소프트웨어를 이용한 마비말장애 화자의 일련운동속도 분석 (Analysis of sequential motion rate in dysarthric speakers using a software)

  • Purpose: The primary goal of this study was to discover whether the articulatory diadochokinesis (sequential motionrate, SMR) collected using the Motor Speech Disorder Assessment (MSDA) software module can diagnose dysarthria and determine its severity. Methods: Two subject groups, one with spastic dysarthria (n=26) and a control group of speakers (n=30) without neurological disease, were set up. From both groups, the SMR was collected by MSDA at a time, and then analyzed using descriptive statistics. Results: For the parameters of syllable rate, jitter, and the mean syllable length (MSL) at the front and back, the control group displayed better results than the dysarthria patients. Conclusions: At the level of articulatory diadochokinesis, the results showed that the use of MSDA software in clinical practice was generally suitable for quickly recording the parameters of syllable rate, jitter, and mean syllable length.

구음장애 환자용 영우글자판 개발 (Development of Youngwoo Keyboard for Dysarthria Patients)

  • 본 논문은 구음장애 환자들이 겪고 있는 말하기 장애를 돕기 위한 영우글자판을 개발하였다. 기존의 구음장애 환자가 의사소통을 하는 방법은 자음과 모음이 적힌 글자판과 안구 마우스 및 의사소통 보조 애플리케이션이 있으나 한 문장당 5분 이상이 시간이 지체되어 소통에 많은 어려움이 있었다. 하지만 기존의 방법과 달리, 제안하는 방법은 구음장애 환자가 안구의 움직임으로 문장을 완성할 수 있도록 하여 의사소통을 할 수 있도록 개선하였다. 본 논문의 결론 및 향후 연구과제에서는 영우글자판을 위한 교육적 활용 가치를 분석하였으며, 이 훈련이 루게릭 환자가 있는 가족들에게 큰 도움을 줄 수 있다는 것을 보여주었다. 본 연구의 기대효과는 구음장애 환자들이 갖고 있는 의사소통을 원활히 함으로써 가족의 애로사항을 개선할 수 있을 것으로 판단된다.

Acoustic analysis of fricatives in dysarthric speakers with cerebral palsy

  • This study acoustically examines the quality of fricatives produced by ten dysarthric speakers with cerebral palsy. Previous similar studies tend to focus only on sibilants, but to obtain a better understanding of how dysarthria affects fricatives we selected a range of samples with different places of articulation and voicing. The Universal Access (UA) Speech database was used to select thirteen words beginning with one of the English fricatives (/f/, /v/, /s/, /z/, /ʃ/, /ð/). The following four measurements were taken for both dysarthric and healthy speakers: phoneme duration, mean spectral peak, variance and skewness. Results show that even speakers with mild dysarthria have significantly longer fricatives and a lower mean spectral peak than healthy speakers. Furthermore, mean spectral peak and variance showed significant group effects for both healthy and dysarthric speakers. Mean spectral peak and variance was also useful for discriminating several places of articulation for both groups. Lastly, spectral measurements displayed important group differences when taking severity into account. These findings show that in general there is a degradation in the production of fricatives for dysarthric speakers, but difference will depend on the severity of dysarthria along with the type of measurement taken.

  • 1. Objective This case study reports a Taeeumin patient experiencing hand tremor, heat flash, and mild dysarthria who improved with Yeoldahansotang medication. 2. Method The patient's subjective and objective symptoms were observed daily, and the VAS scores for the main symptoms (hand tremor, heat flash, and mild dysarthria) were recorded daily throughout hospitalization period. 3. Result The patient's general conditions improved by the end of hospitalization, and VAS score dropped dramatically from VAS 10, VAS 10, VAS 10 to VAS 0, VAS 0.5, VAS 3 for hand tremor, heat flash, and mild dysarthria, respectively. 4. Conclusion The patient responded well to Yeoldahansotang, one of the main medicaments for the Joyeol (dry-heat) symptomatic pattern, which indicates that Yeoldahansotang can be used to treat the Joyeol symptomatic pattern even when the textbook indications are not evident as long as the Joyeol pathology is recognizable from other symptoms.

뇌성마비로 인한 마비말장애 성인의 자음 오류 분석 (Consonant Confusions Matrices in Adults with Dysarthria Associated with Cerebral Palsy)

  • The aim of this study was to analyze consonant articulation errors produced by 90 speakers with cerebral palsy (CP). Phonetic transcriptions were made for 37 single-word utterances containing 70 phonemes: 48 initial consonants and 22 final consonants. Errors of substitution, omission, and distortion were analyzed using a confusion matrix paradigm showing the visualization of error patterns. Results showed that substitution errors in initial and final consonants were most frequent, followed by omission and distortion. Consonant omission occurred more frequently on final consonants. In both initial and final consonants, the within-place errors were more prominent than the within-manner errors. The current results suggest that consonant confusion matrices for dysarthric speech may provide useful information for evaluating speech intelligibility and developing automatic speech recognition system of adults with CP associated dysarthria.

특발성 단독설하신경마비 1례 (A Case of Idiopathic Isolated Hypoglossal Nerve Palsy)

  • Isolated hypoglossal nerve palsy is a rare clinical condition and it causes deviation of the tongue and dysarthria. A 50-year-old female presented with tongue deviation to the left and mild dysarthria. She had no remarkable past medical history except several recent upper respiratory infections. On examination, the other cranial nerves were intact and she had no focal neurological signs. The findings of MRI and MR angiography were normal. Cerebrospinal fluid analysis revealed only mild elevation of protein. We diagnosed her as suffering with idiopathic isolated hypoglossal nerver palsy and we administered steroid therapy. The dysarthria was improved, but the tongue deviation still remained at 50 days after onset. We report here on a rare case of idiopathic isolated hypoglossal nerve palsy.

이완성 구음마비 환자의 운율적 특성 연구 (Some Prosodic Characteristics of Flaccid Dysarthria)

  • In the previous studies, some characteristics of flaccid dysarthria patients have been studied mainly in two aspects: their difficulties in articulation and their metrical dysfunction. Therapeutic research on the articulation impediment of the patients have been carried out extensively (Yorkston, 1981). However, their phonetic characteristics have been less well-studied. The aim of this paper is to measure and describe some phonetic differences between the normal speaker group (six speakers) and the flaccid dysarthria patient group (six speakers in three different degreed of severity). Two types of short sentences comprising of subject-object-verb, i.e. declarative and yes-no question sentences, were recorded to investigate some phonetic characteristics of these two groups of speakers. The two groups (normal group vs. patient group) show differences in yes-no question boundary tone (H% vs. HL%), pitch range (wide vs. narrow), duration (short vs. long) and intensity (strong vs. weak) of sentence final verb endings in Korean.

CPAP 치료 프로그램이 이완형 마비성구어장애자의 과대비성 개선에 미치는 효과 (The Effects of CPAP Therapy on Hypernasality in a Flaccid Dysarthric Patient: A Case Study)

  • This study aimed at investigating the effects of CPAP therapy on hypernasality in flaccid dysarthria. The subject was a flaccid dysarthric patient with hypernasality. An A-B-A experimental research design was used. The therapy program was conducted 16 sessions. During CPAP therapy, the subjects was required to repeat single-word utterances in the form VNCV. The results showed some improvement in decreasing hypernasality.

  • Objective: The purpose of this clinical study is to report the effect of Banhahoobak-tang in the case of a 57-year-old woman with multiple cerebral infarction causing dysphagia and dysarthria diagnosed as pseudobulbar palsy. Methods: A patient was treated with Korean medicine, including herbal medication, acupuncture, and language therapy. We focused on the effect of Banhahoobak-tang by evaluating improvements in symptoms by examining times of aspiration and changes in Speech Mechanism Screening Test scores. Results: After three months of Korean medicine treatment and language therapy, there was improvement in the patient's symptoms, including dysphagia and dysarthria. Conclusions: This study suggests that Korean medicine treatment could be effective for improving the symptoms of pseudobulbar palsy in patients. Specifically, herbal medicine (Banhahoobak-tang) could be an effective choice for treating dysphagia.

A Case Report on the Use of Korean Medicine Treatment for a Patient with Dysarthria Caused by Progressive Bulbar Palsy (PBP)

  • Objectives : To introduce a rare case of a patient with Progressive Bulbar Palsy (PBP) and suggest the possibility of treatment using electroacupuncture and Korean Medicine. Methods : A 61-year-old man with PBP, complaining of dysarthria, was treated with electroacupuncture and Korean Medicine, from June 16 to July 10, 2015. Improvements in symptoms were measured using the speech mechanism screening test (SMST), measurement of tongue and orbicularis oris motility, and speech handicap index (SHI). Results : The scores of SMST, motility of tongue and orbicularis oris showed a tendency for gradual improvement with 25 days of Korean Medicine Treatment after admission, but conversation was still impossible. In SHI scores, one point increased in the speech domain and one point decreased in the psycho-social domain, and three points increased in the other domain. Conclusion : Electroacupuncture and Korean Medicine Treatment improved dysarthria caused by PBP, but not completely. Korean Medicine Treatment seems effective in the management of accompanying symptoms such as black hairy tongue, dry mouth, and general condition. The symptoms of PBP are similar to those of amyotrophic lateral sclerosis and there is controversy regarding the classification of PBP. The most important aspect of treating a patient with PBP is an early diagnosis and devising appropriate rehabilitation strategies.
