Effect of Doxycycline on the Acute Lung Injury Induced by Gut Ischemia/Reperfusion (장의 재관류로 유도된 급성폐손상에서의 Doxycyclin의 효과)

  • Background : Phospholipase $A_2$ ($PLA_2$) has been known to be involved in the pathogenesis of acute lung injury (ALI) including ARDS. Since doxycycline has the property of inhibiting secretory group II $PLA_2$, the therapeutic effect of doxycycline hyclate was investigated for gut ischemia/reperfusion (I/R)-induced ALI in Sprague-Dawley rats. Methods : ALI was induced in Sprague-Dawley rats by clamping of the superior mesenteric artery for 60 min, followed by 120 min of reperfusion. To confirm the pathogenetic mechanisms of this ALI associated with neutrophilic oxidative stress, we measured bronchoalveolar lavage (BAL) protein content and lung MPO, and performed cyto-chemical electron microscopy for detection of free radicals, assay of $PLA_2$ activity and cytochrome-c reduction assay. Results : In gut I/R-induced ALI rats, protein leakage, pulmonary neutrophil accumulation, free radical production and lung $PLA_2$ activity were all increased. These effects were reversed by doxycycline hyclate. Conclusion : Doxycycline appears to be effective in ameliorating the gut I/R-induced ALI by inhibiting $PLA_2$, thereby decreasing the production of free radicals from neutrophils.

Studies on the Selective Media for Bifidobacterium infantis Maeil-K9 Using Various Carbon Sources and Antibiotics (Bifidobacterium infantis Maeil-K9 균주의 당 발효 특성 및 항생제 내성을 이용한 선택배지 개발연구)

  • To differentiate commercial bifidobacteia for Bifidobacterium lactis Bb-12, Bif. longum Bb-536 and Bif. infantis Maeil-K9, we studied the various carbon source, the nitrogen sources and antibiotics. Amygdalin and fructose were good candidates for carbon sources, and tryptone was suitable for nitrogen sources to design a new selective media for three commercial bifidobacteria. In the case of the amygdalin-containing medium as carbon sources, Bif. lactis Bb-12 and Bif. infantis Maeil-K9 showed good growth, and in fructose-containing medium, Bif. longum Bb-536 showed good growth. In antibiotics resistance study, the addition of 1 mg/L doxycyclin was very effective for differentiation of each bifidobacteria. Doxycyclin did not affect the growth of Bif. lactis Bb-12 and Bif. infantis Maeil-K9, but Bif. longum Bb-536 was completely inhibited by doxycyclin. Finally to confirm the selection capability of newly designed selective media, temperature-shocked bifidobacteria were cultured on them. As the results, fructose or doxycyclin containing medium showed for high growth for temperature-shocked bifidobacteria, but amygdalin containing medium showed low growth of temperature-shocked bifidobacteria.

Pleurodesis with Autologous Blood Plus Sclerosing Agents (자가혈과 늑막유착제를 이용한 늑막유착술 -치험 2례 보고-)

  • The patients with prolonged air leak after lung resection surgery were generally treated by pleurodesis with sclerosing agents such as talc, tetracyclin, doxycyclin, bleomycin, vibramycin, and OK432. However, for the case like dead space resulted by incomplete reexpansion of the remaining lung, chemical pleurodesis has shown to be not as effective as expected. If the patients keep the chest tube for long period of time, the risk of thoracic empyema would increase. Most thoracic surgeons have experienced prolonged airleak which developed after lung resection. Pleurodesis with autoblood was reported as an effective method in treatment of patients with prolonged airleak. The mechanism of blood pleurodesis may be direct obliteration of BPF and reduction of dead space by clot. Therefore we successfully treated the two patients with prolonged airleak using the autoblood plus OK432 or vibramycin.
