두 마리 고양이에서의 염증성 용종 예 (Inflammatory Polyps in 2 Cats)

  • 고양이의 귀에서 발생하는 염증성 용종은 중이 또는 귀이두관에서 유래하여 인두 또는 외이도관으로 파급되는 비종양성, 염증성 성장을 말한다. 두 마리의 2세령 암컷 고양이(Russian blue)가 이도 삼출물, 외이의 가피 형성, 머리 또는 귀를 흔드는 증상을 약 2-3주간 나타내었다. 이도에서 외과적으로 적출한 두 개의 종괴를 검사하였다. 병리조직학적으로 이 종괴들은 증식된 섬모상피 또는 비각화중층편평상피로 피복되어 있으며, 부분적인 미란 또는 궤양이 형성되어 있었다. 종괴는 증식된 결합조직과 광범위한 단핵세포 침윤으로 구성되어 있었다. 면역조직화학 염색을 실시한 결과 약 90%의 단핵세포들은 CD3 양성인 T 림프구로 증명되었다. 종괴에 대한 중합효소연쇄반응을 실시하였으나 바이러스 병원체는 모두 음성으로 확인되었다. 따라서 임상 소견 및 병리학적 소견을 토대로 두 종괴는 고양이의 중이에서 유래한 염증성 용종으로 확진되었다.

외이도에 발생한 악성종양에서 유경근육피판을 이용한 결손 재건 (Utilizing Pedicled Muscle Flap for Defect Coverage after External Auditory Canal Cancer Resection)

  • Purpose: Cancer arising from the external auditory canal is a rare disease. A lesion that seems harmless in someway, can be lethal when inadequately excised, the tumor may infiltrate nerves, the parotid and auditory tissues before re-invading the skin. Wide resection of the lesion surrounding the structure and reconstruction with an adequate plan is crucial for the treatment of this disease. Methods: Two patients with external auditory canal cancer were treated with muscle flaps and skin grafts. Lateral temporal bone resection (LTBR) was performed for complete resection of the cancer. The defect cavity was obliterated with highly vascularized tissue using pedicled sternocleidomastoid muscle, and temporalis muscle individually, combined with full thickness skin graft for covering the skin defect of the ear. Results: Clear resection margin was obtained, and both patients showed disease free survival during the follow up. There was no complications of hematoma, infection, flap loss, or wound problem in both patients. Both patient received radiation therapy, there was no osteoradionecrosis or any other complication related to radiation therapy. Conclusion: Utilizing pedicled muscle flaps for managing defects after wide resection of the external auditory canal cancer is an effective method for managing this difficult disease.

  • An osteoma of the external auditory canal(EAC) is an uncommon benign tumor. If a canal obstruction occurs, ear fullness or conductive hearing loss can arise. But, the association of an osteoma with a cholesteatoma is extremely rare. Therefore, we report a case of a 35-year-old female patient with osteoma of temporal bone that was complicated with EAC cholesteatoma.

Single Dose Topical Application of Clotrimazole for the Treatment of Otomycosis: Is This Enough?

  • Background and Objectives: To determine the efficacy of filling the external auditory meatus with 1% clotrimazole at a single visit for the treatment of otomycosis. Subjects and Methods: This prospective study included 40 patients who were referred to our clinic with complaints of ear itching, pain, and fullness, and were diagnosed with unilateral otomycosis. After cleaning the mycotic hyphae from the external auditory meatus, the ear canal was filled with 1% clotrimazole, using an intravenous catheter and syringe. The patients received follow-up examinations on post-treatment days 7, 15, and 45. Results: The follow-up otomicroscopic examinations revealed that 95% of the ear canals were entirely clean and that all symptoms had resolved. The post-treatment scores of pain, aural fullness and itching were significantly lower than the pre-treatment scores (p<0.01). Conclusions: Filling the external auditory meatus with 1% clotrimazole at a single visit is an easy, efficient, and cost-effective treatment for otomycosis. Additionally, high patient compliance makes this treatment superior to long-term topical therapy.

  • Angiolymphoid hyperplasia with eosinophilia(ALHE) is an uncommon idiopathic condition that presents with isolatedor grouped cutaneous plaques or nodules of the head and neck. Extracutaneous involvement is rare. ALHE is a distinct pathologic entity marked by a proliferation of blood vessels with distinctive large endothelial cells accompanied by a characteristic inflammatory infiltrate that includes eosinophils. The lesion is benign but may be persistent and difficult to eradicate. The authors have recently experiecned a case of angiolymphoid hyperplasia with eosinophilia in a 52-year-old male who had a painless enlarging mass in his right preauricular area and external ear canal for several years. We present this case with the review of literatures.

한국인 성인남자의 개별 이도내 음향응답에 대한 경계요소 해석 (Boundary Element Analysis for Individual Acoustic Responses in Ear Canal of Korean Adults)

  • Individual differences in head-related transfer functions(HRTFs) were calculated using boundary element(BE) models for three Korean adults. The BE models for the individuals were developed from the computerized tomography(CT) images of the individuals. The BE models were composed of the head, pinna, and ear canal. The frequency-dependent impedance boundary conditions were imposed on the skin, hair, and tympanic membrane. The HRTFs calculated from the individual BE models showed large difference above 2 kHz in magnitude and in the locations of peaks and valleys of the frequency spectrums, which should be considered in virtual auditory sound field. The identified individual differences in the HRTFs demonstrate that the developed BE models can be utilized successfully in order to obtain the HRTFs information of individuals.

Efficacy of aromatherapy for the treatment of otitis externa in dogs

  • This study examined the therapeutic effects of aromatherapy for the treatment of otitis externa in dogs. Eleven dogs with otitis externa were examined. The control group (5 dogs) was treated with susceptible antibiotics, and the experimental group (6 dogs) was treated with aroma-oil applied topically to the ear canal. The aroma-oil contained 10 ml sweet almond oil, 0.3 ml bergamot oil, 0.2 ml lavender oil, 0.1 ml tea tree oil and 0.1 ml roman chamomile oil. The blended aroma-oil (0.1 ml) was applied to the ear canal twice daily for 2 weeks. The authors examined the changes in the clinical signs, bacterial count in discharges, total WBC count and neutrophil/lymphocyte ratios in the two groups. The bacterial cell counts in the experimental group were significantly lower at one (p < 0.01) and two weeks (p < 0.05) after treatment than the control group. These results suggest that aromatherapy is an effective and practical treatment for otitis externa in dogs.
