발열성 소아 요로감염에서 Extended-Spectrum ${\beta}$-Lactamase 생성 $Escherichia$ $coli$의 임상적 의의 (Clinical Significance of Extended-Spectrum ${\beta}$-Lactamase Producing $Escherichia$ $coli$ in Pediatric Patients with Febrile Urinary Tract Infection)

  • 목적: 광범위 베타락탐 분해 효소(extended-spectrum ${\beta}$-lactamase, ESBL) 생성 $E.coli$에 의한 지역 사회 요로감염의 빈도가 전세계적으로 증가하고 있다. ESBL은 광범위 cephalosporin 계 항생제와 monobactam 계 항생제를 포함한 다양한 새로운 ${\beta}$-lactam 계 항생제에 대하여 내성을 일으킨다. 본 연구에서는 발열성 소아 요로감염에서 ESBL(+) $E.coli$의 감염 여부가 요로감염의 중증도, 동반 비뇨기계 기형과 연관이 있는지 알아보고자 하였다. 방법: 2008년 1월부터 2010년 10월까지 고려대학교 병원에서 입원 치료한 소아 중 $E.coli$에 의한 발열성 요로감염 소아 290명을 대상으로 하였다. 요 배양검사에서 항생제 감수성 검사 및 ESBL 생성 여부에 따라서 ESBL(+) $E.coli$ 군과 ESBL(-) $E.coli$ 군으로 나누어 입원 전후 발열 기간, 입원 기간, 말초 혈액 내 백혈구 수와 혈청 C-반응성 단백, 농뇨의 동반 여부, 수신증의 유무, 신 스캔에서 초기 신 결손의 유무, 신 반흔의 유무, 방광 요관 역류의 유무 등의 항목들에 대하여 두 군간에 후향적 비교 분석을 시행하였다. 결과: 대상 소아 총 290명 중 ESBL(+) $E.coli$ 군은 32명, ESBL(-) $E.coli$ 군은 258명이었다. 무균 채뇨백으로 검사를 진행한 소아는 56명이었으며, 이를 제외한 소아는 총 234명으로 ESBL(+) $E.coli$ 군은 22명, ESBL(-) $E.coli$ 군은 212명이었다. 무균 채뇨백으로 검사를 진행한 소아를 포함했을 때와 제외했을 때 모두 두 군간에 통계학적으로 유의한 차이를 보이는 항목은 없었다. 결론: 저자들은 발열성 소아 요로감염에서 ESBL 생성 $E.coli$ 의 감염 여부가 요로감염의 중증도 및 동반 비뇨기계 기형과의 관련이 없음을 제시하는 바이다.

소아설사 환아에서의 Escherichia coli O157 분리 (Isolation of Escherichia coli O157 in Children with Diarrhea)

  • 목 적 : Escherichia coli (E. coli) O157은 설사, 출혈성대장염 등을 일으키며, 드물게 용혈성 요독증후군까지 일으키는 세균이다. 미국이나 유럽에서는 세균성설사의 2~3번째로 흔한 원인균이다. 우리나라는 이 세균감염이 거의 없는것으로 알려져 왔으나, 우리나라도 점차 햄버거와 같은 간이음식의 섭취가 늘어나, 이 세균감염이 생겼을 가능성이 있을 것으로 사료되어, 향후 이 세균에 대한 통상적인 변배양 검사의 필요성 여부를 알아보고자 하였다. 방 법 : 1996년 3월부터 1997년 2월까지 6개월 이상된 소아설사 환아의 변배양 검체를 대상으로 하였다. Sorbitol-MacConkey 한천에 배양한 후 형성된 집락 중 무색 집락을 대상으로 E. coli로 동정된 것을 E. coli O157 라텍스 응집검사를 실시하여 응집이 되면 양성으로 판독하였다. 결 과 : 총 317 검체중 11월에 1 검체 (0.3%)에서 Shiga 독소를 생성하지 않는 E. coli O157:NM이 분리되었다. 7세 남아로 2일간의 복통과 1일간의 설사 및 구토를 주소로 입원 한 후 특별한 치료없이 2일후 증상이 호전되어 퇴원하였다. 결 론 : 검사된 모든 대변 검체중 한 검체 (0.3%)만이 E. coli O157이 분리되었으나 Shiga 독소를 생성하지 않는 군주였다. 따라서 이 세균의 분리를 위한 통상적인 변배양 검사는 아직 불필요한 것으로 사료되었다.

요로감염의 주요 원인균과 항생제 감수성의 변화에 관한 고찰 (Changes in Causative Organisms and Antimicrobial Susceptibility of the Urinary Tract Infection)

  • 요로감염은 병원 내 감염의 주요한 부분이며 최근 항생제의 남용 등으로 인해 요로감염에서의 효과적인 항생제의 선택이 제한받고 있다. 연구자들은 본 병원을 방문하여 시행한 요배양검사에서 의미있는 균이 동정된 외래 및 입원 환자의 요로감염의 주요 원인균과 항생제 감수성을 조사하여 요로감염의 치료에 도움을 주고자 하였다. 2010년 1월 1일부터 2015년 12월 31일까지 6년간 본 병원에서 시행한 요배양검사에서 의미 있는 원인균이 동정된 외래 및 입원환자를 대상으로 하였으며 균집락 단위(colony forming unit: CFU)가 $10^5/ml$이상인 환자를 대상으로 하였다. 배양된 동정균주에서 가장 흔한 원인균은 E.coli로 28.1%의 비율이었다. E.coli의 경우 남성에서는 2010년부터 2015년까지 빈도의 차이는 없었으나 여성의 경우 2014년 이후 그 빈도가 점차 감소하였다. 항생제 감수성 결과 가장 흔한 균주인 E.coli의 감수성 검사에서는 ampicillin 72.2%, TMP/SMX (trimethoprim/sulfamethoxazole) 44.9%, ciprofloxacin 41.3%의 내성율을 보였으며, cefazolin 12.4%, 2,3,4세대 cephalosporin에는 5% 이하의 낮은 내성율을 보였다. 요로감염의 원인균은 시간이 지날수록 다양해지고 빈도 또한 변화를 보인다. 따라서 TMP/SMX와 ciprofloxacin 등 기존 항생제에 대한 내성도 또한 비교적 높은 것으로 나타나 향후 요로감염에 대한 경험적인 항생제의 사용에 대해 더욱 신중한 선택을 해야 할 것으로 생각한다.

실험적 감전 Stress 가 Escherichia coli 감염마우스의 기병에 미치는 영향 (Effect of Experimental Electrization Stress on the Pathogenesis of Escherichia coli in Mice)

  • A hypothesis that is a stress condition in animal may cause either enhancement or reduction of the host resistance against microbial infection was experimentally studied. Among of many processes for stress formation an experimental electrization in mice was devised, on the bases of blood picture analysis, and studied the effect of experimental electrization of mice on E. coll infection. The results obtained were as follows. 1. Electrization with ordinary current, A. C. 60 cps., on the path of symmetrical line of both posterior limbs at 20 to 100 volts (less than 10 mA) for 15 to 30 seconds was able to induce a stress reaction in blood pattern without showing any dangers of electrocution, electric burns and other residual signs, and no correlation between blood pattern of the reaction and an amount of current between 20 to 100 volts was observed. As the electrodes, two of 21 gauge hypodermic needles were used, when the electrization each of them were inserted into the center of toe tissue of the both legs. 2. Serum protein fractions following the experimental electrization showed a tendency of a low A/G ratio and a high value of ${\alpha}$-globulin. 3. In the studies on the effect of electrical stress on the pathogenesis of E. coli in mice, a group in which a simultaneous electrization and infection, and a group infected two hours after electrization showed 80 per cent mortality. On the other hand, infection after 20 hours electrization and control groups showed their mortality of 40 and 60 per cent respectively.

수경시설 물놀이에 따른 대장균 감염의 위해도 분석 (Risk Assessment of Escherichia coli Infection from Use of Interactive Waterscape Facilities)

  • Objectives: With the goal of quantifying the risk of children contracting gastroenteritis while playing at interactive waterscape facilities and evaluating the adequacy of current water quality regulations, risk assessment was performed with Escherichia coli as pathogen. Methods: Abundances of E. coli in the waters of interactive water features in South Korea were acquired from survey reports. A gamma distribution describing the volume of water swallowed by children during swimming activities was adopted. Exposure rate and risk were calculated by Monte Carlo simulation and dose-response models for various pathogenic E. coli. Results: E. coli was detected in 25 out of 40 facilities, with range of ~1,600 CFU/100 ml. The abundance fitted an exponential distribution. Simulated exposures ranged ${\sim}1.9{\times}10^{10}$ CFU, varying greater along E. coli abundance than the volume of water. Risk of children being infected by enterohemorrhagic E. coli was high, with range of ~0.85. When E. coli abundance was <200 CFU/100 ml, which is the current government threshold, the risk decreased to <0.43. Although the guideline successfully reduced the risk of adults being infected by a less virulent E. coli strains (<0.03), the risk for children could not be quantified due to lack of dose-response models for those pathogens for children. Conclusions: Under the current guideline, children are at risk of being infected if water is contaminated with by enterohemorrhagic E. coli. For other E. coli strains, the risk appears to be considerably less. The result warrants need for developing dose-response models for children for each pathogenic E. coli strain.

  • The antimicrobial effects of HAp and Ag-HAp was observed using periprosthetic infection bacteria such as Pseudomonas Aeruginosa, Staphylococcus Epidermidis, Escherichia coli (DH5$\alpha$). Ag-HAp showed good antimicrobial effects. TEM study of E. coli with and without Ag treatment in HAp was experimented in order to find the mechanism of Ag in antimicrobial effects. It was observed that the shape of Ag-treated E. coli was changed, the cells walls became inhomogeneous. The vaculoes at cytoplasm formed into E. coli and finally it was discovered by EDAX that there were many dark granules which contain the Ag element inside the cells.

Analysis of Uropathogens of Febrile Urinary Tract Infection in Infant and Relationship with Vesicoureteral Reflux

  • Purpose: This study aimed to investigate the relationship between uropathogens of infants with febrile urinary tract infection (UTI) and vesicoureteral reflux (VUR). Materials and Methods: We analyzed 308 infants hospitalized for febrile UTI between January 2010 and December 2015, and assessed the voiding cystourethrography (VCUG). The medical records, including clinical symptoms, laboratory findings, urinalysis, urine culture tests, ultrasound (US), dimercaptosuccinic acid scan, and VCUG, were retrospectively obtained. The incidences of VUR and high-grade VURs (III, IV, and V) were analyzed in 4 groups categorized by uropathogens and renal US findings. Results: The mean age of 308 infants was $3.29{\pm}2.18months$. The male-to-female ratio was 3.46:1. In urine culture tests, 267 infants (86.69%) showed single bacterial uropathogen; Escherichia coli in 241 infants (78.25%) and non-E. coli uropathogens in 26 infants (8.44%). Multiple distinctive microorganisms were identified as causative uropathogens in 41 infants (13.31%). Abnormal findings of US and VCUG were identified in 216 and 64 patients, respectively. In 308 infants, the incidences of VUR and high-grade VUR were not different among the 4 groups. In 239 male infants, the incidences of high-grade VUR were higher in patients with non-E. coli single or multiple uropathogen and with abnormal US findings (p=0.042). Conclusions: In male infants with non-E. coli uropathogen or multiple uropathogens and with abnormal US findings at febrile UTI, there was an increased chance of finding high-grade VURs on subsequent VCUG tests.

Escherichia coli-Derived Uracil Increases the Antibacterial Activity and Growth Rate of Lactobacillus plantarum

  • Lactobacillus plantarum (L. plantarum) is a representative probiotic. In particular, L. plantarum is the first commensal bacterium to colonize the intestine of infants. For this reason, the initial settlement of L. plantarum can play an important role in determining an infant's health as well as their eventual health status as an adult. In addition, L. plantarum combats pathogenic infections (such as Escherichia coli (E. coli), one of the early pathogenic colonizers in an unhealthy infant gut) by secreting antimicrobial substances. The aim of this research was to determine how L. plantarum combats E. coli infection and why it is a representative probiotic in the intestine. Consequently, this research observed that E. coli releases uracil. L. plantarum specifically recognizes E. coli-derived uracil, which increases the growth rate and production of antimicrobial substance of L. plantarum. In addition, through the inhibitory activity test, this study postulates that the antimicrobial substance is a protein and can be considered a bacteriocin-like substance. Therefore, this research assumes that L. plantarum exerts its antibacterial ability by recognizing E. coli and increasing its growth rate as a result, and this phenomenon could be one of the reasons for L. plantarum settling in the intestine of infants as a beneficial bacterium.

Genotyping of avian pathogenic Escherichia coli by DNA fragment analysis for the differences in simple sequence repeats

  • Avian pathogenic E. coli (APEC) causes severe economic losses in the poultry farms, due to systemic infections leading to lethal colisepticemia. It causes a variety of diseases from air sac infection to systemic spread leading to septicemia. Secondary infection contains opportunistic infections due to immunosuppression disease. Collibacillosis causes the great problems in the poultry industry in Korea. Thus, it is necessary to identify and classify the characteristics of E. coli isolate of chicken origin to confirm the diversity of symptoms and whether they are transmitted among the farms. Fragment analysis is identify the difference in the number of Variable-Number Tandem-Repeats (VNTRs) for genotyping. VNTRs have repeating structure (Microsatellite, Short tandem repeats; STR, Simple sequence repeats; SSR) in the chromosome. This region can be used as a genetic marker because of its high mutation rate. And various lengths of the amplified DNA fragment cause the difference in the number of repetition of the DNA specific site. The number of repetition sequences indicates the separated size of fragments, so the each fragments can be distinguished by specific samples. The results of the sample show that there is no difference in six microsatellite loci (yjiD, aidB, molR_1, ftsZ, b1668, yibA). There are differences among the farms in relation of the number of repetitions of other six microsatellite loci (ycgW, yaiN, yiaB, mhpR, b0829, caiF). Four (ycgW, yiaB, b0829, caiF) of these six microsatellite loci show statistically significant differences (P<0.05). It means that the analysis using four microsatellite loci including ycgW, yiaB, b0829, and caiF can confirm among the farms. Five E. coli samples in one farm have same SSR repetition at all markers. But, there are significant differences from other farms at Four (ycgW, yiaB, b0829, caiF) microsatellite loci. These results emphasize again that the four microsatellite loci makes a difference in the amplified DNA fragments, enabling it to be used for E. coli genotyping.

재래산양의 설사병 병원체 감염률 조사 (Status of diarrhea pathogens from Korean indigenous goat feces)

  • The purpose of this study was to survey on infection status of pathogens of diarrhea from Korean indigenous goat. A total of 800 fecal samples was collected from 50 farms from January to November 2016 and was tested by automatic biochemical machine and polymerase chain reaction (PCR). The overall infection rates of parasitic, bacterial and viral pathogens was 13.0%, 23.0%, 11.3% and the rates of coccidia, Escherichia coli (E. coli), bovine viral diarrhea virus (BVDV), rotavirus and coronavirus were 13.0%, 23.0%, 5.3%, 8.8% and 2.6%, respectively. In the rates of mixed detection, single was 29.8%, double 5.1%, triple 2.8%, quadruple 1.1% in each sample, respectively.
