Reality Orientation Therapy and Spaced Retrieval Therapy of the applied of CERAD-K type cognitive rehabilitation programs effects: for Mild Alzheimer's dementia (현실감각(ROT)과 시간차 회상 훈련(SRT)을 적용한 CERAD-K형 인지재활 프로그램의 효과 : 경증 알츠하이머 치매를 중심으로)

  • The purpose of this study was to investigate the effect of Reality Orientation Therapy and Spaced Retrieval Therapy of the applied of CERAD-K type cognitive rehabilitation program for mild Alzheimer's patients. The tool used to this study was CERAD-K neuropsychological test consists of Boston Naming, a simple mental status, the word list recall, and configure behavior. A non-equivalent control group pretest-posttest design was used. The participants were 85 mild Alzheimer's patients (experimental group: 32, control group: 53) who registered G Health Center, in Gyeonggi province. Those Data were collected from 10:00 to 11:00, 14:00 to 15:00, twice a week on Tuesday and Friday, 8 session, from September 1 to September 30, 2014. Control group was participated daily routine program only in the G public health center.The data were analyzed using SPSS WIN 21.0 program. The results of this study were as follows;Boston Naming(F=3.22, p<.001), simple mental status(F=5.92, p<.001), and configure behavior(F=4.67, p<.001) were accepted, but word list recall(F=.75 p>.05) was not accepted. The study findings indicate that the Reality Orientation Therapy and Spaced Retrieval Therapy of the applied of CERAD-K type cognitive rehabilitation program is effective and can be recommended as intervention for mild Alzheimer's patients.

The Factors Related to Substance Use among Korean Adolescents: Focusing on Sexual Experiences and Risky Sexual Behaviors (한국 청소년의 약물사용 관련요인: 성경험과 위험 성행동을 중심으로)

  • The purpose of this study is to investigate factors related to substance use focusing on the sexual experiences and risky sexual behaviors among Korean adolescents. This study conducted with a secondary data analysis using the Seventh Korea Youth Risk Behavior Web-based Survey 2011 in South Korea. The study participants selected 75,643 Korean adolescents who were recruited anonymously through national web-based survey. Data were analyzed using descriptive statistics, rao-Scott ${\chi}^2$, and logistic regressions considering complex samples. Results indicated that substance use was significantly associated with sexual experiences such as sexual intercourse with the opposite sex (adjusted OR=5.691; 95% CI=4.223-7.670), sexual intercourse with the same sex (adjusted OR=21.180; 95% CI=14.704-30.507) and risky sexual behaviors such as sexual assaults (adjusted OR=9.936; 95% CI=7.217-13.680), date rape (adjusted OR=16.979; 95% CI=11.648-24.751), sexual intercourse after drinking (adjusted OR=6.564; 95% CI=3.986-10.810), unexpected pregnancies (adjusted OR=18.628; 95% CI=8.476-40.936), and sexually transmitted diseases (adjusted OR=21.872; 95% CI=13.920-34.367) in Korean adolescents. Findings of this study show that adolescents who use substances are more likely to engage in risky sexual behaviors. Therefore, intervention program should be designed to consider both substance use and sexual problems and to include the effective approaches at the family and school level.

  • This research analyzes the decision-making status of expected graduates depending on their Holland preference type, as well as the influence career worries have on vocational decision status. By discovering this mediating effect, career counselors can carry out counseling based on the variety of career worries college students have, further supporting self-actualization of the client. As the previous studies on group counseling programs and the effects of career counseling have not yet considered the difference between various types of career worries and carried out identical programs and counseling method, this study is significant in marking the emergence of differentiated counseling intervention and strategy for more effective career counseling. This research is based on a survey conducted on 606 expected graduates attending six 4-year universities in Seoul, Gyeonggi, Chungcheong areas. The students surveyed are in the second semester of their senior (4th)years, during two weeks from november 4, 2013. First, the results showed that the level of career worries and the type of career worries differed between college students that have different types of Holland preference, and such difference was meaningful. Second, examination on the influences that career worries have on different vocational decisions depending on Holland preference types, found that the state of anxiety-free and anxiety toward career and dependence, as one of vocational decision types, together work as mediators. In conclusion, the significance of this research lies on that it reveals the mediating effects that career worries have on vocational decision status of college students.

The Long-Term Effects of High-Frequency Transcutaneous Electrical Nerve Stimulation(TENS) on the Lower Limb Spasticity and the Balance in the Chronic Stroke Patients (장기간 고빈도 경피신경전기자극이 뇌졸중 환자의 하지 경직 및 균형에 미치는 영향)

  • The purpose of this study was to investigate effects of the long-term high-frequency transcutaneous electrical nerve stimulation(TENS) on the spasticity and the balance in chronic stroke patients. Twenty-six subjects with spasticity over lower limbs were allocated randomly into two groups under standard rehabilitation: (1) TENS group, (2) placebo-TENS group. TENS stimulation was applied on the both the gastrocnemius for 30 minutes, 5 days a week for 4 weeks(100 Hz, 0.25 ms, 2 times sensory threshold). The Modified Ashworth Scale(MAS) and Hand-held manual muscle tester were used to assess the ankle plantarflexor spasticity. Balance function under three conditions was measured by using force-plate and the amount of postural sway was assessed; in (1) the condition of standing with eyes opened, (2) with eyes closed and (3) the condition of standing on unstable surface with eyes opened. Both groups showed significant improvement in spasticity and balance function after treatment for 4 weeks(p<.05). Especially, TENS group showed a significant reduction of spasticity compared to placebo-TENS group(p<.05). These results suggested that additional stimulation of a long-term high-frequency TENS to standard rehabilitation induced an improved balance function and a spasticity reduction. The long-term application of high-frequency TENS will be an effective intervention for reducing spasticity and increasing balance ability in the chronic stroke patients.

Comparisons of Fracture Types and Pelvic Angiographic Findings in Hemodynamically Unstable Pelvic Bone Fracture (혈역학적으로 불안정한 골반골 골절 환자에서 골반골 골절 소견과 혈관조영술 소견의 비교)

  • Purpose: Hemorrhagic shock is the leading cause of death in patients with pelvic bone fractures. The majority of blood loss is due to injured pelvic arteries and retroperitoneal veins and to bleeding from the fracture site itself. Pelvic angiography and embolization of injured vessels is an effective way to control continuous bleeding. However, identifying the bleeding focus in hemodynamically unstable patients before diagnostic intervention is difficult. The purpose of this study was to determine the correlation between fracture patterns in hemodynamically unstable patients with pelvic fractures and later pelvic angiography findings. Methods: We performed a retrospective study of 21 hemodynamically unstable patients with pelvic fractures admitted to our emergency department between April 2001 to April 2006. All 21 patients underwent pelvic angiography. Pelvic fractures were assessed according to the Tile's classification and the degree of injury was assessed using the Injury Severity Score (ISS) and Revised Trauma Score (RTS). The hemodynamic status of the patients was defined using vital signs, base excess, and blood lactate. Fracture patterns were compared with hemodynamic status and angiography findings. Results: In the 5year study period, 21 hemodynamically unstable pelvic bone fracture patients were admitted; ten were men (47.6%), and 11 were women (52.4%). The mean age was 41.1 years (range: ${\pm}20.1$). Of the 21 embolization was performed in 6 patient (28.6%): 1 patient of the 5 unstable pelvic bone fracture patients (20%), and 5 patients of 16 the stable pelvic bone fracture patients (31.3%). There were no significant differences between the RTS (p=0.587) and embolization rate (p=0.774) for either the stable patients or the unstable patients. Patients with arterial injury on angiography had a lower RTS compared with patients without arterial injury but there was no significant difference in ISS between the two groups. The angiographic injured sites were five internal femoral arteries and one external femoral artery. Conclusion: The findings in this study suggest that the pelvic fracture pattern in hemodynamically unstable patients with pelvic fractures does not correlate with pelvic angiography findings.

Integrated Schistosomiasis and Soil-Transmitted Helminthiasis Control over Five Years on Kome Island, Tanzania

  • Integrated control strategies are important for sustainable control of schistosomiasis and soil-transmitted helminthiasis, despite their challenges for their effective implementation. With the support of Good Neighbors International in collaboration with National Institute of Medical Research, Mwanza, Tanzania, integrated control applying mass drug administration (MDA), health education using PHAST, and improved safe water supply has been implemented on Kome Island over 5 years for controlling schistosomiasis and soil-transmitted helminths (STHs). Baseline surveys for schistosomiasis and STHs was conducted before implementation of any integrated control strategies, followed by 4 cross-sectional follow-up surveys on randomly selected samples of schoolchildren and adults in 10 primary schools and 8 villages, respectively, on Kome islands. Those follow-up surveys were conducted for impact evaluation after introduction of control strategies interventions in the study area. Five rounds of MDA have been implemented from 2009 along with PHAST and improved water supply with pumped wells as other control strategies for complementing MDA. A remarkable steady decline of schistosomiasis and STHs was observed from 2009 to 2012 with significant trends in their prevalence decline, and thereafter infection rate has remained at a low sustainable control. By the third follow-up survey in 2012, Schistosoma mansoni infection prevalence was reduced by 90.5% and hookworm by 93.3% among schoolchildren while in adults the corresponding reduction was 83.2% and 56.9%, respectively. Integrated control strategies have successfully reduced S. mansoni and STH infection status to a lower level. This study further suggests that monitoring and evaluation is a crucial component of any large-scale STH and schistosomiasis intervention.

Cost-Effectiveness Analysis of a Hyperlipidemia Mass Screening Program in Korea (성인 고지혈증 선별 검사의 비용-효과 분석)

  • Objective : Until now, there have been no evidence-based guidelines produced for the mass screening of hyperlipidemia cases in Korea. This study was done to find the most efficient strategy for a hyperlipidemia-screening program among Korean adults, Method : Seven alternative strategies for hyperlipidemia screening were formulated and compared ir terms of cost-effectiveness. Cost and effectiveness were estimated from social perspectives and using a two-stage screening process (initial testing and additional testing for positives from the first test). A computerized database (based on persons who had visited a health promotion center in one teaching hospital for a routine health check-up) was used to determine the cost and the outcome of various strategies. Official data was used in calculating direct and indirect costs. Effectiveness was measured according to the number of persons who needed clinical intervention for hyperlipidemia. A stratified analysis, considering age group and sex, was then done. Sensitivity analyses, focusing on several uncertain parameters, were also done. Results : Of the seven test alternatives available, the most cost-effective strategy was a screening program, which consisted of an initial test of total cholesterol, high-density lipoprotein cholesterol and triglyceride. There was some variation in the rank of the cost-effectiveness ratios for the seven alternatives dependent on age group or gender. Conclusions : Current hyperlipidemia screening practice, for National Health Insurance beneficiaries, tests only the total cholesterol level with a cut-off value of 260mg/dl as an initial screening test. It is not the best strategy for cost-effectiveness, and should be modified. Different screening strategies taking age group and sex into account should be developed and used for the efficient mass screening of hyperlipidemia cases among Korean adults.

The effect of community based participatory obesity management program in obese school children (학령기 비만아동을 위한 지역사회 참여형 비만관리 프로그램의 효과)

  • The aim of this study was to develop a community based participatory obesity control program and measure the effects of the program on obesity, exercise habit, eating habit and life habits among obese elementary school children. The community based participatory obesity control program was provided for the experimental group of $4^{th}$ grade elementary school students for 10 weeks. The program included obesity related health educations and counseling once a week, jump rope classes twice a week and folk game classes held once a week. Method: There were 24 students with obesity in the experimental and 27 in the control groups. The collected data were analyzed by the Kolmogorov-Smirnov test, ${\chi}^2$-test, independent t-test and repeated measure ANOVA. Results: After 10weeks of participation in the program, the experimental group showed significantly lower Body Mass Index (p<.001) and waist hip ratio (p=.036), significantly higher exercise habit (p=.004), eating habit (p=.003) and life habit (p=.001) than those in the control group. Conclusion: This study provides evidence that community based participatory obesity management program is effective nursing intervention for managing child obesity.

The Effect of Mental Practice with Task Activities on Upper Limbs Movement Speed and Accuracy of Ideomotor Apraxia Patients : Single Subject Experimental Research (상상연습과 과제활동 병행 치료가 관념운동 실행증 환자의 상지 움직임 속도와 정확성에 미치는 효과: 단일사례 실험연구)

  • Objective : The purpose of this study is to investigate the effect of mental practice with task activities through upper limbs movement speed and accuracy in ideomotor apraxia patient. Methods : As a single subject research design with multiple baseline across individuals, the patients were two ideomotor apraxia patients, employed in this study. The employed program included receiving mental practice and task activities, measuring change of the total performance times, time to put down the last cups and amount of water in cups for 4 weeks, including baseline and intervention periods. The subject were measured by Ideomotor apraxia test and MFT were analyzed. The analyses were performed using visually and two-standard deviation band method. Results : All subjects were a reduction in symptoms of apraxia, but noticeable change in upper limbs function was not there. Total time of performance from 376.67 to 355.24 seconds, 391.33 seconds to 434.60 seconds, reduced. Put down the cup of performance time from 15.67 to 13.30 seconds, 20.80 seconds to 10.73 seconds, reduced. The amount of water transferred from 52.38ml to 49.70ml, 50.89ml to 50.09ml, all results were closer to 50ml. Conclusion : As a result of this study, mental practice with task activities can be an effective method to ideomotor apraxia patients. This study would serve a fundamental data on ideomotor apraxia patients to the further studies.

  • This research utilized the Grounded Theory and Giorgi's phenomenological research method to analyze the experience of battered women who are maintaining a non-violent marriage relationship. Due to the low accessibility of subjects and the distinctiveness of the topic, I have selected 4 participants and conducted in-depth interviews. As for data analysis results, there were 5 superior component elements and 19 inferior component elements for the experience of maintaining a non-violent marriage relationship and these appeared according to time. Thus, it can be analyzed as follows: 'the vicious cycle stage of violence: caught in a trap,' 'stage of breakaway from vicious violence cycle: making the self-rescue measures,' 'entry to a new track: retrieval of autonomy,' 'stage of forming a non-violence track: preventing the recurrence of the violence,' 'stage of maintaining non-violent track: conversion to non-violent relationship.' This research result will be useful in seeking an effective social welfare intervention plan for successful non-violent relationship in order to help 50% of the battered women from family violence who wish to maintain marriage.
