Analysis on the Decrease of Planning Target Volume in the Case of Lung Radiation Surgery with the Application of Respiratory Gated Radiotherpy Method (폐암 환자의 방사선수술 시 호흡연동 방사선치료 방법의 적용을 통한 계획용표적체적 감소 효과 분석)

  • The application of a respiratory gated radiotherpy method to the lung radiation surgery was evaluated compared with the conventional method in which the whole tumor motion range is considered in the delineation of PTV (Planning target volume). The four dimensional CT simulation images were acquired for the five NSCLC (Non-small cell lung cancer) patients for radiation surgery. The respiratory gated plan was prepared with the 50% phase CT images and the conventional method was planned based on the ITV (Internal target volume) which include all the target volumes created in each phase CT images within a whole respiratory period. The DVH (Dose volume histogram) of OAR (Organ at risk) which calculated in each method was compared for the evaluation of the plan properness. The relative decrease of OARs' DVH were verified in the application of respiratory gated method. The average decrease rate were $16.88{\pm}9.97%$ in the bronchus, $34.13{\pm}19.15%$ in the spinal cord, $28.42{\pm}18.49%$ in the chest wall and $32.48{\pm}16.66%$ in the lung. Based on these results, we can verified the applicability and the effectiveness of the respiratory gated method in the lung radiation surgery.

Histological Changes in Rat Testis by Injection of Hypertonic Saline (고장성 식염수 주사에 의한 흰쥐 정소의 해부학적 변화)

  • Recent evidence has revealed that the intratesticular injection of hypertonic saline(20%) resulted in a chemically castrated state such as nadir testosterone levels in rats. To confirm the efficacy of this simple saline-injection method further, we investigated the changes in the gross and microscopic anatomy of testis. Our study comprised three groups; intact(control) group, orchidectomy group and saline-injection (experimental) group. Single dose of hypertonic saline (sterilized, $750{\mu}{\ell}/testis$) were directly administered into both testis of adult rats (about 300 g BW). Bilateral orchidectomy was performed at the same day of saline injection. Following 30 days post-injection, reproductive tissues were surgically removed, weighed and fixed for histological examination. The body weights were not changed in both orchidectomy group and saline-injection group when compared to those in intact group. The wet weights of testis were significantly decreased in saline-injection group when compared to those in intact group. The wet weights of epididymis and seminal vesicle and prostate were significantly decreased in orchidectomy group and saline-injection group when compared to those in intact group. Macroscopically, the testes exerted slight atrophy and the tunica albuginea seemed to be intact in saline injection group. Histologically, however, larger parts of testicular tissue underwent necrosis and were barely recognizable after hematoxylin-eosin staining. In the same section, only the opposite part of the injection site was stained showing abnormal state of cell layers mostly fibrosis and infiltrated leukocytes. Sloughing of immature germ cells from the basement membrane along with shedding cells in the intraluminal space was notable in most seminiferous tubules from the saline injected testis. The present study confirmed that the direct injection of hypertonic saline into testis can induce a castration-like, testosterone-depriving effects on accessory sex organs. Our findings suggest that the efficacy of this less expensive and minimally invasive method seems to be almost even with that of conventional orchidectomy and chemical castration, though more in-depth evaluation should be supported.

Oxidation of Geosmin and 2-MIB in Water Using $O_3/H_2O_2$: Kinetic Evaluation (오존과 과산화수소를 이용한 Geosmin과 2-MIB 산화: 동력학적 평가)

  • Unpleasant tastes and odors in drinking water cause same problems for water utilities across Korea. Even though tastes and odors do not create health problems, they are main concerns for consumers who determine the safety of their drinking water. In this study, two different odor producing compounds(geosmin 2-MIB) in the Nakdong river water and rapid sand filtered rater were treated by advanced oxidation of $O_3/H_2O_2$ process. The experimental results showed that the removal efficiency of geosmin with the use of 5 mg/L of $O_3$ and $H_2O_2$ was higher than efficiency with the use of $O_3$ alone for both the raw water and the sand filtered water. And in general, the removal efficiency of geosmin was higher than 2-MIB in the sand filtered water. Under the range of $O_3$ concentration $0.5\sim2.0$ mg/L, the removal rate constants(k) of geosmin for the raw and sand filtered waters, and the one of 2-MIB in the sand filtered water were increased rapidly as doses of $O_3$, and $H_2O_2$, increased. The removal rate constants(k) do not increase any more when $H_2O_2/O_3$ ratio increases above the optimum ratio. The optimum ratio of $H_2O_2/O_3$, dose was $1.0\sim2.0$ for both geosmin and 2-MIB. The removal rate constant(k) becomes lower when OH radical consuming materials are present in raw water. The half-life of geosmin decreased rapidly as the $O_3$ and $H_2O_2$ doses increase in the sand filtered water. The half life decreased about 8.5 times with the use of 2 mg/L of $O_3$ and 10 mg/L of $H_2O_2$ than with the use of 2 mg/L of $O_3$ alone for the sand filtered water.

Toxic evaluation of phenanthrene and zinc undecylenate using the population growth rates of marine diatom, Skeletonema costatum (해산규조류(Skeletonema costatum)의 개체군 성장률을 이용한 phenanthrene와 zinc undecylenate의 독성평가)

  • We evaluated the toxic effects of phenanthrene (PHE) and zinc undecylenate (ZU) on the population growth rate (r) of the marine diatom, Skeletonema costatum. The r of S. costatum was determined after 96 hrs of exposure to PHE (0, 25, 50, 100, 200 and 300 mg L-1) and ZU (0, 5, 10, 15, 20 and 25 mg L-1). The results showed that r in the control (the absence of PHE and ZU) was greater than 0.04, while r in the treatment groups decreased with increasing PHE and ZU concentrations. PHE and ZU were shown to reduce r in a dose-dependent manner, with significant decreases occurring at concentrations above 50 and 10 mg L-1, respectively. The EC50 values of r in PHE and ZU exposure were 136.13 and 16.95 mg L-1, respectively. The no observed effect concentrations (NOEC) were 25 and 5 mg L-1, and the lowest observed effect concentrations (LOEC) were 50 and 10 mg L-1. These results indicated that concentrations of greater than 50 mg L-1 of PHE and 10 mg L-1 of ZU in marine ecosystems induced a toxic effect on the r of S. costatum. These results can serve as useful baseline data for the establishment of safety concentrations of PHE and ZU in marine ecosystems.

Evaluation on Usefulness of Applying Body-fix to Liver Cancer Patient in Tomotherapy (간암환자의 토모치료시 Body-fix 사용유무에 따른 유용성 평가)

  • Purpose: In every time radiation therapy set up errors occur because internal anatomical organs move due to breathing and change of patient's position. These errors may affect the change of dose distribution between target area and normal structure. This study investigates the usefulness of body-fix in clinical treatment. Materials and Methods: Among 55~60 aged male patients who has hepatocellular carcinoma in area of liver's couinaud classification, we chose 10 patients and divided two groups by using body-fix or not. When applying body-fix, we maintained a vacuum of 80 mbar pressure by using vacuum pump (Medical intelligence, Germany). Patients had free breathing with supine position. After working to fuse and consist MV-CT (megavoltage computed tomography) with KV-CT (kilovoltage computed tomography) obtained by 5 times treatments, we compared and analyzed set up errors occurred to (Right to Left, RL) of X axis, (Anterioposterio, AP) of Z axis, (Cranicoudal, CC) of Y axis. Results: Average Set up errors through image fusion showed that group A moved $0.3{\pm}1.1\;mm$ (Cranicoudal, CC), $-1.1{\pm}0.7\;mm$ (Right to Left, RL), $-0.2{\pm}0.7\;mm$ (Anterioposterio, AP) and group B moved $0.62{\pm}1.94\;mm$ (Cranicoudal, CC), $-3.62{\pm}1.5\;mm$ (Right to Left, RL), $-0.22{\pm}1.2\;mm$ (Anterioposterio, AP). Deviations of X, Y and Z axis directions by applying body-fix indicated that maximum X axis was 5.5 mm, Y axis was 19.8 mm and Z axis was 3.2 mm. In relation to analysis of error directions, consistency doesn't exist for every patient but by using body-fix showed that the result of stable aspect in spite of changes of everyday's patient position and breathing. Conclusion: Using body-fix for liver cancer patient is considered effectively for tomotherapy. Because deviations between group A and B exist but they were stable and regular.

Patient Position Verification and Corrective Evaluation Using Cone Beam Computed Tomography (CBCT) in Intensity.modulated Radiation Therapy (세기조절방사선치료 시 콘빔CT (CBCT)를 이용한 환자자세 검증 및 보정평가)

  • Purpose: Cone beam computed tomography (CBCT) using an on board imager (OBI) can check the movement and setup error in patient position and target volume by comparing with the image of computer simulation treatment in real.time during patient treatment. Thus, this study purposed to check the change and movement of patient position and target volume using CBCT in IMRT and calculate difference from the treatment plan, and then to correct the position using an automated match system and to test the accuracy of position correction using an electronic portal imaging device (EPID) and examine the usefulness of CBCT in IMRT and the accuracy of the automatic match system. Materials and Methods: The subjects of this study were 3 head and neck patients and 1 pelvis patient sampled from IMRT patients treated in our hospital. In order to investigate the movement of treatment position and resultant displacement of irradiated volume, we took CBCT using OBI mounted on the linear accelerator. Before each IMRT treatment, we took CBCT and checked difference from the treatment plan by coordinate by comparing it with the image of CT simulation. Then, we made correction through the automatic match system of 3D/3D match to match the treatment plan, and verified and evaluated using electronic portal imaging device. Results: When CBCT was compared with the image of CT simulation before treatment, the average difference by coordinate in the head and neck was 0.99 mm vertically, 1.14 mm longitudinally, 4.91 mm laterally, and 1.07o in the rotational direction, showing somewhat insignificant differences by part. In testing after correction, when the image from the electronic portal imaging device was compared with DRR image, it was found that correction had been made accurately with error less than 0.5 mm. Conclusion: By comparing a CBCT image before treatment with a 3D image reconstructed into a volume instead of a 2D image for the patient's setup error and change in the position of the organs and the target, we could measure and correct the change of position and target volume and treat more accurately, and could calculate and compare the errors. The results of this study show that CBCT was useful to deliver accurate treatment according to the treatment plan and to increase the reproducibility of repeated treatment, and satisfactory results were obtained. Accuracy enhanced through CBCT is highly required in IMRT, in which the shape of the target volume is complex and the change of dose distribution is radical. In addition, further research is required on the criteria for match focus by treatment site and treatment purpose.

A Study on Migration of Monomers from Kitchen Utensils Including PA, PU, ABS, and Acrylic Resin Plastics (폴리아마이드제 등 조리기구 중 모노머의 이행에 관한 연구)

  • The purpose of our paper was to investigate the migration level of 4,4'-MDA(4,4'-methylenedianiline), 2,4-TDA(2,4-toluenediamine), aniline, acrylonitrile and methylmeth acrylate from plastic cookwares into food simulants and to evaluate the safety of each monomers. The test articles for monomers were PA (polyamide) items for 4,4'-MDA, 2,4-TDA and aniline, PU (polyurethane) items for 4,4'-MDA, ABS (acrylonitrile-butadiene- styrene) items for acrylonitrile, and acrylic resin items for methylmethacrylate. All the article samples of 321 intended for contact with foods were purchased in domestic market. 4,4'-MDA, 2,4-TDA and aniline were analyzed by LC-MS/MS (liquid chromatography -tandem mass spectrometer), acrylonitrile by GC-NPD (gas chromatography-nitrogen phos phorus detector) and methyl methacrylate by GC-FID (gas chromatography-flame ionization detector). The migration level of monomers were within the migration limits of Ministry of Food and Drug Safety (MFDS). As a result of safety evaluation, our results showed that the estimated daily intake (EDI, mg/kg bw/day)s were $2.39{\times}10^{-9}$ and $1.20{\times}10^{-9}$ for 4,4'-MDA and 2,4-TDA of PA, $4.32{\times}10^{-9}$ for acrylonitrile of ABS and $2.27{\times}10^{-7}$ for methylmethacrylate of acrylic resin. Reference Dose (RfD, mg/kg bw/day) of acrylonitrile and tolerable daily intake (TDI, mg/kg bw/day) of methacrylate were established respectively as 0.001 by EPA (US Environmental Protection Agency) and as 1.2 by WHO (World Health Organization). When comparing with RfD and TDI, the EDIs of acrylonitrile and methylmethacrylate accounted for $4.32{\times}10^{-4}%$ and $1.89{\times}10^{-5}%$ respectively.

Evaluation of $^{14}C$ Behavior Characteristic in Reactor Coolant from Korean PWR NPP's (국내 경수로형 원자로 냉각재 중의 $^{14}C$ 거동 특성 평가)

  • This study has been focused on determining the chemical composition of $^{14}C$ - in terms of both organic and inorganic $^{14}C$ contents - in reactor coolant from 3 different PWR's reactor type. The purpose was to evaluate the characteristic of $^{14}C$ that can serve as a basis for reliable estimation of the environmental release at domestic PWR sites. $^{14}C$ is the most important nuclide in the inventory, since it contributes one of the main dose contributors in future release scenarios. The reason for this is its high mobility in the environment, biological availability and long half-life(5730yr). More recent studies - where a more detailed investigation of organic $^{14}C$ species believed to be formed in the coolant under reducing conditions have been made - show that the organic compounds not only are limited to hydrocarbons and CO. Possible organic compounds formed including formaldehyde, formic acid and acetic acid, etc. Under oxidizing conditions shows the oxidized carbon forms, possibly mainly carbon dioxide and bicarbonate forms. Measurements of organic and inorganic $^{14}C$ in various water systems were also performed. The $^{14}C$ inventory in the reactor water was found to be 3.1 GBq/kg in PWR of which less than 10% was in inorganic form. Generally, the $^{14}C$ activity in the water was divided equally between the gas- and water- phase. Even though organic $^{14}C$ compound shows that dominant species during the reactor operation, But during the releasing of $^{14}C$ from the plant stack, chemical forms of $^{14}C$ shows the different composition due to the operation conditions such as temperature, pH, volume control tank venting and shut down chemistry.

Effect of ${\alpha}$-Interferon Treatment on Serum ALT Levels in Patients with Chronic Hepatitis C (만성 C형 간염에 대한 ${\alpha}$-interferon 치료 후 ALT치의 변화)

  • The prognosis of chronic hepatitis C is very variable. In some, the disease is progressive and cirrhosis can develop from chronic hepatitis C. Hepatitis C virus(HCV) may act as a trigger towards hepatocellular carcinoma in patients with cirrhosis. Interferon has been used for the treatment of chronic hepatitis C in abroad, 16 patients with chronic C liver disease were treated with ${\alpha}$-interferon (alfa-2b; "Intron A" Schering Corp. Kenilworth. NJ). All patients were given ${\alpha}$-interferon in subcutaneous doses of 3 million units three times weekly for 1 to 9 months. During therapy, CBC and ALT levels were checked weakly to monthly. After therapy, patients were followed for 1 to 8 months. Among 16 patients treated with ${\alpha}$-interferon, progressive decrease of ALT levels was observed in 14(87.5%). In 11 patients(68.8%), ALT levels fell into the normal range during therapy, and in 9 of 11, within one month after therapy, 6 months after the completion of therapy in 4 of 9 patients(44.4%) whose ALT levels were in the normal range, ${\alpha}$-interferon seems to have effect in controlling disease activity in patients with chronic hepatitis C. But the changes in the usage of ${\alpha}$-interferon, dose and duration, long term follow up and more convenient and simple tests for HCV detection are recommended for the better effect and the exact evaluation on the effect of ${\alpha}$-interferon therapy in patients with chronic hepatitis C.

Effect of Gamma Irradiation on the Microbiological and Physicochemical Quality Changes of Steamed Waxy Corns during Storage (감마선 조사에 의한 증자 찰옥수수의 저장 중 미생물 및 이화학적 특성변화)

  • Effect of gamma irradiation on the microbiological and physicochemical characteristics of steamed waxy corns was determined in order to develop preservation techniques to enhance shelf-life during storage at room or low temperature. The initial total microbial count, and yeast & molds steamed waxy corns before irradiation were $2.1{\times}10^3CFU/g,\;1.9{\times}10^3CFU/g$, respectively. However, the total microbial count were completely inactivated and the count of yeast and molds were only 34 CFU/g at 3 kGy treatment When non-irradiated steamed waxy corns were stored at $25^{\circ}C$ for 7 months, the growth of total microbial counts and yeasts & molds were rapidly increased up to $7.1{\times}10^6CFU/g\;and\;1.9{\times}10^7CFU/g$, respectively. However, the total microbial counts at 3 kGy treatment and yeast & molds at 5 kGy treatment for 7 month storage were completely inactivated Similar result was observed at $15^{\circ}C$ and $4^{\circ}C$ storage. The Hardness of the irradiated steamed waxy corns and irradiation dose were not significantly different from those of the non-irradiated steamed waxy corns before storage, but the hardness of non-irradiated steamed waxy corns was significantly increased during the storage, compared to those of irradiated corns. The water content of both irradiated and non-irradiated steamed waxy corns after 7 month storage were significantly decreased from $65{\sim}60%$ to 30% but the changes of water content among treatments or irradiation doses were not significantly different during storage. Sugar contents in the irradiated steamed waxy corns were less free than those of non-irradiated samples and the amount of free sugar slowly increased as storage temperature increased. Sensory evaluation result showed that total sensory scores in irradiated steamed waxy corns were better than those of non-irradiated samples and total sensory scores were increased as irradiation doses increased.
