중증외상 환자에서 심부정맥혈전 발생에 관한 고찰 (Development of Deep Vein Thrombosis in Severely Injured Multiple Trauma Patients)

  • Purpose: Deep vein thrombosis and pulmonary thromboembolism are major causes of death after severe multiple trauma. Although various means of prevention have been presented and utilized, still, there are no standard guidelines for anticoagulation of multiple trauma because of some contraindications. Methods: A retrospective study of adult major trauma patients whose injury severity scores (ISSs) were over 16 and who had visited one university hospital in Daegu city was performed. We compared some features of patients diagnosed DVT or PTE with those of patients without DVT by computed tomography or ultra sonography. Those features included accompanying various kinds of intracranial hemorrhages, possibility of ambulation, emergent operation, early transfusion, and suspicious symptoms. Results: The mean age of the 58 subjects included in this study was $50.9{\pm}17.2years$, the mean ISS was $22.7{\pm}6.0$, and the mean hospital stay was $55.2{\pm}37.9days$. Ten(17.2%) patients had emergent surgery, and 44(75.9%) experienced delayed surgery. Early transfusion was needed in 34(58.6%) patients. Among the 18 patients diagnosed with DVT, accompanying intracranial hemorrhages were noted in 8(44.4%) patients; one of the 8 also had PTE. Among the same 18 patients, early transfusions were required in 11(61.1%) patients; one of the 11 also had PTE. Conclusion: The risk of DVT is increased in cases of severe multiple trauma, and many difficulties in applying anticoagulants are experienced. Though we need additional studies to decide proper prophylaxis for DVT and PTE, if the patient's general condition permits, a screening test for DVT as soon as possible could be an effective method to reduce the possibility of a bad outcome.

A Nomogram for Predicting Non-Alcoholic Fatty Liver Disease in Obese Children

  • Purpose: Non-alcoholic fatty liver disease (NAFLD) ranges in severity from simple steatosis to steatohepatitis. Early detection of NAFLD is important for preventing the disease from progressing to become an irreversible end-stage liver disease. We developed a nomogram that allows for non-invasive screening for NAFLD in obese children. Methods: Anthropometric and laboratory data of 180 patients from our pediatric obesity clinic were collected. Diagnoses of NAFLD were based on abdominal ultrasonographic findings. The nomogram was constructed using predictors from a multivariate analysis of NAFLD risk factors. Results: The subjects were divided into non-NAFLD (n=67) and NAFLD groups (n=113). Factors, including sex, body mass index, abdominal circumference, blood pressure, insulin resistance, and levels of aspartate aminotransferase, alanine aminotransferase (ALT), γ-glutamyl transpeptidase (γGT), uric acid, triglycerides, and insulin, were significantly different between the two groups (all p<0.05) as determined using homeostatis model assessment of insulin resistance (HOMA-IR). In our multivariate logistic regression analysis, elevated serum ALT, γGT, and triglyceride levels were significantly related to NAFLD development. The nomogram was established using γGT, uric acid, triglycerides, HOMA-IR, and ALT as predictors of NAFLD probability. Conclusion: The newly developed nomogram may help predict NAFLD risk in obese children. The nomogram may also allow for early NAFLD diagnosis without the need for invasive liver biopsy or expensive liver imaging, and may also allow clinicians to intervene early to prevent the progression of NAFLD to become a more advanced liver disease.

  • This study investigates the fundamental properties of high strength concrete made with various blame values of cement, manufactured by the particle screening method in a pulverizing process. Test showed that concrete using lower blame of cement, such as large particle (L) and both ordinary and large particle (OL), increased the fluidity of fresh concrete. As tine progressive, it was noticeable that the specimens using ordinary cement (OPC) gradually decreased the fluidity, but the other specimens showed the sudden decline until 30 minutes, which is followed by a gradual decrease after 60 minutes. For the setting time, higher blaine of cement accelerated the initial and final setting time, especially concrete using minute size of cement (M) was 10 hours faster than OPC. Compressive strength of L exhibited similar value at 1 days as to that of strength in OPC at 3 days. Importantly, the specimens using M also revealed the similar strength value, regardless of curing temperature between $-5^{\circ}C$ and $20^{\circ}C$, which means that using this minute particle of cement in concrete can secure the strength development even in the lower temperature circumstance. Therefore it is clear that using OPC+M simultaneously at cold weather concreting can resist the early frost and develop the early strength of concrete.

Central Auditory Processing Tests as Diagnostic Tools for the Early Identification of Elderly Individuals with Mild Cognitive Impairment

  • Background and Objectives: Mild cognitive impairment (MCI) is a disorder that usually occurs in the elderly, leading to dementia in some progressive cases. The purpose of this study is to examine the utility of central auditory processing tests as early diagnostic tools for identifying the elderly with MCI. Subjects and Methods: This study was conducted on 20 elderly patients with MCI and 20 healthy matched peers. The speech perception ability in a quiet environment and in the presence of background noise and also temporal resolution were assessed by using Speech Perception in Noise (SPIN) and Gap in Noise (GIN) tests, respectively. Results: The results indicated that the ability to understand speech in a quiet environment did not differ significantly between the two groups. However, SPIN at the three signal-to-noise ratios and the temporal resolution scores were significantly different between the two groups (p<0.001). Conclusions: Individuals with MCI appear to have poorer speech comprehension in noise and a lower temporal resolution than those of the same age, but without cognitive defects. Considering the utility of these tests in identifying cognitive problems, we propose that since the GIN test seems to be less influenced by intervening factors, this test can therefore, be a useful tool for the early screening of elderly people with cognitive problems.

학교 집단 요검사로 조기 진단된 IgA 신증 환아의 임상적 특징 (The Characteristics of IgA Nephropathy when Detected early in Mass School Urine Screening)

  • 목적: 면역글로불린A(IgA) 신증은 다양한 임상양상을 보이는 가장 흔한 원발성 사구체 신염으로, 적극적인 치료로서 단백뇨와 사구체손상을 줄여야한다. 저자들은 학교 집단 요검사를 통해 진단된 IgA 신증의 특징을 알아보고자 하였다. 방법: ${\bigcirc\bigcirc}$대학교병원 소아청소년과에서 조직검사를 받고 IgA 신증으로 진단받은 환자 64명을 내원 당시의 증상 유무에 따라 두 군으로 나누어 후향적으로 연구하였다. 결과: 증상 없이 학교신검에 의해 진단된 37명과 혈뇨 등으로 내원하여 진단된 27명을 대상으로 하였다. 각 군의 평균 연령은 각각 $10.8{\pm}2.7$세 및 $9.5{\pm}3.4$세였고, 각 군 모두에서 남아가 많았다. 증상 발현 후 내원까지의 기간과 조직 검사까지의 기간은 증상군에서 더 짧았다. 임상 증상은 증상군에서 육안적 혈뇨(P<0.001)와 부종(P=0.008)이 더 많았다. 혈액검사에서 증상군의 백혈구 수가(P=0.007) 많았고, 혈색소 값은(P=0.007) 더 낮았으며 BUN과 크레아티닌 값은 차이가 없었다. 혈청 알부민 값은 증상군에서 학교신검군에 비해 더 낮았고(P=0.006), 두 군에서 치료기간은 차이가 없었고, 치료약제는 ACEI가 가장 많았다. 전체 환아군을 대상으로 광학적 신조직 검사분석 시, 육안적 혈뇨기왕력이 있는 군에서 없는 군에 비해 좀 더 심한 병변을 보였으며, 단백뇨의 중증도와 병변의 중증도와는 상관이 있었다. 결론: 학교신검군 및 증상군 모두 조기 진단된 IgA 신증으로 임상적 및 조직학적인 큰 차이를 볼 수 없었다. 학교신검은 조기에 IgA 신증을 선별하는데 유용하지만, 조기진단이 치료나 예후에 어떤 영향을 주는지는 향후 보다 많은 대상자들의 장기간 추적관찰이 더 필요할 것으로 생각된다.

  • Gorlin-Goltz syndrome is an autosomal dominant disorder with a high degree of penetrance. It is characterized by basal cell carcinomas, odontogenic keratocysts, palmar and plantar pits and ectopic calcifications of falx cerebri. The presence of two major and one minor criteria or one major and three minor criteria are necessary to establish a diagnosis. Early diagnosis and treatment of Gorlin-Goltz syndrome, as well as family screening and genetic counseling are essential as it may be associated in 10% of patients with aggressive basal cell carcinoma and malignant neoplasias. We report here a patient with Gorlin-Goltz syndrome.

  • Electrical Designs for Application Specific Integrated Circuits (ASIC) has undergone a change recently with the advent of the sub-wavelength lithography. The optical projection with 193 nm wavelength has been further extended with the use of immersion and other techniques. The competing trends for printing smaller design features have been discussed in this paper with the discussion of the electrical layout analysis to find unfriendly design features. The early knowledge of the unfriendly design features allows remedial actions in time for better yield on the wafer. There are existing standard design qualification criteria being used in the design and fabrication community, but they seem to be insufficient to guarantee defect free designs. This paper proposes an integrated approach for screening the layout with multiple aspects: layout geometry based, graphical analysis and process model based verification. The results have been discussed with few example design features from the 28nm design layout.

장애아전담시설기관의 장애아에 대한 신체성장 및 발달평가 (Growth and Development of Children with Disabilities in Registered Early Educational Institutions)

  • Purpose: The purpose of this study was to look for physical growth and developmental retardation of the disabled children in registered educational institutions. Method: The participants in this study were 103 children. The data was collected from January to September 2006. Body weight, height and head circumference were measured and the Denver II developmental screening tool was used to measure personal-social development. Results: For 40.8% of the children body weight was less than the average for Korean children of the same age, for 49.5% height was lower than average and for 44.7% head circumference was lower. For 84.5% of the children there was a delay in personal-social development, for 87.4% a delay in fine motor-adaptive development, for 80.6% a delay in speech development and for 81.6% a delay in gross motor development. Conclusion: Based on the findings of this study, it is necessary to develop an individual developmental programs for these children based on their growth and development status.

  • Transcranial Doppler(TCD) is an important diagnostic tool for evaluating the patients with stroke. It has some advantages and unique role when compared with other neuroimaging modalities. Recent development of transcranial color-coded Doppler(TCD) improves the limitation and pitfalls of TCD. The current indications of TCD are as follows: 1. Screening and evaluation of the intracranial major vessels 2. early detection and follow-up of vasospasm due to SAH 3. emboli detection (high-imtensity transient signals, HITs) 4. dignosis and follow-up of subclavian steal 5. evaluation of intracranial collaterals when the extracranial ICA has severe stenosis or occlusion 6. evaluation of cerebral perfusion pressure (intracranial pressure) 7. evaluation of arteriovenous malformation 8. diagnosis and follow-up of arterial dissenction 9. diagnosis and follow-up of venous sinus thrombosis (experimental).
