응급실에서 전원된 급성기 근골격계 환자의 통증에 미치는 침치료의 효과: 증례보고 (Effects of Acupuncture on Acute Musculoskeletal Pain Transferred from Emergency Department: Case Report)

  • Objectives : The aim of this study was to report the effects of acupuncture on the patients with severe acute musculoskeletal pain transferred from emergency department(ED). Methods : Three patients were transferred from ED of western medicine, and treated with Korean medicine treatments composed of acupuncture mainly and cupping, herbal steaming, and herbal decoction additionally. Results : All patients showed pain-reduction and relaxation both physically and mentally immediately after the first acupuncture treatment. In cases 1 and 2, the pain numerical rating scale(NRS) decreased after the day of admission from 8 to 4 and 10 to 2, respectively. In case 3, pain NRS also gradually decreased from 8 to 4 after three days. Conclusion : Acupuncture was effective for immediate pain reduction in patients with severe acute musculoskeletal pain transferred form ED. Further studies are warranted to confirm the immediate pain-reduction effect of acupuncture on acute musculoskeletal patients visiting or transferred from ED.

Comparison of the Effects of Early and Conventional Defoliation on Fruit Growth, Quality and Skin Color Development in 'Fuji' Apples

  • We compared the quality of 'Fuji' apples (Malus ${\times}$ domestica) from trees whose leaves were not removed (no artificial defoliation; NAD) with apples from trees that underwent early defoliation (ED, treated in mid September and early October) and conventional defoliation (CD, treated in early and mid October). The experiment was conducted in three consecutive years using 15-year-old 'Fuji' apple grafted on Malus prunifolia. Fruits were harvested on November 7, 16 or 12 in 2011, 2012 and 2013, respectively. Compared to NAD treatment, ED and CD treatment reduced the fresh weight by 4.7% and 0.6%, respectively. The soluble solids content of NAD apples ($14.4^{\circ}Brix$) was slightly higher than that of CD ($14.1^{\circ}Brix$) and ED ($14.0^{\circ}Brix$) apples. Soluble sugar content, flesh firmness, water-core index, and titratable acidity were not affected by defoliation treatment regardless of treatment timing. The skin blush index of NAD apples (2.3) was inferior to that of CD (3.3) and ED (3.4)- treated apples. Furthermore, artificial defoliation treatments increased skin redness ($a^*$) and yellowness ($b^*$) and significantly improved the degree of skin blush compared to NAD fruits.

식물병원균 생육을 저해하는 Aspergillus terreus로부터 분리한 향균물질의 특성 (Characterization of an antifungal compound isolated from an antagonistic fungus Aspergillus terreus against phytopathogenic fungi)

  • 온실 토양으로부터 주요 작물 병원균의 생육을 억제하는 길항곰팡이를 분리, 동정하였고, 그 균이 분비하는 향균물질을 분리하여 활성을 petriplate assay로 검정했다. 길항균의 foot cell, 분생포자 및 MY20 배지상의 균사 등 형태학적 특징에 근거하여 이 균을 Aspergillus terreus로 동정하였다. A. terreus의 배양여액을 chloroform으로 추출하고, 그 추출물을 column chromatography와 thin layer chromatography로 향균물질을 순수분리하였으며, 화학구조는 butyrolactone I이었다. 이 물질의 주요 작물병원균에 대한 $ED_{50}$은 Botrytis cinerea, Phytophthora capsici. Pythium ultimum, Rhizoctonia solani, 그리고 Fusarium oxysporum에 각각 9.7, 42.6, 23.3, 13.7, 102.7 ppm으로 나타났다.

수종 한약재가 혈관에 미치는 영향 (Effects of Several Herbs on the Blood Vessel)

  • Rhizoma Arisaematis, Lignum Akebiae, Rhizoma Zedoariae, Cortex Eucommiae, Folium Perillae, Radix Sophorae Subprostratae, Radixi, Radix Ledeboutriellae, Rhizoma Atractylodis, Herba Ephedrae, Radix Puerariae and Radi Aconitx Bupleuri have been used in Korea for many centuries as a treatment for various disease. The purpose of the present study is to determine the effect of several herbs on norepinephrine(NE) induced blood vessel contraction in rabbits and pigs. Rabbit(2 kg, male) were killed by $CO_2$ exposure and a segment (8-10mm) of each rabbit was cut into equal segments and mounted in a tissue bath. Contractile force was measured with force displacement transducers under 2-3 g loading tension. The dose of norepinephrine(NE) which evoked 50% of maximal response $(ED_{50})$ was obtained from cumulative dose response curves for NE $(10^{-6}{\sim}10^{-3}M)$. Contractions evoked by NE $(ED_{50})$ were inhibited significantly by Rhizoma Arisaematis, Lignum Akebiae, Rhizoma Zedoariae, Cortex Eucommiae, Folium Perillae, Radix Sophorae Subprostratae and Herba Ephedrae in abdominal aorta. Contractions evoked by NE $(ED_{50})$ were inhibited significantly be Lignum Akebiae, Rhizoma Zedoariae, Cortex Eucommiae, Herba Ephedrae, Radix Puerariae and Radix Bupleuri in femoral artery. Contractions evoked by NE $(ED_{50})$ were inhibited significantly by Radix Sophorae Subprostratae, Radix Aconiti and Herba Ephedrae in renal artery. These results indicate that each herb can relax NE induced contraction of rabbit and pig blood vessel selectively, and that this relaxation relates to Gui-Gyung(歸經).

6-(3,4-디클로로페닐)아미노-7-클로로-5,8퀴놀린디온의 항진균작용 및 안전성 평가 (The Evaluation of Antifungal Activities and Safeties of 6-(3,4-Dichlorophenyl)amino-7chloro-5,8-quinolinedione)

  • 6-(3,4-Dichlorophenyl)amino-7-chloro-5,8-quinolinedione (RCK50) was tested for antifungal activities in mice systemically infected with Candida albicans. The therapeutic potential of RCK50 was also assessed in comparison with ketoconazole. CK50 had $ED_{50}$ 0.22${\pm}$0.01mg/kg. Ketoconazole as a positive control had $ED_{50}$ 6.00${\pm}$1.70mg/kg. Intraperitoneally administered RCK50 at the $ED_{50}$ for 7 days and 14 days reduced Candida albicans colony count in the kidneys and liver. And administered RCK50 at the $ED_{50}$ for 14 days improved survival rates. The genotoxicities of RCK50 had been evaluated. RCK50 was negative in Ames test with Salmonella typhimurium and chromosomal aberration test in CHL cells. RCK50 did not show any clastogenic effect in mouse peripheral blood and was negative in mouse micronucleus assay. These results indicate that RCK50 has no genotoxic potential under these experimental conditions. Acute oral toxicity studies of RCK50 were carried out in ICR mice of both sexes. RCK50 did not show acute oral toxicities and $LD_{50}$ values were over 2,850mg/kg in ICR mice.

Analysis of Errors on Death Certificate for Trauma Related Death

  • Purpose: This study was to investigate errors of death certificate (DC) issued for patients with trauma. Methods: A retrospective review for DC issued after death related to trauma at a training hospital trauma center was conducted. Errors on DC were classified into major and minor errors depending on their influence on the process of selecting the cause of death (COD). All errors were compared depending on the place of issue of DC, medical doctors who wrote the DC, and the number of lines filled up for COD of DC. Results: Of a total 140 DCs, average numbers of major and minor errors per DC were 0.8 and 3.7, respectively. There were a total of 2.8 errors for DCs issued at the emergency department (ED) and 5.4 errors for DCs issued beyond ED. The most common major error was more than one COD on a single line for DCs issued at the ED and incompatible casual relation between CODs for DCs issued beyond ED. The number of major errors was 0.5 for emergency physician and 0.8 for trauma surgeon and neurosurgeon. Total errors by the number of lines filled up for COD were the smallest (3.1) for two lines and the largest (6.0) for four lines. Conclusions: Numbers of total errors and major errors on DCs related to trauma only were 4 and 0.8, respectively. As more CODs were written, more errors were found.

How Effective Are Radiation Reducing Gloves in C-arm Fluoroscopy-guided Pain Interventions?

  • Background: The physician's hands are close to the X-ray field in C-arm fluoroscopy-guided pain interventions. We prospectively investigated the radiation attenuation of Proguard RR-2 gloves. Methods: In 100 cases, the effective doses (EDs) of two dosimeters without a radiation-reducing glove were collected. EDs from the two dosimeters-one dosimeter wrapped with a glove and the other dosimeter without a glove-were also measured at the side of the table (Group 1, 140 cases) and at a location 20 cm away from the side of the table (Group 2, 120 cases). Mean differences such as age, height, weight, radiation absorbed dose (RAD), exposure time, ED, and ratio of EDs were analyzed. Results: In the EDs of two dosimeters without gloves, there were no significant differences ($39.0{\pm}36.3{\mu}Sv$ vs. $38.8{\pm}36.4{\mu}Sv$) (P = 0.578). The RAD ($192.0{\pm}182.0radcm^2$) in Group 2 was higher than that ($132.3{\pm}103.5radcm^2$) in Group 1 (P = 0.002). The ED ($33.3{\pm}30.9{\mu}Sv$) of the dosimeter without a glove in Group 1 was higher than that ($12.3{\pm}8.8{\mu}Sv$) in Group 2 (P < 0.001). The ED ($24.4{\pm}22.4{\mu}Sv$) of the dosimeter wrapped with a glove in Group 1 was higher than that ($9.2{\pm}6.8{\mu}Sv$) in Group 2 (P < 0.001). No significant differences were noted in the ratio of EDs ($73.5{\pm}6.7%$ vs. $74.2{\pm}9.3%$, P = 0.469) between Group 1 and Group 2. Conclusions: Proguard RR-2 gloves have a radiation attenuation effect of 25.8-26.5%. The radiation attenuation is not significantly different by intensity of scatter radiation or the different RADs of C-arm fluoroscopy.

의도적인 중독 환자들의 음주 여부에 따른 중독 중증도 지수의 비교 (Comparison of Poisoning Severity Score (PSS) according to alcohol co-ingestion in intentional poisoning patients)

  • Purpose: Alcohol ingestion enhances impulsivity and aggression, and has been proven to have a close relationship with suicide. This study investigates whether alcohol co-ingestion affects the Poisoning Severity Score (PSS) grade in patients with intentional poisoning. Methods: We conducted a retrospective analysis of intentional poisoning patients who visited the emergency department (ED) from January 1 to December 31, 2020. Patients were divided into non-drunken and drunken groups. We collected the data based on the medical records of the patients and serum ethanol level results recorded during initial blood tests at the ED. To grade the PSS, the highest score was assessed through clinical signs and test results during the hospital stay. A comparative analysis was conducted between the two groups. Results: A total of 277 patients were included in the study. 163 (58.8%) were in the non-drunken group, and 114 (41.2%) were in the drunken group. The PSS grade showed a significant difference between the two groups (p=0.002). While grade 1 (mild) was observed more in the non-drunken group, grade 2 (moderate) and grade 3 (severe) were seen more in the drunken group. In an ordinal logistic regression analysis, alcohol co-ingestion (adjusted odds ratio [aOR] 2.557, 95% confidence interval [CI] 1.554-4.208, p<0.001) was considered to be a risk factor for a higher PSS grade. There was no significant correlation between the serum ethanol level and the PSS grade. (p=0.568) Conclusion: Intentional poisoning patients with alcohol co-ingestion had a higher PSS. Hence close observation and aggressive treatment in the ED is warranted in such cases.

양식산 볼락, Sebastes inermis Cuvier의 성장 특성 (Growth Characteristics of Cultured Dark-Banded Rockfish, Sebastes inermis Cuvier)

  • 본 연구의 목적은 양식산 볼락, Sebastes inermis Cuvier의 자 치어 시기부터 성어에 이르기까지의 성장 특성을 조사하여, 본 종에 대한 수산생물학적 연구 및 양식기술개발을 위한 기초 자료를 제공하는데 있다. 이를 위하여 250마리의 어미로부터 생산된 동일한 일령의 자어를 대상으로 산출 직후부터 1,350일째까지의 성장 특성을 조사하였다. 볼락에 있어서 산출 후 경과 일수(days after parturition, X)에 대한 전장(total length, TL)의 성장은 $TL=0.0217X\;(r^2=0.8867)$, 그리고 체중(body weight, BW)의 성장은 $BW=0.0001X^2+0.0713X\;(r^2=0.9858)$의 관계가 있었다. 또한 전장에 대한 체중의 상대성장은 $BW=0.0072TL^{3.4373}\;(r^2=0.9945)$의 관계가 있는 것으로 나타났다. 산출 후 경과일수(X)에 따른 눈지름(eye diameter, ED) 성장은 $ED=-2E-5X^2+0.0326X$ ($r^2=0.9886;$ X, 산출 후 기일)의 관계식에 따랐다. 전장에 대한 두장(head length, HL), 주둥이길이 (snout length, PHL) 눈지름(ED), 후두부길이(postorbital head length, PHL)와의 관계는 각각 $HL=0.2652TL\;(r^2=0.9758),\;SL=0.0613TL\;(r^2=0.9602),\;ED=0.0752TL\;(r^2=0.9715)$, 및 $PHL=0.2652TL\;(r^2=0.9501)$로 나타났다.

지리산오갈피, 가시오갈피, 오갈피나무, 오가나무 추출물의 항염증, 피부장벽개선, 항노화 효능 비교 (Comparison of Anti-inflammatory, Skin Barrier Improvement, and Anti-aging Efficacy of Eleutherococcus divaricatus var. chiisanensis and various Eleutherococcus Genus Extract)

  • 활성산소로 인한 염증 유발과 그로 인한 장벽손상은 꾸준히 주름생성의 원인으로 지목되어 왔다. 본 연구에서는 오갈피나무속에 속해있는 지리산오갈피와 가시오갈피, 오갈피나무, 오가나무 4종의 추출물에 대한 항산화, 항염증 및 피부 장벽개선, 항노화 관련 기능성 화장품 소재로서의 활용가치를 검증하고자 유효 지표성분 탐색과 효능 분석을 수행하였다. 먼저, 오갈피나무속 식물인 지리산오갈피와 가시오갈피, 오갈피나무, 오가나무 각각을 50% 에탄올을 용매로 추출, 농축, 동결건조 하여 시료를 제조하였다. 유효 지표성분탐색을 위해 주요 지표 성분인 eleutheroside E와 오갈피나무속에서 주로 검출되는 폴리페놀류 중 chlorogenic acid, caffeic acid 에 대하여 고성능 액체크로마토그래피를 통해 성분 함량을 측정하였으며, 오갈피나무속 중 지리산오갈피에서 eleutheroside E와 chlorogenic acid의 함량이 가장 높은 것을 확인하였다. 항산화활성은 DPPH 라디칼 소거활성이 지리산오갈피에서 가장 높았으며, 항염증 효능의 경우 LPS로 염증이 유발된 대식세포 내 NO 생성 억제 효능은 가시오갈피, 오가나무, 오갈피나무 추출물 100 ㎍/mL에서 각각 25%, 26%, 32% 일 때 지리산오갈피 추출물 100 ㎍/mL는 40% 억제 효능을 보였고 pro-inflammatory cytokine으로 알려진 IL-6 발현 억제 효능은 가시오갈피, 오가나무, 오갈피나무 추출물 100 ㎍/mL 농도에서 각각 20%, 36%, 23% 일 때 지리산오갈피 추출물 100 ㎍/mL는 41% 억제 효능을 보였다. 또한, 지리산오갈피 추출물 100 ㎍/mL에서 피부장벽 관련 인자인 filaggrin, involucrin의 발현이 각각 2.5 배, 1.6 배 증가되었으며, 주름관련 인자인 콜라겐분해효소는 29% 억제 효능을 나타내어 오갈피나무속 4 종 중 지리산오갈피는 피부의 염증 억제와 장벽강화, 주름개선용 화장품소재로써 활용가능하리라 판단된다.
