A Preliminary Study on Clinical Decision Support System based on Classification Learning of Electronic Medical Records

  • We employed a hierarchical document classification method to classify a massive collection of electronic medical records(EMR) written in both Korean and English. Our experimental system has been learned from 5,000 records of EMR text data and predicted a newly given set of EMR text data over 68% correctly. We expect the accuracy rate can be improved greatly provided a dictionary of medical terms or a suitable medical thesaurus. The classification system might play a key role in some clinical decision support systems and various interpretation systems for clinical data.

Characteristics of MnxSi1-xTe Compound Studied by Electron Magnetic Resonance and Other Experiments

  • The magnetic and other physical characteristics of $Mn_xSi_{1-x}Te$ have been investigated by electron magnetic resonance (EMR), X-ray diffraction (XRD) and other experiments. $Mn_xSi_{1-x}Te$ is found to have corundum structure for manganese contents up to 10% and also to be ferromagnetic for temperatures below 80 K. While ferromagnetic resonance signal coexists with the usual paramagnetic resonance signal, invariance of the g-factor inferred from the electron paramagnetic resonance signals throughout all temperature ranges clearly confirms that the manganese ions are in the electronic 3d5 state. The temperature dependence of EMR line-width is the same as other diluted magnetic semiconductors. From the EMR signals relaxation times $T_2$ and $T_1$ of $Mn_xSi_{1-x}Te$ compounds are estimated to be about $4.4{\times}10^{-10}s$ and $9.3{\times}10^{-8}s$ respectively and are found to vary slightly with temperature or composition change. Exchange narrowing of the EMR line-width becomes dominant for the sample in which the substitution ratio, x = 30%. For one sample, in which x = 0.5%, spin glass-like behavior is indicated by EMR signals for temperatures lower than 60 K. This behavior may authentic for samples within a certain range of x.

  • 국내 대형병원 중심의 통합의료정보시스템은 2000년 초반을 기점으로 부터 처방전달시스템(OCS : Order Communication System)중심에서 전자의무기록(EMR : Electronic Medical Record)시스템을 포함한 시스템으로 확장발전하고 있다. 하지만, 오늘날의 고도화된 의료정보시스템은 정보 사용자의 요구도가 복잡 다양해짐에 따라 단위 시스템으로 개발되어 점차 통합되는 과정에 이르렀고, 이에 따라 대형병원과 같은 복잡한 조직체계에 적합한 의료정보시스템의 권한관리는 해결되어야 할 큰 문제점이다. 또한 다양한 사용자의 적절한 권한관리의 부재로 인하여 개인정보 및 민감정보 노출의 심각성 또한 대두되었다. 따라서 이 논문에서는 과거 병원정보시스템(HIS: Hospital Information System)의 문제점들을 분석하여 의료정보운영 환경에서의 효율적이고 적합한 권한관리 시스템을 제안한다. 또한 통합의료정보시스템의 특성에 맞는 적합한 권한 관리를 개발하여 대형병원 EMR 시스템에 적용된 사례를 기술한다. 제안된 시스템은 차세대 EMR을 비롯한 통합의료정보시스템에 맞는 권한관리 솔루션 개발에 기반이 된다.

AHP를 이용한 전자의무기록시스템 선정 평가에 관한 연구 (A Study on the Evaluation of Electronic Medical Record Systems using the AHP)

  • The evolution of information technology and proliferation of hospital management and managerial applications of computing has led to change in the characteristics, uses and evaluations of software for the hospital management. With the growing proliferation of microcomputer use and the value-added for management strategies, more and more software has been massively developed, produced and distributed for the hospital industry. The user is faced with an increasingly difficult choice in the evaluation and selection of software. For many reasons, users frequently must rely on expert evaluations of the technical functions and quality of software. The objectives of this study are to provide selection criteria for an Electronic Medical Record (EMR) and to develop an evaluation framework for the Hospital Information Systems. The major findings of our study are as follows (1) the identification of EMR evaluation characteristics (2) the design and development of EMR selection model and (3) the evaluation of the importance for EMR characteristics using Analytic Hierarchy Process (AHP). We identify 6 characteristics and 22 sub-characteristics of the EMR, calculate their weights, and decide the best configuration. Especially, the AHP methodology can be applied to gather knowledge from multiple experts. Because AHP can 1) facilitate the participation of multiple experts 2) increase group productivity and therefore result in both quantitatively and qualitatively superior outcomes than that of a single individual's work 3) provide a mechanism for reconciling conflict from multiple expert 4) validate the acquired knowledge, providing consistency of facts, and 5) enhance the accuracy reliability of the acquired knowledge increase through of the reliability provided by consensus across multiple experts. Although some further research is required, the proposed model can be regarded as a basis for the selection of EMR.

지문인식 기반의 전자의무기록 시스템 인증 모델 (An Authentication Model based Fingerprint Recognition for Electronic Medical Records System)

  • 의료정보는 환자에게 중요한 개인정보로써 반드시 보호돼야 한다. 특히 전자의무기록에 접근할때, 의료인의 강화된 신원확인에 대한 인증방식이 필요하다. 기존의 공인인증서 기반 인증모델은 개인키 관리, 권한위임 등 문제점으로 전자의무기록의 특성을 반영하지 못했다. 본 논문에서는 전자의무기록 시스템에 의료인이 접근하는 경우 지문인식 기반 인증 모델을 적용하여 강화된 인증방식을 제안한다. 전자의무기록의 지문인증 모델은 의료업무의 특성을 반영하여 개인키 관리, 권한위임 문제를 원천적으로 해결하였다.

Characteristics of EMR emitted by coal and rock with prefabricated cracks under uniaxial compression

  • Crack instability propagation during coal and rock mass failure is the main reason for electromagnetic radiation (EMR) generation. However, original cracks on coal and rock mass are hard to study, making it complex to reveal EMR laws and mechanisms. In this paper, we prefabricated cracks of different inclinations in coal and rock samples as the analogues of the native cracks, carried out uniaxial compression experiments using these coal and rock samples, explored, the effects of the prefabricated cracks on EMR laws, and verified these laws by measuring the surface potential signals. The results show that prefabricated cracks are the main factor leading to the failure of coal and rock samples. When the inclination between the prefabricated crack and axial stress is smaller, the wing cracks occur first from the two tips of the prefabricated crack and expand to shear cracks or coplanar secondary cracks whose advance directions are coplanar or nearly coplanar with the prefabricated crack's direction. The sample failure is mainly due to the composited tensile and shear destructions of the wing cracks. When the inclination becomes bigger, the wing cracks appear at the early stage, extend to the direction of the maximum principal stress, and eventually run through both ends of the sample, resulting in the sample's tensile failure. The effect of prefabricated cracks of different inclinations on electromagnetic (EM) signals is different. For samples with prefabricated cracks of smaller inclination, EMR is mainly generated due to the variable motion of free charges generated due to crushing, friction, and slippage between the crack walls. For samples with larger inclination, EMR is generated due to friction and slippage in between the crack walls as well as the charge separation caused by tensile extension at the cracks' tips before sample failure. These conclusions are further verified by the surface potential distribution during the loading process.

개인키 위탁관리 서버를 이용한 전자의무기록 지문인증 모델 (An Fingerprint Authentication Model of ERM System using Private Key Escrow Management Server)

  • 의료정보는 환자에게 중요한 개인정보로써 반드시 보호되어야 하는 중요 정보이다. EMR((Electronic Medical Records) 시스템은 개인정보와 의료정보가 유출될 경우, 환자의 사생활 침해 등 매우 심각한 피해를 초래할 수 있어 EMR 시스템의 의료정보는 사용자 접근에 관한 제어 및 통제 강화 등 높은 보안성이 요구되는 시스템이다. 특히 의료인이 전자의무기록에 접근할 때, 보안이 강화된 신원확인에 대한 인증방식이 반드시 필요하다. 그러나 기존의 공인인증서 기반의 인증모델은 개인키 관리, 권한위임 등의 문제로 인해 전자의무기록의 보안 특성을 반영하지 못하였다. 본 연구에서는 기존의 전자의무기록(EMR) 시스템 접근 시 문제점을 해결할 수 있는 보안이 강화된 지문인식 기반 인증 모델을 제안한다. 제안한 인증 모델은 PEMS(Private-key Escrow Management Server)를 이용한 EMR 지문인증 모델로서, 개인키 위탁 프로토콜과 개인키 인출 프로토콜을 적용하여, 개인키 관리와 권한위임 문제를 해결할 수 있도록 하였다. 제안한 인증 모델은 성능 실험을 통해 기존의 공인인증서 기반 인증에 비해 수행시간 단축된 것을 확인할 수 있었고, 기존 전자서명 비밀번호 방식을 대체 가능하며, 사용자의 편의성이 증가된 장점이 있다.

  • 의료 서비스의 효율성과 질은 개인의 의료 정보의 안전한 보호와 투명한 관리에 크게 의존하며, 이는 디지털 시대에 더욱 중요해지고 있다. 현재의 EMR 시스템은 중앙 집중식으로 운영되어 개인의 의료 정보에 대한 소유권 및 투명성 부족으로 인해 문제가 발생하고 있으며 데이터 공유 및 업데이트 과정에서 지연과 오류가 발생할 수 있다. 이러한 문제를 해결하기 위해, Hyperledger Indy 기반의 분산 신원 관리(DID)와 전자 의료 기록(EMR) 통합 시스템을 제안한다. 이 시스템은 의료 정보의 소유권을 개인에게 확실히 보장하고, 의료 정보의 접근성과 활용도를 높이는 데 목표를 두고있다. 개인은 이 시스템을 통해 자신의 의료 정보를 직접 관리하고, 필요한 경우 해당 정보를 투명하게 공유할 수 있게 되어, 의료 서비스의 효율성을 높일 수 있다. 또한, 이 시스템은 의료 정보를 안전하게 보호하고, 투명하게 관리하여, 의료 서비스의 투명성을 높이고, 개인의 의료 정보에 대한 통제력을 강화한다. 따라서, 이 시스템은 의료 서비스의 질 향상, 개인의 의료 정보 보호, 그리고 의료 서비스의 효율성 향상에 크게 기여할 것이다.

균형성과표를 활용한 전자의무기록시스템의 성과측정 모형개발 (Development of the Performance Measurement Model of Electronic Medical Record System - Focused on Balanced Score Card -)

  • The purpose of this study are suggest to performance measurement model of Electronic Medical Record(EMR) and Key Performance Index(KPI). For data collection, 665 questionnaires were distributed to medical record administrators and insurance reviewers at 31 hospitals, and 580 questionnaires were collected(collection rate: 87.2%). Regarding methodology, Critical Success Factor(CSF) and index of the information system were derived based on previous studies, and these were set as performance measurement factors of EMR system. The performance measurement factors were constructed by perspective using BSC, and analysis on causal relationship between factors was conducted. A model of causal relationship was established, and performance measurement model of EMR system was proposed through model validation. Analysis on causal relationship between performance management factors revealed that utility cognition of the learning & growth perspective factor had causal relationship with job efficiency(${\beta}=0.20$) and decision support(${\beta}=0.66$) of the internal process perspective factors, and security had causal relationship with system satisfaction(${\beta}=0.31$) of the customer perspective factor. System quality had causal relationship with job efficiency(${\beta}=0.66$) and decision support(${\beta}=0.76$) of the internal process perspective factors, all of which were statistically significant(P<0.01). Job efficiency of the internal process perspective had causal relationship with system satisfaction(${\beta}=0.43$), and decision support had causal relationship with decision support satisfaction(${\beta}=0.91$) and job satisfaction (${\beta}=0.74$), all of which were statistically significant(P<0.01). System satisfaction of the customer perspective had causal relationship with job satisfaction(${\beta}=0.12$), job satisfaction had causal relationship with cost reduction(${\beta}=0.53$) of the financial perspective, and decision support satisfaction had causal relationship with productivity improvement(${\beta}=0.40$)of the financial perspective(P<0.01). Also, cost reduction of the financial perspective had causal relationship with productivity improvement(${\beta}=0.37$), all which were statistically significant(P<0.05). Suitability index verification of the performance measurement model whose causal relationship was found to be statistically significant revealed that $X^2/df=2.875$, RMR=0.036, GFI=0.831, AGFI=0.810, CFI=0.887, NFI=0.838, IFI=0.888, RMSEA=0.057, PNFI=0.781, and PCFI=0.827, all of which were in suitable levels. In conclusion, the performance measurement indices of EMR system include utility cognition, security, and system quality of the learning & growth perspective, decision support and job efficiency of the internal process perspective, system satisfaction, decision support satisfaction, and job satisfaction of the customer perspective, and productivity improvement and cost reduction of the financial perspective. In this study, it is expected that the performance measurement indices and model of EMR system which are suggested by the author, will be a measurement tool available for system performance measurement of EMR system in medical institutions.
