노인요양시설 요양보호사를 위한 낙상예방교육프로그램이 낙상관련 지식, 낙상관련 부담감 및 낙상예방 돌봄행위에 미치는 효과 (Effects of the Fall Prevention Education Program (FPEP) for Caregivers in Elderly Care Facilities on Fall-related Knowledge, Fall-related Burden, and Caring Behaviors for Fall Prevention)

  • 본 연구는 노인요양시설 노인에게 직접 돌봄을 제공하는 요양보호사가 시설노인의 낙상을 예방하고 낙상으로 인한 상해를 최소화하기 위해 노인요양시설의 요양보호사를 위한 낙상예방프로그램을 개발하였으며 낙상예방교육프로그램이 요양보호사의 낙상관련 지식, 낙상관련 부담감 및 낙상예방 돌봄행위에 미치는 효과를 검증하고자 하였다. 본 연구의 낙상예방교육프로그램은 4주 프로그램으로 주 1회 80분씩 총 4회 실시하였다. 프로그램 효과를 검증하기 위해 2012년 7월 16일부터 8월 31일까지 C지역의 A장기요양시설 요양보호사 실험군 24명, B장기요양시설 요양보호사 대조군 23명, 총 47명을 대상으로 프로그램 적용 전 후 사전 사후 조사를 실시하였다. 수집된 자료는 SPSS/Win 19.0 프로그램을 이용하여 실수와 백분율, 평균과 표준편차, ${\chi}^2$-test와 Fisher's exact probability test, independent t-test, ANCOVA로 분석하였다. 연구결과, 낙상예방교육프로그램을 받은 노인요양시설의 요양보호사는 교육을 받지 않은 요양보호사에 비해 낙상 예방교육프로그램 적용 후 낙상관련 부담감이 감소하였으며, 낙상관련 지식과 낙상예방 돌봄행위가 증가하였다. 따라서 본 연구에서 개발된 낙상예방교육프로그램은 노인요양시설 요양보호사의 낙상관련 지식과 낙상예방 돌봄행위를 증진하고 낙상관련 부담감을 완화하는 적절한 프로그램으로 노인노인요양시설 노인의 낙상예방을 위한 요양보호사의 교육자료로서 활용 가치가 있다 하겠다.

  • Purpose: The purposes of this study was to develop a comprehensive community-based fall prevention program and to test the effects of the program on the muscle strength, postural balance and fall efficacy for elderly people. Methods: The design of this study was a nonequivalent control group pretest-posttest design. There were 28 participants in the experimental group and 29 in the control group. The program consisted of balance exercises, elastic resistance exercises and prevention education. The program was provided five times a week for 8 weeks and each session lasted 90 minutes. Data were analyzed using ${\chi}^2$-test, independent t-test and paired t-test using the SPSS program. Results: Muscle strength of the lower extremities, postural balance and fall efficacy scores significantly improved in the experimental group compared to the control group. Conclusion: These results suggest that this program can improve lower extremity muscle strength, postural balance and fall efficacy in elders. Therefore, this program is recommended for use in fall prevention programs for elders living in the community.

입원 환자에서 STRATIFY의 예측 타당도 메타분석 (Predictive Validity of the STRATIFY for Fall Screening Assessment in Acute Hospital Setting: A meta-analysis)

  • Purpose: This study is to determine the predictive validity of the St. Thomas Risk Assessment Tool in Falling Elderly Inpatients (STRATIFY) for inpatients' fall risk. Methods: A literature search was performed to identify all studies published between 1946 and 2014 from periodicals indexed in Ovid Medline, Embase, CINAHL, KoreaMed, NDSL and other databases, using the following key words; 'fall', 'fall risk assessment', 'fall screening', 'mobility scale', and 'risk assessment tool'. The QUADAS-II was applied to assess the internal validity of the diagnostic studies. Fourteen studies were analyzed using meta-analysis with MetaDisc 1.4. Results: The predictive validity of STRATIFY was as follows; pooled sensitivity .75 (95% CI: 0.72~0.78), pooled specificity .69 (95% CI: 0.69~0.70) respectively. In addition, the pooled sensitivity in the study that targets only the over 65 years of age was .89 (95% CI: 0.85~0.93). Conclusion: The STRATIFY's predictive validity for fall risk is at a moderate level. Although there is a limit to interpret the results for heterogeneity between the literature, STRATIFY is an appropriate tool to apply to hospitalized patients of the elderly at a potential risk of accidental fall in a hospital.

후마네트 운동프로그램이 노인의 낙상 관련 체력요인과 인지기능에 미치는 영향 (The Effects of Fumanet Exercise Program on Fall Down-Related Physical Fitness Factor and Cognitive Function in Elderly People)

  • Purpose: This study examined the effects of the fumanet exercise program on the cognitive function and fall down-related physical fitness factor in elderly people. Method: The study period was May 2-June 24, 2016. The participants included 20 elderly persons (control group 10, experimental group 10) who registered at the day-care Center in G. City. Descriptive statistics were used for the analysis of the control and experimental groups' general characteristics. The chi-square and Mann-Whitney U tests were conducted to identify the two groups' general characteristics' and dependent variables' homogeneity. The Wilcoxon signed-rank test was used to compare the pre-and post-intervention cognitive function and fall down-related physical fitness factor, and a Mann-Whitney U test was used to analyze the rate of cognitive function and fall down-related physical fitness factor changes between the groups after the intervention. Result: The participants' fall down-related physical fitness factors (balance, gait, leg strength) increased and memory in the cognitive function sub-area improved in the participants who participated in the fumanet exercise program. The amount of change in gait was significantly different between the control and experimental groups. Conclusion: The fumanet exercise program included gait training using the learn, remember and repeat phased steps. Therefore, this study proposes using the fumanet exercise program to improve the elderly's cognitive function and physical fitness factor.

한국 노인의 대사증후군 요인 질환과 낙상과의 관련성: 퇴원손상심층조사를 이용하여 (The Relationship between Metabolic Syndrome Factor Diseases and Falls in Korean Elderly: Using National Hospital Discharge In-depth Injury Survey)

  • Objectives: The purpose of this study is to identify the relationship between metabolic syndrome factor diseases and falls in the elderly aged 65 years or older and use them as basic data to reduce the risk of falls. Methods: The method of this study was to compare the injury-related characteristics of the fall and non-fall groups with a factor disease of metabolic syndrome in Korea over 65 years of age. Data from the 14th National Hospital Discharge In-depth Injury Survey in 2018 were used to conduct the study. A total of 7,991 data were analyzed using SPSS 23.0. Results: Among the total injuries, the fall group with metabolic syndrome factor disease accounted for 69.0% and the non-fall group 31.0%. Falls occurred in 86.3% of households. In the fall group with metabolic syndrome factor disease, the number of females was 1.9~2.1 times higher than that of males. Compared to 65~69 years of age, the incidence of falls was 1.4~1.5 times higher in 70~79 years, 1.7~2.2 times higher in 80~89 years, and 2.5~3.6 times higher in 90-year-olds and older. In NISS, the incidence of falls was 1.7 times higher in moderate compared to mild. In principle diagnosis, the incidence of falls was 2.2 times higher in S40-S99 compared to S00-S19. Conclusion: The elderly with metabolic syndrome factor disease should continue to promote health through light exercise that can strengthen muscle strength to prevent falls.

The Effect of Lumbar Muscle Strengthening Exercise Types on Balance and Walking Ability of Elderly

  • The purpose of this study is to examine the effect of stabilization, resistance exercise, and combined exercise for lumbar on balance and walking ability of elderly. This study selected 22 subjects of over 65 years old elderly women who satisfy the study condition from A, B and C Senior Citizen's Center, in Daegu. The subjects were divided into groups; 7 for resistance exercise group, 8 for stabilization exercise group, and 7 for combined exercise group(resistance and stabilization exercise). The exercises were conducted for 60min a day, three times a week for 12 weeks. Balance and walking ability were checked before the exercise, 6 weeks later, and 12 weeks later. First, all lumbar muscle strengthening exercises were effective for static balance which changed according to exercising period. In dynamic balance, the resistance exercise group showed significant improvement in sit to stand. Stabilization exercise group showed significant improvement in all factors. The combined exercise group showed significant improvement in sit to stand and timed up and go. There was no difference between the exercise types. Second, the resistance exercise group showed significant change in Cadence which changed according to exercising period. The combined exercise group showed significant improvement in all factors. Between the exercise types, combined exercise was a bit more effective than resistance exercise. According to the result of 12 weeks of lumbar muscle exercise, combined exercise is considered to be the most effective exercise to prevent fall as it helps balance and walking ability slightly more than other exercises. Therefore, this study can understand the risk factors for fall accidents that frequently occur among elderly and adopt the combined exercise to prevent fall which in turn will prevent secondary problems occur from fall accidents and improve quality of life of elderly.

  • Purpose: The purpose of this study was to evaluate the effects of BeHaS exercise program on pain, balance, and falling efficacy in elderly with osteoarthritis. Method: A convenience sample of 52 elderly people with osteoarthritis who lived in C city were recruited through the public health center. For this quasi-experimental study design, 31 of the elders were assigned to the experimental group which received the BeHaS exercise program twice week for eight weeks, and 24 were assigned for the control group (no intervention). Outcome measures were numeric pain scale, standing on one leg for balance, fall efficacy scale. Analysis was performed with SPSS version 12.0 program. Results: Pain was decreased significantly in the experimental group as compared to the control group, Balance and fall efficacy were improved significantly in the experimental group as compared to the control group. Conclusion: The BeHaS exercise program can be effective in improving pain, balance, and fall efficacy in elders with osteoarthritis.

식후저혈압 위험 노인의 식전 물 섭취가 식후 혈압변화에 미치는 효과 (Effect of Water Drinking on the Postprandial Fall of Blood Pressure in the Elderly)

  • Purpose: The purpose of this study was to examine preventive effects of water drinking on postprandial fall of blood pressure in the elderly. Method: Participants were 25 elders who had experienced postprandial fall of systolic blood pressure of more than 15mmHg in a previous study. Within subject repeated measures design using random order allocation was used to control extraneous variables among participants. During the experiment, each participant had to drink 400mL water before lunch, whereas when in the control, they only had their lunch. Blood pressure and heart rate were measured for both times before lunch and at 15 minutes intervals up to 90 minutes after lunch. Data were analyzed using descriptive statistics, repeated measures of ANOVA, paired t-test and Bonferroni adjustment as Post-hoc analysis. Results: Water drinking before lunch elicited significant pressor effects by increasing systolic and diastolic pressure $13.9{\pm}4.7/5.2{\pm}2.2mmHg$ compared blood pressure during the control p<.005, p<.022. Heart rate, however, did not change significantly at either time. Conclusion: Drinking water before meal is recommended for elders who experience postprandial hypotension. However, the appropriate volume of water to prevent postprandial fall in blood pressure should be further studied.

  • Purpose: This study was conducted in order to investigate the effects of an aerobic exercise program on mobility, fall efficacy, balance, and stress in the elderly at senior centers. Methods: This research was conducted as a quasi-experimental pretest and posttest control and experimental comparison study. The subjects were 41 senior residents (Exp.=18, Cont.=23) who aged above 65 at senior centers in S City. The period of time for data collection and intervention was from August 25 to December 5, 2008. Results: Mobility (t=-3.10, p<.01) in the experimental group were significantly higher than those in the control group without an aerobic exercise program. However, fall efficacy (t=1.28, p=.207), balance (t=-.53, p=.602; t=.36, p=.723), stress (t=-1.32, p=.199) in the experimental group was not significantly higher than that in the control group. Conclusion: These findings confirmed that aerobic exercise programs make a contribution to improving mobility in the elderly at senior centers.

요양시설 노인의 낙상 실태와 영향 요인 (Incidence and Predictors of Falls in Institutionalized Elderly)

  • Purpose: The purpose of this study was to determine the incidence and influencing factors of falls in the institutionalized elderly. Method: A descriptive survey design was used with a convenience sampling of 430 institutionalized elderly. Data were collected using a structured questionnaire. Results: The falls rate after admission in institution was 8.4%. Types of injury after falls were mostly contusion and fracture. Interventions for fall prevention were environmental management and regular evaluation of risk factors for falls. Fall experience was significantly different according to admission periods. Type of falls was significantly different according to state of ADL. Predictors for falls were longer admission periods and dependent state in ADL. Conclusion: Falls is important health related problem in the institutionalized elderly, so effective fall prevention program is needed for their health promotion.
