A Study on Prescription Intention of Botanical Drug in Korean Medicine Doctors (한의사들의 천연물 신약 처방의사에 관한 연구)

  • Objectives : The purpose of this study was to examine the prescription intention of Korean medicine doctors on botanical drug.Methods : The subjects in this study were 340 Korean medicine doctors in Korean medical institutions, on whom a survey was conducted from July 1 to August 31, 2014.Results : The factors that affected the prescription decision making of the Korean medicine doctors were drug superiority, level of Interest, Recognition, marketing of sales associates of pharmaceutical companies and satisfaction with information provided by those companies. When the internal consistency of the variables was measured, that was above 0.8.Conclusions : Accordingly, well-planned education and promotion efforts are required to encourage Korean medicine doctors to put more prescription intention in botanical drug. This study had some limitations : First, the subjects were selected in a manner to allow for the convenience of this researcher, and the findings might not be generalizable. Second, there was a problem with the time for the study in that there were quite fierce disputes on botanical drug at that time. Third, the majority of the subjects run their own hospitals, and the number of them was small. So it's not possible for them to represent every Korean medicine doctor.

Biotin-Conjugated Block Copolymeric Nanoparticles as Tumor-Targeted Drug Delivery Systems

  • To achieve targeted drug delivery for chemotherapy, a ligand-mediated nanoparticulate drug carrier was designed, which could identity a specific receptor on the surfaces of tumor cells. Biodegradable poly(ethylene oxide)/poly$({\varepsilon}-caprolactone)$ (PEG/PCL) amphiphilic block copolymers coupled to biotin ligands were synthesized with a variety of PEG/PCL compositions. Block copolymeric nanoparticles harboring the anticancer drug paclitaxel were prepared via micelle formation in aqueous solution. The size of the biotin-conjugated PEG/PCL nanoparticles was determined by light scattering measurements to be 88-118 nm, depending on the molecular weight of the block copolymer, and remained less than 120 nm even after paclitaxel loading. From an in vitro release study, biotin-conjugated PEG/PCL nanoparticles containing paclitaxel evidenced sustained release profiles of the drug with no initial burst effect. The biotin-conjugated PEG/PCL block copolymer itself evidenced no significant adverse effects on cell viability at $0.005-1.0{\mu}g/mL$ of nanoparticle suspension regardless of cell type (normal human fibroblasts and HeLa cells). However, biotin-conjugated PEG/PCL harboring paclitaxel evidenced a much higher cytotoxicity for cancer cells than was observed in the PEG/PCL nanoparticles without the biotin group. These results showed that the biotin-conjugated nanoparticles could improve the selective delivery of paclitaxel into cancer cells via interactions with over-expressed biotin receptors on the surfaces of cancer cells.

Awareness of Adverse Drug Reaction Reporting System in General Population (일반인에서의 의약품 부작용보고제도 인식도)

  • Background: Safety of drugs has become a major issue in public healthcare. Spontaneous reporting of adverse drug reaction (ADR) is the cornerstone in management of drug safety. We aimed to investigate the awareness and knowledge of spontaneous ADR reporting in general public of Korea. Methods: A total of 1,500 study subjects aged 19-69 years were interviewed with a questionnaire for their awareness and knowledge related to spontaneous ADR reporting. Computer assisted telephone interview was performed from 27th February 2013 to 4th March 2013. Target population was selected with quota sampling, using age, sex, and residence area. Healthcare professionals such as physicians, pharmacists, and nurses were excluded. The survey questions included awareness of spontaneous ADR reporting, opinions on ways to activate ADR reporting, and sociodemographic characteristics. Results: Overall awareness of spontaneous ADR reporting system was 8.3% (${\pm}2.53%$) among general population of Korea. Major source from which people got the information regarding ADR reporting was television/radio (69.9%), followed by internet (19.3%), and poster/brochure (6.1%). Awareness level differed between age groups (p<0.0001) and education levels (p<0.0001). Upon learning about the ADR reporting system, 88.5% of study subjects agreed on the necessity of ADR reporting system, while 46.6% thought promotion through internet and mass media as an effective way to activate ADR reporting. Conclusion: The overall awareness of spontaneous ADR reporting should be enhanced in order to establish a firm national system for drug safety. Adequate promotions should be performed targeting lower awareness groups, as well as various publicity activities via effective channels for the general population.

The Effects of Prevention Programs on Drug Use in Korean Adolescents (청소년 약물남용 예방교육 프로그램의 효과)

  • Background and purpose : Drug abuse is one of the most serious problems among the Korean adolescents today and has been associated with delinquent behavior in adolescents. The number of adolescents who abuse drugs is increasing yearly. Solutions to the problem, however, have not been well developed. The purpose of this study was to determine the effectiveness of a drug use prevention program which was developed by researchers. The program was a modified DARE(Drug Abuse Resistance Education) program for Korean adolescents. Methods: This study used an experimental, one-group pretest-posttest design with a convenience sample of 122 middle school students who were at one middle school located in Seoul. Data were collected using a self-administered questionnaire where ‘knowledge and attitude about drug use’, ‘social skill’, and ‘self-esteem’ were measured. The program had 12 sessions to improve knowledge and attitudes towards drugs, decision making skills, social resistance skills, management of stress and self-esteem of students. Teaching strategies for each session were varied lectures, group discussion. role-playing, question-and-answer sessions and audiovisual materials. Paired t-test was used to analyze the differences between the pre- and post-test scores on knowledge of and attitudes towards drug use, social skills, and self-esteem. Results : There were significant differences in knowledge and attitudes about drug use between pretest and post-test (p<.05), but no significant differences in social skill and self-esteem(p>.05). Conclusion: Our results showed that the drug use prevention program was not effective in promoting social-skill and self-esteem of Korean adolescents. These findings might be due to the respondents being in the period of early adolescence (puberty) and not being good at group discussion. So, further study is needed to develop a drug use prevention program appropriate to students 9 and 10 years old, and to encourge small group discussion.

A Study on Drug awareness information, Usage and Misusage on Elderly Inpatients of Veterans Hospital (보훈병원입원노인환자의 약물정보인식, 약물사용 및 오용실태에 관한 연구)

  • This study is descriptive research, attempts to determine the status of elderly inpatients of veterans hospital' of drug awareness information, drug use status and drug Misusage. The subjects were 220 patients of aged 65 and over only five metropolitan areas nationwide in drug use in the Veterans Hospital, national merit, the study is from May to June. SPSS Win 18.0 program was used as the data analysis and a statistical statement by mean, standard deviation, t-test and ANOVA, and multiple regression analysis. As a results, there was showed a statistically significant level 'age', 'type of marriage', 'education', 'triage', 'health status' as the general characteristics of the subjects. It showed the 'age' of the variables that influence recognition in the area of drug awareness information, and drug Misusage and drug use status factors seemed to affect the 'health status'. Therefore, It need for a comparative study of elderly inpatients and inpatients of veterans hospital, the research should be qualitative research of veterans medical services to the on reasonable support measures in the Veterans Affairs health care system and national merit.

  • Many scientists have been interested in drug delivery system (DDS) which improves medical treatment for curing a disease. Transdermal drug delivery (TDD) that is one of the DDS offers several advantages over the traditional methods. For this reason, the study of TDD has been investigated in various field. In this paper, principle of transdermal delivery and penetration enhancers into the skin including in vitro and in vivo data have been studied.

The Development and Effects of WEB Instruction Programs for Drug Abuse Prevention in Korean Adolescents (청소년의 약물남용예방을 위한 웹 활용 학습 프로그램 개발 및 효과)

  • The purpose of this study was to develop, through the integration of instructional theory, a Courseware and to investigate the effectiveness of a web-based computer assisted instruction(WBI) program for preventing drug abuse, a serious problem for youth problem. During the first stage of this study done "Drug Abuse Prevention" Courseware was developed based on, Gagn & Brigg's instructional design theory, Keller's ARCS theory and the CAI model of Hannafin & Peck. For the second stage, the courseware was used to provide education for students adolescents in drug abuse prevention. This study used an quasi-experimental, one-group pretest-posttest design with a convenience sample of 36 male high school students who were at one high school located in Seoul. Data were collected using self-reported questionnaires which included a learning achievement tool, the Keller's IMMS (Instructional Material Motivation Survey), on attitudes to drug use, and on responses to the WBI instruction. Prior to the experiment, the "drug abuse prevention" learning method and the procedures of the study were explained to the students, and then the learning achievement of the subjects was measured as a pretest. The students were then given 2 weeks WBI utilizing the courseware. A post-test which included the pre-test learning achievement questionnaire and a survey of learning motivation and attitudes toward drug were given two weeks after the education was completed. The data analysis was done using SPSS/PC. Paired t-test was used to analyze the differences between the pre-test and post-test scores for learning achievement. The results of the analysis are as follows: There were significant differences in learning achievement between the pre-test and post-test(t=-18.62, p=0.000). The hypothesis, that learning achievement will be higher, after the class has used the courseware, than before was supported. The scores for learning motivation and attitudes toward drugs were also higher than the results of existing studies. In conclusion, this study suggests that WBI is an effective learning method in the prevention of drug abuse for adolescents as it can be used for self-learning and repeated learning as assisted instruction. Recommendation would be given that further research needs to be develped in the courseware by cognitive learning style and by multimedia courseware and virtual reality system.

Status of Hypertensive Patients' Drug-taking in Health Center and Its related Factors (보건소 방문 고혈압환자의 약물복용 실태와 관련요인)

  • The purpose of this study is to show that, before registration of chronic degenerative diseases and enforcing management system in health centers, the management of hypertension could achieved comprehensively and continuously by knowing the realities of drug-taking and its related factors those who are being cared for in health centers. For one year, a questionnaire about drug-taking realities of hypertensive patient was distributed at 8 health centers in Taegu from March 24. 1997 to April 24. 1997. The questionnaires were then collected and analysed. The results are as follows: Of 691 patients, 77.4% of patient were taking medicine regularly every day. The reasons why patients weren't taking medicine every day was as follows: the blood pressure became normal(34%, the highest of the group), patients forgot(28.2%), it was difficult to buy the medicine(15.4%), there was no effect even though the patient took medicine continuously. The experience of medical treatment outside of health centers was 28.9%. The types and percentages of alternative medicine are herbs (50.5%), health food(24.5%), folk remedy(13.7%). Among them, 44.6% of patients didn't know if the medicine was effective. Medical treatment places excluding health centers was pharmacies(63.5%-the highest rate). In simple analysis, the older one was the more regularly one took the medicine, but there was no statistical relations. Of health activities, patients eating low-salt diet showed that they took the medicine regularly. Nonsmoking patients and those who experienced drug side-effect and those who doubted doctor's prescriptions and instructions showed that they took alternative, excluding modem medicine. Anti-hypertensive drug medication status according to recognition, attitude, experiences of hypertension showed that patients who thought that they should take anti-hypertensive drug during the whole life time took the medicine regularly(82.3%). The reasons for patients turning to alternatives varied. In case of having subjective symptom(34.1%), those who thought they knew the hypertension well(36.6%), they decided to use alternatives. In multiple logistic regression analysis the key statistics were as follows: The older patients, patients who ate low-salt diet, patients who thought that they should take anti-hypertensive drug during the whole life time, all showed that they took medicines regularly. And also patients who experienced drug side-effect, doubted prescriptions and instructions, and patients who had subjective symptoms, patients who thought that he knew the hypertension very well, all showed higher rate of using alternative medicine. In the future, at the health centers, to register and manage hypertension patients effectively, we must educate patients about regular drug-taking, and alternatives without doctor's prescriptions.
