  • Six cases of congenital heart disease were operated on by means of cardiopulmonary bypass between December, 1975 and April, 1976. Two cases of ventricular septal defects (VSD), two cases of VSD, associated with ruptured aneurysm of sinus Valsalva, two cases of atrial septal defects (ASD) and one case of pulmonic stenosis with patent ductus arteriosus were operated. Sarns roller pumps and Bentley Temptrol oxygenators were used for extracorporeal circulation. Pump oxygenator was primed with Ringer's lactate solution, 5% dextrose in water, mannitol, and ACD blood. Flow rate ranged from 2.0 to $2.4L/M^2/min$. Bicarbonate was added to the oxygenator with estimated amount as 15 mEq/L/hr. Venous catheters were introduced into superior and inferior vena cava, and oxygenated blood was returned to the body through aortic cannula inserted into ascending aorta. Moderate hypothermia ($30^{\circ}C$) was induced by core cooling. Aorta was cross clamped for 15 minutes and released for 3 minutes, and repeated clamping when necessary. Atrial and ventricular septal efects were closed by direct sutures. Aneurysms of sinus Valsalva ruptured into the right ventricle were repaired through right ventriculotomy by d:rect closure with Dacron patch reinforcement. Cardiopulmonary bypass time varied from 66 to 209 minutes, and aorta cross clamping time ranged from 13 to 56 minutes. Postoperative bleeding was minimal except one case who needed for evacuation of substernal hematoma. Intra- and postoperative urinary output was satisfactory. Acid-base balance, partial pressure of $O_2$, electrolytes, and hematological changes during intra- and post-perfusion period remained at the acceptable ranges. No mortality was experienced.

  • In order to identify the effect of soil salinity on saturated hydraulic conductivity in reclaimed paddy soils, we established the soil columns uniformly packed with soils collected at every 20 cm up to 60 cm from the reclaimed paddy area with high and low salinity which has been cultivated rice plants for the last 30 years. The soil textures were sandy loam and loamy sand for high-salinity and low-salinity topsoils, respectively. For high-salinity and low-salinity soils the ECes were ranged from 25.2 to $37.8dS\;m^{-1}$ and 3.0 to $3.4dS\;m^{-1}$ while the ESPs were ranged from 7.70 to 20.84 % and from 5.12 to 11.33 %, respectively. The bulk densities of the soil columns were adjusted to $1.15{\pm}0.03g\;cm^{-3}$. The results of the soil column experiments shows that the stabilized saturated hydraulic conductivity of low-salinity soil was $0.62cm\;hr^{-1}$ at the topsoil while there were little water flow at the bottom of the soil columns packed with high-salinity soils. After removal of $Na^+$ ions with $1N\;NH_4OAc$ from the high-salinity soil, Ksat of the saline soil was drastically increased to $0.23cm\;hr^{-1}$. Soil columns of high-salinity topsoil treated with four different concentration of NaCl influent after removal of soluble and exchangeable cations with $1N\;NH_4OAc$ show Ksat in the range of $0.1{\sim}0.15cm\;hr^{-1}$ and the Ksat slightly decreased as the concentration of NaCl influent was increasing. Conclusively, we could assume that $Na^+$ can be significantly contributed to the saturated hydraulic conductivity in newly reclaimed sandy soil.

  • Objectives The aim of this study done as pilot study is to analyze the current state of patients who have low back pain through registry. Methods This study is done under approval of Gil Oriental Medical Hospital of Gachon University, Oriental Medical Hospital of Sangji University, Jecheon Oriental Medical Hospital of Se-Myung University IRB. Among subjects who signed the consent form by their own will, we decided whom to register as subjects of this study by the standard of selection and exception. We collected the sociological investigation, character of low pack pain, degree of symptom which they felt before and after the hospitalization from registered subjects. Results 1. The number of the subjects is 16. The average age is 41.0. 9 of them are female, 7 of them are male. Most of the subjects have history illness which has connection with low pack pain. 2. According to the patient's free description of the back pain, 6 of them suffered throbbing pain. And 8 of the patients have chronic pain, 6 of them have intermittent pain of back pain analysing the character of the low back pain. They answered the pain lasted for 47.6 minutes on average. 3. About the change on the average R.O.M. of L-spine, R.O.M. of Lateral bending, Extension, Flexion, Rotation has increased after leaving the hospital compared with before hospitalization. 4. The amount of discomfort or strength of pain, which was checked by VAS on the day of leaving the hospital, has decreased than they were before the hospitalization. And there was the improvement on the dysfunction score and EQ-5D. Conclusions Through this study, we specifically analyzed the symptoms of the low back pain by accumulating the analysis about the symptoms using several indicators and description which is freely spoken by patients about their symptoms. Further research is expected to complete multi-center registry by building registry and by using it, to get various epidemiologic informations about low back pain.

Treating Idiopathic Parkinson's Disease with Bonoesikpungjigyeong-tang: A Case Study on "Delayed On" and "Wearing Off" Phenomena after Long-term Levodopa Intake (레보도파 약효 지연 현상 및 약효 감소 현상으로 인한 자세불안정 및 전신탈력감을 호소하는 특발성 파킨슨병 환자에 대한 보뇌식풍지경탕 치험 1례)

  • A 69-year-old female diagnosed with idiopathic Parkinson's disease and treated with anti-Parkinsonian medications, including levodopa, for 7 years presented with complaints of wearing-off phenomenon and delayed onset of medication efficacy, resulting in postural instability and feeling of severe exhaustion. The patient underwent a 50-day course of treatment with Bonoesikpungjigyeong-tang formula. The effects of the treatment were evaluated by recording the average time to onset of medication efficacy, the average time to medication wearing-off, and discomfort due to wearing-off phenomenon. Postural instability was assessed using the Numeric Rating Scale (NRS). The patient's overall quality of life was assessed using the EQ-5D-3L. After 50 days of treatment, the average time to onset of medication efficacy decreased, and the average time to medication wearing-off increased. Discomfort due to wearing-off phenomenon and postural instability decreased. This case suggests that Bonoesikpungjigyeong-tang might be an alternative for improving discomfort caused by long-term levodopa intake.
