Ectopic ureters and urachal remnant in a dog (딴곳요관증과 요막관 잔존증 증례 보고)

  • An intact male, 3-month-old, Great Pyrenees mountain dog, weighing 7.3 kg with a history of severe hematuria during 2 months was referred. Hematology showed mild leukocytosis. There were no remarkable findings in abdominal radiographs due to ingesta and gas in the gastrointestinal tracts. Ultrasonographs showed bilateral hydronephrosis, bilateral dilated and tortuous ureters, and hyperechoic sludges in the urinary bladder (UB). And a dilated cavity was identified cranial to the UB. Excretory urography and cystography were performed to examine the cause of hydronephrosis and an abnormal cavity cranial to the UB. Bilateral ureters were tortuously dilated and opened into the craniodorsal part of the UB. An abnormal cavity was connected to the UB at the left cranioventral part of the UB. Therefore bilateral ectopic ureters and urachal remnant were diagnosed.

A Tuberculoma in the Left Lower Lobe of Lung That Was Erroneously Diagnosed as Ectopic Liver (이소성 간으로 오진된 좌폐 하엽의 결핵종)

  • Tuberculoma of the lungs is not an uncommon finding, but an ectopic liver in the lung is extremely rare. Pulmonary tuberculosis presenting as tuberculoma can be diagnosed radiologically, but its definite diagnosis is established by confirmation of the acid-fast bacillus or the unique histology. We report here on a case of tuberculoma of the left lower lobe that was erroneously diagnosed as ectopic liver by ultrasono-guided fine needle aspiration biopsy. An understanding of the normal variants of the liver can prevent a patient from undergoing an unnecessary invasive procedure.

  • Ectopic mediastinal parathyroid adenomas or hyperplasias account for up to 25% of primary hyperparathyroidism cases. Most abnormal parathyroid glands are found in the superior mediastinum within the thymus and can be removed through a cervical incision; however, a few of these glands are not accessible using standard cervical surgical approaches. Surgical resection has traditionally been performed via median sternotomy or thoracotomy. However, recent advancement in video-assisted thoracic surgery techniques has decreased the need for sternotomy or thoracotomy to remove these ectopic parathyroid glands. Here, we report a successful case of video-assisted thoracoscopic removal of a mediastinal parathyroid adenoma.

  • Pleomorphic adenoma is the most common benign tumor of the salivary gland. Pleomorphic adenoma occurs most commonly in the parotid gland but it may involve other salivary gland such as submandible or lingual. We report an ectopic pleomorphic adenoma in the subcutaneous layer of the face. A woman presented with a mass of the nasolabial fold. After excision of the mass, it was revealed as an pleomorphic adenoma pathologically. An ectopic pleomorphic adenoma which was located in the subcutaneous layer of the face is very rare in medical literature.

  • Small cell lung cancer(SCLC) is frequently associated with paraneoplastic syndromes, which occur in approximately 20% of patients at presentation. Clinical Cushing's syndrome secondary to ectopic ACTH production is uncommon, occurring in approximately 5% of all SCLC patients. However, biochemical evidence of hypercortisolism can be detected in up to 50% of patients. Patients with Cushing's syndrome from ectopic ACTH production show hypertension, weakness, hyperglycemia, and hypokalemic metabolic alkalosis, but differ from patients with classic Cushing's disease in that symptoms develop more rapidly. Ectopic ACTH production is associated with a poor response to chemotherapy, short survival, and a high risk of treatment-related complications. We report a case of Cushing's syndrome associated with ectopic corticotropin production in 59-year-old male patient with extensive stage of SCLC.

Effects of Astragalus membranaceus on Surgically Induced Endometriosis in Rats (황기가 자궁내막증 유발 흰쥐에 미치는 영향)

  • Purpose : Astragalus membranaceus is a herb which has effectiveness of energy increase, protection against pathogenic and organ immune response control. Endometriosis has been thought to be related with immune response in uterus. Therefore in this study, the effects of Astragalus membranaceus on endometriosis were investigated. Methods : The endometrial tissue was autografted to rat's small intestine. Rats with surgically induced endometriosis was administerd with Astragalus membranaceus extracts for 40 days orally or rectally. We observed the size of ectopic uterine implants at the mesenteric with the naked eye and examined the concentrations of estradiol, progesterone, $TNF-{\alpha}$ and IL-2, IL-4, IL-6 and IL-10 in serum. Also, we inquired out the efficiency between oral and rectal administration groups. Results : The size of ectopic uterine implants of oral and rectal treated groups were much smaller than those of control groups. The concentration of estradiol, progesterone. $TNF-{\alpha}$ and IL-2 were significantly decreased in oral administration experimental group compared with the control group. The concentration of progesterone were significantly decreased in rectal administration experimental group compared with the control group. The efficiency of oral administration group was better than that of rectal administration group in the serum concentration of estradiol, $TNF-{\alpha}$ and IL-2. Conclusion : Our data suggest that Astragalus membranaceus reduce the size of ectopic uterine implants at the mesenteric wall and inhibit the growth of ectopic uterine implants. Judging from the above results, it can be suggested that Astragalus membranaceus should be a useful agent for inhibiting the proliferation of uterine endometrial tissue. May be, oral administration of Astragalus membranaceus is more effective than rectal administration for inhibiting the proliferation of endometriosis.

Ectopic Human Fasciola hepatica Infection by an Adult Worm in the Mesocolon

  • We report here an ectopic case of Fasciola hepatica infection confirmed by recovery of an adult worm in the mesocolon. A 56-year-old female was admitted to our hospital with discomfort and pain in the left lower quadrant of the abdomen. Abdominal CT showed 3 abscesses in the left upper quadrant, mesentery, and pelvic cavity. On surgical exploration, abscess pockets were found in the mesocolon of the sigmoid colon and transverse colon. A leaf-like worm found in the abscess pocket of the mesocolon of the left colon was diagnosed as an adult fluke of F. hepatica. Histologically, numerous eggs of F. hepatica were noted with acute and chronic granulomatous inflammations in the subserosa and pericolic adipose tissues. Conclusively, a rare case of ectopic fascioliasis has been confirmed in this study by the adult worm recovery of F. hepatica in the mesocolon.

Ectopic expression of $ARR1{\Delta}DDK$ in tobacco: alteration of cell fate in root tip region and shoot organogenesis in cultured segments

  • A specific deleted version of ARABIDOPSIS RESPONSE REGULATOR1 (ARR1) lacking the signal receiver domain (1.152 amino acids)-coding sequence, referred to as $ARR1{\Delta}DDK$, was amplified using Arabidopsis thaliana cDNA prepared from adult leaves and transferred into the genome of Nicotiana tabacum cv. Samsun under the transcriptional control of a ${\beta}$-estradiol-inducible expression system. The ectopic expression of $ARR1{\Delta}DDK$ affected the morphology of transgenic seedlings and their segments in vitro. In the presence of an inducer, ${\beta}$-estradiol, ectopic expression of $ARR1{\Delta}DDK$ induced only the formation of soft, pseudo-bulbous tissue in the root tip region of intact seedlings, which appeared similar to callus generated on a hypocotyl segment in the presence of 2,4-D and 6-benzyladenine (BA), both at $1\;{\mu}M$. Those callus tissues on the root tip region could not generate shoots unless $1\;{\mu}M$ BA was supplied. In segment culture, ectopic expression of $ARR1{\Delta}DDK$ induced calluslike tissue around the cut-end of cotyledon and hypocotyl segments with occasional shoot formation, suggesting that the expression of $ARR1{\Delta}DDK$ could substitute for the effects of cytokinin on these segments. Additionally, treatment with only ${\beta}$-estradiol induced NtWUS, a WUS ortholog in tobacco, which was detected during the process of callus tissue formation in the root tip region and also in cotyledon or hypocotyl segments. These findings suggest that the NtWUS might be associated in the transdifferentiation process caused by the functional regulation of $ARR1{\Delta}DDK$ in transgenic tobacco seedlings.

Extracorporeal Membrane Oxygenation in the Patient with Cardiopulmonary Resuscitation Failure after Open Heart Surgery. - A case report - (개심술 후 심폐소생술 실패환아에서의 체외막산소화 치험 1례)

  • We describe a case of successful extracorporeal membrane oxygenation(ECMO) in a small infant with cardiopulmonary resuscitation(CPR) failure after an open heart surgery. A 35-day-old male infant weighing 4.4 kg who had congestive heart failure and pulmonary hypertension underwent patch closure of ventricular septal defect without any intraoperative event. Postoperative course was unremarkable in the intensive care uint for about 5 hours before the junctional ectopic tachycardia developed. Sudden cardiac decompensation with bradycardia occurred about 50 minutes after the development of junctional ectopic tachycardia. He was put on ECMO by arterial cannulation at the ascending aorta and by venous cannulation at the right atrial appendage after 4 hours' CPR. The hemodynamics were stable with enough urine output during ECMO. He was weaned from ECMO 38.5 hours after initiation. Delayed sternal closure was attempted. He was extubated on postoperative day 7 and discharged home on postoperative day 21 without any neurologic sequelae.
