Instrumental Activities of Daily Living, Leg Muscle Strength, Cognitive and Visual Function According to Demographic Variables and the Experience of Falling in Community Resident Elderly Koreans (지역사회 재가 노인의 인구학적 특성과 낙상경험에 따른 일상생활 수행능력, 하지근력, 인지기능 및 시각기능)

  • Purpose: To investigate the instrumental activities of daily living (IADL), leg muscle strength, cognitive and visual functions according to the participants' demographic characters and experiences of falling in Korean elder who are residents in the community. Method: Data were collected from 351 participants aged 65 or more, who were ambulatory, A structured questionnaire and several physical function measurements were used to collect the data which were analyzed using the SAS program. Results: The participants' IADL scores were significantly different according to their age, educational level and living situation. The leg muscle strength of the elders were significantly different according to gender and age, and were also related the risk of falling. Cognitive scores were significantly different according to gender, age, education level and marital status. Visual acuity and depth perception were significantly different according to age, education level, and marital status. Conclusion: Demographic variables and experience of falling were identified as variables related to IADL, leg muscle strength, cognitive and visual function of elders. Further study with a larger and nation-wide sample is needed to identify the variables related to the physical and cognitive functions of elders.

  • A major accident occurring on the bed is falls that occur during at times when the care of nurses or protectors is inadequate, which is fatal to patients or the elderly. In particular, Enuresis or sleepiness caused by sleep apnea increases the risk of falls. Therefore, it is very important to detect falls and sleep apnea of patients without infringing privacy in the bed to patient's safety and accident prevention. In this paper, we reviewed the technologies developed for bed monitoring and implemented a non-intrusive monitoring system. The Occupancy Sensor allows the temperature of the bed and surrounding area to be extracted to enable track of the patient's motion. The Doppler Radar detects the patient's movements at normal times and the respiration state when patients have no movement during sleeping. It is specially designed for real-time monitoring of falling and respiration during sleeping through contactless multi-sensing while solving patient's privacy problems.

Effect of a PNF Training Program on Functional Assessment Measures and Gait Parameters in Healthy Older Adults (PNF 훈련 프로그램이 건강한 노인의 기능적 평가 측정과 보행에 미치는 효과)

  • Purpose: The purpose of this study was to examine changes in functional assessment measures (FAMs) and spatiotemporal gait parameters (STGPs) in healthy older adults before and after a 6-week intervention of a proprioceptive neuromuscular facilitation (PNF) training program. Methods: Thirty healthy older adults (mean age: $73.37{\pm}1.21$ age range: 65-79) were randomly assigned either to an experimental group (participating in a 6-week intervention of PNF training) or a control group (only performing daily activities). Participants in the control group did not receive any training program. Performance was assessed by recording changes in the FAMs and STGPs using functional assessment tools and GAITRite. Results: Participants in the PNF group showed significantly improved functional assessment measures and increased stride length, cadence, velocity, and step width (p<0.05). No significant differences in FAMs and STGPs were found in the control group (p>0.05). Conclusion: Participation in a PNF training program improves FAMs and STGPs, thereby increasing the ability of healthy older adults to maintain dynamic balance during functional performance and gait. These findings support the use of PNF training programs as effective fall-prevention programs for the elderly.

A Study on Microbial Pollution of Indoor Air at Elderly Care Facilities (노인요양시설(老人療養施設)의 실내공기(室內空氣) 중 미생물(微生物) 오염(汚染)에 관(關)한 연구(硏究))

  • 우리나라에서의 실내공기 중 미생물에 대한 연구는 사무실, 유치원, 극장, 도서관등 다중이용시설과 병실 내 공간에 존재하는 곰팡이를 비롯한 오염 미생물 수에 대한 조사가 이루어지고 있으나 노인요양시설을 대상으로 한 바이오에어로졸이 포함되어 있는 미생물의 종류에 대한 연구는 아직 미비한 실정이다. 따라서 본 조사에서는 1개 광역도 1개 도농복합도시 지역의 노인요양시설을 대상으로 실내 미생물 오염상태가 건강에 미치는 영향과 적절한 실내 환경유지를 위한 미생물 오염 방지 및 개선방안에 대한 기초자료를 제공하고자 노인요양시설의 실내 미생물의 오염실태를 조사하였다. 2007년 4월 1일부터 5월 31일까지 2개월 동안 1개 광역도 1개 도농복합도시 지역의 노인요양시설 5곳을 대상으로 실내 외의 공기 중 바이오에어로졸 조사하기 위해 관성충돌 채취법을 적용한 미생물 채취기인 air IDEALTM(Biomerieux)를 이용하여 면양혈액한천배지와 Sabouraud Dextrose Ager를 사용하여 채취하여 배양하였다. 배양하여 분리 동정한 결과 다음과 같은 결론을 얻었다. 1. 각 시설별 전체 미생물의 분리는 300 L 실내에서 S 요양원이 263 cfu/m3로 가장 많았고, U 요양원이 123 cfu/m3로 가장 낮은 수가 분리되었다. 진균의 수는 300 L 실내에서 G 요양원이 73 cfu/m3로 가장 많았으며, C 요양원은 40 cfu/m3로 가장 낮은 비율로 분리되었다.

Effects of Taping Therapy and Passive Range of Motion Exercises on Shoulder joint, Hand dexterity in Elderly (테이핑요법이 노인의 견관절 통증과 관절가동범위, 손의 민첩성에 미치는 효과)

  • 본 연구는 견관절 통증이 있는 노인을 대상으로 테이핑 요법과 수동적 관절가동범위운동이 견관절의 통증, 관절 가동력, 손의 민첩성에 미치는 효과를 확인하기 위하여 실시하였다. 연구대상자는 B시의 D복지관을 이용하는 65세의 이상의 노인들 중 견관절 통증을 호소하는 노인 40명이었고 연구기간은 4주간 총12회로 테이핑 요법과 수동적 관절가동범위운동을 적용하였다. 연구를 시작하기 전과 후에 견관절의 통증, 견관절의 가동범위, 손의 민첩성을 측정하였다. 통증정도는 시각적 상사척도를 사용하여 측정하였고, 견관절의 가동범위는 인체각도기를 사용하여 측정하였다. 유의수준은 p<.05로 설정하였다. 그 결과, 테이핑과 수동적 관절가동범위 운동을 적용 후에 노인의 견관절 통증은 감소하였고 견관절의 가동력과 손의 민첩성이 증가하였다. 노인의 견관절 통증의 감소정도와 손의 민첩성 정도는 테이핑군과 관절 범위운동군 간에 차이가 나타나지 않았다. 노인의 견관절 가동범위의 변화정도는 관절범위운동군보다 테이핑군에서 견관절 신전과 외전에서 관절의 범위가 증가하는 정도가 더 큰 것으로 나타났다. 따라서 테이핑요법과 수동적 관절범위운동은 노인의 견관절 통증을 감소시키고, 관절가동범위, 손의 민첩성과 같은 신체적인 기능의 개선을 가져온다고 할 수 있겠다. 또한 테이핑의 적용이 견관절 운동범위 증가에는 더 효과적이라고 할 수 있다.

Correlation Analysis between MBI and MMSE after Exercise Program for Dementia Elderly (치매노인을 위한 운동프로그램 적용후 MBI와 MMSE 사이의 상관관계분석)

  • 29 dementia patients over 60 years of no in the nursing home were assigned to execute rehabilitation program a month for 3 months. 1 evaluated MBI scores and MMSE scores and analyzed correlation between both scores. Analyses of results were as follows : 1. In the experimental group with rehabilitation program. ADL and cognitive function were improved significantly(p<0.05). 2. In the control group with no rehabilitation program. ADL was decreased significantly(p<0.05), cognition was not changed (pgt;0.05). 3. There were significant rises in MBI scores and MMSE scores in the experimental group(p<0.05). 4. There was a significant fall in MBI scores in the control group(p<0.05). a no change in MMSE scores in the control group(p>0.05). 5. There was strong correlation between ADL and cognition in the experimental group and the control group. 6. Inverse correlation revealed between the experimental group and the control group in ADL. 7. Inverse correlation revealed between the experimental group and the control group in cognition.

  • 한국은 급격한 고령화가 진행 중에 있다. 세계적으로도 유례가 없는 급격한 고령화에 노인시설 및 병원에서 생활하는 고령자의 수 역시 증가하고 있는 추세이다. 시설이나 병원에서 고령자의 샤워수발은 수발자에게는 매우 힘든 노동이 되고 있다. 따라서 본 연구에서는 샤워 수발자의 불편함을 해소할 수 있는 와상형 자동 샤워링 시스템을 개발하는 것을 연구목적으로 한다. 와상형 자동 샤워링 시스템을 개발하기 위하여 본 연구에서는 기존 국외제품의 노즐위치 문제점을 분석하고, 수발자의 수발행위가 최적화될 수 있는 최적노즐위치 구현을 위한 샤워링 테스트, 샤워링시뮬레이션 등을 사용하여 최적노즐위치를 구현하였다. 또한 적은 노즐 수로 넓은 범위를 효과적으로 세정할 수 있는 스윙노즐을 개발하여 시제품에 적용하였다. 샤워링 테스트의 노즐위치 최적화결과를 샤워링 시뮬레이션에 적용하여 노즐위치 및 분사각의 최적화를 수행하였고, 이를 실제 시제품 설계에 적용하였다. 본 연구에서는 와상형 샤워링 시스템의 개발에 있어 사용자의 수발부담을 경감할 수 있는 가장 중요한 설계요소를 노즐위치 및 분사각으로 정의하고, 노즐위치 및 분사각도의 최적화를 중심으로 와상형 자동샤워링 시스템 개발 연구를 수행하였다. 본 연구의 사용자 중심 제품개발과정은 앞으로 다양한 고령친화제품을 개발하는데 있어 수발자의 부담을 감소시키고, 고령사용자의 자립생활을 지원하는데 있어 적용 가능할 것으로 사료된다.

Co-occurrence Network Analysis of Keywords in Geriatric Frailty

  • Purpose: The aim of this study is to identify core keyword of frailty research in the past 35 years to understand the structure of knowledge of frailty. Methods: 10,367 frailty articles published between 1981 and April 2016 were retrieved from Web of Science. Keywords from these articles were extracted using Bibexcel and social network analysis was conducted with the occurrence network using NetMiner program. Results: The top five keywords with a high frequency of occurrence include 'disability', 'nursing home', 'sarcopenia', 'exercise', and 'dementia'. Keywords were classified by subheadings of MeSH and the majority of them were included under the healthcare and physical dimensions. The degree centralities of the keywords were arranged in the order of 'long term care' (0.55), 'gait' (0.42), 'physical activity' (0.42), 'quality of life' (0.42), and 'physical performance' (0.38). The betweenness centralities of the keywords were listed in the order of depression' (0.32), 'quality of life' (0.28), 'home care' (0.28), 'geriatric assessment' (0.28), and 'fall' (0.27). The cluster analysis shows that the frailty research field is divided into seven clusters: aging, sarcopenia, inflammation, mortality, frailty index, older people, and physical activity. Conclusion: After reviewing previous research in the 35 years, it has been found that only physical frailty and frailty related to medicine have been emphasized. Further research in psychological, cognitive, social, and environmental frailty is needed to understand frailty in a multifaceted and integrative manner.

Risk Factors of Malnutrition by Age in Hospitalized Older Adults (입원 노인의 연령별 영양불량 위험요인)

  • Purpose: Malnutrition affects all age groups, but older adults are particularly more vulnerable to nutritional deficiencies. This study evaluated the age-specific factors affecting malnutrition in hospitalized older adults. Methods: A retrospective study was conducted on inpatient elderly people who received artificial nutrition from 2010 to 2017. Data of demographics, diagnosis, type of nutrition therapy, number of comorbidity, fall risk assessment, Acute Physiologic Assessment and Chronic Health Evaluation (APACHE) score, and intensive care unit admission were collected. Malnutrition was defined as a body mass index (BMI) of less than 18.5 kg/m2. Patients were classified as the young-old (65~74 years old), the old-old (75~84 years old), or the oldest-old (85 years old or older). Results: A total of 7,130 older adults were included, and 4,028 patients were classified as the young-old, 2,506 into the old-old, and 596 into the oldest-old. Proportion of malnutrition was higher in the oldest-old compared to the other groups. In multivariate analysis, parenteral nutrition, alcohol, and high risk of falls were factors affecting malnutrition in all groups. Parenteral nutrition and alcohol in the young-old, high risk of falls in the old-old, and male sex in the oldest-old were the factors affecting malnutrition by the age group. Conclusion: Older age was the most significant factor affecting malnutrition. Specific strategies by age are needed to improve nutritional status in hospitalized older adults as influencing factors for malnutrition vary among different age groups.

Usability Test for Motion Tracking Gait Assistive Walker

  • Background: This study evaluates the usability of the Motion-Tracking Gait Assistive Walker (MTGAW) designed for elderly individuals and those with disabilities, identifying areas for improvement through interviews with physical and occupational therapists. Design: A survey study involves the usability test for MTGAW. Methods: Usability evaluations were conducted with 37 physical therapists and occupational therapists. The process included explanation, product usage, satisfaction surveys, and interviews. A satisfaction survey covering 19 items across safety, maneuverability, usability, and management areas was administered. Individual interviews identified areas for improvement. Results: Overall, high satisfaction was reported across the four areas, but interviews highlighted the need for improvements, such as addressing discomfort due to slow speed and enhancing safety measures to prevent rear-end falls. Adjusting the walker's height and width to suit the user's physique was also suggested. Conclusion: MTGAW enhances walking support and hand movement freedom but needs refinement in speed control, fall prevention, and customization based on the user physique. Future efforts should focus on developing an improved MTGAW, considering recommendations from physical therapy experts, and conducting studies to analyze its clinical effectiveness for commercialization.
