언더라이팅시 흔하게 접하는 심전도 소견과 위험 평가 (Common ECG pattern and underwriting risk assessment)

  • ECG is included in certain medical examinations of insurance application, ECG has low specificity and sensitivity. So ECG is not usually used to diagnose specific diseases. But, ECG is not invasive and costs low. So ECG is usually used in underwriting. Actually in underwriting we meet various ECG patterns and diagnosises. Understanding of various ECG patterns is different between insurance medicine and clinical medicine. So We have to learn various ECG patterns and effects on mortality and morbidity. First considerations of ECG readings are age, sex, blood pressure, family history, smoking historyalcohol history and hyperlipidemia. These are predictors for possibility of disease. Also it is important to review recording ECG with proper skill. In this review I consider several ECG diagnosises that we meet frequently, which is, LVH, RVH, ST abnormalities, LBBS, RBBB, A-B blocks, several kinds of arrhythmia. We have to consider long term mortalities and morbidities of specific ECG patterns although applicants have no symptom and sign. And then we have to make underwriting manual according to specific ECG diagnosises and patterns and underwrite precisely ECG patterns according to insurance products. Nowadays coronary heart disease and other heart diseases are increasing in Korea. So we have to learn various ECG patterns and research mortalities and morbidities of abnormal ECG patterns. Also we have to apply to more broad, precise underwriting skills about ECG patterns and diagnosises.

바이너리 파일 디스크립션 방식을 이용한 ECG 변환 시스템 (ECG Conversion System Using Description Method for Binary Files)

  • In this paper, we develop a descriptor based on binary ECG conversion system that supports the conversion of a binary ECG format into XML-based HL7 aECG for the interoperability of ECG. HL7 aECG is a XML based standard for interoperability of ECG waveform. For the conversion of variety of ECG formats, we propose a binary ECG description mechanism. In order to describe binary ECG more efficiently, we develop a XML-based Binary ECG Description (BED) Language. One of the powerful features of using the XML-based ECG Description mechanism is that ECG data can read from various formats without the modification of source code of conversion system, and consequently it reduces conversion system maintenance costs.

  • In this paper, we develop a new conversion method for implementing binary ECG(Electrocardiogram) conversion scheme to improve our previous research works that supported the conversion of binary ECG files into HL7 aECG for enhancing interoperability of ECG data. HL7 aECG is a XML-based standard for interoperability of ECG waveform. To improve the performance of ECG data conversion, we utilize a compilation-based ECG conversion method on binary ECG files. Our new method supports both flexibility of BED-based ECG conversion mechanism and the performance of direct conversion mechanism.

헬스케어 가방의 ECG 센서 전극 위치에 따른 신호 분석 (Signal Analysis According to the Position of the ECG Sensor Electrode in Healthcare Backpack)

  • Heart rate is one of the most important signal to monitor the health condition of the patient or exerciser. Various wearable devices have been developed for the continuous monitoring of ECG signal from human body during exercise. Among these, ECG chest belt has been widely used. However wearing chest belt with ECG sensor is uncomfortable in normal life due to the electrode contact between metal electrodes of ECG sensor and skin of the human body. So we develop the royal healthcare backpack that can measure ECG signal without skin contact by using capacitor-type ECG sensor. The position of the measurement point is critical to collect a clear ECG signal in the capacitive ECG measurement from backpack. Various tests were conducted to find the optimal ECG measurement position which has less noise and could get strong and clear ECG signal during exercise, walking, hiking, mountain climbing and cycling.

상호연동성을 지원하는 XML기반의 심전도 데이터 관리를 위한 메타데이터 시스템 (Metadata System for XML-based ECG Management Supporting Interoperability)

  • 본 연구에서는 의료정보시스템 간에 상호연동성을 지원한 수 있는 XML 기반의 심전도(electrocardiography, 이하 ECG) 데이터의 효율적인 저장구조 및 관리방법을 제시하고, 웹기반으로 ECG 정보서비스를 제공할 수 있는 ECG 메타데이터 시스템을 구현한다. ECG 메타데이터 시스템은 ECG 데이터와 메타데이터의 저장, 검색, 그리고 pdf 파일형식의 리포팅 서비스 기능을 포함한다. ECG 데이터의 저장구조는 심전도 데이터의 특성과 액세스 특성을 분석하여 비분할 저장방식을 사용하였고, 효율적인 검색기능을 제공하기 위해 HL7 aECG 데이터로부터 메타데이터를 추출하여 인덱스로 활용하였다. ECG 리포팅 서비스를 제공하기 위해 템플릿 메커니즘을 이용하여 텍스트 XML 형태의 데이터를 여러 데이터 포맷으로 변환한다.

  • 본 연구에서는 생체신호 중 비침습적으로 측정이 가능하고 많은 건강정보를 포함하고 있는 ECG(electrocardiogram)신호를 일상생활 중 보다 편리하게 모니터링 할 수 있는 시스템을 구현하고자 하였다. 이를 위하여 벨트형 ECG전극 시스템을 개발하였으며, 배터리로 구동 가능한 초소형 저전력 ECG측정시스템을 구현하였다. 또한 측정된 ECG신호의 무선전송을 위하여 Zigbee호환 무선센서노드를 이용하여 초저전력 무선데이터 통신부를 구성하였고 PC상에서 ECG신호를 모니터링하기 위한 프로그램을 구현하였다. 그리고 ECG측정 시 움직임에 따라 발생하는 동잡음의 제거를 위하여 이동평균필터(moving average filter)를 이용하여 기저선 변화를 추출하였고 이를 적응필터의 참조신호로 사용하여 동잡음을 제거하였다. 실험 결과 본 연구에 의해 구현된 ECG전극 및 계측시스템을 통해 활동상태 에서도 ECG계측 가능성을 확인하였으며, 제안한 적응신호처리기법을 통해 활동 중 ECG측정에서 동잡음의 최소화가 가능함을 확인하였다.

  • In this paper, we would like to discuss the signal processing and the algorithm for ECG analysis. The ECG gives us information about the condition of the heart muscle, because myocardial abnormality or infarction is inscribed on the ECG during myocardial depolarization and repolarization. Analyzing the ECG signal, we can find heart disease, for example, arrhythmia and myocardial infarction, etc. Particularly, detecting arrhythmia is more important, because serious arrhythmia can take away the life from patients within ten minutes. The wavelet transform decomposes the ECG signal into high and low frequency component using wavelet function. Recomposing high frequency bands including QRS complex, we can detect QRS complex and eliminate the noise from the original ECG signal. To recognize the ECG signal pattern, we adopted the curve-fitting partially and statistical method. The ECG signal is divided into small parts based on QRS complex, and then, each part is approximated to the polynomials. Comparing the approximated ECG pattern with some kinds of heart disease ECG pattern, we can detect and classify the kind of heart disease.

바이너리 심전도 파일 포맷의 효율적인 디스크립션을 위한 시각적 도구 설계 (Design of Visual Tool for Efficient Descripting of Binary ECG File Formats)

  • 본 연구에서는 BED(Binary ECG Description)기반의 바이너리 심전도(ECG) 변환시스템은 ECG 데이터의 상호연동성을 지원하기 위해 바이너리 ECG 파일을 XML기반의 HL7 aECG로 변환하는 시스템이다. 그러나, BED 시스템을 처음 사용하는 초보자는 XML을 사용하여 BED 문서를 작성하기 어렵다. 본 논문에서는 초보자들이 BED 문서를 쉽게 작성할 수 있도록 지원하는 시각적 도구인 BED 스튜디오를 구현한다. 이 시스템은 세부분, 즉 비주얼 편집기, XML트리 편집기, 그리고 포맷검증 뷰어로 구성된다. 포맷검증뷰어는 사용자가 비주얼 인터페이스를 사용하여 작성한 BED 문서를 바이너리 ECG 파일과의 포맷 매칭을 시각적으로 검사할 수 있도록 지원하여 BED 문서 작성시 발생할 수 있는 오류 가능성을 감소시켰다.

일상생활 중 건강모니터링을 위한 착용형 심전도계측 시스템 개발 (Development of the wearable ECG measurement system for health monitoring during daily life)

  • In this study, wearable ECG measurement system was implemented for health monitoring during daily life. A wearable belt-type ECG electrode worn around the chest by measuring the real-time ECG is produced in order to minimize the inconvenience in wearing. The measured ECG signal is transmitted via an ultra low power consumption wireless data communications unit to personal computer using Zigbee-compatible wireless sensor node. The ECG monitoring program is developed at end user which is personal computer. The measured ECG contains many noises mainly due to motion artifacts. For ECG signal processing, adaptive filtering process is proposed which can reduce motion artifacts efficiently and accurately than digital filter. The experimental results show that a reliable performance with high quality ECG signal can be achieved using this wearable ECG monitoring system.

스마트폰과 연동한 휴대용 심전도계 (A Portable ECG System Coupled with a Smartphone)

  • The electrocardiogram(ECG) and heart rates are essential for diagnosing heart disease. However, conventional portable ECG devices are possible to only measure heart rates or have limitations in how and where they are measured. In this paper, a portable ECG system in which ECG waveforms and heart rates are displayed on smartphone screens is developed. A smartphone is used as display equipment instead of a computer screen for continuous monitoring. The developed ECG system filters and amplifies detected analog ECG signals. Next, it converts the amplified analog ECG signals into digital signals, then transmits to the smartphone via Bluetooth communication. This ECG system can display and store biomedical signals on a smartphone through the application. As a result, the waveform and heart rates of the developed portable ECG system has been confirmed to be similar to those of existing medical devices.
