A Case of Dual Ectopic Thyroid (이중 이소성 갑상선 1예)

  • Ectopic thyroid is an uncommon embryologic aberration characterized by the presence of thyroid tissue in a site other than its usual pre-tracheal location. Dual ectopic thyroid is extremely rare, only few cases have been reported in the literature. Physical examination, laboratory test and radiologic examinations are usually performed for the diagnosis and therapeutic plan of dual ectopic thyroid. We report a 36-year old female with dual ectopic thyroid in lingual and hyoid regions but without thyroid tissue in its normal area. The diagnosis was made through pathologic examination after surgical resection.

Not a neuroendocrine tumor: A case of hepatocellular carcinoma in ectopic liver tissue in the pancreas

  • Hepatocellular carcinoma (HCC) accounts for most of the hepatic neoplasms and can also occur in ectopic liver tissue. We present a case of a 55-year-old male complaining of weight loss. The imaging studies reported a 2.9 cm nodule in the pancreatic body, with a neuroendocrine tumor diagnosis by cytology. A corpo-caudal pancreatectomy was performed. Pathology showed a well-differentiated HCC developed in ectopic liver tissue with free margins and no lymph node metastases. HCC presenting in ectopic liver tissue is rare. In this case, the preoperative study did not establish the diagnosis, warranting the need for suspicion of this neoplasm.

Paragonimiasis in the Abdominal Subcutaneous Tissue: A Case Report (복부 피하조직으로의 폐흡충증 이소기생 치험례)

  • Purpose: Paragonimiasis is infectious disease occurred by Paragonimus Westermani, which invades into human body as a final host. Habitual eating the freshwater crab or crawfish unboiled is one of the reason of infection. Paragonimiasis raged in 1970s in Korea, Japan, China and other Asian countries but the incidence decreased rapidly. Once people eat infected second host, parasite penetrates the duodenal wall and migrates to the lung. During this migration period, the parasite can migrate to other organ, such as brain, spinal cord, liver and subcutaneous tissue, but the cases are rarely reported. The objective of our study is to present our experience of the ectopic migration of parasite to the subcutaneous tissue of the abdomen, which was easily treated with excision and Praziquantel medication. Methods: A 59-year-old woman who likes eating unboiled freshwater crab was diagnosed as Paragonimiasis 15 months ago. Her symptoms were fever and cough, and she was treated with Praziquantel medication. 3 months after discharge, she visited our hospital with left pleuritic chest pain, cough with fever, and palpable mass formation on left lower quadrant of the abdomen. Wedge resection of the left lung and Praziquantel medication was maintained for a week. Nevertheless, fever persisted after the treatment. The patient received total excision of the abdominal soft tissue mass, and the fever was relieved. Results: Pathologic findings of the mass showed multiple cyst and abscess formation with crystal structure which is suspicious parts of the parasite or calcified egg shells. Uncontrolled fever was relieved after the operation, and there was no evidence of recurrent Paragonimiasis and ectopic migration for 1 year follow up period. Conclusion: Ectopic migration of Paragonimus is rare, but multiple organ can be involved. Patient with Paragominiasis who was refractory in fever control after Praziquantel medication or surgical evaluation of the lung should be considered as ectopic migration of the Paragonimiasis.

Effects of Ouhyul Herbal Acupuncture on Experimentally Induced Endometrosis in Rats (중성어혈약침이 자궁내막증 백서(白鼠)에 미치는 영향)

  • Purpose : Endometriosis is characterized by the ectopic growth of uterine tissue in various extrauterine locations. This study examined the macroscopic, hormonal and immunological effects of Ouhyul Herbal Acupuncture(OHA) therapy on rats with experimentally induced endometriosis. Methods : Endometrial tissue was implanted in the serosal wall of the small intestine in rats. The rats were divided randomly into an experimental group and control group. The experimental group was treated with OHA on Kwanwon(CV4) three times per week and the control group was administrated normal saline every day. After 6 weeks, the size of the ectopic uterine tissue was estimated and the serum progesterone, estradiol and cytokines($TNF-{\alpha}$, IL-2, IL-4, IL-6, IL-l0) concentrations were analyzed. Results : The size of the ectopic uterine tissue in the experimantal group was slightly smaller than that in the control group. The estradiol, IL-4 and IL-6 concentrations were significantly lower and the IL-l0 concentration was significantly higher in the serum of the experimental group than in the control group. There was no significant difference in the concentrations of the other cytokines. Conclusion : These results suggest that OHA might be an effective method for treating dometriosis.

  • Esophageal stenosis due to ectopic tracheobronchial remnants is an uncommon anomaly and only few cases were reported in the literature. We have experienced two case of esophageal stenosis due to ectopic tracheobronchial remnants and performed operative correction. The one patient was a five year old male with a chief complaint of swallowing difficulty from birth and the other was a twenty-three year old female with a slowly incresing symptom of dysphagia for twenty years. both of them were performed surgical correction by esophagectomy of the stenotic portion and esophago-gastrostomy with anti-reflux procedures. The resected specimens of the these two patients showed ectopic tracheobronchial chondroepithelial tissue withinthe esophageal wall histipathologically. Postoperative course was uneventful and have been in good condition without any problems.

  • Ectopic pancreas is defined as an abnormally located pancreatic tissue not sufficiently connected with the normal pancreas, which rarely occurs in neonates. To our knowledge, only a few cases of ectopic pancreas have been reported in newborns in South Korea. We report a case of ectopic pancreas as the cause of intussusception and jejunal atresia in a newborn. This clinical association is extremely rare, and this is the first report in South Korea.

  • Supernumerary ectopic limb(s) (SEL) is a congenital anomaly defined as the presence of accessory limb(s) attached to various body regions. This paper describes a case of SEL with ectopic lung and ectopia cordis in a newborn calf, based on macroscopic, microscopic and radiographic findings. External features of multiple congenital anomalies included an ectopic lung growing over the middle of the backbone and covered with normal haired skin. Ectopia cordis was found in the abdominal cavity and attached to the liver. Two extra abnormal limbs originated separately from within the ectopic lung. Most of the abdominal organs were exposed to the outside through the opened abdominal cavity. Microscopically the ectopic lung tissue had edema in the connective tissue around the bronchus and artery. Changes in other organs included congestion of the renal medulla, infiltration of inflammatory cells (lymphocytes and eosinophils) around the hepatic portal tract, and edema surrounding blood vessels and neurons in the brain. The rudimentary humerus of the forelimb was attached to the thoracic spine, as viewed radiographically. The hindlimb was consisted of an irregularly shaped femur, short tibia and fibula, two tarsal bones, one metatarsal bone, and three phalanges. This is the first description of congenital anomalies involving the SEL, ectopic lung and ectopia cordis in a calf.

Ectopic expression of $ARR1{\Delta}DDK$ in tobacco: alteration of cell fate in root tip region and shoot organogenesis in cultured segments

  • A specific deleted version of ARABIDOPSIS RESPONSE REGULATOR1 (ARR1) lacking the signal receiver domain (1.152 amino acids)-coding sequence, referred to as $ARR1{\Delta}DDK$, was amplified using Arabidopsis thaliana cDNA prepared from adult leaves and transferred into the genome of Nicotiana tabacum cv. Samsun under the transcriptional control of a ${\beta}$-estradiol-inducible expression system. The ectopic expression of $ARR1{\Delta}DDK$ affected the morphology of transgenic seedlings and their segments in vitro. In the presence of an inducer, ${\beta}$-estradiol, ectopic expression of $ARR1{\Delta}DDK$ induced only the formation of soft, pseudo-bulbous tissue in the root tip region of intact seedlings, which appeared similar to callus generated on a hypocotyl segment in the presence of 2,4-D and 6-benzyladenine (BA), both at $1\;{\mu}M$. Those callus tissues on the root tip region could not generate shoots unless $1\;{\mu}M$ BA was supplied. In segment culture, ectopic expression of $ARR1{\Delta}DDK$ induced calluslike tissue around the cut-end of cotyledon and hypocotyl segments with occasional shoot formation, suggesting that the expression of $ARR1{\Delta}DDK$ could substitute for the effects of cytokinin on these segments. Additionally, treatment with only ${\beta}$-estradiol induced NtWUS, a WUS ortholog in tobacco, which was detected during the process of callus tissue formation in the root tip region and also in cotyledon or hypocotyl segments. These findings suggest that the NtWUS might be associated in the transdifferentiation process caused by the functional regulation of $ARR1{\Delta}DDK$ in transgenic tobacco seedlings.

  • Ectopic thyroid gland is a defined as thyroid tissue located in a site other than its usual pre-tracheal location. Ultrasound examination, CT scan, thyroid scan, and thyroid iodine uptake test are all valuable modalities for a differential diagnosis. Although most of cases are asymptomatic, treatment is indicated when there are symptoms related to progressive growth of mass. This article reports a ectopic thyroid cyst in the anterior neck that was similar to deep neck infection accompanied by acute airway obstruction.

A Case of Pseudocyst Originated from Ectopic Pancreas in the Transverse Mesocolon Associated with Colonic Duplication (대장중복증과 동반된 횡행결장간막의 이소성 췌장에서 유래된 가성낭종 1예)

  • We have treated a case of pseudocyst of transverse mesocolon in 3-year-old male child. Operative findings demonstrated that the pseudocyst originated in the transverse mesocolon, and was not connected to the pancreas. A colonic duplication was found incidentally near the pseudocyst. On microscopic examination, ectopic pancreatic tissue was noted in the transverse mesocolon. This pseudocyst was thought to have originated from the ectopic pancreas of the transverse mesocolon. This is the first reported case of pseudocyst originated from ectopic pancreas of the transverse mesocolon, combined with a colonic duplication. The pathogenesis is discussed.
