National Standard Food Composition Tables Provide the Infrastructure for Food and Nutrition Research According to Policy and Industry (식품 영양 연구, 정책, 산업의 기반이 되는 국가표준식품성분표의 활용)

  • The National Standard Food Composition Table published by the Rural Development Administration (RDA) provides the foundations in research, nutrition monitoring, policy and dietary practices in Korea. This databases consists of several sets of data including food descriptions, nutrients, portion weights, and source of data. The National Standard Food Composition Table have been published since 1970 and, recently, new version (8th edition) of Food Composition Table which has quantitative and qualitative nutrient data is released in 2011. In addition, the User-friendly Food Composition Table is divided into adult, children, and elderly categories depending on the subjects because we need different nutrients according to various ages. The Tables of Food Functional Composition is firstly edited in 2009. RDA published the minerals and fatty acids composition table, tables of amino acid, fat-soluble vitamin composition table, and the cholesterol table. The resulting database will be widely used. The users of the databases are from diverse fields, includeing federal agencies, the food industry, health professionals, restaurants, software application developers, academia and research organizations, international organizations, and foreign governments ect. Therefore, consistent improvements of the database is important, so that people can better address such health challenges by providing reliable and accurate data.

Seismic Evaluation of Beam-Column Joint Specimens of RC Special Moment Frames (철근콘크리트 특수모멘트골조의 보-기둥 접합부 실험체의 내진성능평가)

  • This study summarizes the results of a research project aimed at investigating the inelastic rotation capacity of beam-column joints of reinforced concrete special moment frames. All of the test specimens were classified as special moment frame (SMF), based on the design and detailing requirements of the ACI 318-02 provisions. The acceptance criteria, originally defined for steel moment frame connections in the 1997 edition of the AISC Seismic provisions, were used to evaluate the beam-column joints of the reinforced concrete moment frames. A total of 39 test specimens were examined in detail. Most of the joints that satisfy the design requirements for special moment frame structures were found to be ductile up to a plastic rotation of 3% without any major degradation in strength. This is mainly due to the stringent ACI 318-02 requirements for special moment frame joints. The presence of transverse beams increases confinement and shear resistance of joints, which results in better performance than for joints without transverse beams. All of the SMF connections that satisfy the ACI 318-02 limitations on joint shear stress turned out to meet the acceptance criteria.

The Ratio-Based N Staging System Can More Accurately Reflect the Prognosis of T4 Gastric Cancer Patients with D2 Lymphadenectomy Compared with the 7th American Joint Committee on Cancer/Union for International Cancer Control Staging System

  • Purpose: The utility of N classification has been questioned after the 7th edition of the American Joint Committee on Cancer (AJCC)/Union for International Cancer Control (UICC) was published. We evaluated the correlation between ratio-based N (rN) classification with the overall survival of pathological T4 gastric cancer patients who underwent D2 lymphadenectomy. Materials and Methods: We reviewed 222 cases of advanced gastric cancer patients who underwent curative gastrectomy between January 2006 and December 2015. The T4 gastric cancer patents were classified into four groups according to the lymph node ratio (the number of metastatic lymph nodes divided by the retrieved lymph nodes): rN0, 0%; rN1, ${\leq}13.3%$; rN2, ${\leq}40.0%$; and rN3, >40.0%. Results: The rN stage showed a large down stage migration compared with pathological T4N3 (AJCC/UICC). There was a significant difference in overall survival between rN2 and rN3 groups in patients with pT4N3 (P=0.013). In contrast, the difference in metastatic lymph nodes was not significant in these patients (${\geq}16$ vs. <15; P=0.177). In addition, the rN staging system showed a more distinct difference in overall survival than the pN staging system for pathological T4 gastric cancer patients. Conclusions: Our results confirm that rN staging could be a good alternative for pathological T4 gastric cancer patients who undergo D2 lymphadenectomy. However, before applying this system to gastric cancer patients who undergo D2 lymphadenectomy, a larger sample size is required to further evaluate the usefulness of the rN staging system for all stages, including less advanced stages.

Need to Pay More Attention to Attendance at Follow-Up Consultation after Cancer Screening in Smokers and Drinkers

  • Background: Follow-up clinical consultations could improve overall health status as well provide knowledge and education for cancer prevention. Materials and Methods: This is the cross-sectional study using the Korean Community Health Survey (KCHS) 6th edition for 2012, with 115,083 respondents who underwent cancer checkups selected as subjects. Associations between the presence of consultation and the socioeconomic status were determined using statistical methods with the SAS 9.3 statistical package (Cary, NC, USA). Findings: Among the recipients, 32,179 (28.0%) received clinical consultations after cancer screenings. Those in rural areas (odds ratio, OR=0.71, 95% confidence interval (CI), 0.69-0.73) visited follow-up clinics less frequently than did those in urban areas. Starting at the elementary school level, as the education level increased to middle school (OR=1.26, 95% CI: 1.19-1.34), high school (OR=1.29, 95% CI: 1.23-1.36) or college (OR=1.76, 95% CI: 1.65-1.89), the participation rates also increased. When compared with the lowest quartile group, the quartile income level showed a statistical trend and difference as follows: second lowest quartile (OR=1.11, 95% CI: 1.07-1.16), third lowest (OR=1.12, 95% CI: 1.07-1.17) and highest quartile income (OR=1.29, 95% CI: 1.23-1.35). In addition, the people with economic activities (OR=0.87, 95% CI: 0.84-0.90) visited follow-up clinics less frequently than did the others. Current smokers (OR=0.93, 95% CI: 0.89-0.98) and inveterate drinkers (OR=0.88, 95% CI: 0.85-0.94) had a tendency to visit less often than did non-smokers and other drinkers with all cancers combined. Interpretation: We suggest primary prevention through lifestyle modifications including smoking and drinking, and environmental interventions may offer the most cost-effective approach to reduce the cancer burden.

Review of Acupuncture Manipulation in Clinical Trials (침치료가 사용된 임상연구에서의 수기법 활용에 대한 고찰)

  • Objectives : This study aims to review the actual use of acupuncture manipulation in clinical trials and to discuss the directions of studies, to be conducted in the future. Methods : Through five search engines, clinical trial papers were collected, which had involved the use of acupuncture treatment since 2001 when STRICTA was published. The selected papers were classified based on whether acupuncture manipulation and De-Qi were stated. The comparison status of domestic and foreign, status by year, target diseases, and therapeutic interventions were also investigated. Results : 54 % domestic and 69 % foreign papers mentioned acupuncture manipulation or De-Qi. Among them, acupuncture manipulation was used in 40 % domestic and 53 % foreign papers. There was no significant difference around 2010 when the revised edition of STRICTA was published. The use of acupuncture manipulation was the highest in studies that targeted musculoskeletal disorders. Besides that, the use of acupuncture manipulation was higher in cases where acupuncture was used for therapeutic interventions than it was for basic manipulation or combination therapy. Conclusion : Compared to the importance of acupuncture manipulation and De-Qi, there was actually a smaller number of papers that clearly suggested information on acupuncture manipulation and De-Qi. The reason is considered to be a lack of studies to establish the mechanism and effectiveness of each acupuncture manipulation method and studies that objectified the amount of stimulation incurred by the acupuncture manipulation methods. It is necessary to properly utilize acupuncture manipulation by actively conducting these studies.

A Literature Review of Effectiveness on the Gongjin-dan (Gongchen-dan) (공진단의 효과에 대한 문헌적 고찰)

  • Objectives The purpose of this study was to investigate the effectiveness of the Gongjin-dan (Gongchen-dan, here in after GJD) in order to obtain the evidence for clinical application. Methods The GJD-related articles published from 1990 to 2013 were searched using "Korean Traditional Knowledge Portal", "Oriental Medicine Advanced Searching Integrated System (OASIS)", "Korean Association of Medical Journal Edition (Koreamed)", "Research Information Services (RISS4U)", "Korean Medicine Database (KMbase)", "National Discovery for Science Leader (NDSL)", "PubMed", "China National Knowledge Infrastructure (CNKI)". The search keywords were "Gongjin-dan", "Gongchen-dan". Thirty-nine articles were obtained. After excluding the eighteen article which did not meet inclusion criteria, finally twenty-one articles were included; five clinical articles and sixteen experimental articles. Results In clinical studies, GJD has the various effectiveness in cardiovascular diseases, alcoholic hepatitis, mild dementia, anemia. Also experimental studies related to the GJD show a variety of effects, such as anti-oxidative activity, neuroprotective activity, hepatoprotective activity, anti-inflammatory activity, immunological activity, reproductive recovery activity with fewer side-effects. Conclusions It has been suggested that there are various effects of GJD in treating a wide-range disease. However, in order to put GJD to use for many kinds of diseases in more reasonable ways, it is needed to publish well-design clinical trial based on the variety of results of experimental studies.

Progress and Prospect of Research on Old Maps in Korea (우리나라 고지도의 연구 동향과 과제)

  • In Korean academic societies, old maps has not yet been properly investigated in terms of their genealogy, classification, detailed place names, historical backgrounds and the other aspects. With publication of the bibliographies and papers on old maps reserved in museum and library, the scope of research enlarged gradually its scope from 1970s. In 1980s, with the development of theoretical geography, scientific analysis were applied to investigate the projection method of Daedongyeo-jido. The 1990s proved a prominent decade for researches. The photo-copies of old maps enabled researchers to investigate the in-depth comparative study. The more important thing is that old maps became to be powerful instrument in the research of historical geography, such as territorial disputes and marine name(東海). And county old maps compiled by region became to be regional-cultural contents of local areas. Important issues in old map research in Korean academic societies are about Cheonha-do which is unique old world map in Korea, grid-system projection in old county maps and the genealogy of Daedongyeo-jido(manuscript and block print edition). This study shows that bibliography of all old maps preserved in each library and museum should be standardized. This could enable the exchange of information of old maps between institutes. The more important thing is that conciliation of human, social and natural sciences should be applied in the research of old maps.

New Prognostic Significance of Malignant Pleural Effusion In Patients with Non-Small Cell Lung Cancer (비소세포폐암의 예후 결정에 있어 악성 흉수의 새로운 의의)

  • Several studies showed that the survival rate of stage IIIB disease with malignant pleural effusion is worse than stage IIIB disease without malignant effusion. But, malignant pleural effusion was considered T4. To analyze changes the survival time for malignant pleural effusion, in the seventh revision of TNM classification for lung cancer. The records of all patients had to have either a histological or cytological diagnosis of non-small cell lung cancer (NSCLC), who were admitted to Wonkwang university hospital between January 2004 and December 2006 were reviewed retrospectively. We evaluated the survival time of 187 patients with advanced lung cancer with and without malignant pleural effusion. This included the pleural effusion or nodule M1 a (pleural dissemination, currently classified as T4), nodule(s) in the other lung M1 a (contralateral lung nodule, currently classified as M1), nodule(s) with the same lobe as the primary tumor T3 (currently classified as T4), other T4 factors T4 (T4 MO anyN), and extrathoracic sites of disease M1b (distant metastasis, currently classified M1). Among the 187 patients, T4anyNMO was 57 patients in the current TNM classification. In the next edition of the TNM classification, T4MOanyN-T4 (excluding same lobe nodules) was 12 patients, pleural dissemiantion-M1a was 45 patients, contralateral lung nodule(s)-M1a was 7 patients, and metastatic disease-M1b was 55 patients. We compared the survival time for these groups. Survival time was 11 months, 8 months, 11 months, and 4 months. The survival time of malignant pleural effusion was shorter than other T4 factors without pleural effusion. But, there was no remarkable difference in statistics due to small cases (p=0.23). We strongly suggest that malignant pleural effusion in advanced NSCLC will be categorized with metastatic disease.

  • Medical electrical equipment - Part 1: General requirement for basic safety and essential performance of MFDS was revised as 3th edition and Medical electrical equipment Part 2-54: Particular requirements for the basic safety and essential performance of X-ray equipment will be expected to be announced as notification. Therefore this technical report was written to introduce provision of the particular requirements, replacement, addition, amendment. The purpose of this particular requirements is to secure requirements for basic safety and essential performance of X-ray equipment for radiography and radioscopy. X-ray high voltage generator, mechanical protective device, protection against radiation is included in this particular requirements. Medical electrical equipment - Part 1, Part 1-2, Part 1-3 is applied to this particular requirements. If the requirements is announced as notification, It is expected to widen understanding for basic safety and essential performance of X-ray equipment for radiography and radioscopy and play a part to internationalize of medical equipments.

Performance Evaluation of Domestic Prototype Dose Area Product Meter SFT-1 (국산 프로토타입 면적선량계 SFT-1의 성능평가)

  • The importance of radiation dose display of medical X-ray equipment was emphasized, while third edition of IEC(International Electrotechnical Commission) 60601 started to apply. The existing medical X-ray equipment selected a method for attaching the DAP(Dose Area Product) meter when the dose display. However, because the DAP meter was dependent on all of the income, And it did not yet produced in Korea. So, we received the support of Seoul R&BD Program(Grants No. C1152055) to produce DAP meter prototype of the Domestically technology. In this study, the performance of this prototype was evaluated by comparing the German company's product Evaluation item was an electronic capture performance, radiation dose dependence, radiation quality dependence, energy transmittance, repeatability, light transmittance of 6 entries. And IEC 60580 was based on this evaluation. Evaluation results were electronic capture performance intrinsic error 9.5%, radiation dose dependence limits of variation 1%, repeatability coefficient of variation 2%, energy transmittance 91% each assessment was passed. However radiation quality dependence limits of variation 29%, light transmittance 55% was less than acceptance criteria.
