Review of Communal Housing for the Elderly in the UK (영국의 노인공동생활주택에 대한 검토)

  • The purpose of this study was 1) to review communal housing in the UK, 2) to consider the policy implications for elderly communal housing in Korea. The research methods used were 1) literature review about communal housing and related policy in the UK 2) field survey in the UK 3) interpretative suggestion for the proper policy implication to develope communal housing for the elderly in Korea. Sheltered housing in the UK had been developed as communal housing for the elderly with special needs since the 1970s. The type of sheltered housing were category 1 and category 2. Very sheltered housing with more facilities and meal services was added in 1980s. Sheltered housing was evaluated as the most humanistic solution for older people in the UK in 1980s. Because of the policy of moving institutional care to community care, sheltered housing became less in demand because of more options for older people including being able to stay in their own home. So new completion of sheltered housing by registered social landlords reduced saliently. Sheltered housing already totalled over half million units in which 5% of all elderly over 65 still lived and a small quantity of private sector for sale schemes emerged in the 1990s. The reason why the residents moved to sheltered housing was for sociable, secure, and manageable living arrangements. In general the residents were satisfied with these characteristics but dissatisfied with the service charge and quality of meals, especially in category 2.5 schemes. The degree of utilisation of communal spaces and facilities depended on the wardens ability and enthusiasm. Evaluation of sheltered housing indicated several problems such as wardens duty as a \"good neighbour\" ; difficult-to-let problems with poor location or individual units of bedsittiing type with shared bathroom ; and the under use of communal spaces and facilities. Some ideas to solve these problems were suggested by researchers through expanding wardens duty as a professional, opening the scheme to the public, improving interior standards, and accepting non-elderly applicants who need support. Some researchers insisted continuing development of sheltered housing, but higher standards must be considered for the minority who want to live in communal living arrangement. Recently, enhanced sheltered housing with greater involvement of relatives and with tied up policy in registration and funding suggested as an alternative for residential care. In conclusion, the rights of choice for older people should be policy support for special needs housing. Elderly communal housing, especially a model similar to sheltered housing category 2 with at least 1 meal a day might be recommended for a Korean Model. For special needs housing development either for rent or for sale, participation of the public sector and long term and low interest financial support for the private sector must be developed in Korea. Providing a system for scheme managers to train and retrain must be encouraged. The professional ability of the scheme manager to plan and to deliver services might be the most important factor for the success of elderly communal housing projects in Korea. In addition the expansion of a public health care service, the development of leisure programs in Senior Citizens Centre, home helper both for the elderly in communal housing and the elderly in mainstream housing of the community as well. Providing of elderly communal housing through the modified general Construction Act rather than the present Elderly Welfare Act might be more helpful to encourage the access of general people in Korea. in Korea.

Identification of High-risk Groups of Suicide from the Depressed Elderly using Decision Tree Analysis (의사결정나무 분석법을 이용한 우울 노인 중 자살 고위험군 규명)

  • Purpose: The aim of this study is to explore levels of suicidal ideation and identify subgroups of high suicidal risk among the depressed elderly in Korea. Methods: A descriptive cross-sectional design was adopted on secondary data from the 6th (1st year) Korean national health and nutrition examination survey (KNHANES). A total of 239 depressed elders aged 60 or over who participated in the KNHANES. The prevalence of suicidal ideation and its related factors, including sociodemographic, physical, psychological characteristics and quality of life (EQ-5D index) were examined. Descriptive statistics and a decision tree analysis were performed using the SPSS/WIN 23.0 and SPSS Modeler 14.2 programs. Results: Of the depressed elderly, 28.9% had suicidal ideation. Three groups with high suicidal ideation were identified. Predictive factors included perceived stress level, household income level, quality of life and restriction of activity. In the highest risk group were those depressed elderly with moderate and low levels of stress, less than .71 of EQ-5D index and restriction of activity, and 80.0% of these participants had suicidal ideation. The accuracy of the model was 80.8%, its sensitivity 85.9%, and its specificity 68.1%. Conclusion: Multi-dimensional intervention should be designed to decrease suicide among the depressed elderly, particularly focusing on subgroups with high risk factors. This research is expected to contribute itself to the policy design and solution building in the future as it suggests policy implications in preventing the suicide of the depressed elderly.

A Study on the Interregional Actual State and Influential Factors after the Application of Denture Insurance (틀니 보험적용에 따른 지역간 차이와 이용 영향 요인 연구)

  • The purpose of this study is to analyze the factors influencing the regional differences and the denture use behavior after the elderly denture health insurance benefits for the elderly over 65 years old in Seoul and Gangwon provinces. The data was used in the "2015 Community Health Survey". The results of the study are as follows; 1) There is a significant difference in denture use among elderly in Seoul and elderly in Gangwon area. 2) There is significant differences in denture use among the elderly in the Gangwon area. According to the age, household income, basic living conditions, occupation, education level and subjective oral health, There is significant differences in all of 2 areas. 3) Some 16.6% of the elderly in Seoul and some 22.3% of the elderly in Gangwon province answered 'yes'. The reasons for not having the required dental care were the most economical reasons in all of 2 areas. Therefore, the elderly denture health insurance benefit project considering various aspects is necessary.

Current status and future tasks of visiting oral health care services for elders (우리나라 노인 방문 구강건강관리 서비스의 현황과 향후과제)

  • Objectives: This study confirms the current status of visiting oral health-care services for the elderly to draw policy implications for revitalization of the visiting oral health care services in the future. Methods: First, a survey was conducted on health centers about the current status of the elderly visiting oral health-care service and how to revitalize it. Next, the number of oral hygiene services provided to the elderly was checked in the long-term care insurance system. Results: Oral health education (100%) was the most common practice in visiting oral health-care service for the elderly, and the most difficult thing in providing services was the lack of dental hygienists (38.9%). The status of oral health-care services in the long-term care insurance system for elderly revealed that the total number of service claims has been confirmed to be zero since the introduction of the system. Conclusions: Despite the existence of a system that provides elderly visiting oral healthcare services, to revitalize it, the law must be amended to secure a dental hygienist as the main agent of the activity and to further take responsibility for autonomous authority and performance.

A study on patients' satisfaction of physical therapy services in public health center in the point of view of the elderly welfare policy (서울시 보건소 물리치료실 이용자의 만족도 조사 연구 - 노인복지 정책측면에서 -)

  • The purpose of the study is (i) to investigate the operating situation of physical therapy department in public health centers, (ii) to analyze the difference of elderly satisfaction from non-elderly satisfaction of physical therapy services among the public health center visitors, and finally (iii) to suggest facilitating improvement on programs that can supply the elderly health service through public health centers. A questionnaire survey was carried out 212 patients of 25 public health center in Seoul, Korea from 15 July to 10 August 1996. Major results of the study are as follows : 1. Among the respondents. the largest group was the elderly who are over 65 years old (72.6%). 2. The averge of general patients' satisfaction for physical therapy services was 3.98, especially fee of treatment(4.76), attitude of physical therapist(4.33), and reliance of physical therapist(4.20) were higher by 5-score Likert scale. 3. The correlation factors with general patients' satisfaction were process of treatment (r =.6301). results of treatment(r=. 6186), and guide facilities(r=.5999). 4. The results of multiple-regression analysis between general patients' satisfaction and affecting factors identify following variables as the statically significant determinants : general stisfaction comparing with other physical therapy services, physical therapy services using forwardly, fee of treatment. 5. There was no significant defference between elderly and non-elderly patients' satisfaction of physical therapy service, except attitude of physical therapist(t=-2.07), explanation of treatment for disease(t=-2.19), fee of treatment(t=6.29).

Analysis of Influencing Factors on the Outpatient Prescription of Antipsychotic Drugs in the Elderly Patients (노인환자의 항정신병 약물 원외처방 내역에 미친 영향 요인 분석)

  • Background: Most antipsychotic drugs studies have been mainly conducted on side effects, randomized clinical trials, utilization rates, and trends. But there have been few studies on the influencing factors in elderly patients. The purpose of this study was to analyze the influencing factors on the outpatient prescription of antipsychotic drugs in the elderly patients. Methods: Active ingredients of antipsychotic drugs in Korea were selected according to the Korean Pharmaceutical Information Center (KPIC)'s classification. Data source was Korean Health Insurance Review and Assessment Service (HIRA) claims data in 2020 and target patient group was the elderly patient group. We extracted patients who have been prescribed one or more antipsychotic drugs and visited only one medical institution. Data were analyzed using descriptive statistics, chi-square, t-test, negative binomial regression. Results: A number of outpatients were 245,197 and prescriptions were 1,379,092. Most characteristics of patients were 75-85 year's old, female, health insurance type, no disease (dementia, schizophrenia), atypical drugs, cci score (>2) and characteristics of medical institution were neurology in specialty, rural region, general hospitals. Results of regression showed that patient's characteristics and medical center characteristics had significant effect on the outpatient prescription of antipsychotic drugs in the elderly patients. Conclusion: This study suggests that national policy of antipsychotic drugs in the elderly patients, with the consideration of the patients' and medical institutions' characteristics, is needed.

The Relationship between The Psychosocial Characteristics, Family Function, and Activities of Daily Living in the Elderly Females (경로당 여성노인의 심리사회적 특성과 가족기능 및 일상생활수행능력간의 관계)

  • Purpose: To investigate the relationships among stress, depression, family function, and activities of daily living in elderly females. Methods: Using a descriptive correlation for the study design, data was collected from 134 elderly females older than 60 years of age. Result: The level of depression among the subjects was 6.04($\pm5.30$) and level of stress was 1.62($\pm.70$). The performance capacity of the activities of daily living was 17.79($\pm0.87$), and the instrumental activities of daily living was 22.33($\pm2.66$). The family function was 2.82($\pm3.47$). Depression and stress showed a positive correlation(p<.001) and had a negative correlation with family function(p=.025), as the correlation with instrumental activities of daily living was negatively related(p=.008). Stress had a positive relationship with family function(p<.001). and was negatively related to instrumental activities of daily living (p=.041). Conclusion: For health promotion of elderly females, it is critical to make an effort to establish and constantly develop a program, as well as policy for the health of the elderly considering and understanding gender differences.

The Influence of Depression on the Life Satisfaction of the Elderly with Hypertension

  • The purpose of this study was to explain the influence of depression on the life satisfaction of the elderly with hypertension. Also, we were focusing whether there is a mediating effect of subjective health status between depression and life satisfaction of the elderly with hypertension. 2,198 persons with hypertension over 65 years of age were selected from the data of KLoSA(Korean Longitudinal Study of Ageing), administered by The Korea Employment Information Service in 2014. Life satisfaction was measured by 10-Likert. The depression measured by CES-D10. The mediating variable is subjective health status measured by 5-Likert. As the method of analysis, the multiple regressions were used with SPSS 19.0. The result of the study shows that depression has a negative influence on life satisfaction and a negative influence on subjective health status. It was also verified that the subjective health status variable has a mediating effect between depression and life satisfaction. These results present the importance of psychological approach for the health promotion experts intervening with the elderly with hypertension.

Unmet Healthcare Needs Status and Trend of South Korea in 2020 (2020년 미충족의료율과 추이)

  • Unmet healthcare is an important indicator to measure accessibility of healthcare services. To examine the latest status of unmet healthcare needs in South Korea, four different data which is composed of nationally representative sample of South Korean population were used; the Korea National Health and Nutrition Examination Survey (KNHANES, 2007-2020), the Community Health Survey (CHS, 2008-2020), the Korea Health Panel Survey (KHP, 2011-2018), and the Korean Welfare Panel Study (KOWEPS, 2006-2020). The proportion of individuals reporting unmet healthcare needs were 6.4% (KNHANES), 5.4% (CHS), and 12.2% (KHP). Annual percentage change (APC) which identifies trend for the follow-up period was -9.9%, -9.1%, and -5.5%, respectively. The proportion of individuals reporting unmet healthcare needs due to cost were 1.0% (KNHANES), 0.4% (CHS), 2.2% (KHP), and 0.4% (KOWEPS). The APC was -11.3%, -17.0%, -12.2%, and -21.2%, respectively. Overall, the low-income and the elderly population reported a higher rate of unmet health care needs. Although the overall experience rate of unmet medical care due to cost decreased over the past decade, the disparity between the lowest and highest income groups still remained in 2020. Disparity between income levels and age groups is a challenge to address in healthcare system, and these results suggest the need for adequate health coverage for the low-income and the elderly populations.

Effects of Oral Health Behavior and Status of Elderly People in Korea on Mental Health and Quality of Life (한국노인의 구강건강행위 및 상태가 정신건강과 삶의 질에 미치는 영향)

  • This study conducted a secondary analysis using original data of performed by Korea Institute for Health and Social Affairs to determine factors affecting oral health-related quality of life and mental health in the elderly. The research subjects were 1,217 people who were over 65 years old and completed an oral examination and oral questionnaire survey. The results were as follows. First, mental health problems arise in people with an oral health status designated as 'bad' or when they feel 'very uncomfortable' chewing or speaking. Second, the quality of life decreases as problems with chewing and speaking increase. The results reveal that to reduce problems of mental health(stress, depression, suicide) and to enhance the quality of life, there is a need to develope diverse programs for preventive treatment and oral health education. What is the most important is consistent policy support.
