  • Major depressive disorder(MDD) is one of the most common diseases with serious health consequences such as increased morbidity, disability, and mortality. Electroconvulsive therapy(ECT) has been used as a treatment for mental disorder since the 1930s. A growing number of recent publications support the conclusions that ECT is an effective and safe treatment for depressed patients. Dosing strategies, frequency, safety, side effects and efficacy of ECT in MDD will be considered. ECT may be an alternative to treatment with antidepressants.

Treatment of Acute Renal Failure in Neonate (신생아 급성 신부전의 치료)

  • Acute renal failure (ARF) is common in the neonatal period, however, there are no uniform treatment strategies of ARF. The main treatment strategies are conservative management including medical treatment and the renal replacement therapy. Because ARF in the newborn is commonly acquired by hypoxic ischemic injury and toxic insults, removal of all the offending causes is important. Aminoglycoside, indomethacin, and amphotericin-B are the most common nephrotoxic drugs of ARF. To relieve the possible prerenal ARF, initial fluid challenge can be followed by diuretics. If there is no response, fluid restriction and correction of electrolyte imbalance should begin. Adequate nutritional support and drug dosing according to the pharmacokinetics of such drugs will be difficult problems. Renal replacement therapies may be provided by peritoneal dialysis, intermittent hemodialysis, or hemofiltration. New promising agents, bioartificial kidney, and stem cell will enable us to extend our therapeutic repertoire.

  • It is difficult to determine the feeding rate of coagulant in the water treatment process, due to nonlinearity, multivariables and slow response characteristics, etc. To deal with this difficulty, the genetic-fuzzy system was used in determining the feeding rate of the coagulant. The genetic algorithms are excellently robust in complex optimization problems. Since it uses randomized operators and searches for the best chromosome without auxiliary informations from a population consists of codings of parameter set. To apply this algorithms, we made the lookup table and membership function from the actual operation data of the water treatment process. We determined optimum dosages of coagulant(LAS) by the fuzzy operation, and compared it with the feeding rate of the actual operation data.

Antifertility Activity of Dictamnus albus Root Bark

  • The preliminary antifertility test of Dictamnus albus(Rutaceae) showed that a methanol extract of root bark decreased fertility in rats when administered orally on days $1{\sim}10$ postcoitum. Hexane, chloroform, butanol and aqueous fractions from the methanol extract, were tested to produce the result that only the hexane fraction exhibited positive activity. The hexane fraction was subfractionated into acidic, alkaloidal and neutral subtractions and tested. Neutral subtraction showed positive activity. From the neutral subtraction, fraxinellone was isolated as an active principle. The timed-dosing experiments revealed that antifertility activity of fraxinellone appeared to be exerted after tubal exit of the ova to prevent implantations.

  • Residue and recoveries of clindamycin were investigated by reversed-phase high performance liquid chromatography (RP-HPLC) which was infected for the control of streptcoccal infection in Anguilla japonica and Paralichthys olivaceusis. Detection limit was 0.1 ppm. Recoveries of clindamycin in muscles of flounder and eel were 80.4 and 78.8%, respectively. The clindamycin in eel and flounder was detected up to 13 and 15 days after dosing, respectively.

Contributing Factors on Pharmacokinetic Variability in Critically Ill Neonates (신생아중환자의 약동학적 다양성에 영향을 미치는 요인)

  • Neonates have large inter-individual variability in pharmacokinetic parameters of many drugs due to developmental differences. The aim of this study was to investigate the factors affecting the pharmacokinetic parameters of drugs, which are commonly used in critically ill neonates. Factors that reflect physiologic maturation such as gestational age, postnatal age, postconceptional age, birth weight, and current body weight were correlated with pharmacokinetic parameters in neonates, especially preterm infants. Comorbidity characteristics affecting pharmacokinetics in critically ill neonates were perinatal asphyxia, hypoxic ischemic encephalopathy, patent ductus arteriosus (PDA), and renal dysfunction. Administration of indomethacin or ibuprofen in neonates with PDA was associated with the reduced clearance of renally excreted drugs such as vancomycin and amikacin. Therapeutic hypothermia and extracoporeal membrane oxygenation were influencing factors on pharmacokinetic parameters in critically ill neonates. Dosing adjustment and careful monitoring according to the factors affecting pharmacokinetic variability is required for safe and effective pharmacotherapy in neonatal intensive care unit.

Toxic epidermal necrolysis induced by lamotrigine treatment in a child

  • Toxic epidermal necrolysis is an unpredictable and severe adverse drug reaction. In toxic epidermal necrolysis, epidermal damage appears to result from keratinocyte apoptosis. This condition is triggered by many factors, principally drugs such as antiepileptic medications, antibiotics (particularly sulfonamide), nonsteroidal anti-inflammatory drugs, allopurinol, and nevirapine. Lamotrigine has been reported potentially cause serious cutaneous reactions, and concomitant use of valproic acid with lamotrigine significantly increases this risk. We describe a case of an 11-year-old girl with tic and major depressive disorders who developed toxic epidermal necrolysis after treatment with lamotrigine, and who was diagnosed both clinically and pathologically. Children are more susceptible to lamotrigine-induced rash than adults, and risk of serious rash can be lessened by strict adherence to dosing guidelines. Unfortunately, in our case, the patient was administered a higher dose than the required regimen. Therefore, clinicians should strictly adhere to the dose regimen when using lamotrigine, especially in children.

  • The decomposition of hydrogen peroxide was used for supplementing the oxygen during batch xanthan fermentations in a bubble column bioreactor in order to escape the oxygen transfer limitation that occurred at the high viscosity of culture broths. The xanthan production, however, was inhibited reversibly by dosing hydrogen peroxide. On the other hand, fluidized particles of glass beads with 8 mm diameter led to high gas-liquid oxygen transfer rates in three-phase fluidized beds, which resulted in higher space-time yields of the xanthan production compared to in the bubble column bioreactors.

  • Fractional horse power single phase induction motors have no starting torque their own. So there are several ways of starting single phase induction motors. The most common type is the starting capacitor installed in series with the auxiliary winding to increase the starting torque. Also, the auxiliary winding is disconnected once the speed of the motor reaches 70 to 80 [%] of the rated speed. In the conventional systems, this function is conducted by a centrifugal switch. But the mechanical centrifugal switch has many problems such as switch malfunction. This paper presents the new methods of electronic starting switch by inductor to overcome these shortcomings of centrifugal switch.

Pharmaceutical Studies of Cefoperazone Phthalidyl Ester, a Novel Prodrug of Cefoperazone (세포페라존프탈리딜에스텔의 약제학적 연구)

  • A new cephalosporin derivative, cefoperazone phthalidyl ester, were synthesized and investigated in terms of dissolution and absorption properties. In comparison with cefoperazone, its phthalidyl ester showed the following characteristics. The mean dissolution time and variance of retention time were more significantly prolonged in simulated intestinal fluid than those in simulated gastric fluid. After a single oral dosing of both cefoperazone and its ester to rabbits, serum concentrations of cefoperazone were measured by bioassay, and the results showed that the ester exhibited much higher and more sustained blood level than the parent drug. The total area under the curve of cefoperazone phthalidyl ester were 10.8 times greater than that of cefoperazone.
