Pop-up Chart for Managing CRRT Improves the Quality of CRRT Care (전자의무기록 팝업차트를 활용한 CRRT 관리의 질향상 활동)

  • Purpose: The time lag between the decision to initiate continuous renal replacement therapy (CRRT) and its actual initiation remains a major barrier in our intensive care units. We developed a CRRT pop-up chart on EMR for managing CRRT machines. Methods: This study measured time interval between the decision to prepare the CRRT machine and the actual use of the machine before and after using a CRRT pop-up chart. This study conducted a questionnaire of the medical staff to assess the changes in the quality of CRRT preparation. Results: A total of 95 patients on CRRT is analyzed. The time to find an available CRRT machine is decreased by 24.6%. The time to move a CRRT machine to the patient's bedside is decreased by 55.8%. Medical surveys of 44 nurses gave the following results. 1) The time to apprehend machines for 1 to 3 minutes is improved from 29.5% to 81.8%, and the time to apprehend machines over 3 minutes is decreased from 70.5% to 18.2%. 2) The number (6-all) of known machine locations is improved from 22.7% to 63.4%. 3) Interruption of a nurse's work due to telephone calls asking for the possession of movable CRRT equipment also is improved. Scores of 1-4 are improved from 15.9% to 41%. Scores of 5-7 are reduced from 52% to 15.9%. Conclusions: CRRT pop-up chart is shortened the time lag of CRRT machine preparation, reduced the nurse's phone workload and helped to improve the quality of CRRT care.

A Development Study of Common Clinical Document Forms for Traditional Korean Medicine Information Standardization (한의 정보 표준화를 위한 공통 임상 기록 서식 개발 연구)

  • Objectives: The clinical document forms, a format for collecting clinical data, is the most fundamental object of standardization. Doctors must have a mutual understanding of the clinical chart. Methods: Clinical document forms were developed by investigating existing conditions in hospitals and conducting demand surveys, doing literature research, and seeking expert advice for the improvement of version 1.0. In addition, an organization of a network of 19 Oriental medical doctors and nurses, 190 patients, and users of collected and assessed data was formed to come up with version 2.0. Results: The overall format was divided into different portions that the patient, nurse, and doctor must fill out, respectively. The patient's section consists of demographic data, lifestyle details, history, and symptoms. The data to be supplied by the nurse include the patient's vital signs and anthropometric parameters. As for the doctors, they are to supply data regarding the patient's palpitation, the detailed symptoms of the patient's head, ophthalmological and otorhinolaryngological symptoms (mouth), respiration, circulatory organ and chest conditions, digestive-organ conditions (thirst), neuropsychiatric conditions, reproductive system, musculoskeletal system, skin (depilation), etc. Conclusions: Common clinical chart development is the prior question to Traditional Korean Medicine standardization. A web-based clinical document format should be developed to support diagnosis and treatment, and furthermore EMR (electronic medical record system) and EHR (electronic health record) developed. Clinical information could be shared through a network of medical institutions and be useful Traditional Korean Medicine for evidence-based medicine.

  • The electronic acupuncture has the advantage of treating the patient with acupuncture by simultaneously analyzing the patient's physical condition and treatment condition. In this paper, it is proposed and developed an electronic acupuncture algorithm embedded in a multi pad. Especially, it is implemented the control algorithm using the optimal electronic dipstick calculation, sensing pad and DSP system for the patient's body condition. Moreover, in this paper, it is developed SW and HW prototypes that can receive personal information by patient and can receive multi-array type electronc needle guiding procedure based on WEB.

  • Objectives : The study designed to analyze annual, seasonal tendency of outpatients in ophthalmology & otolaryngology & dermatology clinic of Korean Medical hospital from 2012 to 2016. Methods : We investigated annual, seasonal population-social characteristics such as sex or age, departmental characteristics such as ophthalmology, otolaryngology, dermatology, many others, main diagnosis, cost characteristics of treatment period and frequency of treatment those who were received treatment as outpatients in Dunsan Korean Medical Hospital ophthalmology & otolaryngology & dermatology clinic from March 2012 to March 2016. The data was obtained from EMR chart, and statistical analysis was performed using SPSS 19.0. Results : We analyzed population-social characteristics, utilization of new patients in the year, in the season, and we obtained various results. Conclusions : In the analysis of annual, seasonal utilization patterns that outpatients offered in ophthalmology & otolaryngology & dermatology clinic of Korean Medical hospital from 2012 to 2016, the report draws 3 different conclusions.

Processing of dosage units and design of database schema for formulas in Korean medicine ontology (한의 온톨로지 처방의 용량 단위 가공과 데이터베이스 스키마 설계)

  • Objectives : This study aims to propose a processing method for dosage units of medicinal materials and the database schema to manage formula data in Korean medicine ontology. Methods : All dosage units of medicinal materials are collected from the seven textbooks that contain formula data of Korea medicine ontology. Dosages are converted to Arabic numerals and units that are frequently used are converted to representative units. Database schema is designed for processing and managing the formulas and medicinal materials with dosage units. Results : Seven representative units are selected out of 77 units. They will be used in the addition or subtraction of medicinal materials in a formula support system. The remaining units will be made available for references. Conclusions : EMR or chart programs used in clinical hospitals contain formula data that is already standardized. However, the formula data in Korean medicine literature and textbook is not refined, so it is necessary to process the dosages and units of medicinal materials to use in the formula support system. This result is a processing method to utilize the formula data of Korean medicine textbooks and it will be implemented this method in the established formula support system in the future.

  • Due to the convergence between digital devices and the development of wireless communication technology, bit-signal sensor miniaturization, building an Electronic Medical Record (EMR) which is a digital version of a paper chart that contains all of a patient's medical history and the information of Electronic Health Record (EHR), Ubiquitous healthcare (u-Healthcare) that can monitor their health status and provide personal healthcare service anytime and anywhere. Also, the appearance of cloud computing technology is one of the factors that accelerate the development of u-healthcare service. However, if the individual information to be used maliciously during the u-healthcare service utilization, leads to serious problems directly related to the individual's life because if it goes beyond the level of simple health screening and treatment, it may not provide accurate and reliable healthcare services. For this reason, we analyzed a variety of security issues related to u-healthcare service in cloud computing environment and described about directions of secure health information sharing system construction. In addition, we suggest the future developmental direction for th activation of u-healthcare industry.

The Analysis on Annual Utilization Patterns of Inpatients in Korean Medical Hospitals for the Past 10 years (10년간 일개 한의대 부속 한방병원에 입원한 환자에 대한 연도별 이용실태 분석 : 침구의학과를 중심으로)

  • Objectives : This study was designed to clarify population-social characteristics that influence the utilization patterns of hospitalized patients in a traditional korean hospital, thereby providing clinical data which would help further improvements of traditional korean medical service in particular the Acupuncture and Moxibustion. Methods : We investigated population-social characteristics and annual utilization patterns of all patients who were hospitalized for more than 24 hours in a Korean Medical Hospital from January 2005 to December 2014. The obtained data were recorded in the EMR chart and statistical analysis was performed using SPSS 21.0. Additionally, data from the patients admitted to the department of Acupuncture and Moxibustion were analyzed separately. Results : 1. All inpatients had a significant annual difference in age, gender, hospitalized department, and disease code annually but not in re-hospitalization number. Inpatients of the department of Acupuncture and Moxibustion also varied in their age, gender, and disease code annually, but not in re-hospitalization number. 2. Pearson correlation analysis on all inpatients showed that the mean days of hospital treatments had a negative correlation with all variables except medical care insurance. Total cost, cost per day per person and recuperation cost had a positive correlation with all variables except medical care insurance. There was no meaningful relationship between nonrecuperation cost and the variables. 3. Stepwise multiple regression analysis on all inpatients showed that the mean days of hospital treatments had a negative correlation with all variables except automobile insurance. The total hospitalization costs had a positive correlation with both general insurance and medical care insurance. Cost per day per person and recuperation cost had a positive correlation with the females. There was no meaningful relationship between non-recuperation cost and the variables. 4. Pearson correlation analysis on inpatients of the department of Acupuncture and Moxibustion inpatients showed that the mean days of hospital treatments had a positive correlation with all variables except general insurance and automobile insurance.
