Purpose: To investigate early complications after repair of massive rotator cuff tears and to find out factors that compromise the results. Materials and Methods: Fourteen patients who had two or more cuff tendons involved were included. All patients were operated by open acromioplasty and rotator cuff repair. At 3 months after operation, we investigated whether there were any early complications or not. We used ASES scoring system for preoperative and follow up evaluation. In addition, various preoperative factors, such as duration of symptom, degree of tendon retraction, degree of fatty degeneration, and acromio-humeral distance, were compared between the complicated patients and non-complicated patients. Results: At 3 months after operation, the ASES score and pain were improved in any degree in all patients. But 5 patients complained persisting pain, and three of them showed major complications such as re-rupture of rotator cuff or deltoid rupture. But no preoperative factors in complicated patients were significantly different from those in non-complicated patients. Conclusion: None of the preoperative factors were related to the complications. There was a tendency of overestimation of fatty degeneration in MRI. Some factors in surgical technique and rehabilitation were highly suspected to be related to the complications.
Purpose: This study examined the effects of stress vulnerability and parental burnout on the mental health of women with early school-aged children, with a focus on the mediating role of spirituality. Methods: A survey was conducted among 171 women with early schoolaged children in Gyeonggi Province, Gangwon Province, and Seoul. Data were collected from September to December 2022 using the Korean-Symptom Check List 95, the Parental Burnout Assessment, and the Spirituality Assessment Scale. The data were analyzed using structural equation modeling with SPSS/WIN 22.0 and AMOS 20.0. Results: The study model demonstrated a good fit, explaining 40.5% of the variance in mental health through stress vulnerability, parental burnout, and spirituality. Spirituality had a significant direct impact on mental health. Additionally, participants' spirituality directly influenced their mental health, while stress vulnerability and parental burnout indirectly affected their mental health and were mediated through spirituality. Conclusion: Stress vulnerability and parental burnout are negatively associated with mental health, while spirituality partially mediates these effects. Implementing a program to promote spirituality is suggested to assist mothers in recognizing the value and meaning of parenting activities during nursing interventions for mental health.
The circular leaf spot of persimmon is occurred almost every place where persimmon is cultivated, especially the disease outbreak severely in southern part of Korea. The disease reveals unusually long incubation period after pathogen invade into leaf tissue and no practical control measure is available once the symptom has appeared. Most of the farmers just follow the suggested spray schedules calculated on the basis of weather condition of ordinary years. Therefore the damages due to circular leaf spot greatly differ year after year. In this article, we tried to describe and summarized the investigation on the circular leaf spot pathogen, Mycosphaerella nawae, related to disease outbreak such as overwintering of pathogen, inoculum formation and spread, incubation period after infection, and secondary inoculum. With the summary of these results, we suggest the disease cycle of circular leaf spot of persimmon. The pathogen overwinters in diseased leaves as mycelial form or pseudoperithecial premodium. The pseudoperitheria become matured in spring as the temperature raise and forms asci and ascospores. The maturation of pseudoperithecia are closely related to the temperatures during March and early April. The ascospores completely mature in early May and the ascospores released when the pseudoperithecia absorbed enough moisture after rainfall. The release of ascospores are diverse greatly with the variation of maturity of pseudoperithecia. Generally the spore start to release from middle of May to early of July. Duration of ascospore release is depend on the weather condition of particular year, especially amount and number of precipitation. The ascospores produced from pseudoperithecia is known to the only inoculum for circular leaf spot disease. But according to the results obtained from our investigations, the conidia formed on the lesions which incited by natural infection. This conidia are infectious to persimmon leaves and formed identical symptom as natural infection. The time of producing secondary inoculum of circular leaf spot of persimmon is considered too late to develop new disease. Generally the importance of secondary inoculum is low but the conidia produced in early September are competent to develop new disease and new infection also significantly affect to harvest of persimmon. The importance of circular leaf spot disease is recognized well to farmers. The approaches to control of the disease should be initiated on the basis of the knowledges of inoculum dynamics and ecology of disease development. The forecasting system for circular leaf spot is need to be developed.
Complications of the mediastinal tumors and cysts are malignant change, infection, bleeding, local invasion and mass effect to heart, lung and other mediastinal structures. But early surgical excision and proper treatments bring patients to good clinical course and results. Therefore mediastinal tumors and cysts are surgically interesting diseases We report the analysis of the 58 cases of mediastinal tumors and cysts, experienced in the Department of Cardiothoracic Surgery of the Kosin Medical College from July 1979 to June 1989. The results were as follows ; Sex ratio of male to female to female was 1.3: 1. Range of age was from 11 to 64 years and mean age was 34.3 years. The thymomas were 14 cases[24%], the teratomas were 19 cases[33%o], the neurogenic tumors were 10 cases[17%], the cysts were 9 cases[15%], the carcinomas were 3 cases[5%], the thyroid tumor was 1 case[2%], the Castleman’s disease was 1 case[2%] and unclassified tumor was 1 case[2%]. Malignant tumors were 12 cases [21%] of the 58 cases. Most frequent symptom was chest pain and discomfort and relationship of symptom and malignancy was significant. Complete removal of tumor was performed on the 47 cases[92%] and partial excision was 3 cases[6%]. Inoperable cases were treated with anticancer chemotherapy and radiotherapy. Postoperative complications were wound infection, Homer’s syndrome, phrenic nerve palsy, mediastinal hematoma and pleurisy. There was no case of postoperative mortality and good clinical course in surgically completely resected cases.
One hundred twenty patients with acute and chronic pain treated by a low power laser were divided into several groups by their pathology and evaluated according to their response rate to laser therapy through a follow-up study. 1) The ages of the patients were between the early twenties and late forties (71.7%), and there was no differences between sexes. 2) The spinal pathology group was the most common(52.5%) and the articular pathology group occupied next (14.2%). 3) The average duration of Laser therapy was about 20 days and response to the therapy appeared about the eighth day. 4) The response to the therapy in the spinal pathology group appeared about the eighth day and the average duration of therapy was about 18 days. 5) The response to the therapy in the articular pathology group appeared about the eighth day and the average duration of therapy was about 28 days. 6) The response rate of the spinal pathology group was 81.0%, and remarkable symptom relief was noted when compaired to a 58.7% response rate in the control group, 7) The response rate of the articular pathology group was 82.4%, which was similar to the control group. 8) The response rate of the miscellaneous group was 87.0%, and remarkable symptom relief was noted when compaired to a 66.7A response rate in the control group. 9) The mean response rate of all patients treated by a low power laser was 82.5% and that of the control group was 70.5%. Laser therary proved to be an effective treatment modality for acute and chronic pain.
Objectives : Aripiprazole is unique drug among the SGA (Second generation antipsychotics) in its pharmacology and pharmacokinetics,but is similar in clinical efficacy. Aripiprazole acts as a partial agonist at dopamine D2 receptors, activating the receptor but eliciting a reduced response compared to the natural neurotransmitter. There are some side effects of aripiprazole, the most common side effects of aripiprazole are headache, nausea, vomiting, insomnia, tremor, constipation and EPS. Difficulty in opening eyes is not defined EPS yet, but it is a rare but important side-effect symptom of aripiprazole. Methods : This article is about a case of side-effect symptom of aripiprazole, 26-year-old single female suffering from schizophrenia had difficulty in opening eyes while she was taking antipsychotics. During the hospitalization, the relaxation therapy is helpful not only to reduce tension in the eyelids but also to headache. Results : It is important that early recognition of aripiprazole-induced oculogyric dystonia can prevent life-threatening complications. Education medical staff to this easily treatable reaction will improve overall quality of health care. Conclusions : This case notifies the need for awareness of the risk of acute oculogyric dystonia in adolescent female patients receiving aripiprazole.
Objective : The aim of this study is to analyze the clinical symptoms, radiological changes, interval from first operation to symptom recurrence and to propose the proper treatment method for re-operated patients following chronic subdural hematoma surgery. Methods : Between January 1992 and April 2003, 18 of 138patients of chronic subdural hematoma repeatedly underwent surgical treatment. The symptoms, mental status by Bender grade, radiological hematoma size and midline shifting, interval from symptom onset to diagnosis, surgical method and prognosis by Glasgow outcome scale[GOS] between the first attack and the recurrence were compared. Results : The symptoms at the time of recurrence were nearly the same as with the first attack, but two patients[2/18, 11.1%] showed a more declined mentality. In addition, the recurred hematoma sizes were the same or large than those previously found. Many patients were recurred within two weeks[13/18, 72.2%]. Most patients were operated on using the previous burr hole, with the exception of one patient who recurred at a different site. All patients had a good prognosis more than GOS 4 [GOS 4:4, GOS 5:12], but two died due to extracranial complication and infection. Conclusion : These results suggest that the early diagnosis and treatment are important, mostly recurred same symptoms within two weeks. Re-operation using the previous burr hole site is a good method.
Brain edema is a major importance in the pathophysiology of CNS injuries including stroke. Ischemic brain edema results from both cytotoxic edema, which is severe in astrocytes at early stage, and vasogenic edema caused by excessive blood-brain barrier (BBB) permeability. The present study was performed to determine the effect of Rhei Rhizoma on brain edema induced by middle cerebral artery occlusion (MCAO) in the rats. The neurological symptom, total infarct volume and edema index caused by MCAO were measured. The changes of Matrix Metalloproteinase-9 (MMP-9) and inducible nitric oxide synthase (iNOS) immunoreactivities were also observed. We found that Rhei Rhizoma extract improved the neurological symptom and attenuated the total infarct volume and brain edema caused by ischemic insult. Rhei Rhizoma extract also attenuated the expression of MMP-9 and iNOS. This results suggest that Rhei Rhizoma has a protective effect on the brain edema caused by ischemic insult.
In 2007, a bulb rot of Allium victorialis var. platyphyllum Makino caused by Sclerotium rolfsii occurred sporadically in a herb farm at Gyeongsangnam-do, Korea. The symptom initiated with water-soaking lesion and progressed into bulb rot and wilt of a whole plant. Severely infected plants were blighted and killed eventually. White mycelial mats appeared on the lesion at early stage and a number of sclerotia were formed on the bulb. The sclerotia were spherical, $1{\sim}3 mm$ and white to brown. The optimum temperature for the growth and sclerotia formation was $30^{\circ}C$ on PDA. The hyphal width was measured $4-9{\mu}m$. The typical clamp connections were observed on PDA. Based on the symptom, mycological characteristics and pathogenicity to the host plant, this fungus was identified as Sclerotium rolfsii Saccardo. This is the first report on the bulb rot of A. victorialis var. platyphyllum caused by S. rolfsii in Korea.
Purpose: This descriptive study was conducted to identify the level of knowledge of stroke symptoms and risk factors among older adults. Methods: A total of 200 older adults over 65 years of age were conveniently recruited from out patient departments of two hospitals and a health care center from October to November 2008. The level of knowledge was assessed using both open-ended questions and a structured questionnaire based on semi-structured interviews. Data were analyzed by t-tests and ANOVA using the SPSS program. Results: 52.5% of the sample had hypertension and 30% had diabetes. The mean knowledge scores for symptoms and risk factors were $8.4{\pm}3.1$ (out of 15) and $9.5{\pm}3.9$ (out of 16), respectively. The older adults who had lower education, lower family income, and who lived in rural areas were more likely to have less knowledge of stroke symptom and risk factors (p < .05). There was no significant knowledge difference between the older adults who had at least one risk factor and those who had no risk factor for stroke. Conclusions: Educational intervention should be focused on informing older adults who are at risk for stroke about the early symptoms and management of risk factors, especially those who have low education and low social status.
