Outcome and Implication of Establishment and Practice of Action Plan for the Elderly Care Facility in Establishing Risk Management System (노인요양시설의 위험관리시스템 구축활동에서 액션 플랜의 수립과 실행에 따른 성과와 시사점)

  • This research is an empirical case study that suggests the practical practice and output of action plan, the actual performing activity of risk management system established to prevent and respond to risk of the elderly care facility. For this, this research analyzed the action plan practice process and outcome upon 2 years' practice (2014~2015) by A Elderly care facility which has conducted performance activity by establishing risk management system to prevent and respond to danger for the first time in Korea. As the research result, first, risk management system action plan was established in the choice and performance stage of risk prevention and response handling method, the 3rd among 4 staged process of risk management system establishment. Second, as the outcome along with risk management system action plan performance, as the result of comprehending the investigation on risk change for 2 years(2014~2015), risk decreased by 28% in 2015 compared to 2014, displaying effect in risk management activity along with performing action plan. Based on this result, it was determined that action plan for the effective action plan establishment and practice in the elderly care facility should be established with systematic promotion schedule to be well connected with its strategy, achievement goal, and achievement project, etc. based on vision and strategy, instead of being established individually, based on clear matter of responsibility, utilizing such technique as Gantt chart, etc., composing concretely by schematizing in order to view all contents to be practiced clearly.

Health Needs of the Elderly in Long-term Care Facilities: Using RAI-MDS-FC (노인요양시설 입소자의 요양등급별 건강 요구: RAI-MDS-FC를 적용하여)

  • Purpose: This study was to identify health needs of the elderly at nursing homes by long-term care grade. Methods: The health needs of 116 elders at two nursing homes in Seoul were measured with resident assessment protocols (RAPs), activities of daily living (ADL), and cognitive performance scale (CPS), and pain and depression were measured by resident assessment instrument (RAI). Results: With regard to RAPs, 11 out of 18 items had different distribution in the 3 groups significantly. The 1st-grade elders had a higher percentage of 9 items than the 2nd- and 3rd-grade ones but the 3rd-grade ones had the highest health needs related with activity. The 2nd-grade elders had similar health needs to the 1st-grade ones. In terms of functional level, the 1st-grade elders had the highest percentage of CPS and ADL but the 3rd-grade ones had the highest score of pain and depression out of the 3 groups. The standardized assessment instrument to identify specific health needs by the 3 groups should be developed. Therefore, care plans to meet health needs of the 3 groups will be made. Conclusion: It is suggested that nursing care is required to the elderly of the 1st- and 2nd-grade and safe activity and environment to the 3rd-graded ones.

The Effects of Community-Based Self-Help Management Program by Strengthening Self-Efficacy of Post Stroke Elderly Patients (재가 뇌졸중 노인환자에서 자기효능증진 전략을 이용한 자조관리프로그램의 효과)

  • The purpose of this study was to determine the effects of self-help management program which strengthens self-efficacy for post stroke elders who attend day care center in Chongju. One group pretest-posttest design was used, and the subjects were 7 post stroke elders with hemiparesis. The self-help management program consisted of 5 sessions and each session included group education on stroke, ROM exercise, recreation and self-care strategies. Those strategies for encouraging and reinforcing self-efficacy provided verbal persuasion, performance accomplishment and vicarious experience. All subjects were questioned before and after the program about ADLs, IADLs, self-efficacy, self-care behavior and depression. The analysis of data revealed that the program was effective to increase the IADLs, self-efficacy and self-care behavior. However, there was no significant difference between pre and post ADLs and depression. The results indicated that this program may be adopted to improve the physical and psychological function of post stroke elderly patients in community settings.

A Study on the Functional Status in Life and Life Satisfaction for Elderly Residing at Home - Comparing Urban and Rural Elderly - (재택노인의 생활기능상태와 생활만족도에 관한 연구)

  • As the population of elderly in Korea is increasing rapidly since the 1990's and will more rapidly in the 21st Century, the demand of their health care would be a great burden to health care expenditure. Then it would be necessary to contemplate the functional status and life satisfaction for elderly to make them live more independently. The objectives of this study were find out the functional status in life and life satisfaction for the elderly aged 65 or over who had resided in urban area of two Gus in Pusan and rural area of two Myuns in Haman Gun in Kyeognam Province, and to provide basic data for planning systematic health care programme. The study period was two weeks from February 6 to February 18, 1995 and the subjects were 274 elderly of which 143 were urban residents, 131 were rural residents, and the study method was by structured questionnaire. The data were analyzed with SAS/PC/sup +/ programme using Chi-square test, t-test, ANOVA, Pearson's correlation coefficients, and multiple regression analysis. The results were as follows; 1. To see the pattern of living together, those living with partners were the most common, 39.1% ; 37.8% of urban elderly lived with married sons, 32.2% of them lived with partners, but 46.6% of rural elderly lived with partners, 16.8% of them lived with married sons, which showed difference between residence(p<0.005). 2. Elderly who had jobs were 64.1% in rural residents, and 7.7% in urban residents, which showed significant difference(p<0.05). 3. The score of cognitive function of total subjects was 24.7, that of urban elderly was 23.8, and that of rural elderly was 25.7, then it was higher in rural ones and low for old-elders and those who had no jobs. 4. The score of PADL was 26.8 for urban elderly, 30.1 for rural elderly, and that of IADL was 22.2 for urban elderly, 25.6 for rural elderly, which showed higher activities of daily living for rural elderly than urban elderly(p<0.001). 5. The score of domestic performance was 21.9 for urban elderly, and 30.5 for rural elderly, which showed higher score for rural elderly(p<0.001). 6. The score of life satisfaction was 20.7 for urban elderly, 29.8 for rural elderly, then it was higher for rural elderly(p<0.01). 7. As a result of ANOVA for functional status in living by general characteristics; the score of cognitive function differed by age, job; that of PADL differed by age, job, education, and the pattern of living together, that of IADL differed by age, job, and the pattern of living together. The score of domestic role performance differed by age, job, marital status, and the pattern of living together. 8. ANOVA for life satisfaction showed that the score of life satisfaction differed by job(p<0.001) and the pattern of living together(p<0.01). 9. The correlations between functional status in living and life satisfaction showed that the higher the score of cognitive function was(r=0.39), the higher the score of activities of daily living was(r=0.50), and the higher the score of domestic role performance was(r=0.41), the higher the score of life satisfaction. 10. Stepwise multiple regression analysis for life satisfaction pointed out that residence was responsible for 39.9% of the variance. cognitive function was for 5.3%, and domestic role performance was for 1.2%.

Current Status of Sanitary and Nutritional Food Service in Elderly Day Care Center (재가노인복지시설 급식소의 위생·영양관리 실태조사 연구)

  • Objectives: This study was conducted to examine the status of foodservice management, with special interest on sanitary and nutritional food service in elderly day care centers. Methods: A total of 79 employees who managed foodservice facilities in elderly day care centers were included in the survey. The contents of the questionnaire consisted of general characteristics, importance and performance of sanitary and nutrition management, the reasons for poor performance, factors necessary for improvement, and the employee's demand for support. Data analysis was conducted using the SPSS v25.0. Results: Sanitary management showed an average importance score of 4.84 ± 0.40 and a performance score of 4.70 ± 0.61 (t-value: 8.260). The item with the lowest performance score was personal sanitary management (4.58 ± 0.71). In nutrition management, the average importance score was 4.52 ± 0.68, and the performance score was 4.20 ± 1.00 (t-value: 9.609). There were significant differences between the average score of importance and performance in both areas. As a result of an Importance-Performance Analysis, items that were recognized as important but had relatively low performance was "personal hygiene", "ventilation" and "food storage". Also in the nutritional management area, "menu planning for disease management" and "checking the saltiness in the soup" etc. had very low performance with low importance recognition. The items shown in the "low priority" quadrant were those that required professional management skills. In the areas that demanded support in foodservice management, education about sanitary and safe institutional food service had the highest score (4.42 ± 0.74), and all other items showed a demand of 4 points or more. Conclusions: Foodservice managers recognize the importance of foodservice facility management but performance is relatively low. Institutional support is, therefore, needed to improve performance. For items with low importance, it seems necessary to improve awareness of the necessity of these items and to provide education in this regard. To gradually improve foodservice management, continuous provision of education and training in these areas are of great importance.

  • Since the elderly voices include a lot of noise caused by physiological changes in respiration, phonation, and resonance, the performance of the convergence health-care equipments such as speech recognition, synthesis, analysis program done by elderly voice is deteriorated. Therefore it is necessary to develop researches to operate health-care instruments with elderly voices. In this study, a voice activity detection using a symmetric higher-order differential energy function (SHODEO) was developed and was compared with auto-correlation function(ACF) and the average magnitude difference function(AMDF). It was confirmed to have a better performance than other methods in the voice interval detection. The voice activity detection will be applied to a voice interface for the elderly to improve the accessibility of the smart devices.

The Experience of Nursing Staff on the Dementia Patients′ Aggressive Behavior (간호제공자들의 치매노인 공격행동 경험에 대한 연구)

  • Providing care to the dementia elderly with behavioral problem is a major issue in nursing homes today. This study was aimed to explore the nursing staffs′ response to aggressive dementia patients, and the effect that the aggressive behavior had on Nsgstaff. The interviews used a semi-structured questionnaire are were carried out from May to July, 1999. The subjects were 23 nursing personnel working in the nursing homes for dementia elderly. The result are as follows; 1. The types of aggressive behavior cited by the subjects was "physical", "linguistic", and "sexual". 2. The factors that caused the behavior were "symptoms of disease", "under- conditioning", "context of nursing care", "unsatisfied need", "relationship to other patients", "change of outer environment", and "invasion of one′s own privacy". 3. The response of the subjects were "unhappiness", "stress", "anger", "exaggeration", "anxiety", and "fear". 4. The management strategies listed by nursing personnel used to alleviate aggressive behavior were "ignorance" "patience" "leaving the area" "soothing sounds" "verbal punishment" "restriction and isolation" and "various management skill appliance" "adaptation". 5. The effect that aggressive behavior and were "disturbance of relationship to elderly" "decline in the amount and quality of nursing care" "interruption of task performance" "job exhaustion" "desire to leave the job" "physical impact" "stress on the cognition of others" "anxiety about health and one own future" "interference to family life′. The findings of this study will be useful in understanding the difficulties of nursing personnel when confronting the aggressive behavior of dementia partients. It also is useful as basic data in preparing efficient intervention program for these difficulties.

The Smart Medicine Delivery Using UAV for Elderly Center

  • Medication safety and medicine delivery challenge the well-being of the elderly and the management of the elderly center. With the outbreak of COVID-19, the elderly in the care center were challenged by the inconvenience of the medication restocking. The purpose of this paper accentuates the importance of the design and development of an UAV-based Smart Medicine Case (UAV-SMC) to improve the performance of medication management and medicine delivery in the elderly center. The researchers came up with the design of UAV-SMC in the light of the UAV and IoT technology to improve the performance of both Medication Practice Management (MPM) and Low Inventory Detection and Delivery (LIDD). Based on the result, with UAV-SMC, the performance of both MPM and LIDD was significantly improved. The UAV-SMC improves the efficacy of medication management in the elderly center by 26.97 to 149.83 seconds for each medication practice and 9.03 mins for each time of medicine delivery in Subang Jaya Malaysia. This paper only investigates the adoption of UAV-SMC in the content of elderly center rather than other industries. The authors consider integrating the UAV-SMC with the e-pharmacy system in the future. In conclusion, the UAV-SMC has significantly improved the medication management and guard the safety of elderly and caretaker in the elderly in the post-pandemic times.

Current status of long-term care facility workers' physical function improvement activities for the elderly

  • Objective: This study was conducted to provide basic data on physical function improvement activities of elderly care facilities by identifying the degree of physical improvement activities of elderly caregivers. Design: Descriptive correlational study. Methods: The instrument of this study consisted of 12 questions on the general characteristics of the subject. For the assessment of improvement in physical function activities, the measuring tool used for the elderly consisted of a total of 20 questions. For data analysis, Pearson's correlation coefficient & Spearman's rho and multiple regression were used. Results: The higher the age of the subjects, the lower their educational background (r=-0.273, p<0.05), and the higher the probability of having a religion (r=-0.258, p<0.05), the more stable the employment type (r=-0.333, p<0.05). The higher the level of education, the higher the monthly income (r=0.187, p<0.01), and the shorter the career (r=-0.204, p<0.05). The more stable employment, the more unstable duty (r=-0.245, p<0.05), and the more unstable work, the higher the monthly income (r=-0.206, p<0.05) and the longer the career, the higher the monthly income (r=0.247, p<0.05). The more stable the employment, the more activities to improve physical function were found (r=0.341, p<0.05), and the more unstable the duty, the more activities to improve physical function were found (r=0.321, p<0.05), and the higher monthly income, the less physical function improvement activities (r=-0.196, p<0.05). Conclusions: It is necessary to provide regular services by a dedicated physical therapist for physical function improvement activities in order to improve the quality of life of the elderly in the future.

Development of Infection Control E-learning Training Program for Preventing Emerging Infectious Diseases for Long-term Care Facility Care Workers (장기요양시설 요양보호사 신종감염병 예방 원격 감염관리 교육 프로그램 개발)

  • Purpose: This study aimed to develop an infection control e-learning training program for long-term care facility care workers to prevent emerging infectious diseases and evaluate its effectiveness. Method: The program was developed using the analysis design development implementation evaluation (ADDIE) model. The effectiveness of the program was evaluated for 30 care workers. The knowledge and performance of the care workers before and after the program were analyzed by a t-test. Results: In the analysis stages, a literature review on infection control, knowledge and performance of infection control, and education needs was performed, and focus group interviews with ten care workers were conducted. In the design stage, education topics, educational content, and educational methods were selected for the program. A video was produced centered on eight themes. In the development stage, a system for education was developed, and each topic was uploaded. In the implementation stage, the program was applied to 30 care workers, and a questionnaire was administered. In the program's final evaluation, there was a significant difference in infection control knowledge (t=3.06, p=.005), and there was no significant difference in infection control performance. Conclusion: In this study, the necessary topics were finally selected by quantitatively and qualitatively analyzing the educational needs of care workers taking care of the elderly in long-term care facilities. It is necessary to understand the long-term effect and the degree of performance of the observation method in the future.
