A Hybrid EMR System to bridge Source-Oriented and Problem-Oriented Medical Record (SOMR과 POMR의 혼용을 위한 하이브리드 EMR 시스템)

  • 다양한 의료서비스를 정형화되고 효율적인 방법으로 제공하기 위하여 최근 의료기관의 EMR(Electronic Medical Record; 전자의무기록) 시스템의 도입이 급속도로 확산되고 있다. 그러나 국내에서 구축되고 있는 EMR은 초기에 기대했던 EMR의 도입목적을 중분하게 달성하지 못하고 있다. 이는 근본적으로 EMR의 방향성과 구조에 한계를 지니고 있기 때문이다. 국내에서 구축되고 있는 대부분의 전자의무기록시스템은 자료수집원 지향 의무기록 (Source Oriented Medical Record; SOMR) 방식으로 개별 사실을 수집하고 제시하기 위한 관찰자 중심, 서식중심의 시스템이며 차트 이송과 종이를 없애는 것이 주목적이다. SOMR 방식은 EMR의 다양한 도입목적, 특히 임상자료의 효과적 활용을 충족시킬 수 없는 태생적인 한계를 지니고 있기 때문에 문제지향 의무기록 (Problem Oriented Medical Record; POMR) 방식의 EMR과 적절한 혼용전략 (Hybrid Strategy)이 절실히 요구된다. 본 연구의 목적은 SOMR 시스템과 POMR 시스템이 보완적으로 결합된 하이브리드 EMR 아키텍처를 제시하고 아키텍처를 구성하는 데 필요한 기술적 요소들과 그들간의 상호관계 틀 파악한 후 이를 기반으로 개발방법론을 도출하는 것이다. 또한 만성신부전증 환자 사례를 이용하여 앞서 제시된 개발방법론에 따라 프로토타입을 개발하였다. 본 연구는 EMR 시스템이 SOMR 구조에만 편향됨으로써 발생될 수 있는 다양한 문제들을 해결함으로써 진료의 품질 항상, 임상연구의 활성화, 조직적이고 과학적인 진료계획의 수립과 같은 소외되어 왔던 EMR의 목표를 실현할 수 있을 것으로 기대 된다.

  • Since 1990 when order communication system(OCS) was first introduced, the use of information technology in medical service has been widely accepted in order to enhance quality and customer relationship as well as to increase managerial efficiency. Medical information system is rapidly increasing and is trying to make ubiquitous healthcare environment through telemedicine system. Especially, medical profession and government have taken interest in electronic medical record (EMR) system which can digitalize and manage all medical records in hospitals. By recording patient's medical information in real time, EMR system can improve service efficiency and customer service quality including short waiting time, various utilization of clinic information, and reduced cost.

  • In this study, a model in which certification standards were added to the health information management practice program was studied and presented in order to understand the EMR certification standards implemented by the Korea Health and Medical Information Service. In the practice program, the certification standard function for patient information management was added to the health information management education system to practice and understand patient information management that corresponds to the functional standard of the EMR certification system. The EMR certification standard practice program for patient information management is composed of the following certification standards. registration number and personal information management, treatment reservation schedule management, personal information revision history management, identification of people with the same name, integrated management of multiple registration numbers, patient search by identification information, patient search by health care type, surgical procedure consent record and inquiry, record/inquiry of consent form for personal information use, display of life-sustaining medical decision information, registration/inquiry of external medical institution documents, registration and inquiry of external examination results. In this way, by operating and practicing the functions of the health information system according to the certification standards, it is possible to understand and practice the certification standards and details of patient information management in the functional area of the certification standards. In addition, since the function of the EMR certification standard can be checked, it will be possible to improve the management ability of the electronic medical record system of the health information manager in the medical institution.

  • Medical Gateway(MG), that integrates medical devices with its unique data format in various hospital information systems, provides effectiveness in the cost of the management unlike existing systems that uses PC as a gateway to hospital information systems because MG enables to interface medical devices with itself. However, in spite of using MG for integrating medical devices, we need additional programs or systems to interwork with EMR system of each hospital in accordance with the configuration of DB. In this paper, we design and implement the format of medical information and server system for interworking MG with EMR system. The proposed system makes easy exchanging medical data between MG and EMR system. Thus, the proposed system can contribute to a national project to export abroad advanced hospital setup converged with IT.

  • The Electronic Medical Record(: EMR) is to store medical data not in the form of document, but in the data storage. Such EMR can not only solve various problems of document use such as storage/arrangement of and securing space for document, but also make it possible to provide customized-treatment based on large quantity of customer data, so that hospitals can reduce the management cost and also improve the work efficiency. Customers also can receive the great quality of medical service. Owing to such strengths, the EMR has been rapidly introduced and applied to many hospitals and clinics since 1990s. In case of the current health screening system, however, paper forms used for health screening is also stored, on top of EMR. There would be various reasons why it is stored in the form of document. While the EMR used in hospitals is comprised of a unit program performing medical record, the health screening system is comprised of a unit program performing logics related to health screening. For this reason, it might be unavoidable for the health screening system to store document forms. If the EMR function is applied to the health screening system, it is expected to be able to operate more efficient health screening solution.

  • Beginning in 2000, domestic large hospital based integrated health information system has been developed from order communication system to electronic medical record system. However, today's advanced medical information system is integrated with unit of the system because user needs is complex and various. And, the problem is authority management of health information system in complex systems of large size hospital. It is also a serious problem of private information exposure because of user's authority management defect. In this paper, we analyze the problems of past hospital information system and propose an efficient and appropriate management authority in operating environment. It also introduces the instances applied into a large hospital EMR system, developing proper authority management to match the characteristics of the integrated medical information system. The proposed system is based on solutions of authority management system suitable for integrated health information system, as well as the next generation of EMR.

  • The purpose of the present study is to theoretically assess IT Implementation Model of Cooper and Zmud (1990) in a hospital IS use context. A case study was applied to analogical study by interview from several end-users of the information systems at a university hospital. This study presented an EMR(Electronic Medical Record) systems how is initially implemented at an initial stage, continually adopted, adapted, accepted at an adoption stage, and finally rountinized and infused into an organization. Our study also elaborated IT Implementation Model as defining EMR development and its impact on nature of IS use in a hospital. This case study explained the characteristics of EMR and hospital organization context conceptually.

Association between Electronic Medical Record System Adoption and Healthcare Information Technology Infrastructure

  • Objectives: The objective of this study was to investigate the relationship between the level of Electronic Medical Record (EMR) system adoption and healthcare information technology (IT) infrastructure. Methods: Both survey and various healthcare administrative datasets in Korea were used. The survey was conducted during the period from June 13 to September 25, 2017. The chief information officers of hospitals were respondents. Among them, 257 general hospitals and 273 small hospitals were analyzed. A logistic regression analysis was conducted using the SAS program. Results: The odds of having full EMR systems in general hospitals statistically significantly increased as the number of IT department staff members increased (odds ratio [OR] = 1.058, confidence interval [CI], 1.003-1.115; p = 0.038). The odds of having full EMR systems was significantly higher for small hospitals that had an IT department than those of small hospitals with no IT department (OR = 1.325; CI, 1.150-1.525; p < 0.001). Full EMR system adoption had a positive relationship with IT infrastructure in both general hospitals and small hospitals, which was statistically significant in small hospitals. The odds of having full EMR systems for small hospitals increased as IT infrastructure increased after controlling the covariates (OR = 1.527; CI, 1.317-4.135; p = 0.004). Conclusions: This study verified that full EMR adoption was closely associated with IT infrastructure, such as organizational structure, human resources, and various IT subsystems. This finding suggests that political support related to these areas is indeed necessary for the fast dispersion of EMR systems into the healthcare industry.

A Preliminary Study on Clinical Decision Support System based on Classification Learning of Electronic Medical Records

  • We employed a hierarchical document classification method to classify a massive collection of electronic medical records(EMR) written in both Korean and English. Our experimental system has been learned from 5,000 records of EMR text data and predicted a newly given set of EMR text data over 68% correctly. We expect the accuracy rate can be improved greatly provided a dictionary of medical terms or a suitable medical thesaurus. The classification system might play a key role in some clinical decision support systems and various interpretation systems for clinical data.

  • These days it is becoming more and more common to find electronic medical screening systems installed in Oriental hospitals and clinics. This is a relatively new development for the practice of traditional Oriental medicine. Specifically, Pulse detection machines are being utilized in order to help determine a patient's disease scientifically. However, identifying and diagnosing the specific disease correctly for each patient is still very difficult in Oriental medicine. The intention of this paper is to propose a solution which uses two separate Electronic systems working together to produce a better likelihood of finding the correct diagnosis for each patient. It is proposed that an EMR intelligent electronic chart system be developed and employed, which would utilize both Pulse wave system and a tongue detection system at the same time, in order to solve the problem. Computer simulation results have proven to show that EMR systems used in hospitals and clinics are more efficient and yield a more accurate diagnosis than traditional methods.
