  • Purpose: The purpose of this study is to search effect that GCM joint treatment gets to range of motion of cervical, lumbar, trunk and anke. And cervical gets in ankle joint. Methods: Estimated body deformity using GCM body type assesment chart then measured range of motion of each region. After control group did as act freely after do experiment pre measurement control group did post measurement. After control group did as act freely after do experiment premeasurement, control group did postmeasurement. Each region was measured by measurer who each subject person differs. Experimental group did GCM joint treatment and all measurements each region by measurer who each subject person differs three times measure postmeasurement after premeasurement. When measure with each measurement, measured after leave and walk time interval for 10 minutes. Result: For the analysis of the result of experiment, the results is change amount comparison increased to keep in mind except cervical flexion and both ankle joint's dorsiflexion after experiment of experimental group. In experimental group, cervical, lumber and ankle joint of range of motion was significantly increased(p<.05).

족근 관절 유합술 후 족부 잔여 운동범위 (Retained Range of Motion of the Foot after Arthrodesis of the Ankle Joints)

  • The purpose of this study Is to assess the retained range of motion[RROM] of the foot after arthrodesis of the ankle joint, which might help designing the angle of arthrodesis. We reviewed the results including the RROM in 14 cases of ankle arthrodesis. Underlying causes included trauma group(six posttraumatic arthritis) and disease group(three osteoarthritis, three Charcot joints, one chronic synovitis and one pyogenic arthritis). We measured the RROM on the lateral roentgenograms of the ankle in full dorsiflexion and plantarflexion, The results were assessed by the AOFAS hindfoot scale[score]. The RROM in sagittal plane was $23.4{\pm}5.3$ degrees and the score was 71.5 points on average. The RROM and the score were $26.8^{\circ}{\pm}2.1$, 81.2 points respectively in trauma group and $20.8^{\circ}{\pm}5.6$, 64.3 points in disease group; $26.6^{\circ}{\pm}2.3$, 83.4 points in cases younger than 40 years of age and $21.6^{\circ}{\pm}5.7$, 64.9 points in the older. The mean score was 77.3 points in cases whose RROM were $24^{\circ}$ or more and 61 points in cases of less RROM; 69.6 and 73.4 points in cases with and without adjacent degenerative arthritis respectively. The score was 69.7, 73.3 and 71.5 points in cases whose angle of arthrodesis was in dorsiflexion, neutral, plantarflexion respectively. In conclusion, after ankle arthrodesis RROM of the foot was $23.4^{\circ}$, and the more the RROM, the higher the score. In cases with sufficient amount of RROM, the angle of fusion on sagittal plane might not influence the result significantly.

Comparison of the Immediate Effect of Ankle and Hip Joint Thera-band Exercise on the Balance Ability

  • PURPOSE: This study compared the effect of training ankle joint and hip joint thera-band exercise on balance. METHODS: The participants were divided into two groups of 11 each. Group A performed hip exercise after ankle exercise, and Group B performed ankle exercise after hip exercise. Using a green thera-band, the dorsiflexion and plantarflexion and hip flexion and hip extension were exercised repeatedly for 15 seconds three times with a five-second rest between each set. After the exercise and measurement of one area were complete, the exercise and measurement of the other area were performed at one-day intervals. The balance ability was assessed using a Tetrax and Y-balance test and repeated three times; the best values were taken. RESULTS: In the stability index (ST) of the static balance, the hip joint exercise group (HTG) during the follow-up of normal eye open (NO) revealed notable improvement over the ankle joint exercise group (ATG), and in the follow-up of the normal eye closed (NC), the ATG showed significant improvement over the HTG. In the pillow with eye closed (PC) follow-up, the ATG showed significant improvements over the HTG. At the left (Lt) and Y-balance test (YBT), the ATG showed significant improvements in the follow-up over the HTG (p <.05). CONCLUSION: In static balance, the ATG showed significant improvement in the follow-up of NC and PC over the HTG. In the dynamic balance, the Lt. dynamic balance on the non-dominant side in the ATG showed significant improvement in the follow-up over the HTG.

보행 속도 변화에 따른 발목 관절의 운동학적 분석과 하퇴 근육의 근전도 분석 (The kinematic analysis of the ankle joint and EMG analysis of the lower limbs muscle for the different walking speed)

  • The purpose of this study was to analyze the kinematic variables of ankle joints and EMG signal of the lower limbs muscle activity for the different walking speed. The subjects were 6 males of twenties. It was classified into three different walking speed-0.75m/s, 1.25m/s, 1.75m/s. The walking performances were filmed by high speed video camera and EMG signal was gained by ME3000P8 Measurement Unit. Tibialis anterior(TA), Gastrocnemius medial head(GM), Gastrocnemius lateral head(GL), Ssoleus(SO) were selected for the dorsiflexion and plantarflexion of the ankle joint. The result of this study were as follows: 1. In the gait cycle, The time parameters for the phases were showed significant difference without the terminal stance phase and terminal swing phase for the different walking speed. 2. The angle of ankle joint was no significant difference for each time point and MDF, MPF but increasing walking speed the angle had the increasing pattern slightly. 3. The angular velocity of ankle joint was showed the significant difference for LHC, RTO, RKC, LHU, MPF and MDF point along the walking speed. 4. TA was showed about 2-3 times muscle activity at the 1.75m/s than 1.25m/s in some phases. And it was showed the similar muscle activity between the 0.75m/s and 1.25m/s but, showed a little much muscle activity in the 0.75m/s. GM was showed about 2-3 times muscle activity in the 1.75m/s than 1.25m/s, and even much muscle activity at the 0.75m/s than 1.25m/s in some phases. GL was showed increasing pattern of muscle activity specially in the initial swing phase as the walking speed increased. SO was showed about 3 times muscle activity in the 1.75m/s than 1.25m/s during the plantarflexion of ankle joint. It was showed the similar muscle activity between the 0.75m/s and 1.25m/s but, showed a little much muscle activity in the 1.25m/s.

  • Objective : The purpose of this study is to evaluate the effects of hip joint exercises and orthotics on RCSP, ankle's range of motion, and core muscle strength of middle school students with pes planus. Method : Out of the original pool of 200 students, 60 students with pes planus (RCSP < -2) were selected for the study. The selected 60 students were then divided into four groups. The first group was a combined orthotics and exercise group (12 students), the second was the orthotics-only group (9 students), the third was the exercise-only group (8 students), and the last was the control group (10 students). Exercise groups worked out twice a week for 60 minutes per session over 8 weeks. The independent variables were corrective hip joint exercises and orthotics. The dependant variables consisted of kinematic and kinetic variables. The kinematic variables were RCSP, and ankle's range of motion (dorsiflexion and plantarflexion). The kinetic variables were muscles forces that consist in core muscle strength, which are hip joint adduction, abduction, and flexion muscles forces. Statistical analysis was performed via SPSS 18.0 with multivariate analysis of covariance (MANCOVA) and a paired t-test was used. Results : The left foot was more responsive to the treatments, both exercise and orthotics, than the right foot. RCSP improved significantly in the left foot for the first and third groups. Only the first group significantly improved hip joint adduction, abduction, and flexion muscles' strengths. As for the ankle's range of motion of the left foot, plantarflexion showed improvement when treated with exercise, orthotics, or both. Conclusion : This study found that exercise is more effective in correcting RCSP and foot orthotics is more effective in reinforcing core muscle strength. Future studies should expand on these results to examine the relationship between the ankle, hip, and pelvis.

Comparison of the Difference of Ankle Range of Motion and Hip Strength in Both Legs of Fencing Athletes with Ankle Instability

  • Background: Ankle sprain in the Lead Leg Side (ALLS) is common in fencing athletes, and studies comparing the ankle range of motion (ROM) and strength of both legs are insufficient. Objectvies: To compare the ankle ROM and hip strength between two legs in fencing athletes who has ankle instability in the lead leg side. Design: Cross-sectional design. Methods: Seven fencing athletes with ankle instability participated in this study, and they randomly assigned into ankle in the Lead Leg Side (ALLS) and ankle in the Rear Leg Side (ARLS). Instability was determined by the Cumberland Ankle Instability Tool (CAIT), and then joint ROM and hip muscle strength were measured. Results: There were significant differences in dorsiflexion ROM, hip strength (extension and abduction) between the ALLS with ankle instability and ARLS (P<.05). Conclusion: This study suggests that the ankle ROM and hip muscle strength of ARLS are greater than ALLS in fencing athletes with ankle instability.

Immediate Effects of Joint Mobilization Techniques on Clinical Measures in Individuals with CAI

  • Objective: Epidemiological research shows that 47 to 73% of athletes suffer from recurrent ankle sprains. Joint mobilization techniques (JMT) implemented in correcting may be beneficial in the management of ankle injuries. The purpose of this study is to examine the immediate JM on ankle complex as clinical measures in individuals with chronic ankle instability (CAI) through intervention. Method: Thirteen subjects with CAI (8 males and 5 females) participated in this study. Each subject tried total four alignments (Navicular drop test: NDT, Standing rearfoot angle: SRA, Tibia torsion: TT, and dorsiflexion range of motion: DFROM). The participants were performed the 10 meter shuttle run after JMT for post-task. Finally, it was tried to compare between pre-post tasks after shuttle run. Results: SRA and DFROM after intervention showed significant differences. SRA (p=.026), and DFROM (p=.034). Conclusion: We concluded that the JMT has resulted in improvement in SRA, DFROM. Increased DFROM and varus shapes of foot would be closed kinetic chain, indicating that reduce the risk factors of ankle sprain. Future study needs to be conducted in order to measure the effects of prolonged intervention of JMT.

족관절 골절 후 발생한 원위 경비 인대의 이소성 골형성 (Heterotopic Ossification of Distal Tibiofibular Syndesmosis after Ankle Fractures)

  • Purpose: To evaluate the effect on clinical course of heterotopic ossification of distal tibiofibular syndesmosis after ankle fractures. Materials and Methods: From June 2001 to May 2004, we found nine cases of heterotopic ossification of distal tibiofibular syndesmosis after ankle fractures. There were 8 male patients and 1 female patient; their mean age was 42 years old. There were 6 Weber type B and 2 Weber type C fractures, and there is 1 case with posterior malleolus fracture only. Among them, 8 ankle fractures were operated. Follow up period was averaged for 14 months. We were able to review radiographs at initial injury and to review clinical menifestation and radiographs at last follow up. We used an ankle-hindfoot scoring system of AOFAS which combined symptom, function and alignment with maximum score of 100 point. Results: In all cases ankle dorsiflexion and plantarflexion were not significantly different from that of the contralateral side. The patients who had developed heterotopic ossification in distal tibiofibular syndesmosis had a similar functional score. The mean ankle-hind foot score was 94 points. Conclusion: We concluded that the heterotopic ossification of distal tibiafibular syndesmosis after ankle fractures had little effect on clinical course and range of motion of ankle joint.

쇼트트랙 스피드 스케이트 운동시 하지 관절각 분석 (A Study on Joint Angle of Lower Extremity during Short Track Speed Skating)

  • This study, which is conducted on two groups of skilled people and unskilled people, is purposed to analyze joint angle of lower extremity during short track speed skating in order to find out efficient body position and propose ideal training method. Short Track Speed Skating has a wide range of research areas, but their findings indicate a promising area for further research. Targeting 8 people who are skilled in short track speed skating and 8 people who are not skilled in it, this study analyzes three dimensional images using super-high speed camera to compare differences between these two groups of people. In this study, 6 sections of body positions including right foot push-off, right foot recovery, right foot basic position, left foot push-off, left foot recovery, and left foot basic position were analyzed using 8 super-high speed cameras of VICON. These body positions were analyzed in order to find out joint angles of the hip joint, the knee joint, and the ankle joint. In the section 4 of the inner and outer parts of the left hip angles showed significant difference, and most of the sections of knee joint angles also showed significant difference. In the section 1 and 2 of the plantar flexion, dorsiflexion of ankle joints showed significant difference(p<.001) It was found out that there were differences between groups of skilled people and unskilled people in terms of lower extremity's joint angles such as angles of the hip joint, the knee joint, and the ankle joint.

Effect of Ankle Joint Mobilization and FES on Change of Ankle Movement and the Quality of Gait in Patients with Hemiplegia

  • This study was conducted to investigate the effect of FES and ankle mobilization on the ankle motion and the quality of gait of chronic hemiplegic patients with limited ankle joint motions. As research subjects, 24 chronic hemiplegic patients who could walk independently, regardless of assistive aids, were selected. Then, 8 subjects received mobilization randomly and 8 subjects received FES and 8 subjects received mobilization and FES, at the same time. The dorsiflexion PROM significantly increased in the group of mobilization therapy, mobilization and FES all together(p<.01). There were statistically significant differences among the three groups(p<.01). The 10m walking test significantly decreased in the group of mobilization therapy, mobilization and FES all together(p<.05). There were statistically significant differences among the three groups(p<.01). The gait velocity significantly increased in the group of mobilization therapy, FES therapy, mobilization and FES all together(p<.05). There were statistically significant differences among the three groups(p<.01). The stride length significantly increased in the group of mobilization therapy, mobilization and FES all together(p<.05). There were statistically significant differences among the three groups(p<.05). In conclusion, these findings demonstrate that rather than only using one treatment technique, applying mobilization and FES together brings a more satisfactory result to hemiplegic patients with limited ankle joint motions.
