Heart Rate Variability in Patients with Coronary Artery Disease (관상동맥질환 환자의 심박동변이도)

  • This study is based on previous information regarding reduced cardiac vagal activity in patients with coronary artery disease(CAD), on reduced variance(SDNN : standard deviation of all normal RR intervals), low-frequency power(LF), and the complexity of heart rate variability(HRV) in patients with chronic heart failure(CHF), and on the normalized high-frequency power of HRV is the highest in the right lateral decubitus position among 3 recumbent postures in patients with CAD, However, nothing is known about the nonlinear dynamics of HRV for the 3 recumbent postures in patients with CAD. To investigate the linear and non-linear characteristics of HRV in patients with CAD, 29 patients as CAD group and 23 patients as control group were studied. Electrocardiogram(ECG) with lead II channel was measured on these patients for 3 recumbent postures in random order. The HRV from ECG was analyzed with linear method(for time and frequency domains) and nonlinear method. The lower the high-frequency power in normalized unit(nHF) in the supine or left lateral decubitous position, the higher the increase in nHF when the position was changed from supine or left lateral decubitous to right lateral decubitous. Among the 3 recumbent postures in patients with severe CAD, the right lateral decubitus position was observed to induce the highest vagal modulation, the lowest sympathetic modulation, and the highest complexity of human physiology system.

  • Cardiovascular disease is the leading cause of death worldwide and is caused by a variety of causes. The highest risk factor for cardiovascular disease is high blood pressure, which has no obvious symptoms, but if left untreated, it causes several complications. In order to treat hypertension, medication and regular exercise are required. In people with high blood pressure, excessive physical activity can put a great strain on the heart and lead to cardiovascular disease. Therefore, there is a need for an exercise intensity monitoring system through PTT measurement that can perform exercise at an appropriate intensity. In this study, we implemented a PTT change monitoring system according to exercise intensity by calculating PTT through ECG and PPG measurement. The implemented system differentiates the R-peak of the ECG and P-peak of the PPG, and calculates the PTT using the time difference between R-peak and P-peak. A running experiment was conducted to monitoring PTT change according to exercise intensity. As a result of the experiment, low intensity PTT is 0.313s, moderate is 0.220s, high is 0.188s, it was confirmed that the PTT decreased as the exercise increase increased.

Measurement of Cardiac Pulse Transit Time using Photoplethysmography Sensor (광전용적맥파 센서를 이용한 맥파전달시간의 측정)

  • In this study, we implemented the pulse transit time (PTT) system to examine usefulness of the monitoring method of distensibility and elasticity using photoplethysmography sensor in vivo. PTT is defined as the time interval between the peak of QRS complex in ECG signal and the maximum slope point of photoplethysmography. these two signals were converted to digital data by means of AID converter, then PTT was evaluated by heartbeat using PC. Results of analysis were displayed as a graph using spline interpolation method. The variance of PTT was measured repetitiously to verify efficiency of PTT system in resting state and hyperemic state. Repeated measurement of PTT was not same value but showed that coefficients of correlation were related with each other as 0.8302 (P<0.01) in resting state. And also repeated measurement of PTT showed significant correlation as 0.868 (P<0.01) in the hyperemic state. These result showed that PTT is reflect on transient pressure variance in the artery and is very useful method for the evaluation of prognosis of the hypertension and arteriosclerosis.

  • Many kinds of electrodes have been developed in various forms and shapes for measurement of bio potential signal. Textile electrode has benefit of collect long tenn data monitoring because of it is non-consciousness, convenient and do not occur skin irritation. However, It is very difficult to acquire available data due to high impedance of electrode and unstable skin-electrode contact which generate motion artifact. Also snap button which usually used as mediator between textile and measurement device cause change of electrical characteristics. In this paper, we inflated textile electrode to stabilize contact and add conductive silver paste between textile and snap button to improve conductance. To compare the performance of two methods, flat or inflated and add conductive paste or not, four types of electrodes are tested on each impedance and SNR by ECG measurement. In result, the first type electrode which flat and non-conductive paste showed the worst performance and the last type electrode which is inflated shape and contain conductive paste show the best performance.

  • The multi-channel analog signal to digital signal conversion is increasing in the field of IoT and medical measurement equipments. It has chip area and power consumption constraints to use a few single or 2_channel ADC for multi_channel application. This paper described to design and implement a proposed comb filter for multi-channel, 24bit ADC. The function of proposed comb filter is verified by matlab simulation and the FPGA test board. It was fabricated using SK Hynix 0.35㎛ CMOS standard process. The performance and chip size is compared with the existing design method that uses integrator/differentiator and FIR construction. The proposed comb filter is expected to use the IoT product and medical measurement equipments that require multi-channel, low power consumption and small hardware size.

A Systematic Review of the Attributes of Interior Design Affecting User's Positive Emotions Measured via Bio-Signals (생체신호 기반 사용자의 긍정적인 감정에 영향을 미치는 실내디자인 특성에 관한 문헌고찰)

  • Environmental conditions are known to impact human health and behavior, emotions such as pleasure, anxiety, and depression, and reduce stress. Interior design that elevates emotional comfort and satisfaction can help improve mental health and well-being. This study is a systematic review that analyzed previous empirical studies that explored the effect of interior design elements on the user's emotional response which is quantitatively evaluated by bio-signal and qualitatively evaluated through self-reported questionnaire surveys. This paper aims to derive the attributes of interior design and biometric indicators that affect the user's positive emotion through the synthesis of previous studies and to confirm the feasibility of measuring bio-signals as an objective evaluation tool for architectural design and as a quantitative research method. As a result of the review, the biometric data from EEG, fMRI, ECG, EMG, GSR, and eye-tracking were used to measure the participants' emotional responses, which were manifested as positive or negative depending on certain attributes of interior design such as the form, color, lighting, material and furniture. The attributes of interior design related to the positive emotional response were the curved shape, high ceiling, openness of space, and subdued tone colors. Standard lighting conditions and wooden spaces were related to stress reduction in terms of comfort and relaxation. The free arrangement of furniture was related to the user's positive emotions. On the other hand, consistent experimental protocols could not be found, and although the sample sizes of the studies were small, the studies have demonstrated the feasibility of the emotional response measurement by using the biometric data. Therefore this method can be a useful objective tool in the measurement of human-centric data in architectural design, and to develop the evidence-based design to induce positive emotions and minimize stress.

Development of a Pulse Wave Velocity Measurement System and Assessment of the System Reproducibility for the Diagnosis of Arteriosclerosis (동맥경화 진단을 위한 맥파전달속도 측정시스템 개발 및 재현성 평가)

  • Background: PWV is determined by dividing the distance by the time taken for the pulses traveling between two measuring sites, used as a marker of arterial stiffness and an important indicator for cardiovascular disease. Methods: A PWV measurement system, which offers a non-invasive, simple method of measurement, and simultaneous recording of six signlas(ECG, PCG and four pulse waves from carotid, femoral, radial and dorsalis pedis arteries) was developed. Seventeen healthy subjects with a mean age of 33 years(22 to 52) without any cardiovascular disease were participated for the experiment. Two observers(A and B) performed two consecutive measurements from the same subject in a random order. For the evaluation of stability and accuracy of the PWV measurement system, reproducibility of PWV from between-observer were also evaluated. Results: PWV $values(Mean{\pm}SD)$ measured by A were $7.07{\pm}1.48m/s$, $8.43{\pm}1.14m/s$ , $8.09{\pm}0.98m/s$ for aorta, arm, and leg, respectively. The values obtained from B were $6.76{\pm}1.00m/s$, $7.97{\pm}0.80m/s$, and $7.97{\pm}0.72m/s$ for aorta, arm, and leg, respectively. Between-observer $differences(mean{\pm}SEM)$ from the aorta, arm and leg were $0.14{\pm}0.15m/s$, $0.18{\pm}0.10m/s$ and $0.07{\pm}0.10m/s$. Reproducibility coefficients(2SD) from the aorta, arm, and leg were 0.62m/s, 0.84m/s and 0.86m/s. Correlation coefficients were significantly higher in aortic PWV, 0.93, compared to the coefficients for arm and leg. Coefficient of variance which reflects the reproducibility of the system ranged from $4.4{\sim}5.8%$ in all regional PWV. , Conclusion: Reproducibility of PWV in the study shows that the developed system has reliable and reproducible characteristics. The PWV measurement system used for the study offers comfortable and simple operation and provides accurate analysis and results with high reproducibility. Results of the PWV measurement system could contribute to various clinical applications in the future.

Improvement of the Accuracy of Wrist Noninvasive Blood Pressure Measurement Using Multiple Bio-signals (다중 생체 신호를 통한 손목 혈압 측정의 정확도 향상)

  • The blood pressure measuring equipment, which is being supplied and used most widely by being recognized convenience and accuracy now generally, is oscillometric blood pressure monitor. However, a change in blood pressure is basically influenced by diverse elements such as each individual's physiological status and physical condition. Thus, the measurement of blood pressure, which used single element called oscillation in blood pressure of being conveyed to cuff, is not considered on physiological elements such as cardiovascular system status and blood vessel stiffness index, and on external elements, thereby being quite in error. Accordingly, this study detected diverse bio-signals and body informations in each individual as the measurement subject such as ECG, PPG, and Korotkoff Sound in order to enhance convenience and accuracy of measuring blood pressure in the complex measurement equipment, thereby having extracted regression method for compensation in error of oscillometric blood pressure measurement on the wrist, and having improved accuracy of measuring blood pressure. To verify a method of improving accuracy, the blood pressure value in each of SBP, DBP, MAP was acquired through 4-stage experimental procedure targeting totally 51 subjects. Prior to experiment, the subjects were divided into two groups such as the experimental group for extracting regression method and the control group for verifying regression method. Its error was analyzed by comparing the reference blood pressure value, which was obtained through the auscultatory method, and the oscillometric blood pressure value on the wrist. To reduce the detected error, the blood pressure compensation regression method was calculated through multiple linear regression analysis on elements of blood pressure, individual body information, PTT, HR, K-Sound PSD change. Verification was carried out on improving significance and accuracy by applying the regression method to the data of control group. In the experimental results, as a result of confirming error on the reference blood pressure value in SBP, DBP, and MAP, which were acquired through applying regression method, the results of $-0.47{\pm}7.45$ mmHg, $-0.23{\pm}7.13$ mmHg, $0.06{\pm}6.39$ mmHg could be obtained. This is not only the numerical value of satisfying the sphygmomanometer reference of AAMI, but also shows the lower result than the numerical value in SBP : $-2.5{\pm}12.2$ mmHg, DBP : $-7.5{\pm}8.4$ mmHg, which is the mean error in the experimental results of Brram's research for verifying accuracy of Omron RX-M, which shows relatively high accuracy among wrist sphygmomanometers. Thus, the blood pressure compensation could be confirmed to be made within significant level.

Measurement of cardiac output during treadmill exercise by impedance cardiography with a new ensemble average (새로운 앙상블 평균법에 의한 임피던스 심장기록법의 트래드밀 운동 중의 심박출량 측정)

  • In this study, a new ensemble average technique was developed to measure cardiac output during treadmill exercise. Each dZ/dt peak (C point) was used as a starting point for ensemble averaging, instead of conventionally used R wave of ECG in order to prevent the peak dZ/dt waveform from blurring. In ease of using R wave as a reference, time interval from R wave to the peak of dZ/dt varies for each heart beat. Stroke volume, heart rate, and cardiac output of five male were successfully measured with Balke protocol using the new ensemble average technique.

Development of Portable Wireless Emergency Units and Integrated System (휴대용 무선 응급 단말기 및 통합 시스템 개발)

  • In this study, we developed five mobile units and an integrated system which can manage vital signs from each unit using Bluetooth wireless communication. The five kinds of mobile unit were so designed that each has different function to be applied according to the condition of patient properly. The mobile units can measure ECG signal of single or 12 channel, blood pressure, pulse and SpO2 signal from a patient. Also, to reduce the uncomfortable measurement, several types of units such as belt type, wrist type and necklace type were designed. Our proposed system can integrate and monitor several biological signals from different patient by using Bluetooth wireless communication simultaneously. The developed system was evaluated in the simulated emergent situation and showed the system can monitor 5 patients in maximum according to the data quality. It showed the possibilities that the developed system can be used effectively for emergency situation or in- or out-hospital transport of patient. In future, with the combination of mobile communication technique, a patient who is in emergency situation can be provided with proper first-aid and a doctor can pile information of patient and give better diagnosis and treatments.
