Quality of Life in Malaysian Colorectal Cancer Patients : A Preliminary Result

  • Objective: Rapidly increasing colorectal cancer (CRC) incidence in Malaysia and the introduction of cutting edge new treatments, which prolong survival, mean that treatment outcome measures meed to be evaluated, including consideration of patient's quality of life (QoL) assessment. There are limited data on QoL in CRC patients, especially in Malaysia. Therefore, this study was performed focusing on cancer stages and age groups. Methods: The cross sectional study was conducted from June to September 2011 at three public tertiary hospitals with the EORTC QLQ C-30 questionnaire in addition to face to face interview and review of medical records of 100 respondents. Results: The mean age was 57.3 (SD 11.9) years with 56.0% are males and 44.0% females, 62% of Malay ethnicity, 30% Chinese, 7% Indian and 1% Sikh. Majority were educated up to secondary level (42%) and 90% respondents had CRC stages III and IV. Mean global health status (GHS) score was 79.1 (SD 21.4). Mean scores for functional status (physical, emotional, role, cognitive, social) rangeds between 79.5 (SD 26.6) to 92.2 (SD 13.7). Mean symptom scores (fatigue, pain, nausea/vomiting, constipation, diarrhea, insomnia, dyspnoea, loss of appetite) ranged between 4.00 (SD 8.58) to 20.7 (SD 30.6). Respondents role function significantly deteriorates with increasing stage of the disease (p=0.044). Females had worse symptoms of pain (p=0.022), fatigue (p=0.031) and dyspnoea (p=0.031). Mean insomnia (p=0.006) and diarrhea (p=0.024) demonstrated significant differences between age groups. Conclusion: QOL in CRC patients in this study was comparable to that in other studies done in developed countries. Pain, fatigue and dyspnoea are worse among female CRC patients. Given that functions deteriorates with advanced stage of the disease at diagnosis, a systematic screening programme to detect cases as early as possible is essential nationwide.

  • Objectives: To grasp the traditional stream of cancer treatment inherited from the previous doctors. Methods: The author's research has been performed cancer treatment based on JIJU(積聚) taking the original text as a reference. Results & Conclusion: The general three outlines and five detailed rules of cancer treatment have been obtained as follows. The first outline of cancer treatment is that reinforced vital function makes cancer reduce naturally. the second is that Harmless cancer can coexist in human beings and aging with them. and the third is that Elimination and reinforcing therapies should be executed in appropriate era and those therapies should utilize appropriate methods. The first detailed rule of cancer treatment is when using reinforcing therapy, it must applicate mildly and when using elimination therapy, it must applicate calmly. The second detailed rule is that the methods of cancer treatment are different from each cancer stage. The concentration should be made on reinforcing therapy at early stage while reinforcing and elimination therapies must be conducted together at middle stage. At terminal stage reinforcing therapy is the sole method to be taken. The third detailed rule is that the basis property of cancer drug is warm nature and extremely biased property should be avoided and when complication arises (eg. inflamatory disease, cancer fever, etc), cold or cool nature can be applied. The fourth detailed rule is that Cancer drug must have the effect eliminating the blood stasis, phlegm and excessive fluid, all together. The fifth detailed rule is that Physicians have to control patient's stress or stress related symptom and teach patients about right way of taking care of themselves and patients should take hygienic rules with their free will by themselves (eg diet, exercise, stress, etc)

The Difference of the $2^{nd}$ to $4^{th}$ Digit Length Ratio between Type I and Type II Alcoholism (알코올 의존 제I형 및 II형 환자에서 검지-약지 길이 비의 차이)

  • Objectives The second to fourth digit ratio (2D : 4D) is known to reflect testosterone level during intrauterine period. Testosterone is essential for masculinization of brain. Cloninger's type II alcoholism is characterized by early age at onset and largely limited to males. Compared with type I alcoholism, type II alcoholism is thought to be masculine. There is a possibility that the digit ratio of type II alcoholism is low compared with type I. We examined digit length ratio in Cloninger's type I and II alcoholism to understand the effects of intrauterine testosterone on the development of alcoholism. Methods The participants were 87 alcohol dependent patients. We divided two subtypes of patients by Irwin's symptom list, based on Cloninger's concept. We captured the images of both palms by the scanner and measured the lengths of digits by the graphic program. Then we analyzed the digit ratios by independent-sample t-test. Results The mean 2D : 4D of type I was 0.942 (right), 0.952 (left). The mean 2D : 4D of type II was 0.927 (right), 0.940 (left). In both hands, 2D : 4D of type II was significantly lower than type I. Conclusions Type II alcoholism showed masculine type of digit length ratio compared with type I. It suggests the exposure of testosterone during intrauterine period might play an important role in determining the course and feature of alcoholism.

A Study on the Image Enhancement Method of Digital Mammogram in the Wavelet Domain (웨이블렛 영역에서 디지털 맘모그램의 영상향상 방법에 관한 연구)

  • Digital mammogram is effective for detecting the micro-calcification that is early symptom of breast cancer. In the digital mammogram, many image processing techniques have been studied for accurate diagnosis and efficient treatment of micro-calcification lesion. The wavelet based multi-scale method was mainly used to enhance the image contrast. This paper presents an advanced mammography enhancement method which is based both on the brightness and the contrast enhancement in the wavelet domain. The proposed method normalizes a dynamic range using histogram of the image. The brightness is enhanced by modifying coefficients of low frequency components, and the contrast is enhanced by coefficients of high frequency component based on the multi-scale contrast measure. The experiment results show that the proposed method yields better performance of the image enhancement over the existing methods.

  • The common cold is the most common pediatric infectious disease, and the incidence is higher in early childhood than in any other period of life. Treatment of the common cold is largely symptomatic. Thus treatment of oriental medicine for the common cold is very effective and safe. We studied 81 children suffering from common cold Oriental Medicine Hospital in Kyunghee University, and analyzed age, sex, season, clinical manifestation, complications, prescription. The results were as follows: 1. Age and sex distribution of children from 1 to 3 was 37.1%, 3 to 6 was 34.6%, over 6 was 24.7%, male to female ratio was 1.31 : 1 2. Frequency of affection per year, from 3 to 5 was 30.9%, 6 to 9 was 29.6%, over 10 was 27.2%, below 3 was 4.9% 3. Frequency of seasons giving rising to common cold, winter was 40.7%, spring and autumn were 16%, summer was 1.2%. 4. Frequency of complications, empyema and rhinitis were 38.3%, asthma was 21%, pneumonia was 16%, atopic dermatitis was 7.4%. 5. Frequency of Clinical manifestation, cough and sputum were 45.7%, high fever and sore throat were 27.2%, complex symptom was 25.9%. 6. Frequency of prescription, BoAh-Tang-gami(補兒湯加味)was 27.2% BangPungHaeDoc-Tang(防風解毒湯)was 11.1 %, GamiGinHae-Tang (加味鎭咳湯)was 11.1%.

Treatment for Tarsometatarsal Fracture-Dislocation (족근-중족 관절 골절 탈구의 치료경험)

  • Tarsometatarsal fracture-dislocation is uncommon but severe lesion. Since this lesion is sometimes difficult to recognize by roentgenography, it is easily overlooked. Three patients were treated with open reduction and internal fixation with 3.5 mm cannulated screw and K-wire, two had treatment with open reduction and internal fixation with 3.5 mm cannulated screw only and two had treatment with dosed reduction and short leg cast only between January 1994 and May 1996. The duration of follow-up ranged from twelve to twenty-nine months after the diagnosis. Results were assessed by a subjective questiormaire, physical examination, and radiographic analysis. Multiple fixation techniques for maintaining the reduction of tarsometatarsl joint have been introduced. We recent]y used the 3.5 mm cannulated screw for internal fixation of the tarso-first and second metatarsal fracture-dislocation. We think cannulated screw fixation has several advantages; 1. The cannulated screw fixation is more rigid than the K-wire fixation. 2. There is an decreased risk of screw breakage with early weight bearing. 3. It is possible to compress the involved joints, if necessary. There were no disability in all patients. One patient who was treated with delayed open reduction and internal fixation with 3.5 mm cannulated screw and K-wire had a radiographic mild degenerative arthritis. And one patient who was treated with dosed reduction and short leg cast had a mild metatarsus adductus. But. these two patients were symptom free. There was no correlation between the severity of the diastasis and the patient s functional result.

Environmental Factors Affecting Maturation Rate of Pseudorhecia of Mycosphaerella nawae, the Causal Organism of the Spotted Leaf Castin of Persimmon (감나무 둥근무늬낙엽병균(Mycosphaerella nawae)의 위자낭각 성숙에 영향을 미치는 환경요인)

  • The environmental factors affecting maturity of pseudothecia overwintered in iufected leaves by Mycosphaserella nawae was investigated. The pseudothecia in the overwinteringinfected leaves were matured at the end of April when the average temperature was $14^{\circ}C$ and precipitation was enough. Pseudothecia initiation was preceded by the fungal biomass development in a given host. The maturity of the pseudothecia from the leaves defoliated in early October was earlier and higher than those defoliated in November of the previous year. Pseudothecia development was also positively affected by low temperature. The maturity time, as determined by percentage of matured pseudothecia out of prematured ones, was also 20 days earlier, and the rate of maturity was higher for the leaves overwintered in door, than for those kept in greenhouse. We have providedevidence that the conidia play a significant role in the epidemiology of this pathogen. Many pseudothecia were developed in the leaves infected by artificially inoculated conidia and maturation rate of the pseudothecia was not different from that of the typical symptom by ascospores infection of Mycosphaerella nawae.

  • Natural sclerotia of Rhizoctonia solani causing rice sheath blight were inoculated at 10 day intervals from June 15 to July 15 in paddy field, Icheon, Korea. Percentage of infected stems, top lesion height and percentage of. lesion height vs. plant height were higher in the early inoculated plots than in the late inoculated ones. However, rio significant differences among inoculation dates of sclerotia were found on the basis of degree of damage at maturing stage and rice yield. These results suggest that the time of initial symptom appearance under the same inoculum potential may not affect the damage of rice plants by the fungus.

Suppression of UDP-glycosyltransferase-coding Arabidopsis thaliana UGT74E2 Gene Expression Leads to Increased Resistance to Psuedomonas syringae pv. tomato DC3000 Infection

  • Plants possess multiple resistance mechanisms that protect themselves against pathogen attack. To identify unknown components of the defense machinery in Arabidopsis, gene-expression changes were monitored in Arabidopsis thaliana under 18 different biotic or abiotic conditions using a DNA microarray representing approximately 25% of all Arabidopsis thaliana genes (www.genevestigator.com). Seventeen genes which are early responsive to salicylic acid (SA) treatment as well as pathogen infection were selected and their T-DNA insertion mutants were obtained from SALK institute. To elucidate the role of each gene in defense response, bacterial pathogen Pseudomonas syringae pv. tomato (Pst) DC3000 was inoculated onto individual T-DNA insertion mutants. Four mutants exhibited decreased resistance and five mutants displayed significantly enhanced resistance against Pst DC3000-infection as measured by change in symptom development as compared to wild-type plants. Among them, member of uridin diphosphate (UDP)-glycosyltransferase (UGT) was of particular interest, since a UGT mutant (At1g05680) showed enhanced resistance to Pst-infection in Arabidopsis. In systemic acquired resistance (SAR) assay, this mutant showed enhanced activation of SAR. Also, the enhanced SAR correlated with increased expression of defense-related gene, AtPR1. These results emphasize that the glycosylation of UGT74E2 is a part of the SA-mediated disease-resistance mechanism.

Clinical Experience with Vascular Surgery (혈관수술에 대한 임상적 고찰)

  • The vascular surgery is the field that has developed in early 20 century and is progressing nowadays. Recent advance in surgical technique accompanying with excellent medical diagnosis and treatment, prompt angiographic usage, development of variable prosthetic material, and concomitant use of anti-coagulant have made remarkable results of vascular surgery. 83 cases of vascular surgery have been performed at Thoracic and Cardiovascular Surgery Department of Pusan National Unversity Hosaital since 1971 till 1990, for 20 years and their results are followed. Patient ductus arteriosus and Buerger`s disease were omited in this study. 1. The age distribution shows that the fifth and sixth decades are most frequently affected and mean age was 56.1 years old. Male to female ratio is 1: 2.32. 2. Among the 83 cases of all, number of occlusive vascular disease is 46 and that of aneurysmal disease is 33. 3. In clinical manifestation, most common symptom of occlusive disease is pulselessness and pain was next. Mass sensation is most commonly complained by patients of aneurysmal disease. 4. CT scan was more important in diagnosis of aneurysmal diseases and angiogram was more commonly used in occlusive diseases. 5. The common site of arterial occlusion was common iliac artery, femoral artery, aortic bifurcation, and external iliac artery, as its frequency rate. The most commonly affecting portion of aortic aneurysm was abdminal aorta, and descending thoracic aorta and femoral artery were next 6. Preoperative associated diseases were atherosclerosis[41 cases], hypertension[21 cases], valvular heart disease[11 cases], and diabetes mellitus[9 cases], etc, 7. Operative methods in ocllusive diseases were thrombectomy[36.9%], endarterectomy [10.9%], and bypass graft insertion[52.7%]. Among the bypass graft, Y-graft was used in 7 case, straight graft was used in 17 cases, and saphenous venous graft was used in 2 cases. 8. Postoperative complications were developed in 17 cases, and morbidity rate was 36. 9. Eleven patient were died within 1 month after operation, so operative mortality rate was 13.3%. 10. Duration of patency was beteween 7 and 58 months[average 27.5 months] in occlusive diseases and their 5-year patency rate was 56.3%. Duration of patency of aneurysmal disease was 20 months in aveage and their 5-year patency rate was 51.3%. 11. Patients of eleven cases of occlusive disease and two cases of aneurysmal disease required reoperation for variable reason. 12. 35 cases of patient have used anticoagulants: coumadin, ticlid, and persanthin-ASA combination.
