conclusions: Despite the existence of a system that provides elderly visiting oral healthcare services, to revitalize it, the law must be amended to secure a dental hygienist as the main agent of the activity and to further take responsibility for autonomous authority and performance.

  • Long-term care Insurance system for the Elderly was started to respond to the aged society on July 2008 in Korea. So responsibility of the care for the old is being changed from the family to the society. The introduction of that insurance system has brought the growth of demand for care service. To improve the level of quality for care service providers in long-term care, the role of care worker in providers is recognized as important. But welfare system for the care worker seems to be poor. So the Seoul Metropolitan Council is preparing an ordinance for the care worker and the purpose of this study is for the ordinance as well. Questionnaires, interviews, documentary surveys were used to carry out this study. The results of this study are summarized as follows. The job stability is the most important thing for the care worker, so local government has to make efforts to promote the job stability of care worker. A welfare center for care workers is needed for the health for care workers in the local government. And the local government has to supervise service providers more strictly and evaluate them in treatments of care worker.

장기요양보호서비스의 비용분석 (Cost Analysis of Long Term Care Services)

  • 본 연구의 목적은 현재 실시되고 있는 장기요양보호서비스에 대하여 유형별로 비용측면에서 경제성을 분석하고 정책적 함의를 논의하는 것이다. 조사대상자는 2008년 7월부터 시행되고 있는 노인장기요양제도의 적용대상자로 인정받은 65세 이상 노인과 그 부양가족이었고, 주부양자를 대상으로 요양보호서비스유형별로 구분하여 설문조사하였다. 회수된 설문지 중에서 방문요양 155부, 방문간호 67부, 요양시설 108부, 요양병원 92부 총 422부가 본 연구의 분석 표본으로 사용되었다. 조사결과 다음과 같은 사실을 확인할 수 있었다. 첫째, 부양 가계의 가계소득이 높지 않았다. 둘째, 장기요양보험제도가 실시되고 있음에도 불구하고 노인부양가계가 노인을 위하여 직접 지불하는 금액이 여전히 높은 편이다. 셋째, 부양가계가 노인을 위하여 수발하는 데 소요되는 간접비용이 아주 높은 편이다. 특히 재가서비스인 방문요양의 경우엔 평균 756,947원, 방문간호인 경우 594,807원으로 시설서비스에 비하여 간접비용이 상대적으로 아주 더 높았다. 넷째, 장기요양보호 노인에 대한 사회적 비용이 아주 높다. 이 금액은 본 조사에서 확인한 부양가계의 평균가계소득과 비슷한 수준이다. 다섯째, 요양보호서비스 유형별로 부양가계가 부담하는 서비스 비용에서 유의한 차이를 보였다. 여섯째, 부양가계의 직접 비용에서도 요양보호서비스 유형별로 유의한 차이를 보였지만 서비스이용비용보다는 그 차이가 작았다. 일곱째, 사회적 직접 비용은 요양보호서비스 유형별로 아주 유의한 차이를 보였다. 여덟째, 요양보호서비스를 이용하는 노인에 대한 사회적 비용금액이 아주 크고 서비스 유형별 총 사회적 비용이 전체 서비스 이용노인의 평균적인 사회적 비용에 수렴하는 경향을 보였다.

노인장기요양보험제도에 의한 방문간호 표준개발 (Development of Home Visiting Nursing Standards Base on a Long-Term Care Insurance for the Elderly Program)

  • Purpose: The study was aimed at qualitatively enhancing and promoting a home visiting nursing program established in Korea on July 1, 2008, as part of the Long-Term Care Insurance for the Elderly program. Methods: Structural, procedural and consequential aspects of home visiting nursing care wereclassified on the horizontal axis by applying the standard notions for the evaluation of medical care (Donabedian, 1998). At the same time, the home visiting nursing care service support system and the service provision system weredivided on the vertical axis with reference to the accreditation standards for home visiting nursing care organizations suggested by the Joint Commission on the Accreditation of Healthcare Organizations (JCAHO, 2008). The data were collected from June 4, 2008 to October 27, 2008, and were analyzed using SPSS ver. 15.0. Results: Twenty-two (proposed) standards, centered on the standard elements under the conceptual framework of the study, were developed, and comprised structural aspects (n=10), procedural aspects (n=6) and consequential aspects (n=6). Those criteria and indicators underwent two content validity surveys among groups of home visiting nursing care research and training experts. The research produced 22 proposed standards, 50 proposed criteria and 166 proposed indicators. Conclusion: The home visiting nursing care standards developed pursuant to the Long-Term Care Insurance for the Elderly Act and the applicability of these standards need to be verified by home visiting nurses. These proposed standards should prove useful in developing an assessment tool to encourage the qualitative enhancement of visiting nursing care in Korea.
