The Follow-up Study of Changes in Frailty in Elderly Receiving Home Health Care of the Public Health Center

  • Purpose: The purpose of this study was to follow-up the frailty of the old who received home health care by Registered Nurse in Public Health Center over 8 years. Methods: We used the second wave data which was a comprehensive longitudinal data set, Public Health Information System of a public health center located in Seoul from 2010 to 2018. For statistical analysis, a mixed model of repeated measures by R program was used. Results: Frailty (range: 0~31) was getting worse significantly from 5.38 on registration to 6.54 on 4th year, 7.40 on 7th year, 7.69 on 8th year with adjustment for age, sex, economic status, the number of family, and the number diseases. The coefficient of parameters with frailty change was serviced year (β=0.29, p<.001), age (70~79 to 60~69; β=0.98, p=.018) and sex (female to male; β=2.55, p<.001). Conclusion: This study showed that the home visiting health service needs to take attention to aged 70s and over, female. The home health care of public health center need to be extended more practical and effective services in terms of 'community care'and 'ageing in place'.

Oral health care of the elderly in long-term care facilities in Korea (요양보호사가 입소노인에게 제공하는 구강관리실태)

  • Objectives : The aim of this study was to assess the oral health care of the elderly in long-term care facility on caregivers' behaviors. Methods : Cross-sectional study in a cluster sample of 171 caregivers recruited from 17 facilities located in the Province of Gangwon. The Questionnaire was consisted of 16 items contained oral hygiene care, denture care, oral health education and general characteristics(Chronba's ${\alpha}$=0.87). Using SPSS WIN 12.0, descriptive statistics and chi-square test were conducted to examine the subjects general characteristics, the status of oral health education and denture care. Results : 1. The mean age of care givers was 42 years and 88.9% education experience rate was found. 2. Above 70% of them was found in daily oral hygiene care after every meal. They used toothbrush and toothpaste, they keep the toothbrush properly. 3. The denture care was conducted by most of care givers, containing proper storage. But three of ten care givers was cleaned denture by toothpaste or used water only. It was severe at the care givers didn't received oral health education(p<0.05). Conclusions : The contents of oral health education for care givers should contain the denture care(time and the reason shouldn't use toothpaste). Further large-scale longitudinal studies are needed to determine professional oral health care and to develop evidence of the dental hygiene practice for the elderly in long-term care facilities.

The Influence of Gender on Clinical Outcomes in Elderly Patients Underwent Coronary Artery Bypass Grafting Surgery

  • The female has previously been shown to be an independent risk factor for mortality and morbidity after coronary artery bypass grafting surgery (CABG). The aim of this retrospective study is to evaluate gender differences of the perioperative outcomes in elderly patients underwent CABG. Data for seventy elderly patients (>70 years) that underwent CABG (between January 2005 and July 2011) were divided into two groups: male patients (n=33, male group) and female patients group (n=37, female group). Heights, body weights, body surface area and coronary artery obstruction rate (right coronary artery territory) in the female group were lower than those of the male group ($P$ <0.05). History of hypertension, hyperlipidemia, congestive heart failure and percutaneous coronary artery intervention in the female group was higher than that of the male group ($P$ <0.05). Total cholesterol and brain natriuretic peptide levels in the female group were higher than those of the male group ($P$ <0.05). Platelet count in the female group was higher than the male group at preoperative (Pre-OP) period ($P$ <0.05). Erythrocyte count, hematocrit and hemoglobin levels in the female group were lower than those of the male group at Pre-OP period ($P$ <0.05). But, erythrocyte count, hematocrit and hemoglobin levels in the female group were higher than those of the Male group at postoperative (Post-OP) period ($P$ <0.05). Left ventricular ejection fraction in the female group was higher than the male group at Post-OP period ($P$ <0.05). Hospital stay length in the female group was higher than the male group ($P$ <0.05). Post-OP bleeding volume and incidence of ventricular premature contraction in the female group were lower than those of the male group ($P$ <0.05). These results suggest that despite female gender have a greater risk factors and require a longer hospitalization than male, there was no significant difference incidence of mortality and complication.

  • The purpose of this study was to assess dietary intake and food habits of low-income person aged 60 years or eldary. The hundred fifty persons from the suburbs of Jeonju were surveyed between August 1 to 20,1979. Results were as follows: Family environment Approximately 90% of elderly persons surved, lived with their children and grand-children: 5.2% together as a couple; and 4.4%, widowed, lived alone. Nearly 40% of the households has a average monthly incomes of W40,000 to W100,000. Average food expediture accounted for 50 to 70% of total monthly income, thus indicating that the subject families belonged to the lowest socioeconomic level. As pocket money, 74.5% of male subjects had more than W5,100 per month. whereas, 51.4% of female had less than W5,000. Anthropometric measurements: 97.6% of subjects has heights greater than 90% of the Korean standard for their age group, whereas 45.2% of the subjects were 60 to 89% of standard weight. 88% had an arm circumferences only 60 to 89% of the standard. Nutrient intake: Intake of the majority of nutrients was below the recommended allowances, especially for energy, protein, calcium and iron. The energy input ratio of carbohydrate: protein: fat was 73.1-80.9. 13.3-15.8: 4.5-11.5, showing very heavy dependence on carbohydrates for energy needs. The contribution of animal protein was 24.3% of total protein intake, indicating an improper protein diet. Other factors influenced on the nutrient intake: Poor teeth, illness, and poor appetite were always associated with inadequate intake of energy and nutrients. The larger the family size, the lesser intake of nutrients was observed among those elderly.

Correlates of Body Mass Index, Perceived Health Status, and the Needs of Functional Games for the Elderly in Korea (한국노인의 체질량지수, 주관적 건강상태와 기능성게임 요구도의 상관성 조사)

  • Purpose: The purpose of this study was to explore the correlates of body mass index (BMI), perceived health status (PHS), and the needs of functional games for the elderly in Korea. Methods: This is a descriptive study with convenience sampling. The data were collected from 363 older adults in S city who completed questionnaires asking about their BMI, PHS, needs for functional games, and general characteristics. Using the SPSS 20.0 program, data were analyzed. Results: The mean age was 65.4(${\pm}4.75$), ranging between 60-83 years old. Among the 363 subjects, 75.2% who were male, 55.9% chose "functional games are necessary," and 25.3% selected "functional games are absolutely necessary" for the elderly. There were statistically significant correlations between current PHS and comparing health to others (r=.827, p<.001), depression (r=-.330, p<.001), age (r=-.113, p=.032), respectively. In addition, there was a statistically significant correlation between comparing health to others and depression (r=-.292, p<.001). Conclusion: The study findings show that older adults have distinct needs and interests for functional games. Thus, we need to develop senior functional games to help make older adults more active with greater enjoyment to gain more health benefits. Interdisciplinary team approaches would work well for developing functional games in Korean considering the preferences of older people are similar.

An Analysis of Somatotype of Elderly Women with Implications for Clothing Construction (노년층여성의 의복구성을 위한 체형분석 연구)

  • Physical measurements of 181 Korean elderly women, aged 60 through 79, living in the Seoul area, were taken in 1981 in order to analyze their body types and to study clothing construction according to their body types. The study was composed of 117 items in all; 35 items in measurements, 1 items in Rohrer index, 68 items in physical index to stature and bust girth, and 14 items in proportion, Coefficients of correlation among body dimensions were used. The results from this analysis are as follows: 1) The most significant changes in body dimensions in advanced age were the decrease of bust height and the increase of both abdomen girth and waist girth. 2) Distinct physical changes did not appear until advanced senescence(70~79 years of age). 3) In the three somatotype groups(thin, average, and plump) categorized by their Rohrer index, the dimensions that remained unchanged in senescence were back waist length and front waist length ; the dimensions that varied most significantly in senescence were the waist measurements. 4) As a result of the analysis of coefficients of correlation among the body dimensions of the 3 figure groups categorized by the Rohrer index, the fundamental dimensions for establishing the size of ready-to-wear garments of elderly women were the stature, bust girth and center back length in upper garments. 5) The result of the analysis of physical index and proportion of the 3 figure groups showed that the more plump a figure might be, the ratio of depth to width measurements increased; the back waist height was higher and trunk enlarged. 6) The 9 figure categorized by their Rohrer index and stature showed that the percentage of women in the category of“normal figure”decreased more in advanced senescence than during early senescence. 7) An analysis of variation of the 9 figure groups, the stature and other items for a short and thin figure were the smallest, and for a tall and plump figure, the largest.

Postoperative chemoradiotherapy versus radiotherapy alone for elderly cervical cancer patients with positive margins, lymph nodes, or parametrial invasion

  • Objective: Women with cervical cancer (CC) found to have positive surgical margins, positive lymph nodes, and/or parametrial invasion receive a survival benefit from postoperative chemoradiotherapy (CRT) vs. radiation therapy (RT) alone. However, older women may not benefit to the same extent, as they are at increased risk of death from non-oncologic causes as well as toxicities from oncologic treatments. This study sought to evaluate whether there was a survival benefit of CRT over RT in elderly patients with cervical cancer. Methods: The National Cancer Database was queried for patients ${\geq}70$ years old with newly diagnosed IA2, IB, or IIA CC and positive margins, parametrial invasion, and/or positive nodes on surgical resection. Statistics included logistic regression, Kaplan-Meier overall survival (OS), and Cox proportional hazards modeling analyses. Results: Altogether, 166 patients met inclusion criteria; 62 (37%) underwent postoperative RT and 104 (63%) underwent postoperative CRT. Younger patients and those living in areas of higher income were less likely to receive CRT, while parametrial invasion and nodal involvement were associated with an increased likelihood (p<0.05 for all). There were no OS differences by treatment type. Subgroup analysis by number of risk factors, as well as each of the 3 risk factors separately, also did not reveal any OS differences between cohorts. Conclusion: In the largest such study to date, older women with postoperative risk factor(s) receiving RT alone experienced similar survival as those undergoing CRT. Although causation is not implied, careful patient selection is paramount to balance treatment-related toxicity risks with theoretical outcome benefits.

  • Ganglioneuroma (GN) is benign neurogenic tumor arising from ganglia of the sympathetic nervous system. They are mostly found at posterior mediastinum, retroperitoneum, and adrenal gland, whereas only 1-5% occurred in the cervical region. GN usually present as a single, painless and slow-growing mass, but multiple cervical occurrences are extremely rare. An 80-year-old woman came to our clinic complained of posterior neck mass for three years. We performed surgical excision, and it was finally diagnosed as GN. We report the unique and rare disease entity with a brief literature review.

$DaVinci^{TM}$ S robot-assisted esophagectomy in a 83-year-old patient with esophageal cancer (고령의 식도암 환자에서 다빈치 S 로봇을 이용한 식도 절제술)

  • The postoperative respiratory complications such as pneumonia and ARDS, are poor prognostic factors after esophagectomy in patients with esophageal cancer. To avoid these complications, there have been attempts to use minimally invasive approach. Recently introduced daVinciTM S surgical system is used in esophagectomy because of its advantages of minimal invasiveness, clear 3-dimensional imaging and precise handling of robotic arms. We report a 83-year-old esophageal cancer patient who underwent daVinciTMS robot-assisted esophagectomy, laparoscopic stomach mobilization followed by cervical esophagogastrostomy.

Maximal Thymectomy for Myasthenia Gravis Management (중증 근무력증치료를 위한 광범위흉선절제술의 효과)

  • Thymectomy has played a central role in the management of myasthenia gravis. Although both the etiology of myasthenia gravis and the reason for improvement after thymectomy remain incompletely explained, complete removal of the thymus is the logical goal of surgical treatment for this disease.From April 1989 to June 1994, maximal thymectomy was performed for 19 cases of myasthenia gravis at Chonnam National University Hospital. The results were as follows:1.Among the 19 cases, male-to-female ratio was 1:1.4, the age ranged 13 years to 71 years, and a diphasic presentation appeared with a peak in young females and a second peak in elderly males;2.Five cases were classified by modified Osserman`s classification as Group I and Group IIa and 14 cases as Group IIb and Group IIc; 3.Histologic examination of the excised thymus glands revealed normality in 5 cases [26% , thymic hyperplasia in 4 [21% , benign thymoma in 8 [42% , and malignant thymoma in 2 [11% ;4.There was no operative mortalities but two deaths occurred during the follow-up periods due to myasthenic crisis and other causes;5.The clinical improvement and the complete remission rates were 85% and 32%, respectively;6.The clinical improvement and the complete remission rates were not so good in patients with thymomas, beeing 70% and 20%, respectively; and 7.Young women with hyperplasia of the thymic tissue tended to show the best response to thymectomy.
