A Case Report of Otitis Media in Three Children Treated with the Sa-Am Acupuncture Bi-Jung-Geouk(脾正格) and Bojoong-Ikgi-Tang(補中益氣湯) (사암침법(舍岩鍼法)의 비정격(脾正格)과 보중익기탕(補中益氣湯) 복합치료를 통한 소아 중이염환자 치험 3례(例))

  • Obiectives : Otitis media is the disease which indicates every inflammatory situation in the middle ear. This includes the inflammation in the ear canal and mastoid sinus. In this study, the effect of Sa-Am acupuncture Bi-Jung-Geouk(脾正格) and Bojoong-ikgi-tang(補中益氣湯) for three children patients with otitis media were evaluated. Methods : After treatment with the Sa-Am acupuncture Bi-Jung-Geouk(脾正格) and Bojoong-ikgi-tang(補中益氣湯) for 3~6 weeks, we evaluated the effect by the figure of otoscope and complaints of patients. Results : In three cases, the pus in the figure declined dramatically and symptoms of patients got better after 3~6 weeks treatment. Conclusions : The Sa-Am acupuncture Bi-Jung-Geouk(脾正格) and Bojoong-ikgi-tang(補中益氣湯) for otitis media was effective. It will be attempted to the patients with it.

The Clinical Application of Sound-Protection

  • Bekesy measured the sound transformation system of the middle ear 49 years ago. According to his reports, a ratio between the size of ear drum and the size of oval window is 17 : 1, and the lever function of the ossicles is physiologically 1.3 : 1. Therefore, the hearing might be aggravated to 27.5㏈ in the case of the vanishment of 3 ossicles. In 1952, Wullstein reported 5 types of tympanop-lasty and the fourth type among them was especially named for the sound-protection. The oval window is only exposed by the sound pressure and the round window is not exposed. According to the application by this idea, the post-operative hearing might be improved until 27.5㏈. Mean while, in 1942, Onchi verified through his experiment that the results of Bekesy's measurement was not completely conformed to Onchi result. Bekesy measured the sound pressure on the stapes plate of the oval window, on the other hand, Onchi measured the sound pressure on the surface of the perilymph of the oval window after removing the stapes plate(Fig. 1).(Figure omitted) The difference of their experiment is recognized that the impedance of the stapes plate exists or not (Fig. 1). Both Audiogrums are compared as Fig. 2. The result of IV type of tympanoplasty is success ful in 54% as the Table 1. (Table Omitted) The reason of unsatisfactory is caused by the thick and unmovable window-membrane and by the closing of air passage to the round window. The closing of the air passage to the round window is occurred by the adhesion between the grafting membrane and the surface of promontorium. In order to preserve this adhesion, I produce to transplantate the mucous membrane of the lip to the bone surface of tympanic cavity after removing the granulation tissue of the tympanic cavity and to form a membranous canal for the sake of air passage (Fig. 3). (Figure Omitted) The post-operative hearing by this method is shown as Fig. 4, 5. In other words, the post-operative sound pressure entered into the cochlea directly, by way of the oval window only, not by way of the round window, as a theorie of the sound protection. A Study of the Characteristics of the Human External Auditory Canal Using 3-Dimensional Medical Imaging (3차원 의료영상을 이용한 인체 외이도 특징에 관한 연구)

  • Using Digital Imaging and Communications in Medicine(DICOM) and a 3D medical imaging program, the characteristics of the external auditory canal(EAC) were compared. Using images of the ears of 63 different male and female subjects of varying age, this study measured and compared EAC transverse axis lengths, internal diameter circumferences, and upper and lower curvature angles. The findings of the study indicated differences in EAC shapes according not only to age and sex but also to the left and right of the same subject. A comparison between the sexes of the subjects (35 males and 28 females) indicated that, on average, the length of the EAC was 4.75mm longer in males. Based on the lower curvature angle, the interior side of the diameter circumference of the EAC was found to be reduced on average by 37.2% compared to the exterior side. Although the upper curvature angle was on average $25.7^{\circ}$ larger than the lower curvature angle, 4 subjects showed a larger lower curvature angle and large differences between the upper and lower curvature angles were observed in 8 subjects of the younger age group (4~14 years old). This indicated changes in EAC curvature shapes during growth. This study presents a method to raise safety and precision by comparing direct measurements taken through physical means and indirect measurements acquired from existing ear samples. This was possible due to technological developments in which 3D medical image representation technology creates images close to reality, and, through further development, this method is expected to be used for standardization research of EAC shapes.

One Case of Otomycosis with Chronic Suppurative Otitis Media Treated with Korean Medical Treatment (만성 화농성 중이염에 동반된 이진균증 환자 한방 치료 1례)

  • Objectives : Otomycosis is superficial fungal infection of external auditory canal accompanied with itching, pain, discharge, hearing loss, tinnitus and ear fullness. And chronic inflammation like chronic suppurative otitis media exacerbates otomycosis. The purpose of this study is to report one case of Korean medical treatment for otomycosis with chronic suppurative otitis media. Methods : This study was conducted with one case of otomycosis with chronic suppurative otitis media patient hospitalized in Ophthalmology, Otolaryngology & Dermatology Clinic of Korean medical Hospital. This patient was treated with Korean medical treatment including herbal medicine, acupuncture, pharmacopuncture and venesection. After the treatment, we took picture of external auditory canal by otoscope and evaluated subjective symptoms. Results : After the treatment, this patient had improved condition of external auditory canal and subjective symptoms. Conclusions : This study suggests that Korean medical treatment may be effective for otomycosis with chronic suppurative otitis media.

  • This paper represents the role of pinna in localizing target direction. Specially, this paper described what is the role of right-side pinna versus left-side pinna. In this experiment, one side of the pinna function was distorted intentionally by inserting a short tube on the ear canal. The localization error caused by right and left side pinna distortion was investigated. Since a laser pointer showed much less error (0.5%) in localizing target position than FASTRAK (30%) that has been generally used, it was used for the pointing task. It was found that harmonic components were not essential for the auditory target localization, however, non-harmonic nearby frequency components were more important to localize target direction of sound. We have found that the right pinna is one of the most important functions in localizing target direction and pure tone with only one frequency component is confusing for localization.

Genomic Tandem Quadruplication is Associated with Ketoconazole Resistance in Malassezia pachydermatis

  • Malassezia pachydermatis is a commensal yeast found on the skin of dogs. However, M. pachydermatis is also considered an opportunistic pathogen and is associated with various canine skin diseases including otitis externa and atopic dermatitis, which usually require treatment using an azole antifungal drug, such as ketoconazole. In this study, we isolated a ketoconazole-resistant strain of M. pachydermatis, designated "KCTC 27587," from the external ear canal of a dog with otitis externa and analyzed its resistance mechanism. To understand the mechanism underlying ketoconazole resistance of the clinical isolate M. pachydermatis KCTC 27587, the whole genome of the yeast was sequenced using the PacBio platform and was compared with M. pachydermatis type strain CBS 1879. We found that a ~84-kb region in chromosome 4 of M. pachydermatis KCTC 27587 was tandemly quadruplicated. The quadruplicated region contains 52 protein coding genes, including the homologs of ERG4 and ERG11, whose overexpression is known to be associated with azole resistance. Our data suggest that the quadruplication of the ~84-kb region may be the cause of the ketoconazole resistance in M. pachydermatis KCTC 27587.

Evaluation of Thermal Comfort on Protective Clothing Worn in an Radiation Power Plant

  • The purpose of this research was to device thermal comfort model for radiation power plant workers in protective clothing. Three fabrics commonly used in protective workwear were made into coveralls of identical design and were evaluated by adult healthy males in four simulated work environment. It was investigated between the physiological response and subjects comfort according to environmental variance and clothing types. The of simulated work enviro mensent was controlled under four different humidity and temperature of each type. (Temperature 20±1℃, RH 40∼70%±5%, Temperature 30±1℃, RH 40∼70%±5%) An index of physiological response was connected with the thermal comfort designed. Mean skin temperature, skim temperature, Axillasy temperature ear canal temperature, clothing climate, total sweat, blood pressure, and R-R interval were be evaluated. Skin temperature difference ocurring during exercise and rest were significant only with respect to time and regions of the body, This despite physical differences in the three coveralls, particulary mass statistically experiment. Also, an index of subject wearing sensation was designed for thermal comfort after investigation determined the kind of clothes and the type of environment. As a result of this research, two types of multiple regressions was deviced to estimate thermal comfort of the protective clothing.

  • Distortion Product Otoacoustic Emissions (DPOAEs) can be measured in the external ear canal two fold: amplitude and latency, but most DPOAE studies deal with amplitude aspects. The purpose of this study was to investigate the latency of the 2f1-f2 DPOAEs in ears with hearing losses and to see if it could be a clinically useful method to distinguish normal from abnormal ears. For this purpose, DPOAE latency were measured as a function of frequency from 1 to 8 kHz in 30 ears with conductive and sensorineural hearing losses (SNHLs). DPOAEs were recorded with Otodynamic Analyzer ILO92. Results showed that the latency decreased as the frequency increased up to 8 kHz. The mean values of DPOAE latency for ears of SNHLs were shorter at all frequencies when they were compared to the mean values of normal ears. The latency in ears of conductive hearing losses was shorter than normal ears at the selective frequencies, as well. The results support the hypothesis that latency values are shorter in pathological ears.

  • Skull-base osteomyelitis is a rare disease affecting the medulla of the temporal, sphenoid, and occipital bones. In general, it occurs due to external ear canal infections caused by malignant external otitis. Skull-base osteomyelitis usually affects elderly diabetic patients. The patient, a 58-year-old man, was referred for evaluation and management of the left jaw. Clinical examination of the patient revealed pain in the left jaw and mouth-opening deflection to the left. The maximum active mouth opening was measured to about 27 mm. Panoramic, CT, and CBCT revealed bone resorption patterns in the left condyle. Through control of diabetes, continued pharmacological treatment, arthrocentesis, and occlusal stabilization appliance therapy were carried out. The extent of active mouth opening was increased to 45 mm, and pain in the left jaw joint was alleviated. This was a case wherein complications caused by failure to control diabetes induced skull-base osteomyelitis. There is a need for continued discussion about the advantages and disadvantages of arthrocentesis with lavage for patients with skull-base osteomyelitis and other treatment options.

Ceruminous Gland Adenocarcinoma in a Maltese Dog (말티즈견에서의 귀지선암종 1례)

  • A 12-year-old, female, Maltese dog was presented with otic signs, such as pruritus and malordorous discharge. Medical treatment of otitis externa had been carried out for several months at primary animal hospital. Radiography, ultrasonography (US), computed tomography (CT), fine needle aspiration (FNA), and histopathological examination were performed for the diagnosis of ceruminous gland adenocarcinoma. Total ear canal ablation and lateral bulla osteotomy were performed for excision of the tumor. Facial nerve paralysis developed after surgery is getting better through the acupuncture therapy.
