The Effect of elderly Wisdom on Life Satisfaction, Self-Esteem, Psychological Wellbeing and Quality of Child Relationship (노인의 지혜가 삶의 만족도, 자아 존중감, 심리적 복지감, 자녀관계의 질에 미치는 영향)

  • This study is aimed to know whether there are any quality differences among wisdom, life satisfaction, self-esteem, psychological well-being, and child relationship in accordance with the individual characteristics of the elderly. The subjects in this study were 227 old people who were over 65 years old among people using the hall for the elderly or elderly welfare center, worship facilities in Cheonan City or Kyeonggi Province. In addition, the effect that wisdom influences each valuable showed that there were statistically significant in all of the satisfaction of living, self-esteem, psychological wellbeing, the quality of child relationship. The result of research in the interaction effects between sex and age showed the interaction effect.

Prevalence and Clinical Features of Probable REM Sleep Behavior Disorder-An Epidemiological Study in Osan City (유력 렘수면 행동장애 환자 유병률과 임상 양상-오산시 역학 연구)

  • Objectives: REM sleep behavior disorder (RBD) has received little attention in epidemiologic studies. This study aimed to determine the prevalence of probable REM sleep behavior disorder (pRBD) in the elderly population and its clinical features. Methods: A random sample of 1,588 was selected from a roster of 14,050 elderly population living in Osan city. The subjects were asked to fill out the REM sleep behavior disorder screening questionnaire (RBDSQ). Subjects whose score were 5 or higher on RBDSQ underwent a diagnostic phase of person-to-person assessment by experts in RBD. Results: Among 1,588 subjects, 886 elderly subjects participated in the screening phase and 123 subjects were assessed in the diagnostic phase. Eleven subjects were diagnosed as having pRBD, so prevalence was 1.5% (95% CI=0.70-2.30%). The frequency of depression and cognitive decline was significantly increased in patients with pRBD compared to subjects without pRBD, and there was no difference in sleep disturbances between two groups. Conclusions: Probable REM sleep behavior disorder is not rare in the elderly but frequently under-recognized. More attention should be paid to evaluation and treatment of RBD.

Factors Affecting the Intention to Participate in Healthcare Programs among Elders Living Alone (일개시 독거노인의 건강관리 프로그램 참여의도 영향요인)

  • Purpose: The aim of this study is to identify factors affecting elders' intention to participate in healthcare programs. Methods: This secondary data analysis used data of 390 elderly people who lived alone and were enrolled in the Visiting Health Care Center of H-city. In 2014, questionnaires were used to collect data about demographics, social activity, depression, frailty, and intention to participate in healthcare programs. The data were analyzed by descriptive statistics and multivariate logistic regressions. Results: Among the participants, 51.8% intended to participate in the offered healthcare programs. The strongest factor affecting the intention was gender. Women were 4.85 times more likely to participate in the programs than men. The women's intention was associated positively with increased levels of frailty. The men tended to participate in the program as they got older and as the level of frailty and the number of chronic diseases increased. Conclusion: Gender-specific public healthcare programs for vulnerable elders living alone should be developed to maximize their participation in the programs and to promote good health. The healthcare programs must be tailored to the levels of the patients' physical functions.

Prediction Model of Quality of Life in Elderly Based on ICF Model (ICF 모델에 근거한 노인의 삶의 질 예측 모형)

  • Purpose: The purpose of this study was to identify from the International Classification of Functioning model, factors influencing quality of life in elderly persons and to describe the concrete pathway of influence and the power of each variable. Methods: The sample included 334 elders who lived in 5 districts of D Metropolitan City. A structured questionnaire was used and the collected data were analyzed for fitness, using the AMOS 18.0 program. Results: This model was concise and extensive in predicting the quality of life of elders. The research verified the factors influencing quality of life for elders as direct factors such as activity of daily living (ADL) (${\beta}$=.13, t=2.47), leisure activity (${\beta}$=.55, t=5.04), social disengagement (${\beta}$=-.25, t=-2.25), and depression (${\beta}$=-.62, t=-10.86). Indirect factors including economic status (${\gamma}$=.17, p=.009), type of residence (${\gamma}$=.19, p=.004), ADL (${\gamma}$=.12, p=.027) were important factors in predicting quality of life for elders. These variables explained 75.6% of variance in the prediction model. Conclusion: The findings indicate a need for the nursing scientific community to develop intervention programs considering these variables to improve the quality of life for elders.

Factors Affecting Suicidal Ideation by Medical Institutions Users - Focusing on elderly people with chronic diseases (일 의료기관 이용자의 자살생각 영향요인 - 만성질환 노인을 중심으로)

  • The purpose of this study was to investigate the effects of depression, self-esteem and daily life ability on suicidal ideation in elderly people with chronic illness. The subjects were 170 outpatients and inpatients in a general hospital in D city. After conducting questionnaires for data collection, t-test, ANOVA and multiple regression analysis were performed and analyzed using SPSS 12.0 / win program. The results showed that suicidal ideation was influenced by gender, health status, chronic disease diagnosis, and experience of suicide attempt, with a positive correlation with depression and a negative correlation with daily life ability. The suicide attempt experience, depression, self-esteem and daily living ability showed 50.8% explanatory power against suicidal ideation. In order to reduce suicidal ideation, it is necessary to reduce suicidal ideation of elderly people with chronic diseases by managing an intervention program that reduces depression and improves daily life ability.

Relevance between oral health life practices and oral discomforts in elderly school participants in W urban-rural complex city (도농복합도시 W시 노인대학생의 구강건강생활 실천도와 구강불편감과의 관련성)

  • This study aims to analyze the relevance between oral health practices and oral discomforts of the elderly in an urban-rural area by surveying the elderly in welfare centers for the Elderly-continued oral health care program. The result shows that the elderly brush their teeth under regular oral care, and practice oral health life by scaling for the prevention of periodontal disease. Most of the elderly who drink alcohol have experienced implant care and tend to quit drinking after the treatment, and seemed to get their oral discomfort relieved. However, no relevance is found between implant experiences and social discomfort. Furthermore, the elderly who had bad breath expressed pronunciation, taste, pain and chewing discomfort, and social discomfort (P>.05). The educational level of the elderly did not have an impact on oral discomfort, but smoking, chewing and bad breath discomfort seemed to be related to social discomfort(P>.05). Therefore, since oral discomfort of the elderly causes social discomfort which decreases their quality of living, we recommend oral health departments of local governments to help the elderly maintain happy lives by continuing to study the practical use of reducing oral discomfort.

A Study of the Residential Environment, Mood States, ADL and Nursing Need of the Elderly who Live Alone (독거노인의 거택환경과 기분상태, 일상생활활동 및 간호요구도에 관한 연구)

  • This study was conducted to find out residential environment, mood states, ADL(Activity Daily Living) and nursing need of the elderly who live alone. The questionnaire survey was conducted on 154 single household elderly over 65years who live n D-city, Kangwon Province. The data were collected between September 1. 1999 and October 17, 1999 and analyzed by means of SPSS package. The results were as follows; 1. The percentage of the subjects who owned their house was no were than $13\%$., which showed that they still belonged to the economically vulnerable class. As for their residential environment, the average indoor temperature and humidity level were $16.45^{\circ}C\;and\;44.62\%$ respectively during the daytime between September and October, which proved to range within the standard scope. The average indoor illumination level was, however, 43.61 Lux, which showed that their illumination environment was very poor. 2. The average score of their mood states was 3.31. which turned out to be rather low. 3. The mean score of ADL was 1.69, which showed that they didn't have any serious inconveniences in their ordinary lives. 4. The mean score of their nursing need was 275, which proved to be close to the normal level. 5. The higher the room humidity was, the higher their nursing need increased. The higher the degree of illumination level was, the lower the degree of their nursing need was and the higher the level of ADL was. 6. The major factors influencing their moon status were the frequency of their contact with children and room humidity. These two factors could account for $22\%$ of the total mood status. These results suggest that the residential environment should be necessarily considered in the psychological nursing intervention for the elderly who live alone and they should be urgently supported in terms of health policy so that they can maintain the optimum level of illumination during the daytime.

Vitamin A Nutritional Status Determined by Assessing Dietary Intake and Serum Retinol Level among Elderly Adults Living in Asan (아산시 거주 노인들의 비타민 A 섭취 실태 및 레티놀 영양상태)

  • Insufficient dietary intake of vitamin A is one of the major nutritional problems for elderly adults in some parts of Korea. The objective of this study was to determine the vitamin A nutritional status of elderly adults in Asan, Korea by assessing the dietary intake and serum retinol concentration. Five hundred twenty four subjects (218 male and 306 female) over 65 years were recruited from city of Asan. Each subject was interviewed to assess the intake of vitamin A using a 24hr recall method and data were analysed from computer-aided nutrient analysis program. Blood samples after 12hr fasting were collected for serum retinol concentration and reverse phased HPLC with UV detector used. The results showed that subjects did not consume the sufficient amount of energy (82-85% of Korean RDA for male and 77-79% RDA for female) and vitamin A (59% RDA for male and 50% RDA for female). Range for retinol intake was 0 to $4342\;{\mu}g$ a day while that of beta-carotene was 65 to $31595\;{\mu}g$. Serum retinol concentrations were within a normal range for both male ($80\;{\mu}g/dl$) and female ($67\;{\mu}g/dl$) subjects. Many subjects (n=342) consumed less than 50% RDA of vitamin A. However, if retinol intake was high (> $37\;{\mu}g$), even with less than 50% RDA of vitamin A intake, serum retinol concentration was high ($75\;{\mu}g/dl$). Subjects showed normal serum retinol status even with low vitamin A intake. The results suggested that optimal intake ratio of dietary retinol and carotenoid is important to maintain an appropriate serum retinol concentration.

The Study on the Spatial Change in an Aging Society (고령화에 따른 공간변화 연구)

  • The purpose of the study is to come up with counter plans to the spatial change caused by an aging society. To achieve the purpose of this study, research methods are conducted in the literature survey and the census data are compared. This study focuses on an aging society, the current status of space and related problems, based on political economic spatial concepts, and then presents five countermeasures as follows. First, the planning considering the aging populations. Second, increase in total fertility rate and increase population absorption. Third, increased economic vitality of the elderly due to increased participation in the production of senior citizens. Forth, establishment and implementation of regional development plan for the elderly. Fifth, needs to transform the spatial policies of the aged to prepare a large gap in space. The result of this paper proposes the need to change the living space policies and planning to avoid mismatching between them, reducing the aging speed simultaneously. The study is expected to contribute to the establishment of a space plan for areas where the aging population is rapidly increasing.

Effect of Food Commodity Supplementary Program for Low Income Elderly People (저소득층 독거노인의 식품지원 프로그램 효과)

  • This study was performed to evaluate the effectiveness of food commodity supplementary program for low-income, living alone elderly people. The subjects were 57 low income aged females who lived alone in Bucheon city. The food commodity supplementary program lasted for 6 months, supplying one meal per day consisting with grains, meat, vegetables, dairy and fruit. Dietary intake, anthropometric measurements, and blood profiles were assessed. After 6 months of food commodity supplemented period, dietary intakes of protein(p<0.05), calcium(p<0.05), iron(p<0.05) and vitamin B(p<0.001) were significantly increased. The mean weight(p<0.05) and BMI(p<0.05) were increased, and the frequency of BMI over 25kg/m$^2$ was also raised from 54.4% to 59.7%. Serum concentrations of total protein(p<0.001) and albumin(p<0.001) were significantly increased, indicating improvement of protein status. The frequency of anemia was decreased from 45.6% to 26.3%. However, the proportion of dyslipidemia was increased. In general the stable food commodity supplementary program was effective to improve general nutritional status of the poor aged who lived alone, however, nutrient intakes still did not meet RDA for the elderly and the rates of obesity and dyslipidemia were increased. Therefore, local government should provide more active food support program and the selection of food items for commodity should be evaluated at regular intervals.
