A Clinical Study on the Effects of Pain control of Dong-Si acupunture therapy for the patients with lumbago (요각통환자(腰脚痛患者)의 동씨침(董氏鍼) 병행시술(竝行施術)이 통증감소(痛症減少)에 미치는 영향(影響)에 대한 임상고찰(臨床考察))

  • Objective : I planned to write this paper to use more Dong-Si acupunture treatment clinically by comparing Dong-Si acupunture with common acupunture for lumbago. Methods : This study was carried out on 56 patients with lumbago who were hospitalized in Dongshin Univ. Oriental Hospital from 1st, April, 2001 to 31st, Octover, 2001. Group A of 28 patients were taken both common acupunture and Dong-Si acupunture. And Group B of 28 patients were only taken common acupunture. And after 10 days of admission, I checked and compared VAS(Visual Analogue Scale) and improvement index out of these two groups. Results : In improvement index, Group A is $0.3932{\pm}0.1073$ and Group B is $0.2793{\pm}0.1080$. So Group A is thought to be significant(P=0.000). In VAS(Visual Analogue Scale), Group A is $6.9286{\pm}1.4889$ and Group B is $5.0000{\pm}1.4142$. So Group A is also thought to be significant(P=0.0000). Conclusion : The group with both Dong-Si acupunture and common acupunture treatment is more effective than the group with common acupunture treatment.

Effects of Acupuncture therapy on Cerebral Blood flow in Migraineurs (편두통 환자의 뇌혈류에 대한 침치료의 효과)

  • Objective : To investigate the effects of Acupuncture therapy on the migraineur by way of the change of the Cerebral blood flow. Methods : 6 migraineurs were inserted in acupoints on GV16, G20, S8 during 2 weeks. and, Mean Velocity (MV) and Pulsatility Index (PI) of MCA, PCA, ACA, BA were measured before and after treatments of 2 weeks, respectively. Results : MV of MCA of the migraineurs was decreased very much by 13.3%, and PI was decreased much by 10.4%. However, MV and PI of BA changed little. Conclusion : It is observed that Acupuncture therapy reduces pains of migraineur through decreasing Cerebral blood flow like MCA.

  • Background and Purpose : Ceramic based infra-red Therapy has been effective on derma system. The purpose of this case was to know the effect of ceramic based infra-red therap therapy on atopic dematitis. Material and Methods Among initial 25 outpatients with atopic detmatitis who visited Kangnam KyungHee Oriental hospital from Mar. 2000 to Sep. 2002, we excluded 4 outpatients that quited treatment after 1st treatment. We treated ceramic based infra-red therapy, herb medication treatment and Acupuncture women patients. Ceramic based infra-red therapy using GI-2000 system was lighted to under for 30 minutes. SoPungSan(消風散) was one of the effective herb medication on atopic dermatitis in oriental. In acupuncture treatment. We activated Kokchi(LI11, 曲池), Choksamni(S36, 足三里) and inhibited Yanggye(LI5, 陽谿), Yanggok(SI5, 陽谷). Results and Conclusions Ceramic based infra-red therapy, herb medication treatment and acupuncture therapy showed usfull effect on atopic dermatitis.

  • Objective: The main symptoms of the Wallenberg's syndrome are ataxia of gait, clumsiness of ipsilateral limbs, nausea and vomiting, vertigo, visual disturbance such as difficulty in focusing blurred vision diplopia, numbness, dysphagia, hoarseness, hiccup, nystagmus, Homer's syndrome. The purpose of this paper is to report the patient with the Wallenberg's syndrome who was improved by oriental medical treatment. Methods: In terms of exuberance of Yang(vital function) of the liver and stagnation of the liver Ki(energy), we let the patient take Chunghunhwadam-Tang, Hwangryeonhaedok-Tang and Dangkwihwalhyeol-Tang, at the same time, treated with acupuncture, Sa-Am and Dong-Si acupuncture therapy twice a day. Results: We could know that symptoms of the Wallenberg's Syndrome were improved after observing left sensory paralysis was almost recovered, hiccup was completely healed and vertigo was reduced to 1/10. Conclusions: We confirmed that oriental drugs and acupuncture by differentiation of syndromes make the patient with the Wallenberg's Syndrome reduce and improve symptoms he has.

The Research Trends of Pharmacopuncture Therapy on Cervical Pain Caused by Traffic Accidents

  • Objectives: The purpose of this study is to comprehensively review Korean domestic studies and investigate the research trends of pharmacopuncture therapy on cervical pain caused by traffic accidents. Methods: Domestic studies between February 1999 and May 2020 from four Korean databases were searched with combinations of keywords 'cervical pain', 'traffic accident', 'whiplash injury', and 'pharmacopuncture'. Results: 17 studies were selected for review, including 7 randomized controlled trials, 5 retrospective observational studies, 3 case reports, and 2 non-randomized controlled trials. Each study was reviewed by published year, study type, types of pharmacopuncture solutions, selected acupuncture points, dosage of pharmacopuncture solutions, frequency of treatment, concurrent treatments, outcome measurements, and the effectiveness of pharmacopuncture therapy. The results are as follows: (1) It showed that the total number of published studies had increased slightly in the last 10 years compared to the previous decade. (2) The pharmacopuncture solutions used in the studies were in the following order: Bee-Venom (蜂毒), Jungsongouhyul (中性瘀血), Hwangryunhaedoktang (黃連解毒湯) and Soyeom (消炎). Frequently used acupuncture points were GB20, GV21, A-shi point, GV16, EX-B2, and SI15 in order. (3) The most commonly used total injection dosage was 1.0 cc at a time, and the frequency of treatment was twice a week. (4) Concurrent treatments such as acupuncture, herbal medication, physical therapy and Chuna manual therapy were performed in all 17 studies. (5) Pharmacopuncture therapy showed positive effects on cervical pain caused by traffic accidents in all 17 studies reviewed. Conclusion: Pharmacopuncture therapy was effective in cervical pain caused by traffic accidents in all 17 studies selected. Further studies will be needed using more larger scales and more objective data to confirm the effectiveness of pharmacopuncture therapy and to generalize its application.

Effects of Honeybee Venom Acupuncture Therapy on the Poststroke Hemiplegic Shoulder Pain (중풍(中風) 편마비환자(偏痲痺患者)의 견관절(肩關節) 동통(疼痛)에 대(對)한 봉독약침료법(蜂毒藥鍼療法)의 효과(效果))

  • Hemiplegic shoulder pain(HSP) is one of the most frequent and difficult problems affecting poststroke hemiplegic patients. Honeybee venom acupuncture therapy(BVAT) is known for its pain relieving effects in arthralgia. To evaluate the effectiveness of BVAT on HSP, 24 patients were sequential1y allocated into BVA T treatment group and control group and monitored for 4 weeks at time interval of initial(T0), 1 week(T1), 2 weeks(T2) and 4 weeks(T4). In treatment group, 1:10000 honeybee venom solution 0.2㎖ was injected into acupoint(s) following Deqi three times a week. Kyonu(LI15) was used in the first week. Thereafter Kyonu(LI15) and Nosu(SI10) were used. Visual analogue scale of pain severity showed significant decrease in treatment group compared to control group at T2 and T4 evaluation. Painless passive range of motion of shoulder external rotation showed significant increase in treatment group compared to control group at T4 evaluation. Fugl-Meyer Motor Assessment of upper limb motor function and Modified Ashworth scale of the spasticity of upper limb showed no difference between two groups. BVAT showed as an effective therapy in HSP and further extensive clinical studies are expected.

  • By process of treatment for a case which diagnosed as glossopharyngeal neuralgia and admitted from the 4th, August, 2000 to the 1st, September, 2000, the results are as follows. Method & Resuits : This patient was diagnosed as Flaring up of Heart-fire(心火上炎), medicated Gamichengsimyeonja-eum(JiaWeiQingXinLianZi-Yin) and acupunture therapy was taken on proximal acupoints of right throat and parotid and distal acupoints inciuding Yuji(L10, 魚際), Taixi(K6,太谿), Zhaohai(K3, 照海), Hegu(LI4, 合谷) and Taichong(Liv3, 太衝). And negative therapy was taken on some pressure points of Jianzhongshu(SI15, 肩中兪) Jianjing(G21, 肩井) and Futu(LI18, 扶突). As a result, the patient's paroxysmal megalgia was improved. Conclusion : I consider that idiopathic pain diseases including glossopharyngeal neuralgia can improved by Oriental-Western collaborative medical treatment and continuous research must be accumulated subsequently.

  • Objectives : The purpose of this study is to investigate the trends of acupuncture treatment on lateral epicondylitis. Methods : We investigated the recent clinical studies of acupuncture treatment for lateral epicondylitis via searching PubMed from 2007.1.1. to 2016.1.1. This study examined published years, the authors, journals, countries, types of study and classified articles by techniques of acupuncture. Results : 20 theses out of 38 were included, excluding papers duplicated or whose texts are not identified or not satisfying both acupuncture and lateral epicondylitis simultaneously. The theses were constantly published every year. Analyzed by journal, the most published journal was Zhongguo Zhen Jiu. The studies of China had the greatest number of the searched theses. The types of study were reviews, case series, and study protocol. The acupuncture technique used were Hwachim(burning acupuncture therapy), sweet bee venom pharmacopuncture, percutaneous needle electrolysis, electroacupuncture, small needle-knife, combination with local points and Tianzong(SI11), and anti-ashi-point. Conclusions : In order to develop acupuncture treatment of lateral epicondylitis, clinical studies on high level with objective evaluation scale are needed.

The Effect of Combined Acupuncture Treatment on Acute Ankle Sprain: Case Series (급성 족관절 염좌에 대한 복합 침치료의 효과: 사례군 연구)

  • The purpose of this study is to report the effect of combined acupuncture treatment on acute ankle sprain patients. The subjects included in this study were 15 acute ankle sprain patients who visited Jaseng Hospital of Korean Medicine from Oct. 15th, 2012 to Feb. 8th, 2013. We treated 15 patients with combined acupuncture treatment. The treatment consisted of Hwangrunhaedok-tang (Huanglianjiedutang) pharmacupuncture, electroacupuncture (applied to GB39, ST36 on the affected side), acupuncture (applied to TE17, SI6 on the unaffected side with Dong-Qi therapy). To assess the effect of treatment, the numeric rating scale (NRS) was applied before and after treatment. After first combined acupuncture treatment, the NRS scores significantly decreased from $8.33{\pm}0.94$ to $2.26{\pm}0.44$ (p<0.01). We suggested that Combined acupuncture treatment are effective and useful on acute ankle sprain. And, further studies will be needed.

  • Objective : This study was designed to find the most effective and safest way to overcome Lumbago due to blood stasis and sprain involved in a few Meridian Tendino-musculatures by evaluating the effect of two kinds of Dong-gi(Dong-qi) Acupuncture(DGA) and by reporting their side effects. Material : 97 patients of out and admission patients were selected, who were diagnosed with lumbar sprain caused by repetitive bending, heavy weight lifting, unsuitable posture, concussion and falling down and whose Lumbago due to blood stasis and sprain in the concept of oriental medicine. Methods : 97 patients were divided into three groups. One is exclusive DGA group to which DGA and the method retaining needles on the acupoints for about 20 minutes were applied, the other is DGA combined active exercise group in which patients stretched their Meridian Tendino-muscuIatures with their hips moving up and downward repeatedly during DGA, the third is DGA combined passive exercise group in which patients were made to flex or extend their bodies on the auto flexion-distraction table in a prone position, from 10 to 20 degree, during DGA. In each group, bed rest, physical therapy and herbal medicine were used according to symptoms, in addition to DGA. In DGA method, "Su(Shu)" points of the meridian related to the involved Meridian Tendino-musculature were mainly chosen, that is, Sokkol(Shugu, B65), Hugye(Houxi, SI3), ChungJo(Zhongzhu, TE3) were used, for most LBP belonged to Bladder and Gallbladder Meridian Tendino-musculature disorders. Pyong-Bo-Pyong-Sa(Ping-Bu-Ping-Xie) such as Dong-Gi and Yeom-Jeon(Nian-Zhuan) was applied as Bo-Sa method. For evaluation of effectiveness, new score system was devised by severity of pain and range of movement. the score was given twice at patients' first and last visit and the difference between first and last score was regarded as a evaluation scale, the effectiveness was classified into four grade by evaluation scale.(scale : 12-15; excellent, 8-11; good, 4-7; fair, 0-3; bad) Results : 1. Exclusive DGA, DGA combined active exercise and DGA combined passive exercise group showed 97, 87 and 89% in effectiveness. 2. Exclusive DGA, DGA combined active exercise and DGA combined passive exercise group showed no aggravation of pain, range of movement. 3. In blood test of 34 patients, only one patient showed abnormal rise of sGOT, sGPT and $\gamma$-GTP at his first visit and the others didn't show any detrimental change. DGA had no bad influence upon BUN and creatinine of patients. Conclusion : For complex theraphy combining DGA, exercise, physical therapy and Herbal medicine proved to be highly effective on treating lumbago due to blood stasis and sprain, this is expected to be available for clinical use.
