Korean Medical Treatment including Phototherapy after Hip Fracture Surgery: Three Case Reports (고관절 골절 수술 이후 광선치료를 포함한 한방치료 3예 보고)

  • This study is designed to report three cases of postoperative care after hip surgery using conventional Korean Medicine treatments including phototherapy. Three patients undergone hip fracture surgery were treated with acupuncture, herbal medicine, and phototherapy. We used Visual analogue scale (VAS), EuroQol five dimension scale (EQ-5D), and hip disability and osteoarthritis outcome score (HOOS) to evaluate treatment effectiveness. After the treatment, VAS score was improved in all three cases, while EQ-5D and HOOS showed different results by items. EQ-5D (pain/discomfort) and HOOS (pain, symptoms, activities of daily living) were improved in all three cases. EQ-5D (self-care) and HOOS (quality of life) showed improvements in two cases. EQ-5D (mobility, usual activities, anxiety/depression) and HOOS (sports/recreation) were improved in one case. Conventional Korean Medicine treatments including phototherapy can be effectively used to alleviate pain and symptoms after hip fracture surgery.

Health-related Quality of Life of Patients With Diabetes Mellitus Measured With the Bahasa Indonesia Version of EQ-5D in Primary Care Settings in Indonesia

  • Objectives: Diabetes mellitus (DM) is a serious public health issue that places a heavy financial, social, and health-related burden on individuals, families, and healthcare systems. Self-reported health-related quality of life (HRQoL) is extensively used for monitoring the general population's health conditions and measuring the effectiveness of interventions. Therefore, this study investigated HRQoL and associated factors among patients with type 2 DM at a primary healthcare center in Indonesia. Methods: A cross-sectional study was conducted in Klaten District, Central Java, Indonesia, from May 2019 to July 2019. In total, 260 patients with DM registered with National Health Insurance were interviewed. HRQoL was measured with the EuroQol Group's validated Bahasa Indonesia version of the EuroQoL 5-Dimension 5-Level (EQ-5D-5L) with the Indonesian value set. Multivariate regression models were used to identify factors influencing HRQoL. Results: Data from 24 patients were excluded due to incomplete information. Most participants were men (60.6%), were aged above 50 years (91.5%), had less than a senior high school education (75.0%), and were unemployed (85.6%). The most frequent health problems were reported for the pain/discomfort dimension (64.0%) followed by anxiety (28.4%), mobility (17.8%), usual activities (10.6%), and self-care (6.8%). The average EuroQoL 5-Dimension (EQ-5D) index score was 0.86 (95% confidence interval [CI], 0.83 to 0.88). In the multivariate ordinal regression model, a higher education level (coefficient, 0.08; 95% CI, 0.02 to 0.14) was a significant predictor of the EQ-5D-5L utility score. Conclusions: Patients with diabetes had poorer EQ-5D-5L utility values than the general population. DM patients experienced pain/discomfort and anxiety. There was a substantial positive relationship between education level and HRQoL.

  • Design chart solution and table solution were proposed by Choi et al. (2019a), which conducted a parametric numerical study for the bored PHC piles socketed into weathered rocks through sandy soil layers. Based on the Choi et al. (2019a), the new prediction formulae for mobilized capacity components such as total capacity, total skin friction and skin friction of sand at the settlement of 5% pile diameter were proposed in this study. The proposed prediction formulae (EQ-G1) considers pile diameter, relative embedment length and ${\bar{N}}$ (i.e, corrected N) value and their verification results are as follows. The SRF calculated from the new proposed design method was 71~94%, which are greatly improved compared with results by the existing design method. The design efficiency of bearing capacity was in the range of reasonable design except 4 cases, and the design efficiency of the PHC pile was evaluated as 85%. Therefore, it is possible that allowable compressive load (Pall) of PHC pile can be utilized in the resonable design by means of the new proposed method using EQ-G1 equations. And the other new proposed equations of EQ-G2-3 can be utilized approximately in calculation of axial compressive bearing capacity components for prebored PHC pile.

Association between Oral Health Status and Health Related Quality of Life (EuroQoL-5 Dimension) (성인의 구강건강 상태와 건강관련 삶의 질 관련성: EuroQoL-5 Dimension 구성요소를 중심으로)

  • The purpose of this study was to examine the association between oral health status and health related quality of life (HRQoL). The study used a nationally representative sample of Koreans (2013 Korea National Health and Nutrition Examination Survey) aged 19~64 years (n=3,252). Dependent variable was HRQoL, which was assessed with each component of EuroQol-5 dimension (EQ5D). Independent variable was oral health status (oral pain, chewing problem, speaking problem, and perceived oral health). After adjustment for confounders (socio-demographic factors, oral health behaviors, health behaviors, and physical conditions), the risk of having poor HRQoL was greater in adults with poor oral health status. The odds ratio (OR) of having pain/discomfort were 1.50 (95% confidence interval [CI], 1.22~1.86) for respondents with oral pain, 1.72 (95% CI, 1.33~ 2.22) for respondents with chewing problem, 1.79 (95% CI, 1.22~2.62) for respondents with speaking problem, and 1.36 (95% CI, 1.09~1.70) for respondents with poor perceived oral health. The OR of having anxiety/depression were 1.74 (95% CI, 1.28~2.38) when having a chewing problem, 1.72 (95% CI, 1.12~2.64) when having a speaking problem, and 1.54 (95% CI, 1.14~2.08) when rating his/her oral health poor. Among Korean adults, two of the EQ5D components were associated with oral health status. Future study is needed to examine the detailed causal relations between oral health status and HRQoL longitudinally.

Effect of Korean Medicine Combined with Electric Moxibustion in Patients with Traffic Accident-Induced Lumbago

  • Background: The purpose of this study was to investigate electric moxibustion on patients with back pain caused by road traffic accidents. Methods: This was a retrospective study (n = 112) comparing treatment with Korean medicine combined with electric moxibustion (n = 56), and Korean medicine alone (n = 56). Patient gender, age, Numeric Rating Scale (NRS), Oswestry Disability Index (ODI) and EuroQol-5 Dimension (EQ-5D) was recorded for each group at the time of hospital admission. Mean NRS measured weekly, mean ODI and EQ-5D scores were measured 2 weeks post-treatment, and evaluated by paired sample t test. using the Statistical Program for Social Science v. 25.0 for Windows. An independent, two-sample t test was used to test for a significant difference in the decrement of NRS, ODI and increment EQ-5D scores between groups. Results: NRS scores decreased in both groups after 1 week of treatment (electric moxibustion, from $5.13{\pm}0.79$ to $3.86{\pm}0.67$; Korean medicine alone, from $5.18{\pm}0.92$ to $4.30{\pm}0.94$; both p < 0.001). There was a significantly greater reduction in NRS score in the electric moxibustion group ($1.27{\pm}0.59$) than in the Korean medicine alone group ($0.88{\pm}0.61$; p = 0.001). After 2 weeks of treatment, EQ-5D scores increased significantly in the moxibustion group ($0.19{\pm}0.12$) compared with the Korean medicine alone group ($0.13{\pm}0.20$; p = 0.043). After 2 weeks of treatment, NRS and ODI scores decreased in both groups. EQ-5D increased in both groups. Conclusion: We suggest that electric moxibustion treatment may be effective for reducing early-stage back pain in patients with road traffic accident injuries.

Effects of Sweet Bee Venom Pharmacopuncture Combined with Korean Medicine Treatment for Acute Low Back Pain Syndrome Patient: A Case Report (급성 요통 증후군 환자에 대해 봉독약침요법을 병행한 한의학적 치료 효과: 증례보고)

  • Objectives : Acute low back pain syndrome causes pain and poor quality of life. There are various studies of SBV (Sweet bee venom) pharmacopuncture, but few have identified the therapeutic effects for patients unable to walk due to acute back pain. This case series report three cases of acute low back pain syndrome treated with SBV pharmacopuncture combined with Korean Medicine (KM) treatments. Methods : Three acute low back pain syndrome patients with no other acute abnormalities in the imaging were treated by KM treatments including SBV pharmacopuncture, acupuncture, cupping, herbal medicine and physical therapy. The improvement of symptoms was evaluated using Numerical rating scale (NRS), Oswestry disability Index (ODI), EuroQol-5 dimension index (EQ-5D) and EuroQol-visual analogue scale (EQ-VAS). Results : Patients who were unable to walk due to acute back pain were able to walk. There were significant improvements of NRS, ODI, EQ-5D and EQ-VAS after treatment. The patients experienced an average 90% decrease in low back pain after inpatient KM treatment for about 8 days. NRS decreased by an average of 78.9%, ODI decreased by an average of 49.4%, and EQ-VAS increased by an average of 92.6%. In the case 1, EQ-5D decreased in all categories, but in the case 2, it decreased in all categories except for pain/discomfort and anxiety/depression, but in the case 3, it decreased in all categories except anxiety/depression. There was no serious side effect after treatment of SBV pharmacopuncture. Conclusions : KM treatments including SBV pharmacopuncture may be effective for acute low back pain syndrome patients. However, randomized controlled trials are needed in the future to confirm the clinical effects of these interventions.

Health-related quality of life in elderly patients with diabetes mellitus according to age: based on Korea National Health and Nutrition Examination Survey (노인 당뇨병 인지자의 연령에 따른 삶의 질 수준 분석: 국민건강영양조사를 이용하여)

  • Purpose: Although the importance of life satisfaction in patients with chronic diseases has been emphasized, limited studies have been performed on the health-related quality of life (HRQoL) among elderly patients with chronic diseases. This study examined the HRQoL among Korean patients with diabetes aged 65 years or older. Methods: This study used data from the Korea National Health and Nutrition Examination Survey 2008-2016. The analysis included data of 2,353 elderly patients with diabetes aged 65 years or older who were aware that they had type 2 diabetes. Each participant was categorized into one of three age groups (65-69 years, 70-74 years, and ≥ 75 years). The EuroQol-5 Dimensions (EQ-5D) scale was computed to estimate the HRQoL. Multivariate linear regression analysis was used to compare the demographic characteristics, lifestyle characteristics, nutrient intake, and EQ-5D scores between the age groups. Results: The intake levels of most nutrients were similar in the three age groups, but the total energy intake levels were significantly lower, and the sodium intake levels were significantly higher in the older age groups (p < 0.001). In addition, all items of the EQ-5D index, except for anxiety/depression, tended to decrease with age. In particular, the oldest age group (≥ 75 years) showed significantly lower levels on most items and the EQ-5D index than those in the other age groups (p < 0.001). Conclusion: Elderly patients with type 2 diabetes may have low levels of life satisfaction, which worsens with increasing age. Therefore, it is important to develop and manage personalized guidelines and medical systems so that elderly patients have a higher quality of life for the remainder of their lives. The results of this study are expected to be used as basic data for the systematic management of diabetes in elderly Korean patients.

Plant Proteins in Relation to Health-related Quality of Life in South Korean Individuals Aged 50 Years or Older: Korea National Health and Nutrition Examination Survey 2016-2018 (50세 이상 한국인의 식물성 단백질 섭취와 건강 관련 삶의 질과의 연관성: 국민건강영양조사 2016-2018)

  • This study aimed to investigate the association between plant protein intake and health-related quality of life (HRQoL) in Korean individuals aged 50 years or older. Using the 2016-2018 Korea National Health and Nutrition Examination Survey data, 7,956 participants (3,434 men and 4,522 women) were included in the study. HRQoL was measured using the Euro-quality of life five-dimension (EQ5D), composed of physical function, self-care, daily activities, pain/discomfort, and anxiety/depression problems. The EQ5D estimates were converted into an EQ5D index score. The association between daily intake of plant proteins and HRQoL was evaluated using regression analysis. The intake of total plant protein, legume nut seed (LNS), and fermented bean (FERMB) proteins were all positively associated with HRQoL in both men and women (p < 0.01). Of EQ5D's five dimensions, physical function and daily activities were most commonly associated with plant protein intake. Compared to LNS protein or FERMB protein intake, total plant protein intake showed a better association with HRQoL. In conclusion, a significant association was observed between plant protein intake and HRQoL. It suggests that adequate intake of plant protein might be helpful for the maintenance or improvement of HRQoL in Korean adults.

Associations between Quality of Life and Marital Status in Cancer Patients and Survivors

  • Background: The cancer survival rate in Korea has substantially increased, necessitating the management of not only patients with cancer but also longer term survivors. Although the divorce rate has drastically increased in Korea, there is not sufficient research regarding the relationship between changes in marital status and quality of life (QOL) in cancer patients and survivors. Thus, we aimed to examine the relationship between marital status and QOL in such cases. Materials and Methods: This study was performed using the Community Health Survey of 2008 administered by the Korea Centers for Disease Control and Prevention (N=169,328). We used t-tests and Chi-square tests to compare demographic variables between men and women, and analysis of variance (ANOVA) to compare QOL scores among comparison groups. We also performed a multilevel analysis on the relationship between QOL and marital status while accounting for provincial differences. Results: Decline of EuroQOL five dimensions (EQ-5D) in single patients with cancer was greater than in any other marital status group, but there was no statistically significant decline in survivors of cancer with regard to marital status. In the general population, the decline of EQ-5D was higher among single people than married people. Using the EuroQOL visual analog scale (EQ-VAS), single people had higher values than those of other marital status among both patients with cancer and survivors of cancer. In the general population, EQ-VAS values were higher for single people compared to married people. Conclusions: There may be a significant relationship between marital status and QOL in cancer patients and survivors. Policy interventions to manage patients with cancer who experience a decline in QOL as well as marital problems should be conducted.

A study on the correlation between health related quality of life(QOL) and the severity of tinnitus patients (이명증 환자 중증도와 삶의 질의 상관 연구)

  • Objective : This study was done to investigate the correlation between general quality of life(QOL) and disease specific QOL of tinnitus patients. Methods : 2009 National health and nutrition survey(NHNS) data were employed for analyzing the mean estimates of total population and tinnitus sub-group patients, and predicting the regression equation of mapping between EQ-5D and disease severity. Baseline statistical analyses and normality tests were done by using Student t-test, Chi-square test, Shapiro-Wilk test and ladder test. Hierarchial regression analyses were performed using related variables and predicted the optimal regression equations. Statistical significance was achieved if the probability was less than 5%. Results : Firstly, the standardized prevalence of tinnitus patient in South Korea was 18.69% and over 90th age-group was relatively higher than other groups. Mean value of domestic QOL measured by EQ-5D was estimated as 0.9486 and QOL of tinnitus subgroup(0.9169) was lower than the non-tinnitus subgroup(0.9559), significantly. Stratified by age and sex, QOLs of all sub-groups with tinnitus were lower than without tinnitus sub-groups significantly.
