요양병원 간호사의 치매환자 통증관리 수행에 영향을 미치는 요인 (Factors affecting nurse's pain management for patients with dementia)

  • 본 연구는 요양병원 간호사의 치매환자 통증관리수행에 영향을 미치는 요인을 규명하기 위한 서술적 조사연구이다. 대상자는 B시 소재 30개 요양병원의 간호사 168명이며, 자료 수집기간은 2016년 6월 25일에서 7월 15일까지였다. 수집된 자료는 SPSS/WIN 22.0 통계 프로그램을 이용하여, 빈도와 백분율, t-test, ANOVA, Scheff's test, Pearson 상관계수, 다중회귀분석을 시행하였다. 연구 결과 대상자의 치매환자 통증에 대한 지식은 18점 만점에 10.73점으로 낮았으며, 통증관리태도는 평균 $2.98{\pm}0.27$점, 자기효능감은 평균 $3.34{\pm}0.61$점으로 중간보다 높았다, 장애요인은 평균 $2.90{\pm}0.51$점으로 중간 정도였으며, 통증관리수행은 평균 $4.09{\pm}0.52$점으로 높은 수준이었다. 대상자의 특성에 따른 통증관리수행 정도는 연령(p=.046), 요양병원경력(p=.009), 통증교육(p=.004)에 따라 유의한 차이가 있었다. 대상자의 통증지식, 태도, 자기효능감, 장애요인과 통증관리수행의 상관관계를 분석한 결과 태도(r=.21, p=.006), 자기효능감(r=.51, p<.001)과는 양의 상관, 장애요인(r=-.16, p=.035)과는 음의 상관관계를 보였다. 통증관리수행에 영향을 미치는 요인은 자기효능감, 요양병원경력, 통증교육 순이었고 전체 설명력은 30.1%이었다. 따라서 요양병원 간호사의 치매환자 통증관리수행을 높이기 위해서 정확한 통증지식과 자기효능감을 높일 수 있는 프로그램을 개발할 필요가 있다.

Comparative evaluation of efficacy of external vibrating device and counterstimulation on child's dental anxiety and pain perception during local anesthetic administration: a clinical trial

  • Background: This study aimed to evaluate the efficacy of external vibrating devices and counterstimulation on a child's dental anxiety, apprehension, and pain perception during local anesthetic administration. Methods: This was a prospective, randomized, parallel-arm, single-blinded interventional, clinical trial. One hundred children aged 4-11 years, requiring pulp therapy or extraction under local anesthesia (LA), were recruited and allocated equally into two groups (1:1) based on the interventions used: Group BD (n = 50) received vibration using a Buzzy® device {MMJ Labs, Atlanta, GE, USA} as a behavior guidance technique; Group CS (n = 50) received counterstimulation for the same technique. Anxiety levels [Venham's Clinical Anxiety Rating Scale (VCARS), Venham Picture Test (VPT), Pulse oximeter {Gibson, Fingertip Pulse Oximeter}, Beijing, China)] were assessed before, during, and after LA administration, while pain perception [Wong-Baker Faces Pain Rating Scale (WBFPS), Visual Analogue Scale (VAS)] was evaluated immediately after injection. Statistical analysis was performed using the Student's t-test to assess the mean difference between the two groups and the repeated measures ANOVA for testing the mean difference in the pulse rates. Statistical significance was set at P < 0.05. Results: Significant differences in mean pulse rate values were observed in both groups. In contrast, the children in the BD group had higher diminution (P < 0.05), whereas the mean VCARS and VPT scores were conspicuous (P < 0.05). Based on the mean WBFPS and VAS scores, delayed pain perception after LA injection was more prominent in the BD group than in the CS group. Conclusion: External vibration using a Buzzy® device is comparatively better than counterstimulation in alleviating needle-associated anxiety in children requiring extraction and pulpectomy.

  • Purpose: The purpose of the study was to investigate the effects of the Thera-Band exercise program following total knee arthroplasty. Methods: The research design for this study was a nonequivalent control group non-synchronized design. Participants were 30 patients for the experimental group and 30 patients for the control group. The experimental group participated in the Thera-Band exercise program in addition to conventional CPM (continuous passive motion) exercise. The control group received conventional CPM exercise only. Outcome measures were pain, knee flexion range of motion, CRP, and psychological parameters (self-efficacy and fear of falling). Data were analyzed using ${\chi}^2$-test, Fisher's exact test, t-test, and repeated measure ANOVA with SPSS/PC version 21.0. Results: There were significant improvement in self-efficacy, and decreases in pain, and fear of falling in the experimental group compared to the control group. However, no significant differences were found between the two groups for CRP and knee flexion ROM. Conclusion: The Thera-Band exercise program gave an additional benefit over the conventional CPM exercise for patients following total knee arthroplasty, and is recommended for use as an effective nursing intervention for patients after total knee arthroplasty.

Analgesic and anti-inflammatory effect of the aqueous extract of Angelica dahurica

  • Background: Angelica dahurica has been used in various clinical cases. Its taste is hot and its property is warm, dry and nonpoisonous. Its efficacy is to remove wind-damp, cure swelling and edema, exhaust pus, stop itching, rhinitis and leukorrhea. Object: To test through experiment Angelica dahurica's analgesic and anti-inflammatory efficacy. Method: Inject acetic acid as a pain-inducing substance to the mice and measure visceral pain bywrithing reflex. Inject carrageenan that is an edema-inducing substance to the rat's paw and measure volume of edema. Take thermal pain to mice with plantar test and measure paw withdrawal latency. Normal group is non Angelica dahurica-treated group and treated group is Angelica dahurica-treated group. Results: In acetic acid-induced visceral model, treatment with Angelica dahurica suppressed writhing reflex significantlyand dose-dependently. In carrageenan-induced paw edema model, treatment with Angelica dahurica suppressed carrageenan-induced paw edema. In plantar test model, no significant effect on the withdrawal latency of thermal stimulation-induced nociception was observed. Conclusion: Angelica dahurica has analgesic and anti-inflammatory efficacy.

관절염 환자의 자기효능감, 우울 및 건강상태 인지와의 관계 (The Relationship on Self-Efficacy, Depression and the Perception of Health Status in Patients with Arthritis)

  • Purpose: This study was conducted to examine the relationship among self-efficacy, depression and the perception of health status. Method: The subjects of this study consisted of 93 patients with arthritis. The data was collected from July to September, 2004 and it was analyzed with t-tests, ANOVA, Duncan's tests and Pearson Con-elation Coefficients using SAS. Result: Self-efficacy showed significant differences according to the economic status, pain, the number of previous treatment methods, depression, the perception of health status and the number of affected sites. There was a significant correlation between self-efficacy and depression (r=-.48, p<.000), the perception of health status and self-efficacy (r=-.29, p=.01), and perception of health status and depression (r=.34, p.001. Conclusion: Continuous self-management and a proper program on self-efficacy promotion are required for the management of arthritis patients.

체간 조절운동 양식에 따른 만성요통환자의 신체·심리적 기능변화 (Changes of Physicopsychological Function difference on Trunk Control Exercise in Patients with Chronic Low Back Pain)

  • 이 연구는 만성요통환자를 대상으로 요통치료 및 관리를 위하여 정적체간조절운동(요부안정화운동-바닥)과 동적체간조절운동(3차원 체간조절운동)을 실시하고 요통관련 신체 심리 기능 변화를 분석하였다. 만성요통환자 30명을 대상으로 신체적 기능은 균형능력과 요부근활성도를 측정하였고 심리적기능은 신체적 자기효능감을 측정하여 운동 전과 운동 후, 각 운동군간을 비교분석하였다. 균형능력은 전체 지수, 전 후 지수, 좌 우 지수로 측정하였고 요부근활성도는 배곧은근, 바깥빗근, 척추 세움근을 대상으로 측정하였으며 자기효능감은 자기효능감 지수로 측정되었다. 그 결과 두군 모두 신체 심리적기능에서 유의한 개선을 나타내었다. 그러나 신체기능에서 동적체간조절운동군이 더 통계적으로 개선된 것으로 나타났으며 자기효능감은 통계적 차이가 없는 것으로 나타났다. 결론적으로 동적체간조절운동이 정적체간조절 운동보다 신체적 기능 증진에 더 긍정적인 영향을 미친다고 판단된다.

Pregabalin and gabapentin in neuropathic pain management after spinal cord injury: a systematic review and meta-analysis

  • Neuropathic pain after spinal cord injury (SCI) has a significant negative impact on the patients' quality of life. The objective of this systematic review is to examine the safety and efficacy of pregabalin (PGB) and gabapentin (GBP) in the treatment of neuropathic pain due to SCI. PubMed, the Cochrane Library, Embase, Scopus, and the Web of Science were searched up to December 2018. The reference lists of key and review studies were reviewed for additional citations. The quality of the studies was evaluated using the Cochrane Collaboration's tools for assessing the risk of bias. A meta-analysis was performed for primary and secondary outcomes. Eight studies were eligible for inclusion. Meta-analysis of PGB vs. placebo showed that PGB was effective for neuropathic pain (standardized mean difference [SMD] = -0.40; 95% confidence interval [CI]: -0.78, -0.01), anxiety (MD = -0.68; 95% CI: -0.77, -0.59), depression (mean difference [MD] = -0.99; 95% CI: -1.08, -0.89), and sleep interference (MD = -1.08; 95% CI: -1.13, -1.02). Also, GBP was more effective than a placebo for reducing pain. No significant difference was observed between the efficacy of the two drugs (MD = -0.37; 95% CI: -1.67, 0.93). There was no significant difference between the two drugs for discontinuation due to adverse events (risk ratio = 3.00; 95% CI: 0.81, 11.15). PGB and GBP were effective vs. placebos in decreasing neuropathic pain after SCI. Also, there was no significant difference between the two drugs for decreasing pain and adverse events.

The efficacy of selective nerve root block for the long-term outcome of postherpetic neuralgia

  • Background: Several nerve blocks can reduce the incidence of postherpetic neuralgia (PHN) as well as relieve acute zoster-related pain, but the long-term outcome of PHN has not been clearly determined. This study investigated the efficacy of selective nerve root block (SNRB) for herpes zoster (HZ) on the long-term outcome of PHN. Methods: We prospectively conducted an interview of patients who had undergone an SNRB for HZ from January 2006 to December 2016 to evaluate their long-term PHN status. The relationship between the time from HZ onset to the first SNRB and the long-term outcome of PHN was investigated. Results: The data of 67 patients were collected. The patients were allocated to acute ($SNRB{\leq}14days$, n = 16) or subacute (SNRB > 14 days, n = 51) groups. The proportions of cured patients were 62.5% and 25.5% in the acute and subacute groups (P = 0.007), respectively. In logistic regression, an SNRB >14 days was the significant predictor of PHN (adjusted odd ratio, 3.89; 95% confidence interval, 1.02-14.93; P = 0.047). Kaplan-Meier analysis revealed that time from the SNRB to the cure of PHN was significantly shorter in the acute group ($2.4{\pm}0.7yr$) than in the subacute group ($5.0{\pm}0.4yr$; P = 0.003). Conclusions: An early SNRB during the acute stage of HZ (within 14 days) appears to decrease the incidence and shorten the duration of PHN, with a median of 5.0 years of follow-up.

관상동맥질환의 식이이행 예측 요인 (Predicting Factors on Eating Behavior in Coronary Artery Disease Patients)

  • Purpose: This study was aimed to identify the factors that influence eating behaviors in coronary artery disease patients and to create data for nursing which is thought to improve the eating behavior. Method: The study population was coronary artery disease patients who were treated on an outpatient basis. The measurements were eating behavior, diet self-efficacy, perceived-benefits, perceived-seriousness, family support and medical team support. All of the measurement tools above were thoroughly modified to verify validity and reliability. Statistical analysis was done by the SPSS PC 12.0 program. Results: The influencing factors for the eating behavior was diet self-efficacy (${\beta}=0.476$), social support (${\beta}=0.253$), chest pain (${\beta}=0.177$), smoking (${\beta}=-0.173$) and regular exercise (${\beta}=.169$), which explained 46.2%. Conclusion: Eating behaviors of coronary artery disease patients were influenced by diet self-efficacy, family support and the presence of chest pain. Therefore, the development of a program for efficient dietary education that prevents the progression of coronary artery disease is needed.

Efficacy of botulinum toxin in the management of temporomandibular myofascial pain and sleep bruxism

  • Objectives: This study sought to evaluate the efficacy of injecting botulinum toxin into the masseter and temporal muscles in patients with temporomandibular myofascial pain and sleep bruxism. Materials and Methods: The study was conducted based on a clinical record review of 44 patients (36 females and eight males; mean age, 35.70±12.66 years). Patients who underwent the injection of botulinum toxin into the masseter and temporal muscles for the management of temporomandibular myofascial pain and sleep bruxism were included in the study. Patients were diagnosed based on the Diagnostic Criteria for Temporomandibular Disorders. Sleep bruxism was diagnosed according to the criteria defined by the American Academy of Sleep Medicine. The values of the visual analogue scale (VAS) and range of jaw motion, including unassisted maximum mouth opening (MMO), protrusion, and right and left laterotrusion, were observed preoperatively and postoperatively at one-, three-, and six-month follow-up visits. Results: MMO, movements of the right and the left laterotrusion, and protrusion increased significantly (P<0.05), while VAS ratings decreased significantly at the three follow-up points relative to baseline values (P<0.05). Conclusion: Botulinum toxin is an effective treatment for patients with temporomandibular myofascial pain and sleep bruxism.
