운동실조형 마비성구음장애에 적용되는 지각적, 음향학적, 생리학적 도구에 관하여 - 환자사례를 중심으로 - (Perceptual, Acoustical, and Physiological Tools in Ataxic Dysarthria Management: A Case Report)

  • Among the various dysarthric subtypes, diagnosis of ataxic dysarthria is rendered when the speech characteristics include imprecise and irregular articulatory breakdowns, marked degree of speech rate impairment, overall monopitch and monoloudness, and respiratory-articulatory incoordination. Traditionally, speech pathologists have relied only upon their ‘ears’ to describe and evaluate the dysarthric speech. A statement of percentage of correct words identified by a listener do not provide so much more than an index of severity. Within the same perceptual dimension, a carefully constructed speech intelligibility test can specify patterns of errors. The patterns can contain a diagnostic value as well as Provide strategies for remediation. The phonetically transcribed texts on single words and a standard passage, 'kail' produced by an ataxic dysarthria are presented in this report, with an emphasis of the articulatory error analysis. Furthermore,, acoustic tools [e.g., spectrography to measure formant transitions, segment durations, consonant spectra, etc.] are utilized to serve as basic measures that objectively document patients' speech intelligibility, Finally, the treatment methods [e.g., spectrography as a visual feedback, gestural reorganization using pacing method, DAF (Delayed Auditory Feedback)] to modify the dysarthric behaviors are presented.

조음복잡성 및 조음중증도에 따른 마비말장애인의 자음정확도와 말명료도 (Effects of Phonetic Complexity and Articulatory Severity on Percentage of Correct Consonant and Speech Intelligibility in Adults with Dysarthria)

  • This study examined the effects of phonetic complexity and articulatory severity on Percentage of Correct Consonant (PCC) and speech intelligibility in adults with dysarthria. Speech samples of thirty-two words from APAC (Assessment of Phonology and Articulation of Children) were collected from 38 dysarthric speakers with one of two different levels of articulatory severities (mild or mild-moderate). A PCC and speech intelligibility score was calculated by the 4 levels of phonetic complexity. Two-way mixed ANOVA analysis revealed: (1) the group with mild severity showed significantly higher PCC and speech intelligibility scores than the mild-moderate articulatory severity group, (2) PCC at the phonetic complexity level 4 was significantly lower than those at the other levels and (3) an interaction effect of articulatory severity and phonetic complexity was observed only on the PCC. Pearson correlation analysis demonstrated the degree of correlation between PCC and speech intelligibility varied depending on the level of articulatory severity and phonetic complexity. The clinical implications of the findings were discussed.

  • People with somatoform disorder have a number of different symptoms that typically last for several years. Their symptoms can't be traced to a specific physical cause. In people with somatoform disorder, medical test results are either normal or don't explain the person's symptoms. The symptoms of somatoform disorder are similar to the symptoms of other illnesses. People with this disorder may have several medical evaluations and tests to be sure that they don't have another illness. In this case, we described a 56-year old man who was diagnosed as R/O) ALS, because he has many similar symptoms of ALS (amyotrophic lateral sclerosis) such as gait disturbance, dysarthria, dyspnea. But we diagnosed his case as somatoform disorder and treated with herbal medication, acupuncure treatment ,negative cupping and relaxation training. Through these treatment, the patient showed improvement of chief complains and accessory symptoms.

A Case Report of a Stroke Patient with Dysarthria Treated with Korean Medicine

  • This case study examined the effect of Korean Medicine on a 72-year old male with a habitual history of smoking and drinking, and a family history of hypertension who had suffered an acute stroke. A couple of months later he complained of upper extremity weakness and was diagnosed with dysarthria. The patient received Korean Medicine treatment of acupuncture and herbal medicine for 39 days at the Samse Oriental Hospital, in conjunction with physiotherapy and Western medicine. Articulation accuracy, vowel accuracy, alternation and speed of reading sentences were evaluated every 7days. The results showed that the patient had improvement of articulation (10%) and vowel (37%) accuracy, alternation (12%) and speed of reading sentences (28%) suggesting that Korean Medicine treatment contributed to this progress.

연결발화에서 마비말화자의 음질 특성 (Voice Quality of Dysarthric Speakers in Connected Speech)

  • This study investigated the perceptual and cepstral/spectral characteristics of phonation and their relationships in dysarthria in connected speech. Twenty-two participants were divided into two groups; the eleven dysarthric speakers were paired with matching age and gender healthy control participants. A perceptual evaluation was performed by three speech pathologists using the GRBAS scale to measure the cepstrual/spectral characteristics of phonation between the two groups' connected speech. Correlations showed dysarthric speakers scored significantly worse (with a higher rating) with severities in G (overall dysphonia grade), B (breathiness), and S (strain), while the smoothed prominence of the cepstral peak (CPPs) was significantly lower. The CPPs were significantly correlated with the perceptual ratings, including G, B, and S. The utility of CPPs is supported by its high relationship with perceptually rated dysphonia severity in dysarthric speakers. The receiver operating characteristic (ROC) analysis showed that the threshold of 5.08 dB for the CPPs achieved a good classification for dysarthria, with 63.6% sensitivity and the perfect specificity (100%). Those results indicate the CPPs reliably distinguished between healthy controls and dysarthric speakers. However, the CPP frequency (CPP F0) and low-high spectral ratio (L/H ratio) were not significantly different between the two groups.

중심성 뇌교 수초용해증 환자에 관한 한양방 복합치료 1례 (A Case Report of a Central Pontine Myelinolysis Patient Treated with Korean and Western Medicine)

  • Objectives : The purpose of this study is to report the clinical effect of Korean and Western medical treatment on a hemiplegic inpatient with central pontine myelinolysis(CPM). Methods : A patient who was diagnosed with central pontine myelinolysis(CPM) was treated with acupuncture, herbal medicine, Korean physical therapy, along with receiving continuous Western medication and rehabilitation treatment. The patient was evaluated through Manual Muscle Testing(MMT), measurement of grip strength, dysarthria, and dysuresia. The Korean and Western medical treatment was conducted for 4 weeks. Results : After the 4 weeks of Korean and Western medical treatment, MMT grade improved from 2 to 4 and grip strength improved from 6 to 13. Dysarthria and dysuresia also improved. Conclusion : According to these results, this report suggests that Korean and Western medical treatment could be effective in the treatment of central pontine myelinolysis(CPM) patients.

청심연자탕으로 호전된 올리브교소뇌위축증 환자 치험 1례 (A Case Report of a Patient with Olivopontocerebellar Atrophy Improved with Chengsimyeonja-tang)

  • Objectives This study aimed to report significant improvement of dizziness, gait disturbance and dysarthria after treatment with Sasang constitutional medicine treatment on a Taeeumin patient with olivopontocerebellar atrophy. Methods A 53-year-old female diagnosed olivopontocerebellar atrophy about one year ago suffered from dizziness, gait disturbance and dysarthria. The patient was identified as Taeeumin Joyeol pattern and treated with Cheongsimyeonja-tang and Taegeuk acupuncture. The patient's main symptoms were observed using Global Assessment Scale (GAS) during the treatment period. In addition, the Unified Multiple System Atrophy Rating Scale (UMSARS) was used to assess the overall function of patient. Results We observed a significant improvement in symptoms of dizziness, gait disturbance and dysarthria using GAS, and UMSARS score decreased in Part I and II. Conclusions This case showed that Sasang constitutional medicine treatment can be effective treatment for Taeeumin patient with olivopontocerebellar atrophy. We consider that consistent treatment can contribute to improve the patient's quality of life.

스마트 기기를 활용한 구강안면 훈련 영상 프로그램이 마비말장애가 있는 급성 뇌졸중 환자의 구강 구조 및 기능과 조음교대운동속도에 미치는 효과 (The Effects of Orofacial Training Video Program using Smart Device on Oral Cavity Structure and Function, Diadochokinetic Rate in Acute Stroke Patients with Dysarthria)

  • 본 연구의 목적은 스마트 기기를 활용한 구강안면 훈련 영상 프로그램이 마비말장애가 있는 급성 뇌졸중 환자의 구강 구조 및 기능과 조음교대운동속도에 미치는 효과를 알고자 하였다. 본 연구는 마비말장애가 있는 급성 뇌졸중 환자 14명을 대상으로 하였다. 연구에 참가한 모든 대상자들은 실험군과 대조군으로 7명씩 무작위 할당되었다. 두 그룹의 대상자들은 4주 동안 모두 동일하게 보편적인 재활치료를 받았다. 실험군은 스마트 기기를을 활용한 구강안면 훈련 영상 프로그램을 보호자의 감독 하에 추가적으로 30분씩 더 수행하였다. 구강 구조와 기능을 측정하기 위하여 조음기관 구조 및 기능선별검사의 하위항목인 구강 구조와 기능을 평가하였다. 조음교대운동속도를 측정하기 위하여 교대운동속도와 연속운동속도를 평가하였다. 연구 결과, 그룹 내 비교에서 실험군과 대조군은 구강 구조 및 기능, 교대운동속도, 연속운동속도에서 유의한 향상을 보였다(p<.05). 두 그룹 간 변화량 비교에서 실험군은 대조군보다 구강기능과 교대운동속도의 /퍼/와 /터/에서 유의한 향상을 보였다(p<.05). 스마트 기기를 활용한 구강안면 훈련 영상 프로그램은 마비말장애가 있는 급성 뇌졸중 환자의 구강 기능과 조음기관 기능에 긍정적인 효과를 기대할 수 있는 중재방법으로 사료된다.

마비말장애 성인의 자음정확도와 비음치가 말명료도 및 말용인도에 미치는 영향 (Effect of Percentage of Correct Consonants and Nasalance Score on the Speech Intelligibility and Acceptability in Adults with Dysarthria)

  • 본 연구는 표준문단 읽기 과제를 사용하여 얻어진 동일한 말샘플을 통하여 마비말장애 성인의 자음정확도와 비음치가 말명료도 및 말용인도에 미치는 직접적인 영향을 알아보는 것을 목적으로 하였다. 연구 대상은 마비말장애 성인 10명과 일반 성인 16명이었다. 비음치 측정을 위한 과제로는 모음 연장, 구강음과 비강음 문장, 표준문단 읽기를 사용하였으며, 동일한 표준문단을 사용하여 자음정확도를 측정한 후, 평정자 20명에게 시각적 아날로그 척도를 사용하여 말명료도, 말용인도를 평정하게 하였다. 연구 결과는 다음과 같다. 첫째, 구강음과 비강음 문장, 표준문단에서는 마비말장애 성인 집단의 비음치가 일반 성인 집단에 비해 유의하게 높게 나타났으나, 모음연장에서는 유의한 차이를 보이지 않았다. 둘째, 마비말장애 성인의 자음정확도는 말명료도 및 말용인도와 유의한 상관을 보였으나, 비음치는 말명료도 및 말용인도와 유의한 상관을 보이지 않았다. 자음정확도를 통제한 상황에서도 비음치는 말명료도 및 말용인도와 유의한 상관을 보이지 않았다. 셋째, 회귀분석 결과 마비말장애 성인의 자음정확도가 말명료도와 말용인도에 미치는 영향이 유의한 것으로 나타났다. 본 연구의 결과로부터 마비말장애 성인의 자음정확도는 말명료도, 말용인도와 밀접한 관련이 있으나, 비음치는 말명료도 및 말용인도와의 상관이 유의하지 않음을 알 수 있다.

The Effects of a Massage and Oro-facial Exercise Program on Spastic Dysarthrics' Lip Muscle Function

  • This study was to determine the effects of a massage and oro-facial exercise program on spastic dysarthric patients' lip muscle function using an electromyogram (EMG). Three subjects with Spastic Dysarthria participated in the study. The surface electrodes were positioned on the Levator Labii Superior Muscle (LLSM), Depressor Labii Inferior Muscle (DLIM), and Orbicularis Oris Muscle (OOM). To examine lip muscle function improvement, the EMG signals were analyzed in terms of RMS (Root Mean Square) values and Median Frequency. In addition, the diadochokinetic movements and the rate of sentence reading were measured. The results revealed that the RMS values were decreased and the Median Frequency moved to a high frequency area. Diadochokinesis and sentence reading rates were improved.
