Effect of lonizing Radiation on the Host Resistance Against Listeria Monocytogenes Infection and the Cytokine Production in Mice (방사선조사후 마우스에서의 Cytokine 생산능 및 Listeria monecytogenes에 대한 저항성의 변화)

  • Purpose : To evaluate the qualitative immunologic changes by ionizing radiation. we studied the altered capacities of the macrophages and lymphocytes to produce cytokines in conjunction with resistance to Listeria monocytegenes (LM) infection in mice Materials and Methods : BALB/c mice and Listeria monocytogenes were used. The mice were infected intraperitoneally with $10^5LM$ at 1 day after irradiation (300cGy) and sacrificed at 1, 3, 5 days after infection, and then the numbers of viable LM per spleen in the irradiated and control group were counted. Tumor necrosis factor-alpha ($TNF-\alpha$), interferon-gamma ($IFN-\gamma$). interleukin-2 (IL-2), and nitric oxide (NO) were assessed after irradiation. Results : Under gamma-ray irradiation with a dose range of 100-850cGy, the number of total splenocytes decreased markedly in a dose-dependent manner, while peritoneal macrophages did so slightly Cultured peritoneal macrophages produced more $TNF-\alpha$ in the presence of lipopolysaccharide (LPS) during the 24 hours after in vitro irradiation, but their capacity of $TNF-\alpha$ Production showed a decreased tendency at 5 days after in vivo total body irradiation. With 100cGy and 300cGy irradiation, cultured peritoneal macrophages produced more NO in the presence of LPS during the 24 hours after in vitro irradiation than without irradiation. Activated splenocytes from irradiated mice (300cGy) exhibited a decreased capacity to Produce IL-2 and $IFN-\gamma$ with Concavalin-A stimulation at 3 days after irradiation. When BALB/c mice were irradiated to the total body with a dose of 300cGy, they showed enhanced resistance during early innate phase, but a significant inhibition of resistance to LM was found in the late innate and acquired T-cell dependent phases. Conclusion : These results su99es1 that increased early innate and decreased late innate and acquired immunity to LM infection by ionizing radiation (300cGy) may be related to the biphasic altered capacity of the macrophages to produce $TNF-\alpha$ and the decreased capacities of the lymphocytes to produce IL-2 and $IFN-\gamma$ in addition to a marked decrease in the total number of cells.

Usefulness of the Chin Press Maneuver in Assessing the Severity of Obstructive Sleep Apnea Syndrome (폐쇄성 수면무호흡증후군의 진단에 있어 턱 압박술의 유용성)

  • Objectives: Obstructive sleep apnea syndrome (OSA) is a moderately prevalent disorder. Even though much progress has been made in the diagnosis of this disorder, the cost-effectiveness of nocturnal polysomnography is undertermined and physicians and patients are still hesitant to undergo this procedure. The authors wanted to see the validity of chin press/tongue curl maneuver in estimating the severity of OSA which is easy to measure and was originally proposed by Simmons etc. by looking at the correlations between this score and the conventional respiratory disturbance indices. Methods: Forty-three sleep-related breathing disorder patients (28 OSA patients and 15 upper airway resistance syndrome (UARS) patients) who underwent investigation for posssible OSA were studied. Two conventional indices of OSA (apnea/hypopnea index (AHI) and oxygen saturation dip rate (SaO2 dips)), four other sleep variables (lowest SaO2, % of time with SaO2<90% (%SaO2 <90), % of sleep stage 1, mean length of SaO2 dips) and the score of Epworth sleepiness scale (ESS) were compared with the chin press score (CPS) which was newly revised by the author and ranges from 0 to 6. Results: The age of subjects was $45.95{\pm}12.47$ (range 14-76) and their average BMI was $25.98{\pm}3.61$ (range 19.65-37.64). There were no significant differences in age, sex and BMI except repiratory disturbance indices and ESS (p<0.05) between OSA and UARS group. Grouped median CPS of the all subjects was 4.14 (range 1-6). There was a remarkable relationship between CPS and diagnosis category (Likelihood Ratio $X^2$ test; $X^2$=17.41, df=5, p=0.004) and measures of association (Somers' $d=0.65{\pm}0.12$, t=4.83, p=0.000) indicated that CPS increased when the diagnosis changed from UARS to OSA. Spearman's rank correlations between CPS and SaO2 dips (R=0.83), between CPS and AHI (R=0.77) were good (p<0.001). Other variables except mean length of SaO2 dips showed good correlations with CPS as well (p<0.05). Regression analysis indicated that when CPS is 3 there is a provability of 0.35 to have AHI of less than 5. Conclusion: Chin press scores that can be measured easily is well correlation with the conventional sleep apnea indices. They may therefore provide a useful guide in diagnosing obstructive sleep apnea synrome.

Cette $th{\grave{e}}se$ est $commenc{\acute{e}}$ ${\grave{a}}$ ${\acute{e}}crire$ pas pour apprendre ses textes mais pour comprendre la ralation entre les textes et les ${\exp}{\acute{e}}riences$ de l'auteur. L'auteur dramatique et le metteur en $sc{\grave{e}}ne$ du $th{\acute{e}}{\hat{a}}tre$ Young-sun Yun propose certaines orientations de la mort de son $p{\grave{e}}re$ dans ses texts dramatiques: le sentiment de victime sous le pouvoir absolu de son $p{\grave{e}}re$. Chez lui, ${\acute{e}}crire$ le texte dramatique, c'est transmettre les amertumes d'esprit et une forme de $r{\acute{e}}flextion$ vis-${\grave{a}}$-vis de la relation avec son $p{\grave{e}}re$ surtout le $probl{\grave{e}}me$ $inachev{\acute{e}}$ de deuil. c'est-${\grave{a}}$-dire fournir des sentiments coupables et des $anxi{\acute{e}}t{\acute{e}}s$ qui $r{\acute{e}}sident$ au fond de son coeur, tout au long de sa vie. Pour cette objecif, il est ${\acute{e}}vident$ qu'il faudra toujours partir du texte et y revenir. J'ai $essay{\acute{e}}$ de ne pas $r{\acute{e}}duiser$ $l^{\prime}{\acute{e}}tude$ des textes au respect myope des paroles des personnage et rester la $r{\acute{e}}flextion$ pshchologique sur la mort et le processu de deuil. J'ai $essay{\acute{e}}$ d'appuyer mes remarques sur les sources $autoris{\acute{e}}es$: psychanalytiques en la $mati{\grave{e}}re$. C'est pourquoi j'ai cru devoir $pr{\acute{e}}senter$ cette recherche de l'analyse sous la forme d'une $synth{\grave{e}}se$ des principaux concepts de la $th{\acute{e}}orie$ de la pshchologie moderne qui permettent l'analyse des textes dramatiques.(Dany Nobus, David K. Switzer, Fritz Riemann, Giles Deleuze, Sigmund Freud, Julia Kristeva) Et auiss j'ai $essay{\acute{e}}$ de explorer l'imaginaire des paroles dans les textes en ses rapports avec l'auteur et les personnages et entre les personnages. Ainsi conçu, j'ai $analys{\acute{e}}$ les formes de paroles en ce qui concerne la mort de $p{\grave{e}}re$ et le deuil. Cette $mani{\grave{e}}re$ s'est $r{\acute{e}}v{\acute{e}}l{\acute{e}}e$ efficace pour montrer comment des traces de sentiment se transforment dans des textes. Dans les pluparts des personnages chez les textes de Young-sun Yun, on peut voir si $d{\acute{e}}finitivement$ leur cicatrices personnelles, $sensibilit{\acute{e}}s$ $m{\hat{e}}l{\acute{e}}s$, ce 'je-ne-pourrai-rien-contre-mon-$p{\grave{e}}re$' ou 'je-ne-sais-quoi-et-presque-$apr{\grave{e}}s$-la mort-de-mon pere'. Pour $d{\acute{e}}pouiller$ cet aspect, j'ai $montr{\acute{e}}$ qu'il n'y avait pas de $v{\acute{e}}rit{\acute{e}}s$ objectives, mais que la $subjectivit{\acute{e}}$ doit trouver le jeu triangulaire entre l'inconscient de l'auteur par les paroles des textes et par les rapports entre le symbolisant et le $symbolis{\acute{e}}$ dans les $repr{\acute{e}}sentation$ socio-culturelle en $Cor{\acute{e}}e$ d'aujourd'hui.

  • Der neue strukturelle Ansatz der Kommbination eines Rahmen$\ddot{u}$bereinkommens und eines ausr$\ddot{u}$stungsspezifischen Sonderprotokolls bedingt einen neuen organisatorischen Anstz f$\ddot{u}$r die Zusammenarbeit zwischen internationalen Organisationen bei der Schaffung von internationalem Einheitsprivatrecht. So haben hier zwei internationale Organisationen gemeinsam die Verantwortung f$\ddot{u}$r einmultilaterales $\ddot{U}$bereinkommen $\ddot{u}$bernommen: auf der einen Seite UNIDROIT als die internationale Organisation, die generell f$\ddot{u}$r die Vereinheitlichung des Privatrechts kompetent ist; auf der anderen Seite ICAO als die f$\ddot{u}$r die private Luftfahrt zust$\ddot{a}$ndige internationale Organisation. Dieses neue, f$\ddot{u}$r die Luftfahrzeugausr$\ddot{u}$stung praktizierte organisatorische Modell eines joint venture zweier internationaler Organisation bei der Einheitsrechtsetzung, namlich die Betreuung eines allgemeinen privatrechtsvereinheitlichenden Rahmens$\ddot{u}$bereinkommens durch UNIDROIT und die Wahrnehmung der sektorspezifischen Belange in einem ausr$\ddot{u}$stungsspesifischen Sonderprotokoll durch die jeweils zust$\ddot{a}$ndige internationale Spezialorganisation, hat bereits f$\ddot{u}$r die Sektoren der Eisenbahn- und Weltraumausrustung Schule gemacht. Das in Kapstadt beschlossene v$\ddot{o}$lkervertragliche Regelungswerk hat erstmals ein einheitsrechtliches - grunds$\ddot{a}$atzlich weltweite Geltung anstrebendes - Sicherungsrecht geschafen. Dies kann f$\ddot{u}$r die Sachenrechtsintergration einen $\ddot{a}$hnlichen Durchbruch bedeuten, wis das Wiener UN-kaufrechts$\ddot{u}$bereinkommen von 1980 f$\ddot{u}$r das Schuldvertragsrecht. Voraussetzung daf$\ddot{u}$r ist allerdings die juristische Qualit$\ddot{a}$t und Praxisgerechtigkeit des Regelungswerkes und - insbesondere - das Funktionieren des Registersystems. Von wesentlicher Bedeutung f$\ddot{u}$r den Erfolg des $\ddot{U}$bereinkkommens wird auchsein, ob es Rechtssicherheit zu gew$\ddot{a}$hrleisten vermag.

The Variation of Natural Population of Pinus densiflora S. et Z. in Korea(I) -Characteristics of Needle and Wood of Chuwang-san, An-Myeon-do and Odae-san Populations- (소나무 천연집단(天然集團)의 변이(變異)에 관(關)한 연구(硏究)(I))

  • F$\ddot{u}$r die Erforschllng der Variationen bei der Naturpopulationen der Pinus densifiora wurden drei Populationen aus Chuwangsan (Kyong-buk), Anmyondo (Chung-nam) und Odaesan (Kangwon) Ausgewiiblt. Bei je 20 Individuen jeder Population wurden die Variationen von der Eigenschaft der $\ddot{a}$usseren Gestalt, von der anatomischen und morphologischen Eigenschaft der Kiefernadel und von der Holzqualit$\ddot{a}$t an Bohrsp$\ddot{a}$nen untersucht. Das Ergebnis der Untersuchung konnte in den folgenden sechs Punkten formuliert werden. 1. Von den Eigenschaften der $\ddot{a}$usseren Gestalt ist die Geradheit des Baumstamms, der Durchmesser der Zweige und der Winkel der Verzweigung in der Odaesan Population wlinschenswerter als in anderen Populationen. 2. Es wird festgestellt, dass sich die drei Nadelmerkmale, Anzahl der Z$\ddot{a}$hne je 0.5 cm, Anzahl der Stomata-Reihen und Anzahl der Harzkan$\ddot{a}$le, sowohl zwischen den Populationen als auch zwischen den Individuen signignifikant unterscheiden. 3. Bei der Betrachtung des Harzkanal Index, der Schichtanzahl der Hypodermis und der Entfernung der Fibrovasalb$\ddot{u}$ndel wird aufgezeigt, dass sich alle drei gepr$\ddot{u}$ften Populationen geringf$\ddot{u}$gig von der normalen Kiefer unterscheiden. 4. Zwischen den Populationen ergibt sich ein Unterschied im Prozentanteil des Spatho lzes und im spezifischen Geswicht, jedoch zeigt die Breite der Jahresrings und die Tracheidenl$\ddot{a}$nge keinen Unterschied. Das spezifische Gewicht und die Tracheidenl$\ddot{a}$nge der Individuen jeder Population ist signifikant verschieden. 5. Bei dem Alter 11-20 ist der Prozentanteil des sp$\ddot{a}$tholzes am gr$\ddot{o}$ssten und wird aber danach immer geringer bis zum Alter 30; sp$\ddot{a}$ter zeigt sich dann keine sichtbare Ver$\ddot{a}$nderung mehr. Das spezifische Gewicht sich vergr$\ddot{o}$ssert bei der Zunahme des Jahresrings, nimmt aber dann in Anmyondo und Odaesan Populationen beim Alter 21-30 wieder ab. Die Tracheidenl$\ddot{a}$nge der drei Populationen besitzen die Neigung, sich nach der zunahme der Jahresringe zu vermehren. 6. Mit diesen hier behandelten Charakteren werden ganz allgemeinen die Unterschiede der drei Kiefer populationen festgestellt. Diese Ergebnisse sind sehr bemerkenswert betrachtet vom genetischen Gesichtspunkt der waldbaumpopulation.

A Study on Curriculum Improvement of the Korea Army Nursing Academy (국군간호사관학교 교육과정 개선을 위한 기초 연구)

  • 1. Need for and Purpose of the Study. There is an increasing demand for curriculum improvement of the Korean Army Nursing Academy (KANA), since it was upgraded into 4-year institution of higher learning from 3-year one. In particular, it is strongly advocated that the KANA needs the outside expertise for its curriculum improvement-namely not only from the internal military view of points but also from the viewpoints of professional educational society, In line with such a necessity for the study, this study was aimed at 1) analyzing the current actual practices of KANA'S curriculum, 2) investigating the desired practices of KANA'S curriculum, and 3) identifying the discrepancy between the actual and desired practices of curriculum. 2. Problems for the Study This study had 4 problems to be answeared as follows: 1) What are the actual curriculum practices of KANA? 2) What are the desired curriculum practices of KANA? 3) How are the extents of perception of actual and desired curriculum different in four groups (student, faculty & administrator, nurse, and medical doctor in militay hospital) ? 4) What are the restraining forces that impede the change from actual to desired curriculum practices? 5) What are the relationships of curriculum component,』 in actual and desired curriculum practices? 3. Methods and Procedures This study was conducted by means of document analysis in addition to literature review and by means of needs assessment questionnaire which was developed by the researcher. The questionnaire included 62 statments with 7 questions for demographic data collection. The needs assessment questionnaire was managed to a total of 243 subjects (100 students, 46 faculty & administrators, 55 nurses, and 42 medical doctors), The collected data were treated using SPSS computer system so as to calculate mean scores, standard deviations, and correlation coefficients. The significance test was made through t-test and one-way ANOVA. The statistical significance level was set at both .05 and .01 level. 4. Major findings The major findings in this study are as follows: 1) The score of desired practices was significantly greater than that of actual practices, representing a strong need for curriculum betterment. 2) There were significant differences in the perceptions of actual practices as well as desired practices among four groups (student, faculty & administrater, nurse, and medical doctor). 3) The most frequently selected restraining forces were army's inherent character, economical limitation, and educational expertise limitations. 4) Such variables as sex, position attachment to the KANA and grade made a statistically significant effect on the perception of desired curriculum practice, while the variables like marrige, position, and military class made it on the perception of actual curriculum practice. 5) The coefficients among the curriculum components were lower in perception of the actual curriculum practices than those in the desired practices. 5. Conclusions The conclusions based on the major findings of this study are as follows: 1) The current curriculum development procedure of the KANA is not consistent with the theoretical frame of systematic development sarategy of curriculum. 2) There are wide conflicts among the groups who are supposed to participate in curriculnm development, concerning the actual and desired practices of KANN'S curriculum. 3) A great deal of need for curriculum improvement for the KANA is clearly felt, and in particular, in the process of teaching and learning. 4) Each component of curriculum is not intergrated into a whole development procedure, being segregated each other. 5) For better curriculum improvement, such restraining forces as financial and professional limitations should be eliminated. 6. Recommendations 1) For Further Research a. There is a need to replicate this study after in-depth statistical analysis of each item of need assessment questionnaire, and with more representative subjects. b. A study should be conducted which. has its focus on the analysis of restraining forces for the change from actual to desired curriculum practices of the KANA. 2) For KANA'S Curriculum Improvement a. There is a need to promote the professional expertise of the participants in curriculum development and the communication among them. b. It is desirable to establish an institution or section of administration, which is soley in charge of curriculum development. c. To better develop KANA's curriculum not only faculty and administrators but also students should be encouraged to participate in development process, while the military medical doctors' participation should be carefully considered.

Die Problematik auf gesetzliche Terminologie und gewerbliche Nutzung von Drohne (드론의 현행 법적 정의와 상업적 운용에 따른 문제점)

  • Auf die ganze Welt macht unbemannte $Flugger{\ddot{a}}te$(sog.Drohnen) in vielen Bereichen rasch Fortschritte und Anwendungen gezeigt. Nachdem ferngesteuerte Drohnen $urspr{\ddot{u}}nglich$ $prim{\ddot{a}}r$ $f{\ddot{u}}r$ $milit{\ddot{a}}rische$ Zwecke entwickelt wurden, $erh{\ddot{o}}cht$ sich derzeit ihre zivile Nutzung sowohl im Freizeit- als auch im Dienstleistungsbereich(Paketdrohnen, Drohnen-taxi) stetig. Mit der vermehrten Drohnennutzung steigen allerdings auch die damit verbundenen Risiken und Herausforderungen. In Zusammenhang damit stellt sich dann die Frage, ob $gegenw{\ddot{a}}rtige$ Vorschriften im Bereich von Luftrecht zurecht gekommen sind. Es sieht sich gerade der zwei Schwerpunkt $gegen{\ddot{u}}ber$. Erstens kann $Passagierebef{\ddot{o}}rderung$ mit unbemanntem Luftfahrzeug(mehr als 150kg) im $gegenw{\ddot{a}}ritigen$ Luftrecht keine Anwendung finden. Denn das kor. Luftsicherheitsgesetz und sein Durchsetzungsverordnung definieren die Terminologie von unbemannten Luftfahrzeugen und unbemannten $Flugger{\ddot{a}}te$ als "wenn eine Person nicht an Bord geht und ferngesteuert wird". Also soll Drohne nach dieser gesetzlichen Definition nur "ohne Person" geflogen werden. Das besagt ohne Piloten und ohne Passagiere. Zweitens ist unbemannte $Flugger{\ddot{a}}te$(weniger als 150kg) nicht auf Handelsgesetz anzuwenden, auf das ${\ddot{u}}ber$ Anspruchsgrundlage und Zurechnungsnorm des gewerblichen Luftverkehr geregelt ist. Der unbemannte Luftfahrzeuglieferdienst bringt nicht nur die Gefahr einer $Besch{\ddot{a}}digung$ des Frachtguts mit sich, sondern auch die Gefahr von $Bodensch{\ddot{a}}den$ durch Dritte. Gemäß ${\S}$ 896 des Handelsgesetzes ist aber die Anwendung von unbemannte $Flugger{\ddot{a}}te$(weniger als 150kg) $hierf{\ddot{u}}r$ begrenzt, weil unbemannt $Flugger{\ddot{a}}te$ $einschl{\ddot{a}}gig$ in Ultralight $Flugger{\ddot{a}}t$ ist, die im Handelsgesetz ausschließlich besteht. Technische Fortschritt und die dadurch $erm{\ddot{o}}glichten$ kommerziellen Anwendungen werden die Nachfrage nach unbemannter $Flugger{\ddot{a}}te$ wecken. Die Umsetzung der $bez{\ddot{u}}glichen$ Vorschriften sollte auch diese Entwicklung aktiv begleitet und $fr{\ddot{u}}hzeitig$ kommuniziert und erarbeitet werden, damit Hersteller und Nutzer $fr{\ddot{u}}hzeitig$ Planungssicherheit haben.

  • The purposes of this study are to identify the necessity of utilization of electric thermometer, to determine the difference of clinical thermometers to reach maximum or optimum temperature, and to determine the length of time necessary for temperature taking, with Fahrenheit thermometer, electric thermometer, Yu Ⅱ centigrade thermometer, and Kuk ll centigrade thermometer. The first and second comparative Experiments were' conducted from August 25 through September 30, 1973. In the first experiment, Fahrenheit thermometer, which had been accurately teated two times, and electric thermometer have been utilized. These two kinds of thermometers were inserted simultaneously under the central area of the tongue and the mouth kept closed while thermometers were in place. All temperature readings were done at one minute interval until leaching-maximum temperature. These procedures were repeated one hundred times and the data were-analyzed statistically by means of the t-test. In the second experiment, Fahrenheit thermometer, which had been accurately tested two. times, Yu Ⅱ centigrade thermometer, and Kuk Ⅱ centigrade thermometer have been utilized. These three kinds of thermometers were inserted simultaneously under the central area of the. tongue and the mouth kept closed while thermometer were in place. All temperature readings were done at one minute interval until reaching maximum temperature. These procedures were. repeated one hundred times and the data were analyzed statistically by means of the F-ratio Under the eight hypotheses designed for this study, the findings obtained are as follows: 1. There were no significant differences in the maximum temperature between Fahrenheit thermometer and electric thermometer. The mean maximum temperature for Fahrenheit thermometers was 37.06℃ and for electric thermometer was 37.09℃. 2. The placement time to reach maximum temperature taken by Fahrenheit thermometer was significantly shorter than that by electric thermometer. The mean placement time for Fahrenheit thermometers was 4.04 minutes, for electric thermometer was 5.52 minutes. In the case of Fahrenheit thermometers, 45 to 77 percent after 3 to 5 minutes, over 90 Percent after 7 minutes, and 100 percent after 10 minutes, had reached optimum temperature. When the electric thermometer was used, 23 to 54 percent after 3 to 5 minutes, over 90 percent after 9 minutes, and 100 percent after 12 minutes, had reached optimum temperature. 5. There ware no significant differences in the maximum temperature among Fahrenheit thermometer, Yu Ⅱ centigrade thermometer, and Kuk Ⅱ centigrade thermometer. The mean maximum temperature for Fahrenheit thermometers was 36.67℃, for Yu Ⅱ centigrade thermometer, was 33.73℃, and for Kuk Ⅱ centigrade thermometers was 37.76℃. 6. There were no significant differences in placement time to reach maximum temperature among Fahrenheit thermometer, Yu Ⅱ centigrade Thermometer, and Kuk Ⅱ centigrade thermometer. The mean placement time (or Fahrenheit thermometers was 7.77 minutes, for Yu Ⅱ centigrade thermometers was 7.25 minutes, and Kuk Ⅱ centigrade thermometers was 7.25 minutes. In the case of Fahrenheit thermometers, 8 to 24 percent after 3 to 5 minutes, over 90 percent after 11 minutes, and 100 percent after 13 minutes, had reached maximum temperature. When the Yu Ⅱ centigrade thermometer was used, 10 to 27 percent after 3 to 5 minutes, over 90 percent after 11 minutes, an8 103 percent after 13 minutes, had reached maximum temperature. When the Kuk Ⅱ centigrade thermometer was used, 11 to 27 Percent after 3 to 5 minutes, over 90 percent after 11 minutes, and 100 percent after 12 minutes, had reached maximum temperature. 7. There were no significant differences in the optimum temperature(the maximum temperature minus 0.1℃) among fahrenheit thermometer, Yu Ⅱcentigrade thermometer, and Kuk Ⅱ centigrade thermometer. The mean optimum temperature for Fahrenheit thermometers was 36.60℃, for Yu Ⅱ centigrade thermometers was 36.69℃, and Kuk Ⅱ centigrade thermometers was 36.69℃. 8. There were no significant differences in placement time to reach optimum temperature among Fahrenheit thermometer, Yu Ⅱ centigrade thermometer, and Kuk Ⅱ centigrade thermometer The mean placement time for Fahrenheit thermometers was 5.70 minutes, for Yu Ⅱ centigrade thermometers was 5.54 minutes, and for Kuk Ⅱ centigrade thermometers was 5.28 minutes. In the case of Fahrenheit thermometers, 21 to 49 percent after 3 to 5 minutes, over 90 percent after 9 minutes, and 100 percent after 12 minutes, had reached optimum temperature. When the Yu Ⅱ centigrade thermometer was used, 23 to 51 percent after 3 to 5 minutes over 90 percent after 10 minutes, and 100 percent after 12 minutes, had reached optimum temperature. When the Kuk Ⅱ centigrade Thermometer was used, 23 to 57 Percent after 3 to 5 minutes, over 90 percent after 9 minutes, and 100 Precent after 11 minutes, had reached optimum temperature.

A Study of Upper Airway Resistance Syndrome : Clinical and Polysomnographic Characteristics (상기도저항 증후군에 대한 연구 : 임상 및 수면다원검사 특징)

  • Objectives : Upper airway resistance syndrome(UARS) is a sleep-related breathing disorder characterized by abnormal negative intrathoracic pressure during sleep. Abnormally increased negative intrathoracic pressure results in microarousal and sleep fragmentation which underlay UARS-associated complaints of daytime fatigue and sleepiness. Although daytime dysfunction in patients with UARS is comparable to that of sleep apnea syndrome, UARS has been relatively unnoticed in clinical setting. That is why UARS is apt to be excluded in diagnosing of sleep-related breathing disorders since its respiratory disturbance index and arterial oxygen saturation are within normal limits. The current study presents a summary of clinical and polysomnographic characteristics found in patients with UARS. The present study aims (1) to explore characteristics of patients diagnosed with UARS, (2) to characterize the polysomnographic findings of UARS patients, and (3) to enhance the understanding of UARS through those clinical and laboratory characteristics. Methods : This was a retrospective study of 20 UARS patients (male 15, female 5) and 30 obstructive sleep apnea (OSA) patients (male 21, female 9) at the Stanford Sleep Disorders Clinic. We diagnosed patients as having UARS when they met critenia, RDI < 5 characteristic findings of an elevated esophageal pressure($<-10\;cmH_2O$), frequent arousals secondary to an elevated esophageal pressure, and symptoms of daytime fatigue and sleepiness. We used polysomnographic value, which is standardized by Williams et al(1974), as normal control. Statiotical test were done with student t-tests. Results : (1) Mean age of UARS was $41.0\;{\pm}\;14.8$ years and OSA was $50.9\;{\pm}\;12.0$ years. UARS subject was significantly younger than OSA subject (p<0.05). (2) The total score of Epworth Sleepiness Scale (ESS) was UARS $9.7\;{\pm}\;6.3$ and OSAS $11.2\;{\pm}\;6.3$. There was no significant difference between two groups. (3) The mean body mass index was UARS $28.1\;{\pm}\;5.7\;kg/m^2$ and OSAS $32.9\;{\pm}\;7.0\;kg/m^2$. UARS had significantly lower meen body man index than OSAS subjects (p<0.05). (4) The polysomnographic parameters of UARS were not significantly different from those of OSA except RDI(p<0.001), $SaO_2$ (p<0.001) and slow wave sleep latency (p<0.05). (5) Compared with normal control, Total sleep time in UARS subjects was significantly shorter (p<0.001), sleep efficiency index was significantly lower (p<0.001), total awakening percentage was significantly higher (p<0.001), and sleep stage 1 (p<0.001) were significantly higher. (6) OSA patients showed poor sleep quality and distinct abnormal sleep architectures compared with normal control. Conclusions : Conclusions from the above results are as follows : (1) UARS patients were younger and had lower body mass index when umpared with OSA patients. (2) The quality of sleep and sleep architectures of the UARS and OSA patients are significantly different from those of normal control. (3) ESS scores and awakening frequencies of UARS are similar with those of OSA, suggesting that daytime dysfunction of UARS patients may be comparable to those of OSA patients. (4) The RDI and the $SaO_2$ which are important indicators in diagnosing sleep-related breathing disorders, of UARS subjects are close to normal value. (5) According to the the above results, we unclude that despite the absence of $SaO_2$ drops and the absence of an elevated number of apnea and hypopnea, subjects developed clinical complaints which were associated with laborious breathing, elevated Pes nadir, and frequently snoring. (6) Accordingly, we suggest including LIARS in the differential diagnosis list when sleep related breathing disorder is suspected clinically and overnight polysomnographic findings except snoring and frequent microarousal are within normal limits.

Relationship Between Usage Needs Satisfaction and Commitment to Apparel Brand Communities: Moderator Effect of Apparel Brand Image (의류 브랜드 커뮤니티의 이용욕구 충족과 커뮤니티 몰입의 관계: 의류 브랜드 이미지의 조절효과)

  • INTRODUCTION Due to the high broadband internet penetration rate and its group-oriented culture, various types of online communities operate in Korea. This study use 'Uses and Gratification Approach, and argue that members' usage-needs satisfaction with brand community is an important factor for promoting community commitment. Based on previous studies identifying the effect of brand image on consumers' responses to various marketing stimuli, this study hypothesizes that brand image can be a moderate variable affecting the relationship between usage-needs satisfaction with brand community and members' commitment to brand community. This study analyzes the influence of usage-needs satisfaction on brand community commitment and how apparel brand image affects the relationships between usage-needs satisfactions and community commitments. The hypotheses of this study are proposed as follows. H1-3: The usage-needs satisfaction of apparel brand community (interest, transaction, relationship needs) influences emotional (H1), continuous (H2), and normative (H3) commitments to apparel brand communities. H4-6: Apparel brand image has a moderating effect on the relationship between usage-needs satisfaction and emotional (H4), continuous (H5), and normative (H6) commitments to apparel brand communities. METHODS Brand communities founded by non-company affiliates were excluded and emphasis was placed instead on communities created by apparel brand companies. Among casual apparel brands registered in 6 Korean portal sites in August 2003, a total of 9 casual apparel brand online communities were chosen, depending on the level of community activity and apparel brand image. Data from 317 community members were analyzed by exploratory factor analysis, moderated regression analysis, ANOVA, and scheffe test. Among 317 respondents answered an online html-type questionnaire, 80.5% were between 16 to 25 years old. There were a total of 150 respondents from apparel brand communities(n=3) recording higher-than-average brand image scores (Mean > 3.75) and a total of 162 respondents from apparel brand communities(n=6) recording lower-than-average brand image scores(Mean < 3.75). In this study, brand community commitment was measured by a 5-point Likert scale: emotional, continuous and normative commitment. The degree of usage-needs satisfaction (interest, transaction, relationship needs) was measured on a 5-point Likert scale. The level of brand image was measured by a 5-point Likert scale: strength, favorability, and uniqueness of brand associations. RESULTS In the results of exploratory factor analysis, the three usage-needs satisfactions with brand community were classified as interest, transaction, and relationship needs. Brand community commitment was also divided into the multi-dimensional factors: emotional, continuous, and normative commitments. The regression analysis (using a stepwise method) was used to test the influence of 3 independent variables (interest-needs satisfaction, transaction-needs, and relationship-needs satisfactions) on the 3 dependent variables (emotional, continuous and normative commitments). The three types of usage-needs satisfactions are positively associated with the three types of commitments to apparel brand communities. Therefore, hypothesis 1, 2, and 3 were significantly supported. Moderating effects of apparel brand image on the relationship between usage-needs satisfaction and brand community commitments were tested by moderated regression analysis. The statistics result showed that the influence of transaction-needs on emotional commitment was significantly moderated by apparel brand image. In addition, apparel brand image had moderating effects on the relationship between relationship-needs satisfaction and emotional, continuous and normative commitments to apparel brand communities. However, there were not significant moderate effects of apparel brand image on the relationships between interest-needs satisfaction and 3 types of commitments (emotional, continuous and normative commitments) to apparel brand communities. In addition, the influences of transaction-needs satisfaction on 2 types of commitments (continuous and normative commitments) were not significantly moderated by apparel brand image. Therefore, hypothesis 4, 5 and 6 were partially supported. To explain the moderating effects of apparel brand image, four cross-tabulated groups were made by averages of usage-needs satisfaction (interest-needs satisfaction avg. M=3.09, transaction-needs satisfaction avg. M=3.46, relationship-needs satisfaction M=1.62) and the average apparel brand image (M=3.75). The average scores of commitments in each classified group are presented in Tables and Figures. There were significant differences among four groups. As can be seen from the results of scheffe test on the tables, emotional commitment in community group with high brand image was higher than one in community group with low brand image when transaction-needs satisfaction was high. However, when transaction-needs satisfaction was low, there was not any difference between the community group with high brand image and community group with low brand image regarding emotional commitment to apparel brand communities. It means that emotional commitment didn't increase significantly without high satisfaction of transaction-needs, despite the high apparel brand image. In addition, when apparel brand image was low, increase in transaction-needs did not lead to the increase in emotional commitment. Therefore, the significant relationship between transaction-needs satisfaction and emotional commitment was found in only brand communities with high apparel brand image, and the moderating effect of apparel brand image on this relationship between two variables was found in the communities with high satisfaction of transaction-needs only. Statistics results showed that the level of emotional commitment is related to the satisfaction level of transaction-needs, while overall response is related to the level of apparel brand image. We also found that the role of apparel brand image as a moderating factor was limited by the level of transaction-needs satisfaction. In addition, relationship-needs satisfaction brought significant increase in emotional commitment in both community groups (high and low levels of brand image), and the effect of apparel brand image on emotional commitment was significant in both community groups (high and low levels of relationship-needs satisfaction). Especially, the effect of brand image was greater when the level of relationship-needs satisfaction was high. in contrast, increase in emotional commitment responding to increase in relationship-needs satisfaction was greater when apparel brand image is high. The significant influences of relationship-needs satisfaction on community commitments (continuous and normative commitments) were found regardless of apparel brand image(in both community groups with low and high brand image). However, the effects of apparel brand image on continuous and normative commitments were found in only community group with high satisfaction level of relationship-needs. In the case of communities with low satisfaction levels of relationship needs, apparel brand image marginally increases continuous and normative commitments. Therefore, we could not find the moderating effect of apparel brand image on the relationship between relationship-needs satisfaction and continuous and normative commitments in community groups with low satisfaction levels of relationship needs, CONCLUSIONS AND IMPLICATIONS From the results of this study, we draw several conclusions; First, the increases in usage-needs satisfactions through apparel brand communities result in the increases in commitments to apparel brand communities, wheres the degrees of such relationship depends on the level of apparel brand image. That is, apparel brand image is a moderating factor strengthening the relationship between usage-needs satisfaction and commitment to apparel brand communities. In addition, the effect of apparel brand image differs, depending on the level and types of community usage-needs satisfactions. Therefore, marketers of apparel brand companies must determine the appropriate usage-needs, depending on the type of commitment they wish to increase and the level of their apparel brand image, to promote member's commitments to apparel brand communities. Especially, relationship-needs satisfaction was very important factor for increasing emotional, continuous and normative commitments to communities. However the level of relationship-needs satisfaction was lower than interest-needs and transaction-needs. satisfaction. According to previous study on apparel brand communities, relationship-need satisfaction was strongly related to member's intention of participation in their communities. Therefore, marketers need to develope various strategies in order to increase the relationship- needs as well as interest and transaction needs. In addition, despite continuous commitment was higher than emotional and normative commitments, all types of commitments to apparel brand communities had scores lower than 3.0 that was mid point in 5-point scale. A Korean study reported that the level of members' commitment to apparel brand community influenced customers' identification with a brand and brand purchasing behavior. Therefore, marketers should try to increase members' usage-needs satisfaction and apparel brand image as the necessary conditions for bringing about community commitments. Second, marketers should understand that they should keep in mind that increasing the level of community usage needs (transaction and relationship) is most effective in raising commitment when the level of apparel brand image is high, and that increasing usage needs (transaction needs) satisfaction in communities with low brand image might not be as effective as anticipated. Therefore, apparel companies with desirable brand image such as luxury designer goods firms need to create formal online brand communities (as opposed to informal communities with rudimentary online contents) to satisfy transaction and relationship needs systematically. It will create brand equity through consumers' increased emotional, continuous and normative commitments. Even though apparel brand is very famous, emotional commitment to apparel brand communities cannot be easily increased without transaction-needs satisfaction. Therefore famous fashion brand companies should focus on developing various marketing strategies to increase transaction-needs satisfaction.
